INF-γ and il-2 Profile as Therapeutic Markers in Tuberculosis Patients at the Jamot Hospital of Yaounde-Cameroon

Background: Tuberculosis (TB) is one of the top lethal infectious diseases worldwide. In recent years, interferon-γ (INF-γ) release assays (IGRAs) have been established as routine tests for diagnosing TB infection. However, produced INF-γ assessment cannot permit to distinguish active ATB from latent TB infection (LTBI), especially in TB epidemic areas. In addition to IFN-γ, interleukin-2 (il-2), secreted by activated T cells, is involved in immune response against Mycobacterium tuberculosis. This could be involved in the follow up of treatment response. The aim of our study was to determine IFN-γ and il2 cytokines profiles of patients under antituberculosis treatment. Materials and Methods: A six months’ cross-sectional study was conducted at the Jamot Hospital of Yaoundé, from May to August 2021. Sociodemographic and clinical data as well as 5 mL of blood were collected from each participant. INF-γ and il-2 were determined using indirect Enzyme linked Immuno-Sorbent Assay (ELISA) according to the manufacturer’s recommendations and spectrum exam in combination with radiography and GeneXpert were used as standard. P-values Results: The results showed that men were more infected 14/61 (31.8%) with a high presence in active and resistant TB groups. The mean age was 41.3 ± 13.1 years with a 95% CI = [38.2 - 44.7], the age group with the highest infection rate was ranged between 31 and 40 years. The il-2 and INF-γ means were respectively 327.6 ± 160.6 pg/mL and 26.6 ± 13.0 pg/mL in ATB patients, 251.1 ± 30.9 pg/mL and 21.4 ± 9.2 pg/mL in patients with resistant tuberculosis, while it was 149.3 ± 93.3 pg/mL and 17.9 ± 9.4 pg/mL in cured patients, 15.1 ± 8.4 pg/mL and 5.3 ± 2.6 pg/mL in participants presumed healthy (p γ and il-2rates were observed between the different groups. Conclusion: Monitoring the serum levels of INF-γ and il-2 would be useful for the follow-up of anti-tuberculosis patients, particularly in the both cytokines association case.

基于术前CRP、PCT、il-2水平构建胃癌根治术后感染风险预测模型

目的探讨C-反应蛋白(C Reactive Protein,CRP)、血清降钙素原(Procalcitonin,PCT)、白细胞介素-2(Interleukin-2,il-2)水平对胃癌根治术后感染的预测作用,并以此构建风险预测模型。方法前瞻性选取2019年6月-2022年7月在江西省丰城市人民医院收治的胃癌患者106例,均给予根治术治疗,统计患者术后发生感染情况,收集患者一般资料和基础疾病发生情况,在术前取患者的空腹静脉血检测CRP、PCT、il-2水平。使用单因素、多因素Logistic回归分析患者的独立危险因素,并根据术前CRP、PCT、il-2水平构建风险预测模型。结果106例行胃癌根治术患者中发生感染有27例(25.47%),Logistic回归分析结果显示,年龄、吸烟史、分化程度、置管时间、手术时间、合并糖尿病及术前血清CRP、PCT均是胃癌根治术后感染的独立危险因素,il-2为独立保护因素(P2×CRP+1.559×PCT-0.843×il-2)]。结论术前血清CRP、PCT水平在胃癌根治术后感染患者中表达水平较高,il-2水平较低,使用三种血清学指标构建的风险预测模型可为患者行胃癌根治术后预防感染提供依据。

目的探讨瑞芬太尼复合麻醉对腹腔镜下胆囊切除（LC）患者血清il-6、il-2及T淋巴细胞亚群的影响。方法选取无为市人民医院2019年10月至2023年7月收治的122例行LC治疗患者为研究对象，根据麻醉药物不同分为对照组（n=59，七氟烷+丙泊酚复合麻醉）和观察组（n=63，瑞芬太尼+丙泊酚复合麻醉）。比较两组麻醉恢复情况、血清il-6、il-2水平、T淋巴细胞亚群变化及不良反应。结果观察组术后苏醒时间、自主呼吸恢复时间和导管拔除时间均短于对照组，差异具有统计学意义（Pil-6、il-2水平比较，差异无统计学意义（P>0.05）;T1（术后6小时）、T2（术后12小时）、T3（术后24小时）时，观察组il-6水平低于对照组，il-2水平高于对照组，差异有统计学意义（P0.05）；在T1、T2、T3时比较，观察组CD3+、CD4+、CD4+/CD8+高于对照组，CD8+高于对照组，差异有统计学意义（P0.05）。结论与七氟烷复合麻醉比较，瑞芬太尼复合麻醉对LC患者血清il-6、il-2水平及免疫系统的影响较小，可加快术后患者清醒和麻醉恢复，安全性尚可。

结核特异性il-2释放试验在结核感染中的诊断价值探讨

目的基于结核特异性白细胞介素2(il-2)释放试验的结果,探讨il-2在辅助诊断活动性结核(ATB)感染中的作用,以及用于监测治疗效果的价值。方法回顾性分析临床进行了结核感染排查的病例599例,其中确诊为活动性结核感染的病例128例,除外活动性结核感染的病例471例。(1)对与阴性对照共同孵育后上清中il-2(N)检测值制作ROC工作曲线,分析失去抗原刺激后PBMCs持续释放il-2的能力;(2)对抗原刺激后的响应值il-2(T-N),制作ROC工作曲线,分析对活动性结核感染的诊断价值;(3)对比分析两组患者il-2的阳性率;(4)观察响应值il-2(T-N)阴性和阳性转化时间与疾病进程的依从性。结果il-2(N)ROC曲线下面积为0.460,95%CI 0.402~0.516;il-2(T-N)ROC曲线下面积为0.788,95%CI 0.745~0.832;在活动性结核感染组,il-2(T-N)的阳性率显著高于非活动性结核感染组(χ^(2)=110.858,Pil-2发生阴阳性转化的平均时间为3.1个月,时间区间为0.5~7个月。结论PBMCs失去抗原刺激后il-2仅低水平分泌,对诊断ATB没有价值;结核分枝杆菌特异性抗原刺激后的响应值il-2(T-N),用于诊断ATB具有一定准确性;il-2(T-N)阴阳性转化的时间较短,在监测治疗效果方面存在一定价值,监测时间间隔建议为3个月。

考察il-2膀胱内给药、BCG与il-2联合膀胱灌注以及传统丝裂霉素C膀胱内注射在防止防治癌症复发的疗效。方法总共有60名膀胱癌病人,当中30个为观察组,运用il-2膀胱内注射与BCG+il-2联合膀胱灌注医治;另外30位充当对照组,采纳化疗药物治疗方式。运用SPSS 21.0专业统计分析软件,深入研究收集来的数据。结果观察组癌症复发概率仅仅3.33%,明显少于对照组百分之十五;观察组在存活率方面的表现形式显著超过对照组。主要问题表现为轻微到适中的尿道不适感,如尿频、尿急、尿痛,并伴随发烫等症状。结论 il-2膀胱内给药及BCG与il-2联合膀胱浸泡在预防膀胱癌再次发作方面表现出显著效果。这个方法为防止膀胱癌术后再次发作带来了新的途径。然而在实际应用中,还需针对可靠性和实效性进行仔细考察,接着应对种种困难,目的在于为病人给出更有效的治疗方案。

Objective Conflicting data have been generated from previous studies to determine which kind of relationship exists between HIV-1 specific CD8 Tcell responses and HIV-1 viral load or CD4 count over the course of *** this study,153 HIV-1 infected LTNPs were enrolled to investigate the role of HIV-1 specific CD8 T-cell responses in chronic HIV-1 infection among HIV-1 infected former blood *** The patients were stratified into three groups according to CD4 count：CD4≥500 cells/μL;350 cells/μL≤CD4〈500 cells/μL;CD4〈350 cells/μ*** were isolated from the patients＇ anticoagulated blood ***-2 and IFN-γ secretions of CD 8 T cells against 17 HIV-1 consensus B full peptide pools were analyzed by using ICS *** An overall inverse correlation were observed between CD4 count and plasma viral *** no significant difference was observed during the comparisons of frequency/breadth of HIV-1 specific CD8 T cell responses,CD4 count stratification analysis showed that different correlation pattern existed in three strata：as for patients whose CD4 counts were less than 350 cells/μL,no significant correlations were identified between frequency/breadth of HIV-1 specific CD8 T cell responses and CD4 count/viral load;as for patients whose CD4 counts ranged from 350 cells /μL to 500 cells/μL,significant correlation was only observed between the response breadth of il-2＋IFN-γ＋ CD8 T cells and CD4 count;however,as for patients whose CD4 counts were more than 500 cells/μL,direct correlations were identified between il-2＋IFN-γ＋/il-2＋/IFN-γ＋ CD8 T cells and viral load or CD4 *** Universal consistent inverse correlation was only indentified between CD4 count and viral *** relationship between HIV-1 specific CD8 T cell responses and CD4 count/viral load varied in different CD4 strata,which showed that better preserved CD4 T cells were correlated with better CD8 T cell functions.

The Role of il-2, il-4, il-10 and IFN-γ Cytokines Expression in the Microenvironment of Cervical Intraepithelial Neoplasia

Objective: To investigate the role of il-2, il-4, il-10 and IFN-γ cytokines in the microenvironment of cervical intraepithelial neoplasia. Methods: 180 patients participating were enrolled in this trial, where 50 cases are in control group, 50 cases are in low-grade squamous intraepithelial lesion (LSil) group, 50 cases are in high-grade squamous intraepithelial lesion (HSil) group, and 30 cases have cervical carcinoma. ELISA methods are used to detect the expression of il-2, il-4, il-10 and IFN-γ in the lavage fluid, and all data is analyzed using one-way analysis of variance. Results: The expression of il-2 and IFN-γ in cervical lavage fluid gradually decreases with the aggravation of the lesion. The expression of il-4 and il-10 increases by the aggravation of pathological changes. There were statistically significant differences in il-2 level among the four groups. IFN-γ levels are significantly different between the cervical cancer group and the other groups, also between HSil and LSil group, and between HSil and the control group, but no statistically significant difference was observed in IFN-γ e between LSil and the control group. The expressions of il-4 and il-10 between the cervical cancer group and the other groups were significantly different, and also between HSil and the control group. But there was no statistically significant difference between LSil and the control group, HSil and LSil. Conclusion: The cytokines of il-2, il-4, il-10 and IFN-γ in the microenvironment of the cervix play an important role in the occurrence and development of cervical intraepithelial neoplasia. This study may provide the evidence for diagnosis and treatment of cervical intraepithelial neoplasia.

Locoregional il-2 low dose applications for gastrointestinal tumors

AIM: To explore the feasibility of local interleukin 2 (il-2)in patients with different forms of abdominal cancer. This required experimentation with the time interval between il-2 applications and the methods of ***: Sixteen patients with stages Ⅲ and Ⅳ of gastrointestinal malignancies (primary or metastatic) who were admitted to our Department of Gastroenterology were treated with locoregionally applied il-2 in low ***: No major problems applying locoregional il-2 were encountered. In 6 out of 16 patients, a modest but clinically worthwhile improvement was obtained. Adverse effects were minimal, The therapeutic scheme was well tolerated, even in patients in a poor ***: This study demonstrates the feasibility of low dose locoregional il-2 application in advanced abdominal cancer. Local il-2 therapy gives only negligible adverse effects. The results suggest that it is important to apply intratumorally. Local il-2 may be given adjunct to standard therapeutic regimes and does not imply complex surgical interventions. These initial results are encouraging.
