The terminology related to the hydatid cyst is sometimes inappropriate and wrong designations are used, based on incorrect concepts. ＂hydatid cyst＂ is the larval form of the tapeworms of the genus Echinococcus granulosus, a parasite found in the small intestine of carnivores. The disease, called cystic echinococcosis, As among the most neglected parasitic diseases despite the development of new drugs and other treatment modalities. Although all of us know the vital cycle of the parasite and the different aspects of the disease, the designations around the parasite, its evolution and some therapeutic procedures are not uniform. It would be useful, for all of us, to use the same nomenclature and it is absolutely necessary that the nomenclature As correct and universally accepted. In this paper I remember the correct terms related to all aspects of the hydatid cyst＇s nomenclature, including especially the anatomical and surgical terminology, as well as the criteria to define the cyst inactivity and the way to know when the cyst range the inactive stage after therapy.

hydatid cyst of the liver (KHF) is a parasitic condition caused by the development of the larval form of the dog tapenia Echinococcus granulosus. In sub-Saharan Africa, few writings are interested in this pathology, which remains common and constitutes a public health problem in highly endemic countries [1] [2]. We report the observation of a 61-year-old hypertensive patient owner of a dog admitted for abdominal pain without abdominal mass. In whom ultrasound and computed tomography have made it possible to make the diagnosis of hydatid cyst of the liver (KHF). The patient underwent surgery. We performed an associated conservative treatment post-operative albendazole. The post-operative follow-up was simple. In our patient the evolution was good after a setback of more than 2 months. Through this observation and a review of the literature, we insist on the contribution of imaging in diagnosis and treatment, which is essentially surgical.

Introduction: hydatid cyst formation is rare in organs such as muscle, bone and spine. Para-spinal involvement is an uncommon finding with a prevalence of less than 0.5% in the literatures. Presentation of Case: In the present paper, we reported a case of primary hydatid cyst in lumbar para-spinal area. The patient presented with complaint of chronic back pain and swelling in right thoracolumbar area. A suspicious mass was revealed in lumbar spine x-ray presenting as multi-loculated cystic mass on further evaluation by magnetic resonance imaging (MRI). The patient underwent surgery and the cyst was excised completely. Histopathological evaluations confirmed the hydatid disease. Discussion: hydatid disease is a zoonotic infectious disease caused by Echinococcus granulosus. The parasitic cysts can form in any part of the host body. The liver and lungs are the most commonly involved organs. The occurrence of these lesions in certain areas such as para-spinal muscles is rare. The cystic mass is detected by radiologic modalities and is confirmed by pathologic examination. Surgery is the mainstay of treatment. Conclusion: hydatid disease should be on differential diagnosis list facing a cystic lesion in any part of the body especially in the endemic areas. Moreover, radiologic and serologic assessments are important in confirming the diagnosis.

A Nonglycosylated 27 KDa Molecule as Common Antigen between Human Breast Cancer and Echinococcus granulosus hydatid cyst Wall

Background: hydatid cyst, which has anti-cancer activities, is outwardly covered with the cyst wall. It is in close contact with the host tissues and its molecules presented to the immune system. In this work immunological reaction of the sera of breast cancer patients with the hydatid cyst wall antigens has been investigated. Method: For this purpose, sera of patients with breast cancer, hydatid cyst and normal human sera were collected and their reaction with hydatid cyst wall antigens was tested using western immunoblotting technique. Results: All sera of patients with breast cancer, hydatid cyst and also human normal sera reacted with a band in western immunoblotting. However, sera of patients with breast cancer showed reaction with a 27 KDa band. Results of this work also revealed that this band was not glycosylated and may express only in some stages of breast cancer development. Conclusion: Sera of patients with breast cancer cross reacted with a nonglycosylated antigen of hydatid cyst wall. However, more work is recommended to find if this band involves in anticancer activity of the hydatid cyst.

Laparoscopic management of hydatid cysts using long ribbon gauze:An initial experience of 37 consecutive cases

Objective:The laparoscopic approach is becoming the standard of care for many surgical disorders.However,in the case of hydatid cysts,laparoscopic management is challenging due to the risk of spillage of hydatid fluid,which can cause an anaphylactic reaction and recurrence.Here,we report our initial experience with laparoscopic partial pericystectomy of hydatid cysts using long ribbon gauze to decrease intra-operative spillage.Method:This was a retrospective study(between January 2010 and December 2021)in the Department of Surgical Gastroenterology,Sanjay Gandhi Postgraduate Institute of Medical Science,a tertiary care referral center in northern India.Here,we have included 37 consecutive patients with hydatid cysts of the liver and spleen.Diagnosis was made by laboratory and imaging findings(abdominal sonography or contrast enhanced CT scans).All patients were managed with laparoscopic partial pericystectomy.Intraoperatively,a betadine-soaked long ribbon gauze,high-pressure suction canula,and an endo-bag were used in all patients.The collected data included patient demography,location,size,and number of cysts,WHO type,operative time,blood loss,postoperative complications,hospital stay and follow-up.Result:In our series,the mean age was 38.4±13.6 years,15(40.5%)were men and 22(59.5%)were women.The right lobe of the liver was the most commonly affected site(21,56.8%).The mean operative time was 80.0±32.0 min,and intraoperative blood loss was 23.6±11.5 mL.Bile leak was present in 6(16.2%)patients.There was no mortality.The hospital stay was 5(3,9)days,and no recurrence was observed at a median follow-up of 36 months.Conclusion:Laparoscopic partial pericystectomy using this technique is safe in the management of hydatid cysts.Simply,proper packing and safe removal of soaked gauzes can minimize the incidence of postoperative complications and recurrence.

Protoscolicidal and immunomodulatory activity of Ziziphora tenuior extract and its fractions

Objective:To evaluate the scolicidal and immunomodulatory effect of the Ziziphora tenuior(Z.tenuior) extract and its fractions.Methods:Protoscolices were treated with six concentrations(3,5,10,25,50,and 100 mg/mL) of Z.tenuior extract and its fractions(ethanol,petroleum ether,ethyl acetate and chloroform) in periods of 10,20,30,40,50 and 60 minutes,and viability of protoscolices was evaluated using the 1.0%eosin.To examine the immunomodulatory effects of Ziziphora and its fractions on macrophage cells,the non-toxic concentration of extract and different fractions determined by MTT assay,and the Griess reaction was used to measure the level of nitrite as an indicator of nitric oxide by the macrophage cells in 10,100 and 200 μg/mL in 24 hours at 37 ℃.Results:In this study,the Z.tenuior extract at 10 mg/mL concentration was able to kill all protoscolices during 20 minutes.By increasing the concentration to 25 mg/mL,the scolicidal time reduced to 10 minutes.Regarding the effect of different fractions of Z.tenuior,the ethanolic fraction showed the highest scolicidal activity.The extract demonstrated an inhibitory effect on the activity of macrophages and reduced nitric oxide production.Although the petroleum ether and ethanolic fractions of the extract reduced nitric oxide production,nevertheless,this effect was only significant at 10 and 100 ug/mL concentrations(P<0.05).Conclusion:The Z.tenuior extract and its fractions were effective against protoscolices yet the effect of total extract was considerable.Our findings indicates that the extract and its ethanolic and petroleum ether fractions could have anti-inflammatory properties.

Liver hydatidosis is the most common clinical presentation of cystic echinococcosis(CE).Ultrasonographic mass surveys have demonstrated the true prevalence,including the asymptomatic characteristic of the majority of cases,providing new insight into the natural history of the disease.This raises the question of whether to treat or not to treat these patients,due to the high and unsuspected prevalence of CE.The high rate of liver/lung frequencies of cyst localization,the autopsy findings,and the involution of cysts demonstrated in long time follow-up of asymptomatic carriers contribute to this discussion.The decision to treat an asymptomatic patient by surgery,albendazole,or puncture aspiration injection and reaspiration or to wait and watch,is based on conflicting reports in the literature,the lack of complications in untreated patients over time,and the spontaneous disappearance and involution of cysts.All these points contribute to difficulties of individual clinical decisions.The patients should be informed of the reasons and the risks of watchful/waiting without treatment,the possibility of complications,and the risks of the other options.As more information on the natural history of liver hydatidosis is acquired,selection of the best treatment will be come easier.Without this knowledge it would be very difficult to establish definitive rules of treatment.At present,it is possible to manage these patients over time and to wait for the best moment for treatment.Followup studies must be conducted to achieve this objective.

cystic echinococcosis (CE) is a widely endemic helminthic disease caused by infection with metacestodes (larval stage)of the Echinococcus granulosus tapeworm.E.granulosus are common parasites in certain parts of the world,and are present on every continent with the exception of Antarctica.As a result,a large number of people are affected by CE.The increased emigration of populations from endemic areas where prevalence rates are as high as 5-10% and the relatively quiescent clinical course of CE pose challenges for accurate and timely diagnoses.Upon infection with CE,cyst formation mainly occurs in the liver (70%).Diagnosis involves serum serologic testing for antibodies against hydatid antigens,but preferably with imaging by ultrasound or CT/MRI.Treatment methods include chemotherapy with benzimidazole carbamates and/or surgical approaches,including percutaneous aspiration injection and reaspiration.The success of these methods is influenced by the stage and location of hepatic cysts.However,CE can be clinically silent,and has a high risk for recurrence.It is important to consider the echinococcal parasite in the differential diagnosis of liver cystic lesions,especially in patients of foreign origin,and to perform appropriate long-term follow-ups.The aim of this review is to highlight the epidemiology,natural history,diagnostic methods,and treatment of liver disease caused by E.granulosus.

BACKGROUND A collecting duct carcinoma is a very rare, malignant renal epithelial tumor.Distant metastases are present in one third of cases at the time of diagnosis. It is known to have a poor prognosis.CASE SUMMARY A 42-year-old male was sent to our surgery clinic for removal of a 119.2 mm ×108.3 mm encapsulated cystic mass, which was localized in the 8th segment of the right liver lobe. The lesion was first identified on ultrasonography. A computed tomography scan confirmed the presence of a Bosniak type Ⅲ cystic lesion,which affected the liver and convexity of the right kidney. Surgical intervention involved a right nephrectomy, with removal of the cystic mass. The patient was mobilized on the first postoperative day and was discharged after 7 d. The histological and immunohistochemical examination revealed a low-grade collecting duct renal carcinoma, which is a rare variant of papillary carcinoma,with low malignant potential. The patient did not receive chemotherapy and after 21 mo of follow-up, a radiological examination and laboratory analyses showed normal aspects. No relapse or other complications were reported.CONCLUSION To manage renal tumors properly, a correct histopathological diagnosis is crucial,as is early diagnosis and correct surgical treatment.

In this study we reported one case of combined procedure for coronary artery bypass grafting and excision of right pulmonary hydatid cyst. Concerns of possible hydatid systemic dissemination as a result of direct vascular breaches are raised. We suggest that avoidance of cardiopulmonary bypass (CPB) if that possible is beneficial for the treatment. If not possible then the excision and clearance of the hydatid cyst should be done in the first place before going on bypass.
