Objective: The aim of this study was to evaluate correlation between plasma homocysteine and coronary artery disease (CAD) in Indian patients. Methods: This study included 150 patients, 100 subjects in study group with angiographically diagnosed CAD and 50 subjects in control group with a normal coronary angiogram. In the study group, patients were divided into three subgroups viz.: CAD only, CAD with hypertension and CAD with type 2 diabetes mellitus. Plasma homocysteine, lipid profile and other risk factors were compared. Results: Mean homocysteine levels in study group (38.34 ± 15.25 μmol/L) were significantly higher (p Conclusion: The study demonstrated that increased levels of homocysteine are independently related to CAD. However, further studies involving a larger sample size will be required to substantiate the findings of the current study.

Objective: The aim of this study is to evaluate vitamin B12, folate, and homocysteine status in pregnant women in the third trimester of pregnancy and their relationship to fetal birth weight and their correlation to corresponding neonatal cord blood levels, and in addition, to evaluate the possibility of maternal serum homocysteine level as a predictor of low birth weight infants. Subjects and Methods: In this cross-sectional study, a total of two hundred pregnant women in third trimester (≥28 weeks) were recruited. After a detailed obstetrical and medical history, and clinical assessment, participants were subdivided into two groups: Group (A)—pregnant women who delivered average birth weight (ABW) infants and Group (B) for those who delivered low birth weight (LBW) infants between completed 37 and 42 weeks. Results: Vitamin B12 deficiency was observed in 24.1% of the total cohort. The mean vitamin B12 level was significantly lower in group (B) compared to group (A) (195.2 ± 38.9 vs. 225.9 ± 66.59 respectively P = 0.008). The mean level of homocysteine for women in group (B) was significantly higher than those determined from women in group (A) (9.10 ± 5.9 vs. 7.6 ± 3.83 respectively, P = 0.042). On the other hand, the mean folate levels showed statistically insignificant differences between both groups. The mean cord vitamin B12 level was significantly lower in LBW infants in comparison to ABW infants (277 ± 61.93 vs. 312.03 ± 81.87 respectively, P = 0.015), while the mean level of cord homocysteine for LBW infants was significantly higher than those levels determined from ABW infants (7.9 ± 3.79 vs. 6.6 ± 2.09 respectively P = 0.0049). Conclusion: Maternal micronutrients particularly cobalamin deficiency could be significant risk for LBW infants. Hyperhomocysteinemia has been shown to be a predictor for adverse pregnancy outcomes particularly LBW.

Objective:To study the relationship between isolated systolic hypertenson and MTHFR,CBS the keys enzyme of homocysteine metabolism,gene polymorphisms,related variation of homocysteine level,their metabolic factors,so as to screen the predisposing gene and intermediate phenotype of ISH.Methods:From Tianjin hypertension prevention and cure area selected by examinations to get 55 examples,matched control 46 example,apply the molecular biology method to detect MTHFR C677,CBS G919,T833,C341 different site gene polymorphisms,and measurese the concentration of Hcy,folic acid,vitamin B12 and B6 at the same time,then proceed the relativity analysis.Results:The genotype frequency of ISH group is different from that of control group( P homocysteinemia patients in ISH group( P homocysteine so as to boost the onset of ISH.Then we can draw the conclusion that MTHFR and CBS genes may be the predisposing gene of ISH,Hcy my be the improtant intermiediate phenotype.Reasonable complementation of folic acid,vitamin B 6,vitaminB 12 can lower the plasma Hcy level,and defer ISH occurrence and progress.

Background: Vitamin B12 (cobalamin) is an essential micronutrient necessary for DNA methylation and plays role in lipid metabolic reactions. Metformin is the first therapeutic choice for T2DM management. Prolonged use of metformin causes vitamin B12 deficiency due to poor absorption by interfering with calcium-based vitamin B12 absorption. Vitamin B12 deficiency leads to elevated homocysteine levels. The aim of this study was to evaluate serum vitamin B12 and homocysteine levels in type 2 diabetic patients with and without metformin therapy. Methods: A cross-sectional study was conducted on two hundred and thirty diabetic patients (180 males and 50 females). Their ages ranged from (30 - 60 years) living in Saudia Arabia at Al-Madinah Al-Monawarah. Patients were selected at outpatients clinics of Islamic University Medical Center during follow up at internal medicine and endocrinology clinic. The included patients were diagnosed with type 2 diabetes mellitus according to American Diabetes Association (ADA) Criteria. The included patients were categorized into two groups according to treatment with metformin drug. Laboratory measurements included serum level of vitamin B12, serum total homocysteine, serum fasting glucose and serum folate. Blood EDTA samples were used to measure HbA1c and MCV. Neurological examinations were performed to detect presence of peripheral neuropathy using Toronto Clinical Neuropathy Score (TCSS), which is a validated and reliable scale for the diagnosis and staging of diabetic polyneuropathy. Results: There were no statistical differences between the two groups as regard (age, sex, smoking, weight, BMI, systolic blood pressure, diastolic blood pressure, fasting blood glucose, Folate and MCV). There were statistical differences between the two groups as regard (duration of diabetes, duration of metformin therapy, dose of metformin, Serum homocystein and HbA1c). The mean of vitamin B12 (pg/mL) of group 1 (312.65 ± 92.28) was lower than that of group 2 (381.55 ± 88.04). In group 1 number of

Riboflavin has been suggested to act with folate to lower homocysteine (Hcy). However, these interactions may differ among the several known forms of folate. Therefore, we examined the effects of riboflavin interactions with 5-methyltetrahydrofolate (5-MTHF) and tetrahydrofolate (THF) on changes in Hcy and folate derivative levels, under conditions with and without methionine addition. Rat hepatocytes were cultured for 48 hours in medium with or without 2.64 μM riboflavin addition, under the following conditions: 1) without the addition of either methionine or folate;2) with addition of 2 μM folate derivatives [(A): 5-MTHF, (B): THF];3) with addition of both 5 mM methionine and 2 μM folate derivatives [(A): 5-MTHF, (B): THF]. The supernatants were collected at 0, 24, and 48 hours for Hcy and folate derivative measurements. The Hcy lowering effect of 5-MTHF, as well as inhibition of 5-MTHF reduction and THF elevation, appeared to be enhanced by riboflavin addition. THF addition did not lower the Hcy level, regardless of the presence of riboflavin and/or methionine, while THF and 5,10-methenyl THF levels were maintained. Further examination is needed to elucidate the interactive effects of riboflavin and folate derivatives on Hcy and folate metabolism.

homocysteine is an intermediate substance formed during the breakdown of the amino acid methionine and may undergo remethylation to methionine or trans-sulfuration to cystathionine or cysteine. The metabolism occurs via two pathways: remethylation to methionine, which requires folate and vitamin B12;and transsulfuration to cystathionine, which requires pyridoxal-5’-phosphate. The disturbances in the metabolic pathways lead to the accumulation of Hcy, either by insufficient transsulfuration (through CBS mutations or vitamin B6 deficiency) or by a blockage of remethylation. In the latter case, folate or vitamin B12 deficiency may be involved, as well as MTHFR. High levels of Hcy induce sustained injury of arterial endothelial cells, proliferation of arterial smooth muscle cells and enhance activity of key participants in vascular inflammation, atherogenesis, and vulnerability of the established atherosclerotic plaque. Hyperhomocysteinemia has become the topic of interest in recent years. It has been highly associated with increased risk for cardiovascular disorders, such as, atherosclerosis, thromboembolism and dyslipidemia. Women with PCOS show constellation of metabolic syndromes. Obesity, hyperandrogenemia and type 2 diabetes mellitus is the hallmark of PCOS which later becomes the risk factors for cardiovascular disease. Various studies had revealed the presence of increased Hcy level in PCOS women which may or may not be associated with other biochemical parameters. Intense treatment for PCOS can influence homocysteine levels.

Objective： To study the effects and mechanism of homocysteine（Hcy） on Parkinson＇ s disease（PD） induced by 6-hydroxydopamine （6-OHDA） in vivo. Methods：Forty rats were divided into 4 groups. 6-OHDA or the solvent of 6-OHDA was focally administrated to induce PD, 2 h later Hcy or 0.9% sodium chloride was administrated in the ipsolateral substantial nigra（SN）. We Used behavioral testing, Immolunohistochemical techniques, biochemistry techniques to detect the injury of SN. Results：The rotary turns of PD rats induced by 6-OHDA showed significant increase after treatment with Hcy compared with the controls（P 〈 0.05）. Also the numbers of tyrosine hydroxylase（TH）-stained neurons were decreased, and dendrites were fragmented and truncated. Free radicals were increased and antioxidaat enzymes decreased. Conclusion：Focal infusion of Hcy into the SN increased the vulnerability of the doparninergic neurons to 6-OHDA-induced degeneration, it seems that the endangering effect of Hcy is due to exacerbating oxidative stress.

Objective: To observe the concentration of se-rum homocysteine in intellectuals and the re-lated influential factors. Methods: The concen-trations serum homocysteine and saliva cortisol were measured in 138 intellectuals from three cities, Tianjin, Guangzhou and Chengdu in China. All the subjects had senior titles of tech-nical post, aged 40-69 years. Results: The mean value of serum homocysteine concentration in intellectuals was 20.6±0.8μmol/L, higher than the reference value. With the increase of cortisol levels the homocysteine concentrations rise (P homocysteine con-centration was highest in 40-49 years old group. Men had higher homocysteine level than women in this investigation. According to the mean value of homocysteine concentration among different cities, Tianjin was highest, Chengdu medium, Guangzhou lowest. Conclusion: The serum homocysteine concentration of intellec-tuals is higher than the reference value. The stress level, gender and resident cities might contribute to the differences in serum homo-cysteine concentration in Chinese intellectuals.

Objective: To investigate the correlation between plasma hydrogen sulfide (H2S), homocysteine (Hcy), folic acid and vitamin B12 in patients with acute promyelocytic leukemia (APL) before and after treatment. Methods:A total of 26 hospitalized patients with APL were randomly selected as case group and 26 healthy persons as control group. The concentration of H2S, Hcy and folic acid in plasma of case group and control group were measured, respectively. The statistically significant difference was investigated by comparing the acute onset phase and clinical remission of case group with those of healthy control group. Results:The concentration of H2S and Hcy of plasma in patients of case group during acute onset phase significantly increased, and concentration of folic acid significantly decreased, and there were both statistically significant differences as compared with those during clinical remission and those of control group (P0.05). The change in concentration of plasma H2S was positively correlated with that of Hcy and negatively correlated with that of folic acid in patients of case group before and after treatment. Conclusions:The increase of plasma H2S in patients with APL may be related to the changes in concentration of Hcy and folic acid.
