Objective:To evaluate the berries of Phytolacca dodecundra(***) for its effect on histoplasma cupsulatum ***(HCF) and for the treatment of cases of epizootic lymphangitis(***:Samples were collected from un-ruptured nodules of cases of EL at Debre Zeit and Akaki(central Ethiopia).Mycologieal culture and isolation of HCF were performed at the Akliln Lemma Institute of *** screening was done for n-butanol extract of *** to delect alkaloids,saponins,phenolic compounds and *** minimum inhibitory concentrations(MICs) and minimum fungicidal concentrations(MFCs) ol aqueous and n-butanol extracts of *** against FICF were determined by agar dilution *** the in vivo trial.5%simple ointment was prepared from n-butanol extract and applied topically to 24(twelve early and twelve moderate) cases of ***:Phytochemical screening showed that n-butanol extract ol *** was positive lor *** and phenolic compounds but negative for *** MFCs of n-butanol and aqueous extracts of *** were(0.039%-0.078%) and(0.625%-1.250%),*** MFCs of n-butanol and aqueous extracts of *** were(0.078%t-0.156%)and(1.250%-2.500%),*** MIC and MFC of ketoconazole(positive control) was(1.200×10^(-5)%-2.500×10^(-5)%) and(5.000× 10^(-5)%-1.000×10^(-4)%),respectively while growth was observed on free medium(negative control).From the total of 24 treated cases of EL,14(58.3%) responded lo treatment;however,10(41.7%) did not respond to *** was no significant difference in the degree of response to treatment between early and moderate cases(χ~2=0.086:P=***:It can be concluded that n-butanol extract of *** demonstrates antifungal effects while the aqueous extract shows no antifungal activity.

Here,we report the identification of histoplasma causing an unexplained disease cluster in Matthews Ridge,*** March 2019,14 employees of Chongqing Bosai Mining Company,China,working in a manganese mining of Guyana,had unexplained fever,and two of them *** obtained lung and brain tissues as well as the blood samples from the two deceased cases(patient No.1 and 2),and bronchoscopy lavages and cerebrospinal fluid samples from one severe case(patient No.3),*** samples were tested by pathological examination,high-throughput sequencing,and real-time *** detection showed the presence of spore-like structures in the lung tissue of patient No.1,indicating a fungal infection in this *** sequencing identified the existing of *** in the lung tissue sample within 13 ***-generation sequencing identified specific fragments of *** in all of the samples tested(lung,brain and blood serum from the deceased cases,and plasma from the severe case).Real-time PCR assays did not reveal any viral infection related to transmission from bat *** conclude that *** was the causative pathogen of this disease cluster based on epidemiologic,clinical,pathological and nucleic acid evidence.

Primary Histoplasmosis of Larynx: A Case Series and Review of Literature

Objective: Aim of the study was to analyze 10 managed cases of Primary Histoplasmosis of Larynx, over a period of 10 years. Design: Retrospective, non-randomized, interventional case series. Patients and Method: This study was conducted in the Department of Otorhinolaryngology S.M.S Medical College from 2002 to 2011. The study consisted of evaluation of self-reported and referred patients who presented to ENT OPD and emergency. In all the cases a detailed history and clinical examination was done. All the patients underwent fiber optic laryngoscopy and subsequent direct laryngoscopy and biopsy. CT Scans were obtained for few cases. The study was a retrospective analysis of 10 treated cases. Main Outcome and Measures: Success of the treatment was defined as resolution of symptoms, differ-entiation from malignancy, no recurrence and regular follow up. Result: A total of 10 cases were managed between 2002 and 2011 and were reviewed. The main presentation was hoarseness of voice, with lesion simulating malignancy. Most common site of involvement was false cord and aryepiglottic fold (6 cases). Primary laryngeal Histoplasmosis was essentially a disease of adult males in all 10 cases. Treatment was exclusively medical with use of Itraconazole as drug of choice in 7 cases. Conclusion: Isolated laryngeal Histoplasmosis is a rare entity. Because of its simulation with malignancy it needs to be included in the differential diagnosis of neoplasm both benign and malignant. Our study clearly limits this kind of disease to adult males most commonly involving false cord and aryepiglottic fold in an endophytic pattern. Though it looks like cancer;biopsy confirms the diagnosis. The treatment is mainly medical with Itraconazole being the drug of choice.
