Computerized Scheme for histological classification of Masses with Architectural Distortions in Ultrasonographic Images

Architectural distortion is an important ultrasonographic indicator of breast cancer. However, it is difficult for clinicians to determine whether a given lesion is malignant because such distortions can be subtle in ultrasonographic images. In this paper, we report on a study to develop a computerized scheme for the histological classification of masses with architectural distortions as a differential diagnosis aid. Our database consisted of 72 ultrasonographic images obtained from 47 patients whose masses had architectural distortions. This included 51 malignant (35 invasive and 16 non-invasive carcinomas) and 21 benign masses. In the proposed method, the location of the masses and the area occupied by them were first determined by an experienced clinician. Fourteen objective features concerning masses with architectural distortions were then extracted automatically by taking into account subjective features commonly used by experienced clinicians to describe such masses. The k-nearest neighbors (k-NN) rule was finally used to distinguish three histological classifications. The proposed method yielded classification accuracy values of 91.4% (32/35) for invasive carcinoma, 75.0% (12/16) for noninvasive carcinoma, and 85.7% (18/21) for benign mass, respectively. The sensitivity and specificity values were 92.2% (47/51) and 85.7% (18/21), respectively. The positive predictive values (PPV) were 88.9% (32/36) for invasive carcinoma and 85.7% (12/14) for noninvasive carcinoma whereas the negative predictive values (NPV) were 81.8% (18/22) for benign mass. Thus, the proposed method can help the differential diagnosis of masses with architectural distortions in ultrasonographic images.

Thymic epithelial tumors(TETs) are common tumors in human anterior mediastinum with limited biological *** decoding the immune landscape of tumors,we reclassify TETs into three types based on T cell developmental *** uncover the developmental dysfunction and TCR repertoire of tumor-infiltrating T cells by cell ***,we identify the unique subset of tumor cells with distinct epithelial origin in each TETs ***,we demonstrate that KRT14/GNB3+mTECs-like cell accumulation inhibits the T cell positive selection in type 1 TETs,while CCL25+cTEC-like cell promotes T cell positive selection in type ***,although CHI3 L1mTEC-like cell in type3 TETs loses the function of supporting T cell development,it acquires the capacity to induce CD8TRMs-mediated ***,we propose a new molecular classification of human TETs using GNB3 and CHI3 L1 to distinguish the epithelial origin of tumor cells,which is promising in prognostic prediction.

Objective The WHO classification for thymic epithelial tumour(TETs) of the fourth edition is used newly, but its prognostic value is still a controversial issue. Methods In this study, we collected 238 cases from the Fourth Hospital of Hebei Medical University during January 2006 to December 2015 and reviewed retrospectively the clinicopathological data of these patients who were diagnosed definitely as thymic epithelial tumors to assess the prognosis relating to histological classifications, Masaoka’s clinical staging, Myasthenia Gravis(MG) and adjuvant therapy. Results Among the 238 cases, a total of 24 cases were type A thymoma(10.08%), 69 cases were type AB thymoma(28.99%), 44 cases were type B1 thymoma(18.49%), 56 cases were type B2 thymoma(23.53%), 33 cases were type B3 thymoma(13.87%) and 12 cases were thymic carcinomas(5.04%). According to the Massaoka staging, there were 106 patients in stage I(44.54%), 69 patients in stage II(28.99%), 34 patients in stage III(14.28%), 17 patients in stage IV(7.14%).Approximately 48 cases which type B1, type B2 and type B3 thymoma accounted for most proportion concomitanted with MG, with the corresponding, none of thymic carcinomas concomitanted with MG. Cox regression analysis drew the conclusion that Masaoka stage was one of the best factor to assess the prognosis. Besides of this, WHO classification of thymus was an independent and important prognostic factor also. Other indexes, such as gender, age, tumour size, tumour boundary, MG, and adjuvant therapy, had no significant impact for patients’ prognosis. Conclusion Masaoka stage was one of the best factor to assess the prognosis. WHO classification of thymus was an independent and important prognostic factor.
