Tissue adhesive hemostatic microneedle arrays for rapid Hemorrhage treatment

Blood loss by hemorrhaging wounds accounts for over one-third of~5 million trauma fatalities worldwide every year.If not controlled in a timely manner,exsanguination can take lives within a few minutes.Developing new biomaterials that are easy to use by non-expert patients and promote rapid blood coagulation is an unmet medical need.Here,biocompatible,and biodegradable microneedle arrays(MNAs)based on gelatin methacryloyl(GelMA)biomaterial hybridized with silicate nanoplatelets(SNs)are developed for Hemorrhage control.The SNs render the MNAs hemostatic,while the needle-shaped structure increases the contact area with blood,synergistically accelerating the clotting time from 11.5 min to 1.3 min in vitro.The engineered MNAs reduce bleeding by~92%compared with the untreated injury group in a rat liver bleeding model.SN-containing MNAs outperform the hemostatic effect of needle-free patches and a commercial hemostat in vivo via combining micro-and nanoengineered features.Furthermore,the tissue adhesive properties and mechanical interlocking support the suitability of MNAs for wound closure applications.These hemostatic MNAs may enable rapid Hemorrhage control,particularly for patients in developing countries or remote areas with limited or no immediate access to hospitals.

Aneurysmal subarachnoid Hemorrhage(aSAH)is a typical neurosurgical emergency.The patient's prognosis is related to the severity of the initial illness and post-operative complications,especially delayed cerebral ischemia(DCI).DCI,which occurs in up to 30%of patients with subarachnoid Hemorrhage(SAH),usually 2 weeks after Hemorrhage,is one of the leading causes of death and disability in aSAH.

Intracerebral Hemorrhage(ICH)is a severe neurological disease with high morbidity and mortality.[1,2]Infection rates of 26%to 58%have been reported in patients with ICH.[3]Infections not only increase the length of hospital stay and healthcare costs but also increase mortality,worsen functional outcomes,and increase hospital readmission rates.The current study sought to determine the prevalence of and risk factors associated with infections in patients with ICH and to investigate their impact on patient outcome upon discharge from the hospital.

To the Editor:China has the largest number of intracerebral Hemorrhage(ICH)patients of any single country,[1]and efforts to mine,utilize,and share the relevant data are warranted.As a structured diagnosis and treatment pipeline for ICH has been demonstrated to improve patient outcomes,there is an urgent need to construct a large medical database to store relevant data and facilitate data mining,analysis,and sharing.However,traditional stroke databases are dominated by text data without comprehensive imaging data,and few studies have focused on the construction of databases on ICH.[2,3]To integrate multimodal data on ICH and standardize data processing and feedback,the Chinese Intracranial Hemorrhage Imaging Database(CICHID)was established by Peking Union Medical College Hospital in 2019 and is under continuous refinement.Differing from traditional text-based databases,CICHID is a large multimodal data warehouse implementing comprehensive clinical data storage and multidimensional imaging data analysis.

Endovascular treatment of delayed arterial Hemorrhage after radical operation for hilar cholangiocarcinoma

Radical operation for hilar cholangiocarcinoma(HCCA)is the most effective treatment,but high rates of severe postoperative complication and death remain concerns due to the complexity of biliary and vascular anatomy of the hepatobiliary region.De-layed arterial Hemorrhage(DAH)occurring>24 h postoperatively usually causes life-threatening bleeding,and relaparotomy for DAH would be difficult and hazardous because of postoperative adhe-sions and critical general condition.As endovascular technique is less invasive and has improved over the recent decades,clini-cians have begun to prefer endovascular treatment(EVT)to surgi-cal treatment of DAH after hepatobiliary pancreatic surgery.How-ever,to our knowledge,a relatively large series of patients dealing with the outcomes of EVT of DAH after surgery for HCCA has not been reported.Herein,we analyzed the technical and clinical out-comes of EVT in 17 patients to evaluate the efficacy and safety of EVT.

To the Editor:Postpartum Hemorrhage(PPH)is a serious complication of childbirth and a leading cause of maternal morbidity and mortality worldwide.Therefore,the correct and proactive treatment of PPH is key to reducing maternal mortality rates.The Bakri balloon is one treatment method for PPH.Here,we investigated the therapeutic effect,advantages,and disadvantages of the Bakri balloon to provide a theoretical basis for seeking appropriate pathological occasions and optimal timing for placing the Bakri balloon(Cook Inc.,Spencer,IN,USA)and develop a more reasonable and effective method for treating PPH.

Aneurysmal subarachnoid Hemorrhage-related hydrocephalus(aSAH-H)refers to a clinical syndrome of excessive secretion,obstructive absorption,or circulatory disturbance of cerebrospinal fluid(CSF)with subarachnoid Hemorrhage following rupture of aneurysm that leads to excessive accumulation of intracranial CSF and enlargement of ventricles impairing neurological function.aSAH is an important risk factor for hydrocephalus[1,2].According to fluid dynamics,aSAH-H is divided into communicating and obstructive hydrocephalus;according to the time of occurrence.
