BACKGROUND Use of liver allograft with hepatic hemangioma after in vivo resection of hemangioma in living donor liver transplantation(LDLT)has been previously reported.However,there are few reports describing ex vivo backtable resection of hemangioma from liver allografts in LDLT.CASE SUMMARY A 55-year-old male was evaluated as a donor for an 8-month-year old patient with acute hepatic failure due to biliary atresia.Pre-operative contrast enhanced computed tomography revealed a 9 cm hemangioma in segment 4 with vascular variations in the donor.During LDLT,an intra-operative intrahepatic cholangiography was performed to ensure no variation in the anatomy of the intrahepatic bile duct.After intra-operative pathological diagnosis,ex vivo backtable resection of the hemangioma was performed and the liver allograft was transplanted into the recipient.The donor’s and recipient’s post-operative course were uneventful.At the 2-year follow-up,the liver allograft showed good regeneration without any recurrence of hemangioma.CONCLUSION Liver allografts with hemangiomas are an acceptable alternative strategy for LDLT.Ex vivo backtable resection of hemangioma from the donor liver during pediatric LDLT is safe and feasible,and can effectively reduce the operative time and intra-operative bleeding for the donor.

hemangiomas are the most common benign vascular tumours of the pediatric age group.They originate from embryonic remnants of unipotent angioblastic cells.The liver is the most common site for hemangioma in the abdomen.1 The prevalence of hepatic hemangioma(HH)varies from 1%to 20%in the general popula-tion,and it is two to five times more common in women.HH is usually detected incidentally and shows no symptoms;therefore,most of the time it does not require any treatment.2 However,hemangioma may sometimes occur at the atypical location and present with unusual clinical manifestations within the abdominal cavity.Due to these reasons,it leads to diag-nostic confusion,resulting in their inadvertent surgical excision.12 We present here a case of intra-abdominal hemangioma in an infant who presented with the signs and symptoms of intestinal obstruction.

BACKGROUND Giant hepatic cavernous hemangioma with multiple satellite nodules is a rare subtype of hepatic cavernous hemangioma,the most common vascular liver tumor.We report on a tumor with unusual histologic features:(1)Finger-like infiltration pattern;(2)lack of encapsulation;(3)blurred tumor/liver interface;and(4)massive satellitosis-referring to the article“Hepatic cavernous hemangioma:underrecognized associated histologic features”.CASE SUMMARY A 60-year-old man presented with increasing uncharacteristic abdominal discomfort and mildly elevated blood parameters of acute inflammation.Imaging revealed an unclear,giant liver tumor of the left liver lobe.A massive vascular tumor with extensive satellitosis broadly infiltrating the adjacent liver parenchyma was resected via hemihepatectomy of segmentsⅡ/Ⅲ.Histopathological diagnosis was giant hepatic cavernous hemangioma with multiple satellite nodules,featuring unusual characteristics hardly portrayed in the literature.Retrospectively,this particular morphology can explain the difficult pre-and perioperative diagnosis of a vascular liver tumor that is usually readily identifiable by modern imaging methods.CONCLUSION This case emphasizes the exact histological workup of tumor and tumor-induced parenchyma changes in radiologically unclassifiable liver tumors.

Introduction Gastric hemangioma is a rare neoplasm,accounting for0.05%of gastrointestinal tumors[1].Its submucosal location and dense vascular structure make diagnosis challenging and endoscopic investigations are pivotal in its identification.Due to the risk of bleeding,biopsy may not always be feasible.The most frequently reported symptoms of gastric hemangioma are abdominal pain and upper gastrointestinal bleeding[2].In this report,we present a case of gastric hemangioma in which the initial clinical manifestation was gastrointestinal bleeding.Post-operative pathological examination revealed a gastric capillary hemangioma.

Background A number of clinical trials evaluated the efficacy and adverse effects of oral propranolol in the treatment of infantile hemangioma(IH),but the treatment has not yet been standardized.This meta-analysis aims to reevaluate the efficacy and adverse effects of oral propranolol in comparative studies and to provide a reliable basis for clinical administration in the therapy for IH.Methods Data were obtained from PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infra-structure and Wanfang database,from inception to December 1st,2018.The pooled risk ratios(RR)with 95%confidence intervals(95%CI)were calculated and used to evaluate the effect size.The meta-analysis was performed using the random-effects model due to heterogeneity between the studies.The Cochrane Collaboration 6 aspects of bias,methodological index for non-randomized studies and the Newcastle-Ottawa Scale were used to assess the risk for bias.Sensitivity analysis,publication bias and subgroup analysis were performed.Results Eighteen unique studies involving 2701 unique children were included in the analysis.The response rate was reported in 18 trials,which compared oral propranolol with other treatments.The heterogeneity was statistically significant(P<0.00001,P=95%).The difference in the response rate was statistically significant(RR=1.40,95%CI 1.13-1.75)while compared with the controls.However,no significant difference in the adverse events rate(RR=0.78,95%CI 0.45-1.34)and relapse rate(RR=1.45,95%CI 0.66-3.16)were found.Otherwise,the subgroup analysis indicated that the RR was 1.64(95%CI 0.24-11.36)for low-dose propranolol(1 mg/kg/day),1.42(95%CI 1.12-1.80)for medium dose(2 mg/kg/day)and 1.46(95%CI 1.17-1.82)for high dose(3 mg/kg/day),but the high dose had higher adverse events rate than medium dose,with 3.60%and 86.22%,respectively.The effectiveness of propranolol therapy among cases of treatment duration less than 6 months(RR=1.24,95%CI 1.05-1.47)was inferior to that of treatment duration greater than or equal to 6 mon

Objective Penile vascular anomalies(PVAs)or hemangioma can arouse patient concern about aesthetics and cause symptoms like bleeding and sexual dysfunction.However,its low incidence and the deficiency of large-volume studies hinder urologists from making informed decisions.This study aimed to investigate the clinical features and treatment experience of PVAs at the Seventh Medical Center of PLA General Hospital,Beijing,China.Furthermore,by systematically analysis of studies on PVAs in Chinese people,we aimed to provide novel insights on the management of this condition.Methods We retrospectively investigated clinical features and pathology of surgery-treated PVAs at our center.Moreover,by systemically reviewing the literature from PubMed and the three largest medical databases(China National Knowledge Infrastructure,Wan Fang,and Chinese Medical Journal Database)in China,we analyzed the clinical features and various therapies of PVAs in Chinese people.Results Between March 1,2018 and March 1,2023,a total of 356 cases with vascular anomalies were treated with surgery at out center.Only seven(2.0%)cases had lesions involving the perineum and external genitalia.All the seven cases were pathologically benign and demonstrated no recurrence over a follow-up period of 1-52 months(median 14 months).A total of 410 cases from 44 studies were selected in the cumulative analysis.Most patients(92.4%)diagnosed with PVAs were asymptomatic,and 68.8%of the patients were treated with sclerotherapy.As to the pathology,57.1%were venous malformation.Conclusion The most common PVA is venous malformation and the majority of patients are asymptomatic.Sclerotherapy and laser have emerged as viable options for treating small lesions.Surgery still has its role in treating large lesions and obtaining pathology.Although PVAs often relapse or demand multiple treatments,the prognosis is favorable.

To the Editor:Kasabach-Merritt syndrome(KMS)is a very rare life-threatening aggressive vascular tumor that is related to Kaposiform hemangioendothelioma and tufted angioma.It is characterized by thrombocytopenia and intravascular coagulation that occasionally develops into disseminated intravascular coagulation.[1,2]A rare incidence of diffuse infiltrative pancreatic hemangioma with a high possibility of missed diagnosis of KMS makes it a fatal disease.Here,we described a case diagnosed with diffuse infiltrative hemangioma(DIH)of pancreas accompanied by KMS and provided ultrasonic data for early and accurate diagnosis,and successful treatment experiences with sirolimus.

Background Infantile hemangioma （IH） is the most common benign tumor in children with prevalence in the face and neck. Various treatment options including oral propranolol have been described for IH, but the mechanism of drugs remains enigmatic. The aim of this study was to investigate the pathogenesis and establish a reliable in vivo model of IH which can provide platform for drug exploration. Methods Stem cells from the proliferating hemangiomas （HemSCs） were isolated by CD133-tagged immunomagnetic beads. Their phenotype and angiogenic property were investigated by flow cytometry, culturing on Matrigel, real-time polymerase chain reaction （PCR）, immunofluorescent staining and injection into BALB/c-nu mice. Results HemSCs had robust ability of proliferating and cloning. The time of cells doubling in proliferative phase was 16 hours. Flow cytometry showed that HemSCs expressed mesenchymal markers CD29, CD44, but not endothelial/hematopoietic marker of CD34 and hematopoietic marker CD45. The expression of CD105 was much lower than that of the reported hemangioma derived or normal mesenchymal stem cell （MSC）. Real-time PCR showed that the mRNA levels of vascular endothelial growth factor （VEGF）, basic fibroblast growth factor （bFGF） and matrix metalloproteinase-1 （MMP-1） of HemSCs were higher than that of neonatal human dermal fibroblasts （NHDFs） and human umbilical vein endothelial cells （HUVECs）. After HemSCs were cultured on Matrigel in vitro, they formed tube-like structure in a short time （16 hours） and differentiated into endothelial cells in 7 days. After 1-2 weeks of implantation into immunodeficient mice, HemSCs generated glucose transporter 1 positive blood vessels. When co-injected with HUVECs, the vascularization of HemSCs was greatly enhanced. However, the single implantation of HUVECs hardly formed blood vessels in BALB/c-nu mice （P 〈0.05）. A 21-year-old man presented with an enlarged giant hemangioma on glans penis which also causes an erectile dysfunction （ED） that partially responded to the intracavernous injection stimulation test. Although the findings in magnetic resonance imaging （MRI） indicated a glandular hemangioma, penile colored Doppler ultrasound revealed an invaded cavernausal hemangioma to the glans. Surgical excision was avoided according to the broad extension of the gland lesion. Holmium laser coagulation was applied to the lesion due to the cosmetically concerns. However, the cosmetic results after holmium laser application was not impressive as expected without an improvement in intracavernous injection stimulation test. In conclusion, holmium laser application should not be used to the hemangiomas of glans penis related to the corpus cavernosum, but further studies are needed to reveal the effects of holmium laser application in small hemangiomas restricted to the glans penis.
