Inflammatory bowel disease(IBD)is a non-specific inflammatory disease of the gastrointestinal(GI)tract that is generally accepted to be closely related to intestinal dysbiosis in the host.GI infections contribute a key role in the pathogenesis of IBD;however,although the results of recent clinical studies have revealed an inverse correlation between Helicobacter pylori(H.pylori)infection and IBD,the exact mechanism underlying the development of IBD remains unclear.H.pylori,as a star microorganism,has been a focus for decades,and recent preclinical and real-world studies have demonstrated that H.pylori not only affects the changes in the gastric microbiota and microenvironment but also influences the intestinal microbiota,indicating a potential correlation with IBD.Detailed analysis revealed that H.pylori infection increased the diversity of the intestinal microbiota,reduced the abundance of Bacteroidetes,augmented the abundance of Firmicutes,and produced short-chain fatty acid-producing bacteria such as Akkermansia.All these factors may decrease vulnerability to IBD.Further studies investigating the H.pylori-intestinal microbiota metabolite axis should be performed to understand the mechanism underlying the development of IBD.

Helicobacter pylori(H.pylori)infection is highly prevalent in the human population and may lead to severe gastrointestinal pathology including gastric and duodenal ulcers,mucosa associated tissue lymphoma and gastric adenocarcinoma.In recent years,an alarming increase in antimicrobial resistance and subsequently failing empiric H.pylori eradication therapies have been noted worldwide,also in many European countries.Therefore,rapid and accurate determination of H.pylori’s antibiotic susceptibility prior to the administration of eradication regimens becomes ever more important.Traditionally,detection of H.pylori and its antimicrobial resistance is done by culture and phenotypic drug susceptibility testing that are cumbersome with a long turn-around-time.Recent advances in diagnostics provide new tools,like real-time polymerase chain reaction(PCR)and line probe assays,to diagnose H.pylori infection and antimicrobial resistance to certain antibiotics,directly from clinical specimens.Moreover,high-throughput whole genome sequencing technologies allow the rapid analysis of the pathogen’s genome,thereby allowing identification of resistance mutations and associated antibiotic resistance.In the first part of this review,we will give an overview on currently available diagnostic methods for detection of H.pylori and its drug resistance and their implementation in H.pylori management.The second part of the review focusses on the use of next generation sequencing technology in H.pylori research.To this end,we conducted a literature search for original research articles in English using the terms“Helicobacter”,“transcriptomic”,“transcriptome”,“next generation sequencing”and“whole genome sequencing”.This review is aimed to bridge the gap between current diagnostic practice(histology,rapid urease test,H.pylori culture,PCR and line probe assays)and new sequencing technologies and their potential implementation in diagnostic laboratory settings in order to complement the currently recommended H.pylori management guidelines and s

Background:Helicobacter pylori(H.pylori)eradication has been widely used.The recurrence rate of H.pylori after eradication and its related factors are gaining more and more attention.Our study aimed to determine the recurrence rate of H.pylori infection after successful eradication,and analyze its influential factors.Methods:We prospectively studied 1050 patients with upper gastrointestinal symptoms who were diagnosed as H.pylori infection by gastroscopy and underwent eradication therapies from April 2013 to January 2014.The 13C-urea breath test(UBT)or Warthin-Starry(WS)staining was done at 8 to 12 weeks after the therapy.Patients with successful eradication were followed by repeated UBT or gastroscopy at one year and 3 years after therapy,as well as,questionnaire surveys.Recurrence was considered if the UBTs or WS staining of biopsy were positive.One-year and 3-year recurrence rates were calculated,and analyzed the differences between recurred patients and others in basic data,socio logical characteristics,lifestyle.Results:A total of 743 patients finished the 1-year follow-up,and the 1-year recurrence rate was 1.75%.Of the 607 patients who finished the 3-year follow-up,28 patients recurred,and the 3-year recurrence rate was 4.61%.Analysis of variance showed that lowincome,poor hygiene condition of dining out place,and receiving invasive diagnoses or treatments were significant risk factors for H.pylori infection recurrence.Logistic regression analysis demonstrated that the combination of invasive diagnoses or treatments,the level of income,and the hygiene standard of dining out place were significant and independent influential factors of the recurrence of H.pylori.Conclusions:The 1-year and 3-year recurrence rates of H.pylori infection after eradication therapy are 1.75%and 4.61%.Lowincome,poor hygiene condition of dining out place,and a combination of invasive diagnoses or treatments are independent risk factors of H.pylori recurrence.

Helicobacter pylori(H. pylori) eradication can reduce gastric cancer. However, gastric cancer still develops after eradication, and cases who received eradication therapy are increasing. In this study, we have reviewed the characteristics and predictors of primary gastric cancer developing after H. pylori eradication. In terms of the characteristics, endoscopic, histologic, and molecular characteristics are reported. Endoscopically, gastric cancer after eradication is often depressedtype and shows a gastritis-like appearance, which sometimes makes the diagnosis difficult. Histologically, most gastric cancer after eradication is intestinal type, and non-neoplastic epithelium, also called epithelium with low-grade atypia, is frequently seen over the tumor, which is presumably the cause of the endoscopic gastritis-like appearance. As for molecular characteristics, some markers, such as Ki67, MUC2, and Wnt5a expression, are lower in cancer from patients in whom H. pylori has been eradicated. In terms of predictors, several Japanese studies have reported that severe endoscopic atrophy at eradication is a risk factor for gastric cancer development. Histologic intestinal metaplasia, especially in the corpus, and long-term use of proton pump inhibitors, are also reported as risk factors for gastric cancer after H. pylori eradication. These studies on the characteristics and predictors of gastric cancer development will become the cornerstone for establishing a novel surveillance program based on the gastric cancer risk stratification specific to H. pylori-eradicated patients.

Third generation of quinolones,such as levofloxacin and moxifloxacin,-containing regimens are often used in second-line or rescue treatment of Helicobacter pylori infection.However,the increasing antibiotic resistance to quinolones affects the efficacies of quinolones-containing therapies in recent years.Therefore,there is a need to enhance the effectiveness of quinolonescontaining therapies.Sitafloxacin,a fourth-generation quinolone,and vonoprazan,a novel potassium-competitive acid blocker,are now available as more effective treatment options.The aim of this paper is to summarize the current evidence of quinolone-containing therapies in rescue treatments,and to discuss the importance of drug sensitivity tests or analysis of gyrA mutation before treatments.

Background:Determining the Helicobacter pylori (H. pylori) infection state during the gastroscopic process is important but still challenging. The linked color imaging (LCI) technique might emphasize the mucosal color change after H. pylori infection, which might help the diagnosis. In the present study, we aimed to compare the LCI technique with traditional white light imaging (WLI) endoscopy for diagnosing active H. pylori infection.Methods:We collected and analyzed gastroscopic images from 103 patients in our hospital from November 2017 to March 2018, including both LCI and WLI modes. All images were randomly disordered and independently evaluated by four endoscopists who were blinded to the H. pylori status of patients. In addition, the H. pylori state was determined by both rapid urease test and pathology staining. The sensitivity, specificity, positive prediction value (PPV), and negative prediction value (NPV) were calculated for the detection of H. pylori infection. Moreover, the kappa value and interclass correlation coefficient (ICC) were used to evaluate the inter-observer variety by SPSS 24.0 software.Results:Of the 103 enrolled patients, 27 of them were positive for H. pylori infection, while the 76 patients were negative. In total, 388 endoscopic images were selected, including 197 WLI and 191 LCI. The accuracy rate for H. pylori evaluation in the corpus LCI group was significantly higher than other groups (81.2% vs. 64.3%-76.5%, χ2 = 34.852, P < 0.001). Moreover, the corpus LCI group had the optimal diagnostic power with the sensitivity of 85.41% (95% confidence interval [CI]: 76.40%-91.51%), the specificity of 79.71% (95% CI: 74.38%-84.19%), the PPV of 59.42% (95% CI: 50.72%-67.59%), and the NPV of 94.02% (95% CI: 89.95%-96.56%), respectively. The kappa values between different endoscopists were higher with LCI than with WLI (0.433-0.554 vs. 0.331-0.554). The main challenge in the field of Helicobacter pylori(H.pylori)infection is antibiotic resistance,which influences the efficacy of eradication regimens.Bismuth-containing quadruple therapy has been confirmed as an effective regimen for eradicating H.pylori,especially in strains with antibiotic resistance.High-dose proton-pump inhibitor-amoxicillin dual therapy could decrease the use of unnecessary antibiotics,which is a promising alternative approach.Adjuvant therapy(specific probiotic or vitamin)also showed good results,although more evidence is needed.Novel anti-H.pylori drugs are needed,and the establishment of the H.pylori database is an effective way to acknowledge the real-time information of H.pylori management.This review provides the recent progress of H.pylori treatment,and further studies are needed to address the role of different regimens in improving H.pylori eradication rate,especially in strains with antibiotics resistance.

BACKGROUND Allicin(2-propene-1-sulfinothioic acid S-2-propenyl ester,diallyl thiosulfinate)extracted from garlic,has proven activity against Helicobacter pylori(H.Pylori)infection.In recent years,clinical trials have explored its utility as an add-on therapy with variable outcomes reported.AIM To perform a systemic review of allicin as an add-on treatment for H.Pylori infection and assess its efficacy in randomized controlled trials(RCTs).METHODS Electronic databases including MEDLINE,EMBASE,the Web of Science,the Cochrane Database,the China National Knowledge Infrastructure Database,Chinese VIP Information Databases,Chinese Medical Databases,and the Wan-Fang Database were searched for keywords including“allicin”,“Helicobacter pylori”,“randomized clinical trials”,and their synonyms.A meta-analysis was performed using the fixed-effects model for low heterogeneity and the randomeffects model for high heterogeneity with sensitivity analysis.Bias was evaluated using Egger’s tests.Trial sequential analysis(TSA)was used to evaluate information size and treatment benefits.The Grading of Recommendations Assessment,Development and Evaluation(GRADE)was used to assess the level of quality,and studies were classed as“high quality”,“moderate quality”,“low quality”,and“very low quality”.RESULTS A total of eight RCTs consisting of 867 participants(435 from the allicin group and 432 from the control group)were included.Eradication rate in the allicin group(93.33%,406/435)was significantly higher than that of the control group(83.56%,361/432)[I2=0%,odds ratio(OR)=2.75,95%confidence interval(CI):1.74-4.35,P<0.001].The healing rate of ulcers following H.pylori therapy in the allicin group(86.17%,349/405)was significantly higher than that of the control group(75.87%,305/402)[I2=0%,OR=2.05,95%CI:1.39-3.03,P<0.001].The total remission rate of peptic ulcers across all allicin groups was 95.99%,which was significantly higher than that of controls[95.99%(359/374)vs 89.25%(332/372),I2=0,heterogeneity P=0.84,OR=3.13,95%CI:1.51-6.51,P=0.002

AIM To clarify the role of serum anti-Helicobacter pylori(H. pylori) antibody titers in gastric cancer.METHODS In this cross-sectional study, the effect of patients' baseline characteristics and endoscopic findings on their serum antibody titers were assessed. We evaluated consecutive patients who underwent esophagogastroduodenoscopy and their first evaluation for H. pylori infection using a serum antibody test. We excluded patients with a history of eradication therapy. The participants were divided into four groups according to their E-plate serum antibody titer. Patients with serum antibody titers < 3, 3-9.9, 10-49.9, and ≥ 50 U/m L were classified into groups A, B, C, and D, respectively. RESULTS In total, 874 participants were analyzed with 70%, 16%, 8.7%, and 5.1% of them in the groups A, B, C, and D, respectively. Patients in group C were older than patients in groups A and B. Gastric open-type atrophy, intestinal metaplasia, enlarged folds, diffuse redness, and duodenal ulcers were associated with a high titer. Regular arrangements of collecting venules, fundic gland polyps, superficial gastritis, and gastroesophageal reflux disease were related to a low titer. Multivariate analysis revealed that nodularity(P = 0.0094), atrophy(P = 0.0076), and age 40-59 years(vs age ≥ 60 years, P = 0.0090) were correlated with a high serum antibody titer in H. pylori-infected patients. Intestinal metaplasia and atrophy were related to age ≥ 60 years in group C and D.CONCLUSION Serum antibody titer changes with age, reflects gastric mucosal inflammation, and is useful in predicting the risk of gastric cancer.

Helicobacter pylori(H.pylori)is a gram-negative bacterium that infects approximately 4.4 billion individuals worldwide.However,its prevalence varies among different geographic areas,and is influenced by several factors.The infection can be acquired by means of oral-oral or fecal-oral transmission,and the pathogen possesses various mechanisms that improve its capacity of mobility,adherence and manipulation of the gastric microenvironment,making possible the colonization of an organ with a highly acidic lumen.In addition,H.pylori presents a large variety of virulence factors that improve its pathogenicity,of which we highlight cytotoxin associated antigen A,vacuolating cytotoxin,duodenal ulcer promoting gene A protein,outer inflammatory protein and gamma-glutamyl transpeptidase.The host immune system,mainly by means of a Th1-polarized response,also plays a crucial role in the infection course.Although most H.pylori-positive individuals remain asymptomatic,the infection predisposes the development of various clinical conditions as peptic ulcers,gastric adenocarcinomas and mucosa-associated lymphoid tissue lymphomas.Invasive and non-invasive diagnostic methods,each of them with their related advantages and limitations,have been applied in H.pylori detection.Moreover,bacterial resistance to antimicrobial therapy is a major challenge in the treatment of this infection,and new therapy alternatives are being tested to improve H.pylori eradication.Last but not least,the development of effective vaccines against H.pylori infection have been the aim of several research studies.
