Patient-reported outcomes in subjects with neuroendocrine tumors of the pancreas

AIM: To assess the patient-reported outcomes (PROs) of pancreatic neuroendocrine tumor (PNET) patients. METHODS: Fifty-one consecutive patients (21 male, 30 female, 61.0 ± 10.3 years) with proven PNETs were studied. An SF-12 questionnaire capable of exploring the physical (PCS) and mental (MCS) aspects of daily life was used. Four questionnaires were also used [12 items General health Questionnaire (GHQ-12) for non-psychotic psychiatric disorders, State Trait AnxietyInventory (STAI) Y-1 and Y-2 for anxiety and BDI-for depressive symptoms] to explore the psychological aspects of the disease. Forty-four sex-and age-matched Italian normative subjects were included and evaluated using the SF-12, STAI Y-1 and Y-2 ***: Seven patients refused to participate to the study; they were clinically similar to the 44 participants who agreed to complete the questionnaires. PNET patients had a PCS score (44.7 ± 11.0) were not signifi cantly different from the norms (46.1 ± 9.9, P = 0.610), whereas the MCS score was signifi cantly lower in patients (42.4 ± 13.0) as compared to the norms (48.2 ± 9.8, P = 0.036). GHQ-12 identified 11 patients (25.0%) as having non-psychotic psychiatric *** STAI scores were similar in the patients and inthe normative population. Finally, BDI-identifiedeight patients (18.2%) with moderate depression and 9 (20.5%) with mild depression whereas 27 patients (61.4%) had no ***: The PNET patients had a good physical but an impaired mental component of their quality of life; in addition, mild or moderate depressive symptoms are present in about 40% of PNET patients.

This paper presents a new low delay centralized cooperative spectrum sensing method based on dynamic voting rule using multiple threshold level,for indoor hospital environment,where fading and direct path loss both are together responsible for variation in signal *** the proposed algorithm,weights of Cognitive Radios(CRs)in terms of assigned votecount are estimated based on the sensed energy values with respect to multiple fixed threshold levels,so that the direct path loss can be dealt without further increasing the error,which occurs due to *** indoor hospital like environment,Rician fading model is more ***,formulation,evaluation and analysis of various sensing performance parameters have been carried out considering Rician fading *** analysis and tradeoff among three optimum parameters-energy threshold,total number of CRs participating in sensing and total error rate,are presented with respect to signal to noise ratio(SNR).Finally,the Monte Carlo simulation is designed and implemented considering the two conditions-only fading environment and fading along with direct path loss *** can be observed that in both cases,the proposed spectrum sensing rule outperforms the other contemporary rules.

Raman spectroscopic identification of normal and malignant human stomach cells

Micro-Raman spectroscopy is employed to identify the normal and malignant human stomach cells. For the cancer cell, the reduced intensity of the Raman peak at 1250 cm^-1 indicates that the protein secondary structure transforms from β-sheet or disordered structures to α-helical, while the increased intensity of the symmetric PO2 stretching vibration mode at 1094 cm^-1 shows the increased DNA content. The ratio of the intensity at 1315 cm^-1 to that at 1340^-1 reduces from 1.8 for the normal cell to 1.1 for the cancer cell in the course of canceration, and the ratio of the intensity at 1655 cm^-1 to that at 1450cm^-1 increases from 1.00 for the cancer cell to 1.26 for the normal cell which indicates that the canceration of stomach cell may induce saturation of the lipid chain.

Assessment of the early effectiveness of a stroke unit in comparison to the general ward

Background Stroke unit is the most effective treatment method to benefit stroke patients. Our study is to evaluate the early effectiveness of a hospital stroke unit (SU). Methods Three hundred and ninety-two patients who had suffered from acute strokes and who were admitted to our hospital between December 2001 and January 2003 were recruited for this controlled study. All patients were sent at random to either the SU or the general ward (GW) for treatment. The following indices were measured by: Barthel Index (BI),National Institute of health Stroke Scale (NIHSS),Oxford Handicap Scale (OHS). Results The mean change in BI score between the day of admission and the day of discharge was 20.00±24.36 for the SU group and 10.63±23.59 for the GW group. A difference that is statistically significant ( P =0.000). The mean change in NIHSS score was -2.01±6.61 for the SU group and 0.55±7.44 for the GW group. A difference that is also statistically significant ( P =0.000). Finally,the mean change in OHS score was -0.74±1.04 for the SU group and -0.28±0.98 for the GW group,also a statistically significant difference ( P =0.000). Among SU patients,patient satisfaction was higher ( P =0.000),the rehabilitation success rate was higher ( P =0.000),and there were fewer complications ( P =0.000).Conclusion Compared to GW patients,stroke patients treated in a special SU were able to return to normal daily activities earlier,with better social abilities,and have reduced neurological defects,without increasing the overall economic burden.

Quality assurance and quality control in the laboratory andrology

Quality assurance （QA） and quality control （QC） are fundamental aspects of any laboratory measurement. However, in comparison with other medical disciplines, the need for QA and QC in laboratory andrology has been recognized only recently. Furthermore, there is evidence that the effort required to undertake QA and QC has not been wholly welcomed by some clinicians. Nevertheless, accrediting bodies and regulatory authorities increasingly require evidence that laboratories have effective QA and QC measures in place because both are central to the quality management processes. Following the publication of the 5th edition of the World health Organization Laboratory Manual, existing QA and QC systems will need to be updated to take into account some of the methodological changes recommended by the manual. Three of these are discussed in this commentary; they relate to：（i） the move to infer semen volume from its weight; （ii） the re-classification of sperm motility grades from four to three; and （iii） the publication of a lower reference limit for morphology of 4% （with a corresponding 95% confidence interval of 3%-4%）. The importance of QA and QC in all laboratory tests, including up and coming new tests to assess sperm DNA integrity, is discussed. The need for adequate initial training and continuing professional development programmes to support laboratory scientists performing andrology is also described.

An investigation on dysphotopsia of a sigma-edged intraocular lens

A sigma-edged design of intraocular lens （IOL） is proposed to minimize the reflected glare images associated with the edge. The optimal sigma inverse edge is investigated when pupil diameter is 5.0 mm by the established Escudero-Sanz＇s wide angle model. The non-sequential ray tracing program of Zemax-EE （Zemax Development Corp., San Diego, USA） is used to investigate the sigma edge of reducing the potential for edge glare phenomena. The results show that sigma-edged design can significantly reduce the reflected glare intensity on retina if the angle of its inverse edge is taken from 20 to 70 degrees.

Diabetes mellitus has been an increasing concern owing to its high morbidity,and the average age of individual affected by of individual affected by this disease has now decreased to *** the high prevalence,it is necessary to address with this problem *** researchers and doctors have now developed detection techniques based on artificial intelligence to better approach problems that are missed due to human *** mining techniques with algorithms such as-density-based spatial clustering of applications with noise and ordering points to identify the cluster structure,the use of machine vision systems to learn data on facial images,gain better features for model training,and diagnosis via presentation of iridocyclitis for detection of the disease through iris patterns have been deployed by various *** learning classifiers such as support vector machines,logistic regression,and decision trees,have been comparative discussed various *** learning models such as artificial neural networks and recurrent neural networks have been considered,with primary focus on long short-term memory and convolutional neural network architectures in comparison with other machine learning *** parameters such as the root-mean-square error,mean absolute errors,area under curves,and graphs with varying criteria are commonly *** this study,challenges pertaining to data inadequacy and model deployment are *** future scope of such methods has also been discussed,and new methods are expected to enhance the performance of existing models,allowing them to attain greater insight into the conditions on which the prevalence of the disease depends.
