Osteonecrosis of the femoral Head(ONFH)is mainly caused by a decrease in the vascular supply to the subchondral bone of the femoral Head,leading to the death of bone cells,degeneration and necrosis of the subchondral bone,and eventually collapse of the femoral Head.^([1])Early and accurate diagnosis is the key to successful hip-preserving therapy,and the size/location of necrosis is the main determinant for ONFH treatment.Previous studies have shown that many hip-preserving therapies achieved satisfactory results in ONFH patients with Association Research Circulation Osseous(ARCO)I–II.When ONFH progresses to ARCO IV with clinical manifestations of osteoarthrosis,total hip arthroplasty(THA)may be the only option.

This study sought to provide an up-to-date review of the importance of anatomy to human identification,focusing on the usefulness of anatomical knowledge about the Head and neck bones and teeth to sex estimation in routine forensic anthropology methods.A detailed search of osteology applications in forensic sex estimation was conducted through the electronic databases for the 10 years prior to July 2020.Relevant articles and classic literature on the subject were gathered and outlined in this review.Among the available literature,several metric analyses showed accuracy superior to 80%in sexual diagnosis.Angles measured from the inclination of glabellae and analysis of the external frontal bone surface through three-dimensional computer-aided design emerge as reliable cranial indexes for sex estimation.in the mandible,the condylar and coronoid height,bigonial width,and condylion-gonion distance express significant sexual dimorphism.Measurements of the canine are the best option for sex estimation using teeth,as well as the thickness of the dentine or enamel of incisors.The axis vertebra surpasses other neck bones for sex estimation due to its atypical shape and the presence of the odontoid process.Metric analyses based on anatomy can provide reliable accuracy in sexual diagnosis.Adequate training and anatomical knowledge can reduce bias and interobserver differences,and the use of three-dimensional models and computed tomography images can enhance the accuracy of these methods for sex estimation.However,every method should be validated before being applied to a different population.

Methyltransferase like 13(METTL13),a kind of methyltransferase,is implicated in protein binding and synthesis.The upregulation of METTL13 has been reported in a variety of tumors.However,little was known about its potential function in Head and neck squamous cell carcinoma(HNSCC)so far.In this study,we found that METTL13 was significantly upregulated in HNSCC at both mRNA and protein level.Increased METTL13 was negatively associated with clinical prognosis.And METTL13 markedly affected HNSCC cellular phenotypes in vivo and vitro.Further mechanism study revealed that METTL13 could regulate EMT signaling pathway by mediating enhancing translation efficiency of Snail,the key transcription factor in EMT,hence regulating the progression of EMT.Furthermore,Snail was verified to mediate METTL13-induced HNSCC cell malignant phenotypes.Altogether,our study had revealed the oncogenic role of METTL13 in HNSCC,and provided a potential therapeutic strategy.

Background:The clinical outcome of transtrochanteric rotational osteotomy(TRO)for osteonecrosis of the femoral Head(ONFH)remains controversial,and the promising clinical results of several Japanese studies could not be reproduced in American and European studies.Trying to solve controversies on TRO for ONFH rising from apparently conflicting studies,a meta-analysis was conducted to assess the 5-and 10-year hip survival rates(with conversion to artificial joint replacement and radiographic failure as endpoints)after TRO.Methods:All eligible studies were searched in seven comprehensive databases including PubMed,Web of Science,Embase,Cochrane Library,VIP Database,China Knowledge Resource Integrated Database,and Wan Fang Database prior to June 2019.The outcomes evaluated were 5-and 10-year hip survival rates after TRO.The odds ratio and risk difference for the noncomparative binary data with the 95%confidence intervals(CIs)were calculated for each outcome.The included studies were assessed for methodologic bias and potential reasons for heterogeneity were explored.Results:Nineteen studies of TRO for ONFH were eligible for this meta-analysis according to inclusion criteria.Based on the previous report,two calculation methods(Methods 1 and 2)were adopted in this meta-analysis.Furthermore,we performed a subgroup analysis of the 5-and 10-year hip survival rates(Method 1)after TRO for ONFH:Asian sub-population and non-Asian subpopulation.Taking conversion to artificial joint replacement as the endpoint,5-and 10-year hip survival rates(Method 1)after TRO for ONFH in the Asian population were 0.86(95%CI=0.82–0.89)and 0.72(95%CI=0.65–0.78),respectively,and 5-and 10-year hip survival rates after TRO for ONFH in the non-Asian population were 0.55(95%CI=0.43–0.67)and 0.42(95%CI=0.28–0.55),respectively.The 5-and 10-year hip survival rates(Method 2)after TRO for ONFH were 0.90(95%CI=0.79–0.95)and 0.89(95%CI=0.81–0.94),respectively.Taking radiographic failure as the endpoint,5-and 10-year hip survival rates after TRO for ONFH wer

BACKGROUND Odontogenic infection is one of the common infectious diseases in oral and maxillofacial Head and neck regions.Clinically,if early odontogenic infections such as acute periapical periodontitis,alveolar abscess,and pericoronitis of wisdom teeth are not treated timely,effectively and correctly,the infected tissue may spread up to the skull and brain,down to the thoracic cavity,abdominal cavity and other areas through the natural potential fascial space in the oral and maxillofacial Head and neck.Severe multi-space infections are formed and can eventually lead to life-threatening complications(LTCs),such as intracranial infection,pleural effusion,empyema,sepsis and even death.CASE SUMMARY We report a rare case of death in a 41-year-old man with severe odontogenic multi-space infections in the oral and maxillofacial Head and neck regions.One week before admission,due to pain in the right lower posterior teeth,the patient placed a cigarette butt dipped in the pesticide"Miehailin"into the"dental cavity"to relieve the pain.Within a week,the infection gradually spread bilaterally to the floor of the mouth,submandibular space,neck,chest,waist,back,temporal and other areas.The patient had difficulty breathing,swallowing and eating,and was transferred to our hospital as an emergency admission.Following admission,oral and maxillofacial surgeons immediately organized consultations with doctors in otolaryngology,thoracic surgery,general surgery,hematology,anesthesia and the intensive care unit to assist with treatment.The patient was treated with the highest level of antibiotics(vancomycin)and extensive abscess incision and drainage in the oral,maxillofacial,Head and neck,chest and back regions.Unfortunately,the patient died of septic shock and multiple organ failure on the third day after admission.CONCLUSION Odontogenic infection can cause serious multi-space infections in the oral and maxillofacial Head and neck regions,which can result in multiple LTCs.The management and treatment of LTCs such as multi-space inf

In recent years,increasing evidence has demonstrated that extra-corporeal shockwave therapy (ESWT) can offer an effective and non-invasive method for the treatment of musculoskeletal disorders,such as shoulder tendinopathies,lateral epicondylopathy of the elbow,greater trochanteric pain syndrome,patellar tendinopathy,Achilles tendinopathy,plantar fasciitis,and bone disorders.[1,2]As a safe,cheap,and non-invasive therapeutic method,ESWT has played a promising role in orthopedic medicine.[3-5] Inspired by this,several researchers have attempted to investigate its use for treating osteonecrosis of the femoral Head.ESWT deserves being recommended as the optimal choice for osteonecrosis of the femoral Head (ONFH).

Background:Head and neck squamous cancer(HNSC)frequently occurs in the clinic.Revealing the role of the genes that correlate with cancer cell outgrowth will contribute to potential treatment target identification and tumor inhibition.Methods:The gene expression profiles and gene ontology of the proton-sensing G-protein-coupled receptor OGR1 were analyzed using the TCGA(The Cancer Genome Atlas)database.The effects of sex,age,race,and degree of malignancy on HNSC were investigated,and the survival times of HNSC patients with high or low/medium expression levels of OGR1 were compared.Methylation of the OGR1 promoter CpG sites was also investigated and OGR1-related genes were analyzed using gene set enrichment analysis.Results:OGR1 is overexpressed in HNSC patients.However,compared with the low/median expression group,the high OGR1 expression group did not have different survival rates.The OGR1 expression level differed across sex,age,race,and degree of malignancy,while the methylation of the OGR1 promoter CpG sites was maintained at a similar level.Gene set enrichment analysis revealed that OGR1 was positively correlated with Head and neck cancer,cisplatin resistance,hypoxia,angiogenesis,cell migration,and TGF-β.Conclusion:The expression of OGR1 correlated with HNSC progression and survival and thus can serve as a potential treatment target and prognostic marker.

BACKGROUND Squamous cell carcinoma of Head and neck(SCCHN) is the fifth most common cancer worldwide. Inhibition of epidermal growth factor receptor signaling has been shown to be a critical component of therapeutic option. Herein, we report a case of durable complete response to erlotinib.CASE SUMMARY An 81-year-old Caucasian male who presented with metastatic poorly differentiated squamous cell carcinoma of right cervical lymph nodes(levels 2 and 3). Imaging studies including(18)F-fluorodeoxyglucose positron emission tomography/computed tomography(CT) and contrast-enhanced CT scan of neck and chest did not reveal any other disease elsewhere. Panendoscopic examination with random biopsy did not reveal malignant lesion in nasopharynx,oropharynx, and larynx. He underwent modified neck dissection and postoperative radiation. Within 2 mo after completion of radiation, he developed local recurrence at right neck, which was surgically removed. Two mo after the salvage surgery, he developed a second recurrence at right neck. Due to suboptimal performance status and his preference, he started erlotinib treatment.He achieved partial response after first 2 mo of erlotinib treatment, then complete response after total 6 mo of erlotinib treatment. He developed sever skin rash and diarrhea including Clostridium difficile infection during the course of erlotinib treatment requiring dose reduction and eventual discontinuation. He remained in complete remission for more than two years after discontinuation of erlotinib.CONCLUSION We report a case of metastatic SCCHN achieving durable complete response from erlotinib. Patient experienced skin rash and diarrhea toxicities which were likely predictors of his treatment response.
