Background The positive influence of most types of exercise has been reported repeatedly,but what the most effective exercise approaches are for improving health-related quality of life(hrqol)in people with cancer remains unknown.The aim of this systematic review and network meta-analysis was to synthesize the evidence from intervention studies to assess the effects of different types of exercise on hrqol during and after cancer treatment.Methods MEDLINE,SPORTDiscus,the Cochrane Library,Web of Science,and Scopus were searched for randomized controlled trials aimed at testing the effects of exercise interventions meant to improve hrqol in people with cancer.Separate analyses were conducted for hrqol as measured by general and cancer-specific questionnaires.We also evaluated whether the effects of exercise were different during and after cancer treatment in both the physical and mental hrqol domains.Results In total,93 studies involving 7435 people with cancer were included.Network effect size estimates comparing exercise intervention vs.usual care were significant for combined exercise(0.35,95%confidence interval(95%CI):0.14–0.56)for hrqol as measured by general questionnaires,and for combined(0.31,95%CI:0.13–0.48),mind–body exercise(0.54,95%CI:0.18–0.89),and walking(0.39,95%CI:0.04–0.74)for hrqol as measured by cancer-specific questionnaires.Conclusion Exercise programs combining aerobic and resistance training can be recommended to improve hrqol during and after cancer treatment.The scarcity and heterogeneity of these studies prevents us from making recommendations about other exercise modalities due to insufficient evidence.

目的了解社区慢性病居民HRQo L水平及影响因素。方法用浙大版SF-36量表,采用分层随机抽样,入户问卷调查,调查区包括1个中心城区和1个新兴城区。结果共调查705人,常见慢性病占36.60%;慢性病患者与非慢性病患者在7个维度的得分有差异;不同患病种类数的生理健康各维度得分均有差异;健康状况的自我判断、NCDs患病数、两周内是否患病、食欲状况和在岗时职业对生理健康的影响显著,Adjusted R2=0.407;健康状况的自我判断、年龄、与家人相处情况和对目前生活状况的满意度对心理健康影响显著,Adjusted R2=0.179。结论社区慢性病患者HRQo L低于非慢性病患者,应通过社区健康管理和改善亲友关系来提高其HRQo L。

Objective： There have been few population-based studies evaluating health related quality of life（hrqol） in rural populations in China, and this study aimed to assess the current status of and risk factors for hrqol in a general rural population in high risk region of esophageal cancer in China.Methods： From November 2015 to September 2016, 12,085 permanent residents aged 45–69 years from 257 villages in the Endoscopic Screening for Esophageal Cancer in China（ESECC） trial（Clinical Trials.gov：NCT01688908） randomly selected from Hua County, Henan Province, China were interviewed. The EQ-5 D-3 L,a generic measure of hrqol, and a questionnaire were used to assess their hrqol and potential risk factors.Results： Among all the participants, 30.62% of the participants reported problems in at least one EQ-5 D dimension. Pain/discomfort（25.52%） was the most frequently reported problem followed by anxiety/depression（7.97%）, mobility（5.82%）, usual activities（2.61%） and self-care（1%）. These rural residents had a mean EQ-5 D index score of 0.948, and lower EQ-5 D index scores were associated with older age, female gender, lower levels of household annual per capita income, living alone, using shallow wells as main source of drinking water, exposure to family members smoking, testiness, unhealthy dietary habits, overweight or obesity, upper gastrointestinal cancer related symptoms and chronic diseases.Conclusions： Rural residents in China have a relatively low quality of life. Health promotion programs in this population should focus on the elderly, especially elderly women and the elderly living alone. Improving basic living circumstances and primary medical care services should be priorities. Results of this study will also serve as the basis for the cost-utility evaluation in our ESECC screening trial.

Purpose An important outcome measure of patient care is the impact on the patient’s health-related quality of life(hrqol).Current ear-specific hrqol instruments are designed for one diagnosis and emphasize different subdivisions such as symptoms,hearing problems,psychosocial impact,and the need for care.The optimal length of the recall period has not been studied.For these reasons,a new survey is needed that would cover most chronic ear diseases.Methods A preliminary 24-item survey(EOS-24)was created.Untreated adult patients(included n=186)with one of seven different chronic otologic conditions from all university hospitals in Finland were recruited to respond to EOS-24 and the 15D general hrqol instrument.The recruiting otologists evaluated the severity of the disease and the disability caused by it.A control group was recruited.Based on the patients’responses in different diagnosis groups,the items were reduced according to pre-defined criteria.The resulting survey was validated using a thorough statistical analysis.Results The relevance and necessity of the original 24 items were thoroughly investigated,leading to the exclusion of 8 items and the modification of 1.The remaining 16 items were well-balanced between subdivisions and were useful in all seven diagnosis groups,thus constituting the final instrument,EOS-16.The most suitable recall period was three months.Conclusions EOS-16 has been created according to the hrqol survey guidelines with a versatile nationwide patient population.The survey has been validated and can be used for a wide range of chronic ear diseases as a hrqol instrument.

Prostate cancer （PCa） is the most common solid-organ cancer in American males and the second most common cause of cancer-related death in men. With the advent of prostate-specific antigen screening, death from PCa continues to decline. However, recent evidence suggests that there is now a trend towards increasing incidence.1 Current screening strategies result in increased incidence of low-risk PCa and importantly, the diagnosis of PCa is becoming more common in younger patients.2 The majority of early-stage PCa patients have a high likelihood of disease free survival after treatment.

研究一lncRNA GAS5、NEAT1、H19和MALAT1与系统性硬化症的相关性研究目的:探讨系统性硬化症(Systemic Sclerosis,SSc)患者外周血单个核细胞中GAS5、H19、NEAT1和MALAT1的相对表达水平与其临床症状之间的相关性。材料与方法:采用病例对照的研究方法,纳入的61例SSc患者和122例健康对照(Healthy controls,HC)。问卷调查收集SSc患者流行病学资料,实时荧光定量聚合酶链式反应测定GAS5、H19、NEAT1和MALAT1在研究对象血样中的表达水平。Mann-Whitney U检验比较lncRNA表达水平,因为其适用于定量资料且不符合正态分布;Spearman秩相关用来分析两个连续型且不符合正态分布变量的相关性。结果:SSc患者与HC的平均年龄分别为(52.10±10.84,50.95±9.72;P=0.470)。SSc患者的GAS5和MALAT1水平低于HC(0.158(0.128,0.241)vs 0.676(0.439,1.676),Phrqol)进行评估同时采用实时荧光定量聚合酶链式反应测定GAS5、NEAT1、H19和MALAT1在研究对象血样中的表达水平。本研究所有分析均用SPSS16.0实现,Graph Pad Prism 8来绘制lncRNA相关图形。结果:随访中GAS5表达水平始终是活动组低于非活动组(P=0.028;P=0.050;P=0.026);且GAS5水平与疾病活动存在负相关(r=-0.518,P=0.023;r=-0.462,P=0.046;r=-0.525,P=0.021)。然而,MALAT1仅在随访M6的时候发现与SSc患者疾病活动度存在负相关(r=-0.462,P=0.046),未发现NEAT1和H19表达水平与疾病活动度有相关

DETERMINANTS OF THE UTILIZATION OF ACUPUNCTURE TREATMENT IN KOREA:USING ANDERSEN'S BEHAVIORAL MODEL

Objective: South Korea is known for its dual health care delivery system, which officially incorporates both Western and traditional Korean medicine.This research aims to find out the determinants of the utilization of traditional Korean medical service, specifically acupuncture service, through Andersen’s behavioral model and quality of life(health-related quality of life, hrqol, EQ-5D).Methods: In this study, we reviewed Korean Health Panel data, a national representative data from 2011.We analyzed the utilization of outpatient acupuncture services within 12 months of 2011.A two-part model, including predisposing, enabling, and need factors of Andersen’s behavioral model in Model 1 and the additional hrqol(EQ-5D Korean index) in Model 2, was analyzed through hierarchal logistic analysis to analyze the frequency of acupuncture service usage among the subjects who visited traditional Korean medical clinics.Results: The frequency of acupuncture service usage among the subjects who visited traditional Korean medical institutions increased as the subjects were female, older, and married, had more numbers of chronic diseases, did not have physical disabilities, and had lower hrqol(EQ-5D Korean index).Conclusion: The important determinants of the frequency oftraditional Korean medicine usage were gender, age, marital status, the number of chronic diseases, and the presence of physical disabilities.The results contribute to our understanding of the characteristics of traditional Korean medicine users and may be used as a basic resource for related policymaking by government officials and medical professionals.

Assessment of Fatigue in Undifferentiated Spondyloarthritis (USpA)

Objectives: This study aimed to evaluate the fatigue in patients with undifferentiated spondyloarthritis [USpA], and its relationship with disease-specific variables, spinal mobility measures and healthy related quality of life [hrqol]. Methods: Eighty patients with USpA and forty healthy subjects were included in this study. The multidimensional assessment of fatigue [MAF] and the generic instrument Short Form 36 [SF 36] were used in patient and control groups to assess fatigue and [hrqol]. Fatigue was also assessed with the Bath ankylosing spondylitis disease activity index [BASDAI] fatigue item. The evaluation included the activity of the disease [BASDAI], functional status [Bath ankylosing spondylitis functional index], and visual analog scale [VAS] of axial and joint pain. Demographics and disease-related data were obtained. Results: Patients with USpA had higher scores in MAF total and in all MAF subgroup scales than controls. All SF-36 subgroups scores were also found to be significantly lower in patients. Severe fatigue experienced 60% in patients. MAF total scores were found to be significantly correlated with morning stiffness, BASDAI, Bath ankylosing spondylitis functional index (BASFI), BASDAI fatigue, VAS-axial, and SF-36 subgroups scores in patients [p hrqol when compared with healthy persons. MAF was found to be related with the clinical and functional status and health-related quality of life of patients with USpA.

Physiological and Psychological Determinants of Quality of Life for Patients after Cardiac Surgery and the Associated Factors

Purpose: This review related to the level of health-related quality of life (hrqol) and post-traumatic stress disorder symptoms (PTSD) for patients after cardiac surgery, and associated risk factors. Also the hrqol, PTSD measures and the related factor. Methods: The search strategy was guided by the review aims. The following databases were searched to retrieve related literature: the Cumulative Index to Nursing and Allied Health Literature (CINAHL) via EBSCO, MEDLINE via PubMed, Ovid, Science Direct, Google Scholar, in addition to the Jordanian Database for Nursing Research. Studies presented in English for the period between 2012 and 2019 were included. Literature was reviewed by using the following strategy: The key word “post-operative hrqol”, “post-operative PTSD”, “prevalence”, “incidence”, “risk factors”, and “cardiac surgery”, with using of Boolean operators “AND”, “OR”, and “NOT” to conduct the search in databases, Then the titles and abstracts were reviewed for their relevancy and irrelevant and duplicated studies were discarded. EBSCO resulted in 180 articles, Ovid 5 articles, and SCIENCE DIRECT 200 articles, articles with the same ideas were sorted, and full text was included in the review. The overall number after sorting was 25 articles that were included in the review. Results: Literature has aggregated the most common risk factors as age, gender, marital status, social support, medical comorbidities, smoking, alcohol use, ICU environment, surgery techniques and physiological indicators, and the most important is preoperative hrqol level. Also, the researcher highlighted several important points related to PTSD after cardiac surgery. Firstly, PTSD appears to be present in approximately 10% of the post-cardiac surgical population, with some estimates reaching as high as 38%. ICU care, fright at the time of the event, perceived threat to life, high levels of sedation and social isolation may all contribute to this developing PTSD. Patients with PTSD are susceptible to poor outcomes following surgery
