Comparison of p38 MAPK, soluble endoglin and endothelin-1 level in severe preeclampsia and hellp syndrome patients

Objective: To compare the levels of p38 mitogen-activated protein kinase (MAPK), soluble endoglin and endothelin-1 in the serum of patients with severe preeclampsia, hemolysis, elevated liver enzymes, and low platelets (hellp) syndrome and normal pregnancies. Methods: This study was an observational analytic cross-sectional study performed at Wahidin Sudirohusodo Hospital, Makassar, Indonesia, in the period of 5th February 2016 to 20th January 2017. P38 MAPK, soluble endoglin and endothelin-1 levels of patients with normal pregnancies, severe preeclampsia and hellp syndrome were measured by enzyme-linked immunoabsorbentassay technique, using kits of human soluble endoglin, endothelin-1 and p38 MAPK, Quantikine immunoassay: R&D System Inc. Results: Level of serum p38 MAPK in hellp syndrome group was higher than in severe preeclampsia and normal pregnancy groups. Soluble endoglin and endothelin-1 levels in pregnancies with severe preeclampsia and hellp syndrome were higher than normal pregnancy but there was no significant difference between these two groups (P>0.05). Levels of p38 MAPK, soluble endoglin and endothelin-1 also had a positive linear correlation with systolic and diastolic blood pressures (Phellp syndrome.

Introduction Preeclampsia is a well known obstetric complication characterized with hypertension and proteinuria whereas hellp syndrome (hemolysis, elevated liver enzymes, low platelet count) which was first stated by Prichard in 1954 and described by Weinstein in 1982, is a multisystemic disease usually seen in the third trimester and in the postpartum period, usually in 48 - 72 hours following delivery [1]. Aim In this study we aim to share our experience in 17 patients with hellp syndrome treated in our clinic between January 2005 and March 2008 in regard of the current literature. Material and Methods 17 patients diagnosed with hellp syndrome between January 2005 and March 2008 were retro-spectively re-evaluated in regard of symptoms, treatment options and prognosis. As such this is a retro-spective study focusing on case reports. Conclusion As noted before main focus of treatment should be on patient stability, in other words, palliative care under intensive care conditions.

Preeclampsia is a pregnancy specific disease characterized by hypertension and proteinuria, by definition, developing after 20 weeks of pregnancy. Here we present an atypical case of severe preeclampsia complicated by generalized convulsions (eclampsia) and hemolysis, elevated liver enzymes, and low platelet count syndrome in an 18 weeks pregnant patient with chronic hypertension. Occurrence of these complications was preceded by upper right quadrant abdominal pain and severe hypertension. In this case no evidence of autoimmunity disorders or molar pregnancy was found.

Thrombocytopenia caused by Hemolysis, Elevated Liver enzymes and Low Platelet count (hellp) syndrome in pre-eclampsia parturients can be associated with substantial maternal and neonatal morbidity. Data on the issue of the safety of neuraxial anesthesia with thrombocytopenia in hellp syndrome is limited. A lower limit of 100,000 per microliter for platelet count was suggested as “safe” for performing neuraxial anesthesia, however there is no supporting data. This lower limit is challenged lately. We present a case of uneventful spinal anesthesia for urgent Cesarean section in a patient with severe pre-eclamsia, hellp syndrome and precipitous platelet drop from 230,000 to 42,000 per microliter.

hellp（hemolysis eevated liver enzymes low platelets）综合征是由Weinstein[1]在1982年首次提出,取其主要临床特征中的溶血（hemolysis）、肝酶升高（eevated liver emzymes）和血小板减少（low platelet）的英文第一个字母命名。hellp综合征可以是妊娠高血压疾病（hypertensivedisorders of pregnancy，HDP）的严重并发症，也可以在无血压升高或无蛋白尿的情况下发生，还可在子痫前期出现临床症状之前发生，临床上多以剖宫产的方式终止妊娠。本文对hellp综合征的诊疗进展及围术期麻醉管理进行简要概述。
