Prevalence and treatment of hypertension in China： impacts of 2017 American College of Cardiology/American Heart Association High Blood Pressure guideline

The definition, recommended treatment thresholds and targets of hypertension have been updated in the recent 2017 American College of Cardiology/American Heart Association(ACC/AHA) guideline for the Prevention, Detection, Evaluation and Management of High Blood Pressure in Adults. However,the impacts of this guideline on Chinese population are currently unknown. In this study, we aim to provide updated data in China using criteria from the updated guideline. A multistage, stratified sampling method was used to obtain a representative sample of 50,171 adults aged 18 years and *** was defined as an average systolic blood pressure !130 mm Hg, or an average diastolic blood pressure !80 mm Hg, or self-reported use of antihypertensive medications in past 2 weeks. The weighted prevalence of hypertension was 60.1%(95% confidence interval [CI] 59.3%–61.0%). The treatment rate and recommended treatment rate were 16.8%(95% CI 16.1%–17.6%) and 53.7%(95% CI52.6%–54.9%), respectively; the gap was more prominent among men and participants from rural ***, the adoption of the 2017 ACC/AHA guideline will lead to a substantial increase in both prevalence and number of patients needing treatment in China. The applicability of this guideline should be carefully evaluated based on evidence from Chinese population.

With the rising epidemic of obesity,metabolic syndrome,and type 2 diabetes mellitus in China,metabolic dysfunctionassociated non-alcoholic fatty liver disease has become the most prevalent chronic liver *** condition frequently occurs in Chinese patients with alcoholic liver disease and chronic hepatitis *** address the impending public health crisis of non-alcoholic fatty liver disease and its underlying metabolic issues,the Chinese Society of Hepatology and the Chinese Medical Association convened a panel of clinical experts to revise and update the“guideline of prevention and treatment of non-alcoholic fatty liver disease(2018,China)”.The new edition,titled“guideline for the prevention and treatment of metabolic dysfunction-associated fatty liver disease(Version 2024)”,offers comprehensive recommendations on key clinical issues,including screening and monitoring,diagnosis and evaluation,treatment,and followup for metabolic dysfunction-associated fatty liver disease and metabolic dysfunction-associated steatotic liver *** dysfunction-associated fatty liver disease is now the preferred English term and is used interchangeably with metabolic dysfunction-associated steatotic liver ***,the guideline emphasizes the importance of multidisciplinary collaboration among hepatologists and other specialists to manage cardiometabolic disorders and liver disease effectively.

Non-muscle invasive bladder cancer(NMIBC)is a major type of bladder cancer with a high incidence worldwide,resulting in a great disease *** and surveillance are the most important part of NIMBC *** 2018,we issued“Treatment and surveillance for non-muscle-invasive bladder cancer in China:an evidencebased clinical practice guideline”.Since then,various studies on the treatment and surveillance of NMIBC have been *** is a need to incorporate these materials and also to take into account the relatively limited medical resources in primary medical institutions in *** a version of guideline which takes these two issues into account to promote the management of NMIBC is therefore *** formed a working group of clinical experts and *** questionnaire investigation of clinicians including primary medical institutions,24 clinically concerned issues,involving transurethral resection of bladder tumor(TURBT),intravesical chemotherapy and intravesical immunotherapy of NMIBC,and follow-up and surveillance of the NMIBC patients,were determined for this *** and recommendations on the management of NMIBC in databases,guideline development professional societies and monographs were referred to,and the European Association of Urology was used to assess the certainty of generated ***,we issued 29 statements,among which 22 were strong recommendations,and 7 were weak *** recommendations cover the topics of TURBT,postoperative chemotherapy after TURBT,Bacillus Calmette–Guérin(BCG)immunotherapy after TURBT,combination treatment of BCG and chemotherapy after TURBT,treatment of carcinoma in situ,radical cystectomy,treatment of NMIBC recurrence,and follow-up and *** hope these recommendations can help promote the treatment and surveillance of NMIBC in China,especially for the primary medical institutions.

Benign prostatic hyperplasia (BPH) is highly prevalent among older men, impacting on their quality of life, sexual function, and genitourinary health, and has become an important global burden of disease. Transurethral plasmakinetic resection of prostate (TUPKP) is one of the foremost surgical procedures for the treatment of BPH. It has become well established in clinical practice with good efficacy and safety. In 2018, we issued the guideline “2018 Standard Edition”. However much new direct evidence has now emerged and this may change some of previous recommendations. The time is ripe to develop new evidence-based guidelines, so we formed a working group of clinical experts and methodologists. The steering group members posed 31 questions relevant to the management of TUPKP for BPH covering the following areas: questions relevant to the perioperative period (preoperative, intraoperative, and postoperative) of TUPKP in the treatment of BPH, postoperative complications and the level of surgeons’ surgical skill. We searched the literature for direct evidence on the management of TUPKP for BPH, and assessed its certainty generated recommendations using the grade criteria by the European Association of Urology. Recommendations were either strong or weak, or in the form of an ungraded consensus-based statement. Finally, we issued 36 statements. Among them, 23 carried strong recommendations, and 13 carried weak recommendations for the stated procedure. They covered questions relevant to the aforementioned three areas. The preoperative period for TUPKP in the treatment of BPH included indications and contraindications for TUPKP, precautions for preoperative preparation in patients with renal impairment and urinary tract infection due to urinary retention, and preoperative prophylactic use of antibiotics. OBJECTIVE:To compare clinical practice guideline recommendations on the use of oral patent Traditional Chinese Medicines(PTCMs) for uncomplicated acute lower respiratory tract infections(ALRTIs)in adults with the existing evidence using results of a systematic review of randomized controlled trials(RCTs).METHODS:A systematic review on RCTs and a systematic review of current guidelines on orally taken PTCMs for uncomplicated ALRTIs were *** Med,Cochrane Library,EMBASE and four Chinese databases were searched from inception to September 2016 for RCTs testing orally taken PTCMs for uncomplicated ALRTIs(excluding pneumonia).Two reviewers independently screened each study,extracted study data,and assessed risk of *** were resolved through discussion or by consultation with a third *** practice guidelines for uncomplicated ALRTIs containing PTCM recommendations were identified and quality *** quality of pooled evidence of the RCTs and the guidelines was assessed with GRADE and AGREE Ⅱ *** consistency of the evidence base in RCTs and the guideline recommendations were then ***:For the systematic review of RCTs,4810 papers were identified,among which 29 RCTs(5093 patients) were included in the *** compared to placebo increased the effective treatment rate of cough(3 trials,949 patients,risk ratio(RR) 2.50,1.16 to 5.43;low certainty);improved assessment of global health(3 trials,948 patients,RR1.70,1.44 to 2.01;low certainty);and increased the effective rate of specific symptom relief(1 trial,478 patients,RR 4.01,2.76 to 5.81;moderate certainty).21 trials(3432 patients) compared effects of different *** the guideline evaluation,29 PTCMs were recommended for the use of uncomplicated ALRTIs,of which27 had no supportive evidence from ***:The evidence base of PTCMs for uncomplicated ALRTIs is weak and the guideline recommendations were based on almost no clinical trial evidence.

Why is the Chinese hypertension guideline necessary?

The new Chinese hypertension guideline was eventually published.[1] It has been more than seven years since the previous Chinese hypertension guideline was published in 2011.[2] The guideline committee should also be congratulated that this new Chinese hypertension guideline was for the first time published in full length in the English language in this issue of the Journal of Geriatric Cardiology.[3] This is the fourth Chinese hypertension guideline,[3] after the first,second[4] and third[2] in 1999,2005 and 2011,*** first Chinese hypertension guideline was printed in a *** document was not so sophisticated,but nevertheless played a role in improving management of hypertension at that time in *** treatment rate of hypertension increased substantially from 45.5% in 1991[5] to 81.8% in 2002[6] in patients who were aware of the disease (Table 1).
