In this paper damage assessment based on guided elastic wave propagation phenomenon is presented.guided waves are generated by piezoelectric transducer and registered by scanning laser doppler vibrometer(SLDV).Signal processing is based on the analysis of full wavefield measurements gathered from dense mesh of measurement points spanned over area of investigated samples.Full wavefield measurement approach allows creation of animations presenting the guided wave propagation in the structure.Moreover such approach is suitable for analysis of interaction of guided waves with discontinuities located in structure.In the research attention is paid especially on analysis of phenomenon of S0/A0′guided wave mode conversion due to interaction with investigated discontinuities-teflon inserts and impact damage.The presented work is related to glass fibre reinforced polymer(GFRP)samples.In the research,auxiliary non-destructive testing(NDT)method is also utilized.The aim of this method is to indicate the depth of discontinuity,and to prove that delamination was created in the case of impact damage.Auxiliary method is based on terahertz spectroscopy(THz)where the analysis of propagation of electromagnetic waves in the terahertz band is conducted.THz spectroscopy method can be utilized for damage assessment in the dielectric materials like GFRP.

With the advent of linear echoendoscopes, endoscopic ultrasound (EUS) has become more operative and a new field of oncological application has been opened up. From tumor staging to tissue acquisition under EUS-guided fine-needle aspiration, new operative procedures have been developed on the principle of the EUS-guided puncture. A hybrid probe combining radiofrequency with cryotechnology is now available, to be passed through the operative channel of the echoendoscope into the tumor to create an area of ablation. EUS-guided fine-needle injection is emerging as a method to deliver anti-tumoral agents inside the tumor. Ethanol lavage, with or without paclitaxel, has been proposed for the treatment of cystic tumors in non-resectable cases and complete resolution has been recorded in up to 70%-80%. Many other chemical or biological agents have been investigated for the treatment of pancreatic adenocarcinoma: activated allogenic lymphocyte culture (Cytoimplant), a replication-deficient adenovirus vector carrying the tumor necrosis factor-α gene, or an oncolytic attenuated adenovirus (ONYX-015). The potential advantage of treatment under EUS control is the real-time imaging guidance into a deep target likethe pancreas which is extremely difficult to reach by a percutaneous approach. To date there are no randomized controlled trials to confirm the real clinical benefits of these treatments compared to standard therapy so it seems wise to reserve them only for experimental protocols approved by ethics committees.

Ultrasound-guided ovarian cyst aspiration is an increasingly important means of diagnosingand treating ovarean cysts.We report the results of 140 ultrasound-guided ovarian cyst aspirations in112 symptomatic patients,with an age range of 19-87 years(mean 38).Cysts had benign character-istics and ranged from 4 to 20 cm in diameter.The majority of cysts were aspirated once.In 20 casescysts were aspirated twice,in five cases three times and in one case four times.The cyst aspirate wesexamined cytologically and correlated with histology where available.The only complication was an o-varian abscess,giving a eomplieation rate of 0.7%.Cytology was not found to be useful in excludingneoplastic lesions(sensitivity 46.6%.)The aspirate type was more useful.A serous aspirate was asso-ciated with a cure rate of 73.7%(45 of 61);28%(17 of 61)eventually had surgery;at surgery 9(14.75%)neoplastic lesions were found,all were benign.A haemorrhagic aspirate with no histo~ ofendometriosis was associated with a cure rate of 70.6% (12 of 17);there were 4(24%)neoplastic cystsat surgery,1 of these was malignant.Endometriotic cysts had a recurrence rate of 90%(19 of 21).Withmutinous aspirates 75%(3 of 4)were mueinous neoplasms.Aspiration was performed in 14 post-menopausal patients.Two neoplasms were found in this group and one of these was a malignant neoplasm,which had yielded a haemorrhagic aspirate.The majority of cysts even in the postmenopausal patientswere innocuous.Uetrasound-guided cyst aspiration is safe and useful in the management of ovarian cysts with benigncharacteristics.The finding of a haemorrhagic aspirate is more likely to indicate a neoplastic lesion,andespecially in the postmenopausal patient should be viewed with caution.The risk of missing a malignantlesion in non-haemorrhagie cysts with benign eharacteristies on ultrasound is negligible.

Ovarian cysts in pregnancy if symptomatic may require treatment because of the risks oftorsion,rupture and obstructed labour.There is also a risk that such cysts are neoplastic and thereforepotentially malignant.These lesions ate a management dilemma because of the risks of surgery to bothmother and fetus.Ultrasound-guided ovarian cyst aspiration is being increasingly recognized as a meansof diagnosing and treating ovarian cysts in the non-pregnant patient.There are few published reports onits use in pregnant patients.We report the results of 14 ultrasound guided cyst aspirations in 12 pregnantpatients.The technique and results are dicussed.Two of the 12 cysts(17%)were eventually found to bedue to neoplasms and one of these showed borderline malignancy at histology.Therefore,all cysts must beassessed to ensure that they have benign characteristics,and if not we would recommend these cysts beconsidered for surgery in the second trimester.There were no complications and all the pregnaeies pro-ceeded to term with normal deliveries.It is concluded that ultrasound-guided needle aspiration of ovariancysts can be simply and safely performed in pregnancy.

目的探讨Guide Care管理模式对慢性心功能衰竭(CHF)患者服药依从性、自我护理行为和生活质量的影响。方法选取在我院接受治疗的90例CHF患者为研究对象,采用随机数字表法将其分为对照组和干预组,每组45例。对照组接受常规护理,干预组在对照组基础上给予guided Care管理模式。比较两组护理效果。结果干预组ACEI/ARB、β受体阻滞剂、ACEI/ARB与β受体阻滞剂联用的应用比例及达到指南推荐靶剂量的患者比例均显著高于对照组(P0.05);随访结束时,干预组的躯体领域、情绪领域评分和总分及EHFScBS评分均显著低于对照组(Pguided Care管理模式应用于CHF患者中有助于改善患者的服药依从性,提高患者的自我护理行为和生活质量水平。

AIM: To evaluate the feasibility of using therapeutic ultrasound as an alternative treatment option for organconfined prostate cancer. METHODS: In this study, a trans-urethral therapeutic ultrasound applicator in combination with 3T magnetic resonance imaging(MRI) guidance was used for realtime multi-planar MRI-based temperature monitoring and temperature feedback control of prostatic tissue thermal ablation in vivo. We evaluated the feasibility and safety of MRI-guided trans-urethral ultrasound to effectively and accurately ablate prostate tissue while minimizing the damage to surrounding tissues in eight canine prostates. MRI was used to plan sonications, monitor temperature changes during therapy, and to evaluate treatment outcome. Real-time temperature and thermal dose maps were calculated using the proton resonance frequency shift technique and were displayed as two-dimensional color-coded overlays on top of the anatomical images. After ultrasound treatment, an evaluation of the integrity of cavernosal nerves was performed during prostatectomy with a nerve stimulator that measured tumescence response quantitatively and indicated intact cavernous nerve functionality. Planned sonication volumes were visually correlated to MRI ablation volumes and corresponding histo-pathological sections after prostatectomy. RESULTS: A total of 16 sonications were performed in 8 canines. MR images acquired before ultrasound treatment were used to localize the prostate and to prescribe sonication targets in all canines. Temperature elevations corresponded within 1 degree of the targeted sonication angle, as well as with the width and length of the active transducer elements. The ultrasound treatment procedures were automatically interrupted when the temperature in the target zone reached 56 ℃. In all canines erectile responses were evaluated with a cavernous nerve stimulator post-treatment and showed a tumescence response after stimulation with an electric current. These results indicated intact cavernous nerve functionality. I

目的探讨guided Care管理模型在老年患者药物不良反应风险防控中的作用。方法采用定点连续抽样方法，选取北京协和医院老年病房的患者为研究对象。2015年1—12月期间的住院患者为对照组，2016年1—12月期间住院的患者为观察组。从2016年开始实施guided Care管理模型对老年患者药物不良反应进行风险防控，包括采用Beers标准进行药物不良反应风险评估、查找问题、多学科协作计划改进。比较实施前后住院患者的药物不良反应发生情况。结果2015年药物不良反应发生率为15.6%，2016年药物不良反应发生率为7.6%，差异有统计学意义（χ2=86.78，P〈0.01）。结论实施guided Care管理模型可降低老年患者药物不良反应发生率，保障护理安全。

本文提出了一种简单有效的基于guided Filter滤波的联合增强算法,用于对红外热图像的增强处理。其中guided Filter是一种能够精确提取导引图像任何细节的滤波器。它是最快速的保边滤波器,具有严格的线性计算量而与滤波窗口半径大小无关。本文所提基于guided Filter的联合增强算法,将输入图像分解为基础层和细节层;基础层采用线性AGC压缩动态范围,而对细节层采用guided Filter增强;然后联合计算整体的增强结果图像。本文所提的算法具有很强的细节增强能力,且提升强度可控,不会导致局部增强类算法的halo现象,结果图像整体协调、自然,主要可应用红外热图像的增强处理。也可用于可见光图像的增强。
