Procedural simulation:medical student preference and value of three task trainers for ultrasound Guided regional anesthesia

BACKGROUND: Ultrasound Guided regional anesthesia is widely taught using task trainer models. Commercially available models are often used; however, they can be cost prohibitive. Therefore, alternative "homemade" models with similar fidelity are often used. We hypothesize that professional task trainers will be preferred over homemade models. The purpose of this study is to determine realism, durability and cleanliness of three different task trainers for ultrasound Guided nerve blocks.METHODS: This was a prospective observational study using a convenience sample of medical student participants in an ultrasound Guided nerve block training session on January 24 th, 2015. Participants were asked to perform simulated nerve blocks on three different task trainers including, 1 commercial and 2 homemade. A questionnaire was then given to all participants to rate their experiences both with and without the knowledge on the cost of the simulator device.RESULTS: Data was collected from 25 participants. The Blue Phantom model was found to have the highest fidelity. Initially, 10(40%) of the participants preferred the Blue Phantom model, while 10(40%) preferred the homemade gelatin model and 5(20%) preferred the homemade tofu model. After cost awareness, the majority, 18(72%) preferred the gelatin model.CONCLUSION: The Blue Phantom model was thought to have the highest fidelity, but after cost consideration the homemade gelatin model was preferred.

On the basis of second-order perturbation approximate and modal expansion approach,we investigate the enhancement effect of cumulative second-harmonic generation(SHG)of circumferential Guided waves(CGWs)in a circular tube,which is inherently induced by the closed propagation feature of CGWs.An appropriate mode pair of primary-and double-frequency CGWs satisfying the phase velocity matching and nonzero energy flux is selected to ensure that the second harmonic generated by primary CGW propagation can accumulate along the circumference.Using a coherent superposition of multi-waves,a model of unidirectional CGW propagation is established for analyzing the enhancement effect of cumulative SHG of primary CGW mode selected.The theoretical analyses and numerical simulations performed directly demonstrate that the second harmonic generated does have a cumulative effect along the circumferential direction and the closed propagation feature of CGWs does enhance the magnitude of cumulative second harmonic generated.Potential applications of the enhancement effect of cumulative SHG of CGWs are considered and discussed.The theoretical analysis and numerical simulation perspective presented here yield an insight previously unavailable into the physical mechanism of the enhancement effect of cumulative SHG by closed propagation feature of CGWs in a circular tube.

In last few years,Guided image fusion algorithms become more and more popular.However,the current algorithms cannot solve the halo artifacts.We propose an image fusion algorithm based on fast weighted Guided filter.Firstly,the source images are separated into a series of high and low frequency components.Secondly,three visual features of the source image are extracted to construct a decision graph model.Thirdly,a fast weighted Guided filter is raised to optimize the result obtained in the previous step and reduce the time complexity by considering the correlation among neighboring pixels.Finally,the image obtained in the previous step is combined with the weight map to realize the image fusion.The proposed algorithm is applied to multi-focus,visible-infrared and multi-modal image respectively and the final results show that the algorithm effectively solves the halo artifacts of the merged images with higher efficiency,and is better than the traditional method considering subjective visual consequent and objective evaluation.

BACKGROUND Novel strategies are needed for improving Guided bone regeneration(GBR) in oral surgery prior to implant placement, particularly in maxillary sinus augmentation(GBR-MSA) and in lateral alveolar ridge augmentation(LRA). This study tested the hypothesis that the combination of freshly isolated, unmodified autologous adipose-derived regenerative cells(UA-ADRCs), fraction 2 of plasma rich in growth factors(PRGF-2) and an osteoinductive scaffold(OIS)(UAADRC/PRGF-2/OIS) is superior to the combination of PRGF-2 and the same OIS alone(PRGF-2/OIS) in GBR-MSA/LRA.CASE SUMMARY A 79-year-old patient was treated with a bilateral external sinus lift procedure as well as a bilateral lateral alveolar ridge augmentation. GBR-MSA/LRA was performed with UA-ADRC/PRGF-2/OIS on the right side, and with PRGF-2/OIS on the left side. Biopsies were collected at 6 wk and 34 wk after GBRMSA/LRA. At the latter time point implants were placed. Radiographs(32 mo follow-up time) demonstrated excellent bone healing. No radiological or histological signs of inflammation were observed. Detailed histologic,histomorphometric, and immunohistochemical analysis of the biopsies evidenced that UA-ADRC/PRGF-2/OIS resulted in better and faster bone regeneration than PRGF-2/OIS.CONCLUSION GBR-MSA with UA-ADRCs, PRGF-2, and an OIS shows effectiveness without adverse effects.

Pancreatic pseudocyst(PPC), a common sequela of acute or chronic pancreatitis, was defined by the revised Atlanta classification as "a collection." Endoscopic ultrasound(EUS)-Guided drainage is often considered a standard first-line therapy for patients with symptomatic PPC. This effective approach exhibits 90%-100% technical success and 85%-98% clinical success. Bleeding is a deadly adverse event associated with EUS-Guided drainage procedures, and the bleeding rate ranges from 3% to 14%. Hemostasis involves conservative treatment, endoscopy, interventional radiology-Guided embolization and surgery. However, few studies have reported on EUSGuided drainage with massive, multiple hemorrhages related to severe pancreatogenic portal hypertension(PPH). Thus, the aim of this case report was to present a case using a balloon dilator to achieve successful hemostasis for PPH-related massive bleeding in EUSGuided drainage of PPC. To our knowledge, this method has not been previously reported.

AIM: To evaluate accuracy of axis alignment and refractive results of toric phakic intraocular lens(IOL) implantation using a digital imaging system. METHODS: This retrospective study investigated toric implantable collamer lens(ICL) implantation in 30 eyes of 21 patients with myopic astigmatism more than 2.0 D Guided with digital imaging system. Data were collected during the first week after phakic IOL implantation.RESULTS: Thirty eyes of 21 patients were included in our study. Patients includes 9 males and 12 females. The mean age of the patients was 26.5±7.1(range 21-44)y. The mean preoperative manifest astigmatism was 3.2±1.7(range from 2.25 to 4.75) D. The mean postoperative uncorrected distance visual acuity(UCDVA) were 0.07±0.07(range from 0.1 to 0.0) log MAR. The mean postoperative residual refractive cylinder was 0.25±0.29(range 0-0.75) D. Eyes with postoperative residual refractive cylinder of 0.5 D or less represented 80%(24 eyes). The mean postoperative toric IOL misalignment measured by the OPD scan III was 1.9°±1.45°(range from 0 to 5°). CONCLUSION: Image Guided system allows accurate alignment of toric ICL. This is associated with good postoperative visual acuity and low residual refractive astigmatism which correlates with the precision of toric phakic IOL alignment.

To solve the problem of small amount of machining centers in small and medium flexible manufacture systems(FMS), a scheduling mode of single automated Guided vehicle(AGV) is adopted to deal with multiple transport requests in this paper. Firstly, a workshop scheduling mechanism of AGV is analyzed and a mathematical model is established using Genetic Algorithm. According to several sets of transport priority of AGV, processes of FMS are encoded, and fitness function, selection, crossover, and variation methods are designed. The transport priority which has the least impact on scheduling results is determined based on the simulation analysis of Genetic Algorithm, and the makespan, the longest waiting time, and optimal route of the car are calculated. According to the actual processing situation of the workshop, feasibility of this method is verified successfully to provide an effective solution to the scheduling problem of single AGV.

After removal of the caries or diseased teeth,the alveolar ridge will undergo absorption and atrophy.When the amount of alveolar bone is insufficient,it will cause an inability to perform effective dental implant restoration.In order to control the absorption and promote the repair and regeneration of alveolar ridge,a method of implanting Guided bone regeneration(GBR)membranes at the extraction site is often used.In this study,silk fibroin(SF)and poly-L-lactide lactone(PLCL)were used to prepare bilayered Guided bone regeneration membranes,and its morphology,hydrophilicity,surface roughness and mechanical properties were studied.At the same time,the drug release behaviors and cell compatibility of the bilayered membranes were studied.The results showed that SF/PLCL bi-layered membranes had good mechanical properties and surface hydrophilicity,and the drug-loaded bi-layered membranes had good cell compatibility.The bilayered membranes fabricated in this study are of potential for applying in the oral health field to promote bone regeneration.
