Background Primary malignant germ cell tumors （GCTs） of mediastinum are rare *** introduce our institutional experience in managing patients with primary malignant GCTs of the mediastinum,focusing on the analysis of therapeutic *** A retrospective review was done in 39 consecutive patients with mediastinal malignant GCTs treated in our institution between 1991 and *** A total of 39 patients were enrolled in this study with a median age of 27 *** 5-year overall survival （OS） and progression-free survival （PFS） rates of the whole population were 60.2% and 57.7%,*** by the histology,18 patients （46.2%） had seminoma and 21 patients （53.8%） had nonseminomatous germ cell tumors （NSGCTs）.The 5-year OS rate of patients with seminoma was 87.4% as compared with 36.7% in patients with NSGCTs （P=-0.0004）.The 5-year PFS rate was also significantly higher in seminoma patients （87.4% vs.31.6%,P=-0.003）.For 19 patients with NSGCTs managed with multi-modality treatment,chemotherapy exposure appeared to impact the *** 5-year OS rate was 44.9% in patients with chemotherapy exposure as compared with 20.0% in patients without it （P=0.43）.Conclusion Our study confirmed the significance of systemic chemotherapy in the treatment of primary mediastinal GCTs.

High-dose chemotherapy(HDCT)with autologous hematopoietic stem cell transplantation has been explored and has played an important role in the management of patients with high-risk germ cell tumors(GCTs)who failed to be cured by conventional *** stem cells(HSCs)collected from the peripheral blood,after appropriate pharmacologic mobilization,have largely replaced bone marrow as the principal source of HSCs in *** it is currently common practice to perform tandem or multiple sequential cycles of HDCT,it is anticipated that collection of large numbers of HSCs from the peripheral blood is a prerequisite for the success of the ***,the CD34+cell dose/kg of body weight infused after HDCT has proven to be a major determinant of hematopoietic engraftment,with patients who receive>2×106 CD34+cells/kg having consistent,rapid,and sustained hematopoietic ***,many patients with relapsed/refractory GCTs have been exposed to multiple cycles of myelosuppressive chemotherapy,which compromises the efficacy of HSC mobilization with granulocyte colony-stimulating factor with or without ***,alternative strategies that use novel agents in combination with traditional mobilizing regimens are ***,after an overview of the mechanisms of HSCs mobilization,we review the existing literature regarding studies reporting various HSC mobilization approaches in patients with relapsed/refractory GCTs,and finally report newer experimental mobilization strategies employing novel agents that have been applied in other hematologic or solid malignancies.

Roles of the Y chromosome genes in human cancers

PURPOSE: This study reported our experience with chemotherapy and/or radiotherapy in series of patients who had a clinical diagnosis of cranial germ cell tumors （GCTs） without surgical biopsy confirmation. METHODS: We retrospectively analyzed the outcome of patients who had typical MRI evidence of a pineal or suprasellar mass with or without elevated levels of beta-human chorionic gonadotropin （β-HCG） and/or alpha- fetoprotein （AFP）. The patients were grouped into chemotherapy（n=21 ）, radiotherapy（n=5）, and stereotactic radiotherapy （n=12） based on the primary treatment. RESULTS: Between May 2002 and August 2010, a total of 38 patients were treated. Median age of the patients at diagnosis was 16 years old （range, 4-30 years） with 33 males and 5 females. Twenty-six （68.4%） patients showed elevated levels ofβ-HCG and/or AFP in serum, cerebrospinal fluid, or both. Twelve patients had received prior ventricular- peritoneal shunt. Twenty-one patients received primary chemotherapy resulting in 9 complete response （CR）, 8 partial response （PR）, 2 stable disease （SD） and 2 not evaluate（NA）. The objective response rate（ORR）was 81%. In which, 17 patients followed by adjuvant radiotherapy. Five patients received primary radiotherapy resulting in 3 CR and 2 PR. The ORR was 100%. In which, 1 patient followed by adjuvant chemotherapy. Twelve patients received primary stereotacticradiotherapy（SRS） resulting in 2 CR, 9 PR and 1 SD. The ORR was 91.7%. In which, 4 patients followed by adjuvant chemotherapy after SRS and 8 of 12 patients received salvage chemotherapy when disease recurred. Six （out of 12） patients received whole brain or cranio-spinal radiotherapy after chemotherapy. Chemotherapy was consisted of cisplatin, VP-16/VM-26 and bleomycin（PEB regimen）.The median follow-up time is 29 months （range, 2-98 months）. Male and female differ genetically by their respective sex chromosome composition, that is, XY as male and XX as female. Although both X and Y chromosomes evolved from the same ancestor pair of autosomes, the Y chromosome harbors male-specific genes, which play pivotal roles in male sex determination, germ cell differentiation, and masculinization of various tissues. Deletions or translocation of the sex-determining gene, SRY, from the Y chromosome causes disorders of sex development （previously termed as an intersex condition） with dysgenic gonads. Failure of gonadal development results not only in infertility, but also in increased risks of germ cell tumor （GCT）, such as gonadoblastoma and various types of testicular GCT. Recent studies demonstrate that either loss of Y chromosome or ectopic expression of Y chromosome genes is closely associated with various male-biased diseases, including selected somatic cancers. These observations suggest that the Y-linked genes are involved in male health and diseases in more frequently than expected. Although only a small number of protein-coding genes are present in the male-specific region of Y chromosome, the impacts of Y chromosome genes on human diseases are still largely unknown, due to lack of in vivo models and differences between the Y chromosomes of human and rodents. In this review, we highlight the involvement of selected Y chromosome genes in cancer development in men.

germ cell cancers are the most common solid tumors among men between 15 and 40 ***-seminomatous germ cell tumors(NSGCTs)represent a unique and exclusive cohort of germ cell tumor ***-seminoma can harbor different histologic *** most commonly found histologies are embryonal cell cancer,teratoma,yolk sack tumor and choriocarcinoma,as well as teratocarcinoma and seminoma,in combination with non-seminomatous germ cell tumors histologic *** clinical definition of stage I nonseminoma is the absence of metastatic lesions on imaging and normal tumor *** cure rate for clinical stage I NSGCT is 99%and this can be achieved by three therapeutic strategies:Active surveillance with treatment at the time of relapse,retroperitoneal lymph node dissection or adjuvant *** balancing of these various strategies should always be based on an individual risk profile of NGSCG patient depending on the lymphovascular invasion of the tumor.

Strumal Carcinoid of the Ovary: A Report of Two Cases

We describe two cases of strumal carcinoid of ovary, which is an extremely rare ovarian germ cell tumor composed of an intimate mixture of thyroid and carcinoid tissues. The first case involved a 63-year-old woman, who presented with a 1-month history of abdominal distension and unintentional weight loss (5 kg). Abdominal and chest computed tomography revealed right ovarian tumor and bilateral pleural effusion. Her thyroid-stimulating hormone level was slightly lower 0.475 μIU/mL (normal range: 0.55 - 4.78 μIU/mL);however, the levels of FT3 and FT4 were normal. Intraoperatively, the right ovary was enlarged (8.0 cm × 7.0 cm), with a smooth surface and intact capsule. Bilateral salpingo-oophorectomy with a total abdominal hysterectomy and an appendectomy was performed. The second case was a 54-year-old woman, in whom pelvic mass was recognized 2 months before. Intraoperatively, the left ovary was dumbbell-shaped (20.0 cm × 9.0 cm × 9.0 cm) and connected to two masses (9.0 cm × 8.0 cm and 8.0 cm × 7.0 cm, respectively). Her thyroid function test was normal (0.70 μIU/mL). Total abdominal hysterectomy, partial omentectomy, and pelvic adhesion lysis were performed. In both patients, intraoperative frozen section suggested strumal carcinoid of the ovary (stage IA). Ovarian strumal carcinoid should be included in a differential diagnosis of adnexal masses. The prognosis is considered good.
