Psycho-Social Hazards for Staff in geriatrics and geriatric psychiatry

Context: Exposure to burnout of staff involved with elderly patients is dependent on many factors either personal or linked to the professional environment. Social stress and systemic problems created particularly by difficulties inherent in the French hospital management system and the way people feel it, lead to a risk of burnout. One illustration of this is the rise in suicides at work. Quality of life at work, harassment and psycho-social risks are intimately linked. Affective factors, such as suffering for the medical carers in response to the distress of their patients aggravate the risk of burnout. Methods: We have evaluated these parameters using a self-filled questionnaire form sent to all staff and filled in by computer, anonymously, in 4 establishments, in December 2012 and over the first semester of 2013. After the three factors studied by the ProQOL scale of quality of life at work, to do with burnout, satisfaction compassion and fatigue compassion, 5 other questions were added, connected with a feeling of harassment and several social and demographic matters. Burnout risk was retained on reaching a threshold of 30 for this ProQOL scale item. Results: After multivariate analysis including the parameters of the Stamm scale, harassment and the socio-demographic factors studied, (age, sex, seniority, profession, and work departments) 4 factors are significantly associated with the risk of burnout, one negatively, compassion satisfaction, three positively, compassion fatigue, harassment experience and seniority. Conclusions: The risk of burnout is linked to subjective factors—the way quality of life at work is perceived and harassment experienced. Some professions, such as nurses, are particularly exposed and require these risk factors to be foreseen.

Pramipexole in Treatment Resistant Depression: A Case Review

Pramipexole is a dopamine agonist used in the treatment of Parkinson’s disease and Restless legs syndrome. Although off-label, the use of Pramipexole as an adjunct therapy in treatment resistant depression has recently been documented in the literature with promising results. We present a 75-year-old male with MDD who has failed trials of SSRIs, SNRIs, TCAs, SGA, TMS, Ketamine, and ECT who was initiated on Pramipexole. We discuss, based on existing literature, the probability of a favorable long-term response to Pramipexole and the potential side effects for our patient. We also highlight the need for future studies designed to test the efficacy of Pramipexole in geriatric patients with TRD.

Prevalence of cognitive dysfunction,psychological morbidity and abuse in the community-based elderly population in India

Background The elderly population in India is expected to grow enormously by 2050 owing to an increase in life ***-based data on the prevalence of psychological morbidity,abuse and cognitive dysfunction are *** To determine the prevalence of cognitive dysfunction,psychological morbidity and abuse in the elderly population in a resettlement colony from East Delhi,*** A cross-sectional study was conducted on the elderly population of a resettlement colony,in East Delhi,comprising 5 blocks and 12 subblocks with a total population of 65 *** study was carried out within2 months from April to August *** self-reported questionnaires were administered to detect cognitive dysfunction,psychological morbidity,social support and pattern of *** The mean age of the study subjects was70.29 years(53.8% were males,78.8% were married,86.9% were Hindus and 75% were living with children).A total of 106 subjects(66.6%) belonged to the lower socioeconomic strata with 120(75%) living with *** of the subjects(70.6%) studied below the primary level of *** detected on Goldberg General Health Questionnaire-12,56.9% of the subjects had psychological *** Dementia Assessment by Rapid Test,33.1%of subjects were screened *** pattern of abuse reported was as follows:emotional abuse(16.9%),physical abuse(7.5%),sexual abuse(1.9%) and social neglect(1 8.1%).The social support score was found to be46.22(12.22).Conclusion The results of this community-based study signify that appropriate steps at the policy level need to be undertaken so that abuse as well as neglect can be *** addition,screening the elderly population helps to detect early cognitive dysfunction,psychological morbidity,abuse victims and individuals with poor support.

Cognitive screening for adult psychiatric outpatients: Comparison of the Cognivue® to the Montreal Cognitive Assessment

In this editorial we comment on the article by Cahn-Hidalgo D published in a recent issue of the World Journal of psychiatry 2020;10(1);*** focus on the importance of utilizing psychometrically valid cognitive screening tools when assessing for cognitive decline in older adults in a psychiatric outpatient *** compared the use of Cognivue®to use of the montreal cognitive assessment(MoCA)as a cognitive screening tool.A total of 58 patients aged 55 and over participated in this comparison *** completed cognitive screening on Cognivue®,a new Food and Drug Administration-cleared computer screening device,and the *** results of patient performance using these two instruments were ***(28%)patients screened negative for cognitive impairment on both ***-two(72%)patients screened positive on one or both of the *** was 43%agreement between Cognivue®and the MoCA in identifying patients with cognitive impairment,and individual subtests were weakly *** MoCA was determined to be the preferred instrument due to its high sensitivity and specificity(100%and 87%,respectively)when screening for cognitive *** propose that the use of Cognivue®cognitive screening tool be closely reviewed until more research proves that the test meets the standards for reliability and *** is important for clinicians to remember that screeners should not be used to diagnosis patients with neurocognitive disorders;instead,they should be used to determine whether further evaluation is ***,misdiagnosing of neurocognitive disorders can pose unnecessary psychological and emotional harm to patients and their families and also lead to incorrect treatment and undue healthcare costs.
