Effect of electroacupuncture at different time points on the recovery of gastrointestinal function after surgery for gastrointestinal malignant neoplasms

Objective To observe the effect of electroacupuncture(EA)at different time points during the perioperative period on the recovery of gastrointestinal function after gastrointestinal malignant neoplasms *** Sixty-three patients who needed radical surgery for gastrointestinal tumors were randomized into a control group,treatment group 1(postoperative EA group),and treatment group 2(intraoperative and postoperative EA group).The control group received surgery and conventional Western medicine treatment,and treatment groups 1 and 2 received additional EA treatment at different time *** initial flatus time after the surgery,visual analog scale(VAS)score at different time points after the surgery,the proportion of using patient-controlled analgesia(PCA)after the surgery,and the times of adding analgesics were observed in the three *** The initial flatus time after the surgery was earlier in treatment groups 1 and 2 than in the control group(P0.05).The VAS score was lower in treatment group 2 than in the control group at 6,12,24,and 72 h after the surgery(P0.05).Regarding the times of adding analgesics,it was less in treatment group 2 than in the control group at 12 h after the surgery(Pgastrointestinal function in patients after radical resection of gastrointestinal *** EA during and after the surgery should be superior to postoperative EA regarding the analgesic effect after the surgery.

The effect of folic acid on the development of stomach and other gastrointestinal cancers

OBJECTIVE: To evaluate the roles of folic acid and beta-carotene in the chemoprevention of gastric and other gastrointestinal (GI) cancers. METHODS: In a randomized, double-blind, placebo-controlled trial, a total of 216 patients with atrophic gastritis were randomly assigned to one of the four groups: (1) folate (FA, 20 mg per day plus vitamin B(12) 1 mg, intramuscularly, per month for one year, then 20 mg two times a week plus 1 mg per three months for the next year); (2) natural beta-carotene (N-betaC, 30 mg per day for first year, then 30 mg two times a week for the next); (3) synthetic beta-carotene (S-betaC, administered as in N-betaC); and (4) placebo. Follow-ups continued from 1994 to 2001. RESULTS: A total of 7 new cases of gastrointestinal cancers were diagnosed with 3 stomach, 1 colon and 1 esophageal cancers occurring in the placebo group; 1 stomach cancer in both of the N-betaC and S-betaC groups, and no cancer occurring in FA group. In terms of GI cancers, there was a significant reduction in the FA group, compared with the placebo group (P = 0.04). A similar trend was observed in both N-betaC and S-betaC groups (P = 0.07 - 0.08). Taken together, the three intervention groups displayed a highly significant decrease in occurrence (P = 0.004, vs placebo), and a lower risk for GI cancers (OR = 0.12; 95% confidence interval, 0.03 - 0.51). For development of gastric cancer, any one of the three active-treated groups did not reach statistically significant reduction. The FA group showed obvious improvement of the gastric mucosal lesions with more patients displaying lesions reversed or stable atrophy and inflammation (P = 0.04), reversed intestinal metaplasia (P = 0.06) at the end of follow-up, and reversed displasia (P = 0.017) at 12 months. Two cases of false jaundice were found in beta-carotene groups with no influence on administration, and no side-effects were reported in FA group. CONCLUSIONS: This trial revealed the interventional effect of folic acid

Effects of cooperative nursing and patient education on postoperative infection and self-efficacy in gastrointestinal tumors

BACKGROUND gastrointestinal tumors have a high incidence *** application value of the cooperative nursing care system of medical care has received widespread attention in recent ***,there are few studies on the value of the joint application of collaborative nursing care and self-efficacy *** To explore the effect of cooperative nursing care management/self-efficacy education on postoperative infection and self-efficacy in gastrointestinal tumor surgery *** A total of 102 patients with gastrointestinal tumors treated in our hospital from October 2018 to February 2020 were selected and divided into a conventional group(n=51)and a combined group(n=51)according to the nursing *** routine group adopted routine nursing,and the joint group adopted the medical care cooperative responsibility system nursing management combined with selfefficacy *** self-efficacy scores,coping style scores,self-experience burden scores,and postoperative complication rates of the two groups before and after intervention were *** After intervention,the daily life behavior management,cognitive symptom management,and disease management scores of the two groups were higher than those before the intervention,and those of the combined group were higher than those of the conventional group(all P=0.000).After the intervention,the positive response scores of the two groups were higher than those before the intervention,the negative response scores were lower than those before the intervention,and the combined group was better than the conventional group(all P=0.000).After the intervention,the two groups’emotional,economic,and physical factor scores were lower than those before the intervention,and the combined group was lower than the conventional group(all P=0.000).The incidence of infection in the combined group(1.96%)was lower than that in the conventional group(15.69%)(P=0.036).CONCLUSION Cooperative nursing care management and self-

Surgical intervention for malignant bowel obstruction caused by gastrointestinal malignancies

BACKGROUND Malignant bowel obstruction(MBO)is a common event for end-stage gastrointestinal cancer *** studies had demonstrated manifestations and clinical management of MBO with mixed *** still lack reports of the surgical treatment of *** To analyze the short-term outcomes and prognosis of palliative surgery for MBO caused by gastrointestinal *** A retrospective chart review of 61 patients received palliative surgery between January 2016 to October 2018 was performed,of which 31 patients underwent massive debulking surgery(MDS)and 30 underwent ostomy/by-pass surgery(OBS).The 60-d symptom palliation rate,30-d morbidity and mortality,and overall survival rates were compared between the two *** The overall symptom palliation rate was 75.4%(46/61);patients in the MDS group had significantly higher symptom palliation rate than OBS group(90%vs 61.2%,P=0.016).Patients with colorectal cancer who were in the MDS group showed significantly higher symptom improvement rates compared to the OBS group(overall,76.4%;MDS,61.5%;OBS,92%;P=0.019).However,patients with gastric cancer did not show a significant difference in symptom palliation rate between the MDS and OBS groups(OBS,60%;MDS,80%;P=1.0).The median survival time in the MDS group was significantly longer than in the OBS group(10.9 mo vs 5.3 mo,P=0.05).CONCLUSION For patients with MBO caused by peritoneal metastatic colorectal cancer,MDS can improve symptom palliation rates and prolong survival,without increasing mortality and morbidity rates.

ANALYSIS OF THE N-GLYCOSYLATION OF THESERUM GLYCOPROTEINS DEFINED BY CON A,PHA-E, RCA1 AND WGA IN CHINESE PATIENTS WITH gastrointestinal DISEASES

Glycoproteins from tumor cells isolated by Con A, PHAE and RCAI, were coupled to horse radish peroxidase and then used as reporters to evaluate the oligosaccharides change on serum glycoproteins in digestive tract diseases and individual healthy persons. A value 2 standard deviation above the mean of the healthy person group was considered positive. The results indicated that the positive rates of the oligosaccharides bound with Con A, PHA-E and RCAI were 51 % (61/119) . 70. 6% (84/119) , and 76. 4% (91/119) in patients with gastric cancer: 53. 3% (16/30),46. 7% ( 14/30) and 66. 7% (20/30) in esophageal cancers; 28. 5% (15/49), 49. 0% (24/49) and .46. 9% (23/49) in colorectal cancer; 78. 1% (25/ 32) ,81. 3% (26/32) and 31. 3% (10/32) in gallbladder tumors; 60. 4% (29/48), 54. 1% (26/48) and 39. 5% (19/48) in hepatocellular cancer. The positive rates among the patients with benign diseases were also found to be 12. 7% (16/126), 15. 5% (20/126) and 26. 2% (33/126). However, the expression of the oligosaccharide moieties recognized by WGA on the glycoprotein bound with Con A, was significantly lower in the serum of patients with tumors than in those with benign disease and in healthy individuals (P<0. 01 , P<0. 01).
