Comparison of Color fundus photography, Infrared fundus photography, and Optical Coherence Tomography in Detecting Retinal Hamartoma in Patients with Tuberous Sclerosis Complex

Background： A sensitive method is required to detect retinal hamartomas in patients with tuberous sclerosis complex （TSC）. The aim of the present study was to compare the color fundus photography, infrared imaging （IFG）, and optical coherence tomography （OCT） in the detection rate of retinal hamartoma in patients with TSC. Methods： This study included 11 patients （22 eyes） with TSC, who underwent color fundus photography, IFG, and spectral-domain OCT to detect retinal hamartomas. TSC1 and TSC2 mutations were tested in eight patients. Results： The mean age of the 11 patients was 8.0 ± 2.1 years. The mean spherical equivalent was -0.55 ±1.42 D by autorefraction with cycloplegia. In 11 patients （22 eyes）, OCT, infrared fundus photography, and color fundus photography revealed 26, 18, and 9 hamartomas, respectively. The predominant hamartoma was type I （55.6%）. All the hamartomas that detected by color fundus photography or IFG can be detected by OCT. Conclusion： Among the methods of color fundus photography, IFG, and OCT, the OCT has higher detection rate for retinal hamartoma in TSC patients; therefore, OCT might be promising for the clinical diagnosis of TSC.

Retinal Vascular Morphological Changes in Patients with Extremely Severe Obstructive Sleep Apnea Syndrome

Background：Obstructive sleep apnea syndrome （OSAS） has been shown to generate hypertension and endothelial dysfunction.Retinal vessel is the only vessel that can be observed directly and noninvasively;retinal vascular abnormalities can serve as a predictive marker for the occurrence,clinical course,and prognosis of cardiovascular and cerebrovascular diseases.The objective of this study was to identify the effect of OSAS severity on the morphological changes of retinal vessels.Methods：Adult patients complained of snoring were included in this study.The patients＆#39; general information,polysomnography,and fundus photography parameters including central retinal artery equivalent （CRAE）,central retinal vein equivalent （CRVE）,and arteriole-to-venule ratio （AVR） were collected.Patients were divided into four groups according to their apnea-hypopnea index （AHI） results：Group Ⅰ,AHI ≤5/h;Group ll,5/h 〈 AHI ≤30/h;Group Ⅲll,30/h 〈 AHI ≤60/h;and Group Ⅳ,AHI 〉60/h.Results：A total of 133 patients were included in this study with 111 males （83.5%） and 22 females （16.5%）.Mean age was 41.6 ± 9.9 years,and the mean body mass index was 28.1 ± 4.0 kg/m2.AHI ranged between 0 and 130.8/h with a mean of 39.l ± 30.7/h.There were 24,34,35,and 40 patients in Group Ⅰ,Group Ⅱ,Group Ⅲ,and Group Ⅳ,respectively.Significant differences were found for AHI （F =388.368,P 〈 0.001）,minimal pulse oxygen saturation （F-91.902,P 〈 0.001）,and arousal index （F =31.014,P 〈 0.001） among four groups;no significant differences were found for CRAE （F=0.460,P =0.599） and CRVE （F =0.404,P =0.586） among groups;there were significant differences for AVR between Group l and Group Ⅳ （63.6 ± 5.1% vs.67.2 ± 5.5%,P =0.010） Group Ⅱ and Group Ⅳ （64.5 ± 6.0% vs.67.2 ± 5.5%,P =0.030）,and Group ll and Group Ⅳ （64.7 ± 4.1% vs.67.2 ± 5.5%,P=0.043）.A main group-by-AHI effect was found on the AVR：patients with higher AHI showed higher AVR results （r =0.225,P =0.009）.Multivariate logistic regression analysis was used for multi-variable factors.A group-by-age effect was found on

AIM:To investigate the association of axial length(AL)and ocular factors on AL elongation.METHODS:A retrospective chart review of patients who underwent two or more AL examinations for more than two years.Totally 4 groups were divided according to initial AL(fundus photograph was used to find risk factors associated AL elongation.RESULTS:The mean age of the patients was 47.21±7.79 y.AL remained almost unchanged in the groups with ALfundus photography.

Evaluation of traumatic retinopathy with ultra-wide field imaging under corneal scar or fixed small pupil

AIM： To evaluate the value of ultra-wide field（UWF） imaging in the management of traumatic retinopathy under the condition of corneal scar or fixed small pupil after complicated ocular trauma. METHODS： Twenty-eight patients（28 eyes） with complicated ocular trauma were enrolled in the study from June 2016 to May 2017, including 19 males and 9 females with age ranged from 11 to 64（43.42±12.62）y. All patients were treated with secondary vitrectomy after emergency operation for wound repair of open ocular trauma. Direct ophthalmoscopy and 45-degree fundus photography were taken at each time point of follow up for comparison of findings with UWF images. Routine eye examination including visual acuity, intraocular pressure, slit lamp examination were performed and analyzed as well.RESULTS： Among the 28 traumatized eyes, the positive rate for identification of traumatic retinopathed was 32.1%（9 cases）, 14.9%（5 cases）, and 85.7%（24 cases） with direct ophthalmoscopy, 45-degree fundus photography, and UWF imaging, respectively. The detective rate of UWF imaging under the condition of corneal scar or fixed small pupil was statistically greater than that of 45-degree fundus photography and direct ophthalmoscopy（Bonferroni correction, P〈0.001）. UWF image was obtained in 19 eyes with opaque corneal scar, otherwise their fundus could not be seen by conventional methods. The additional findings of traumatic retinopathies by UWF imaging included periretinal membranes or pre-retinal proliferating strip, retinal holes, hemorrhage in the vitreous or sub-retinal space.CONCLUSION： UWF imaging is superior to traditional fundus photography in the evaluation of traumatic retinopathies under the condition of corneal scar or fixed small pupil after complicated ocular trauma.

A fundus Photographic Method with Narrow Spectral Band Luminance
