BACKGROUND： Previous studies have analyzed cerebral activation and lateralization of cognitive functions, as well as cerebellar function with reference to high-level cognitive processing. However, there has been very little research on systematization and diversification. In particular, there are no reports on cerebellar lateralization, although reliable results have been reported on cerebral lateralization. OBJECTIVE： This study analyzed cerebellar activation and lateralization in relation to verbal and visuospatial tasks using functional magnetic resonance imaging （fMRI）. DESIGN, TIME AND SETTING： A block design for fMRI observation was performed at the fMRI Laboratory, Brain Science Research Center, Korea Advanced Institute of Science and Technology from May 2006 to September 2008. PARTICIPANTS： Sixteen healthy, male, college students, aged （23.3 ± 0.5） years, and 16 healthy, male, college students, aged （21.5 ± 2.3） years, participated in the study, respectively. METHODS： Verbal and visuospatial tasks were presented while functional brain images were acquired using a 3T fMRI system. Verbal analogy testing required the subject to select the word with the same relationship as one of the given words. Verbal antonym testing required the subject to select the word with a different meaning among four words. Visuospatial tasks involved selecting a shape that corresponded to a given figure with four examples, as well as selecting a development figure of a diagram. MAIN OUTCOME MEASURES： Changes in cerebellar activation and lateralization between two cognition tasks. RESULTS： Bilateral hemisphere Iobules VI and IX, right hemisphere Iobule VIII, bilateral hemisphere Iobules Crus I, and vermis Iobule IV, V, and Vl were closely related to verbal tasks in comparison to visuospatial tasks. Conversely, bilateral hemisphere Iobules IV and V, as well as the right hemisphere Iobule VI, were closely related to visuospatial tasks compared to verbal tasks. There was no great differe

functional magnetic resonance imaging evaluation of brain function reorganization in cerebral stroke patients after constraint-induced movement therapy

In this study, stroke patients received constraint-induced movement therapy for 3 weeks. Before and after constraint-induced movement therapy, the flexibility of their upper limbs on the affected side was assessed using the Wolf motor function test, and daily use of their affected limbs was assessed using the movement activities log, and cerebral functional reorganization was assessed by functional magnetic resonance imaging. The Wolf motor function test score and the movement activities log quantity and quality scores were significantly increased, while action performance time in the Wolf motor function test was significantly decreased after constraint-induced movement therapy. By functional magnetic resonance imaging examination, only scattered activation points were visible on the affected side before therapy. In contrast, the volume of the activated area was increased after therapy. The activation volume in the sensorimotor area was significantly different before and after therapy, and the activation area increased and appeared adjusted. In addition to the activated area around the lesions being decreased, there were also some new activated areas, including the supplementary movement area, premotor area and the ipsilateral sensorimotor area. Our findings indicate that constraint-induced movement therapy significantly improves the movement ability and daily use of the affected upper limbs in stroke patients and promotes cerebral functional reorganization.

AIM: To test the hypotheses that diffusion weighed (DW)and transcatheter intraarterial perfusion (TRIP)magnetic resonance imaging (MRI) can each be used to assess regional differences in tumor function in an animal pancreatic cancer ***: VX2 tumors were implanted in pancreata of 6 rabbits. MRI and digital subtraction angiography (DSA) were performed 3 wk following implantation. With a 2-French catheter secured in the rabbit's gastroduodenal artery, each rabbit was transferred to an adjacent 1.5T MRI scanner. DWand TRIP-MRI were performed to determine if necrotic tumor core could be differentiated from viable tumor periphery. For each, we compared mean differences between tumor core/ periphery using a 2-tailed paired t-test (α = 0.05). imaging was correlated with histopathology. RESULTS: Tumors were successfully grown in all rabbits, confirmed by necropsy. On DW-MRI, mean apparent diffusion coeffi cient (ADC) value was higher in necrotic tumor core (2.1 ± 0.3 mm2/s) than in viable tumor periphery (1.4 ± 0.5 mm2/s) (P functional MRI can be used to differentiate necrotic from viable tumor cells in an animal pancreatic cancer model using ADC (DW-MRI) and perfusion (TRIP-MRI) values.

Regional homogeneity analysis of low-frequency blood oxygenation level-dependent signals from neighboring voxels enables the analysis of local neuronal synchrony. Both structural magnetic resonance imaging and resting-state functional magnetic resonance imaging scans were collected from nine Parkinson＇s disease patients with right resting tremor, and from eight age-matched normal controls. Regional homogeneity was compared between Parkinson＇s disease patients and controls. The results revealed that regional homogeneity was increased in several brain regions, including the right precuneus, right superior parietal gyrus, left anterior cingulate cortex, right middle frontal gyrus and right inferior frontal gyrus. Conversely, regional homogeneity was decreased in the cerebellar vermis in Parkinson＇s disease patients compared with healthy controls.

The aim of this study was to examine the secondary visual cortex functional disorder in patients with glaucoma and large pituitary adenoma by functional magnetic resonance imaging, and to determine the correlation between visual field defect and primary visual cortex activation. Results showed that single eye stimulation resulted in bilateral visual cortex activation in patients with glaucoma or large pituitary adenoma. Compared with the normal control group, the extent and intensity of visual cortex activation was decreased after left and right eye stimulation, and functional magnetic resonance imaging revealed a correlation between visual field defects and visual cortex activation in patients with glaucoma and large pituitary adenoma. These functional magnetic resonance imaging data suggest that anterior optic pathway lesions can cause secondary functional disorder of the visual cortex, and that visual defects are correlated with visual cortex activation.

The anatomical organization of the corticopontocerebellar tract （CPCT） in the human brain remains poorly understood. The present study investigated probabilistic tractography of the CPCT in the human brain using diffusion tensor tractography with functional magnetic resonance imaging. CPCT data was obtained from 14 healthy subjects. CPCT images were obtained from functional magnetic resonance imaging and diffusion tensor tractography, revealing that the CPCT originated from the primary sensorimotor cortex and descended to the pontine nucleus through the corona radiata, the posterior limb of the internal capsule, and the cerebral peduncle. After crossing the pons through the transverse pontine fibers, the CPCT entered the cerebellum via the middle cerebral peduncle. However, some variation was detected in the midbrain （middle cerebral peduncle and/or medial lemniscus） and pons （ventral and/or dorsal transverse pontine fibers）. The CPCT was analyzed in 3 dimensions from the cerebral cortex to the cerebellum. These results could be informative for future studies of motor control in the human brain.

Background Writer＇s cramp is a type of task specific idiopathic focal dystonia and has an incompletely understood pathophysiology. The present study utilized functional magnetic resonance imaging （fMRI） to investigate what type of brain activity correlates with writer＇s cramp and its physiological *** Ten patients with writer＇s cramp were age and gender matched with ten nealthy control suojects in a block design. Subjectswere scanned by fMRI while performing three consecutive, visually instructive, tasks with MR Vision 2000：（1） suppositional writing, （2） writing with finger and （3） writing with a pencil. Data was analysed using AFNI software for groups of patients and *** The patients with writer＇s cramp showed significant activations of contralateral basal ganglion （especially the putamen）, motor cortex （primary sensorimotor cortex, supplementary motor cortex, premotor cortex） and ipsilateral cerebellar hemisphere in writing with a pencil compared with controls; whereas there was no obvious difference between patients and controls during writing with finger. Furthermore, these differences exist in the stibtractive activated maps for ＂writing with a pencil＂ minus ＂writing with finger＂ of patients, when the activation of subcortical area and insula in controls *** Abnormal activations of contralateral basal ganglion, motor cortex and ipsilateral cerebellar hemisphere of the patients with writer＇s cramp suggest dysfunction of basal ganglion and subcortical-cortical loop might play a pathophysiological role in writer＇s cramp.

Background The hypothalamus plays a central role in the regulation of metabolism by sensing metabolic demands and releasing regulatory neurotransmitters. This study investigated the response of the hypothalamus to glucose ingestion in rats by blood oxygen level-dependent functional magnetic resonance imaging （BOLD-fMRI） and immunohistochemical techniques to determine the role of the hypothalamus in glycoregulation during disturbances in carbohydrate metabolism. Methods The signal intensity of the hypothalamus was monitored by fMRI for 60 minutes after oral glucose intake in 48 healthy rats （age 14 months）, which included 24 normal weight rats （weighing （365±76.5） g） and 24 overweight rats （weighing （714±83.5） g）. Then, 12 rats （6 normal, 6 overweight） underwent a repeat fMRI scan after consuming an equivalent amount of water without glucose on a separate day. The procedure for fMRI with water intake was the same as for glucose ingestion, fMRI data was processed using time cluster analysis and intensity averaging method. After fMRI the expression of neuropeptide Y （NPY） and 5-hydroxytryptamine （5-HT） in the hypothalamus of all rats was determined by immunohistochemistry. Positive cells for NPY or 5-HT were counted. Results There was a transient, but significant, decrease in fMRI signal intensity in all rats （mean （3.12±0.78）%） in the hypothalamus within 19.5-25.5 minutes of oral glucose ingestion. In overweight rats, the decrease in signal intensity in response to the glucose ingestion was more markedly attenuated than that observed in normal weight rats （（2.2±1.5）% vs （4.2±0.7）% inhibition, t=2.12, P〈0.05）. There was no significant response in the hypothalamus after oral water ingestion. The percentage of NPY positive cells in obese rats were slightly lower than those in control group （21% vs 23% t=0.71, P〉0.05）; but there was no significant difference between the two groups; the percentage of 5-HT positive cells in obese rats were significantly lower than those

Angioplasty and stenting for severe vertebral artery orifice stenosis: effects on cerebellar function remodeling verified by blood oxygen level-dependent functional magnetic resonance imaging

Vertebral artery orifice stenting may improve blood supply of the posterior circulation of the brain to regions such as the cerebellum and brainstem. However, previous studies have mainly focused on recovery of cerebral blood flow and perfusion in the posterior circulation after interventional therapy. This study examined the effects of functional recovery of local brain tissue on cerebellar function remodeling using blood oxygen level-dependent functional magnetic reso- nance imaging before and after interventional therapy. A total of 40 Chinese patients with severe unilateral vertebral artery orifice stenosis were enrolled in this study. Patients were equally and randomly assigned to intervention and control groups. The control group received drug treat- ment only. The intervention group received vertebral artery orifice angioplasty and stenting ＋ identical drug treatment to the control group. At 13 days after treatment, the Dizziness Handicap Inventory score was compared between the intervention and control groups. Cerebellar function remodeling was observed between the two groups using blood oxygen level-dependent functional magnetic resonance imaging. The improvement in dizziness handicap and cerebellar function was more obvious in the intervention group than in the control group. Interventional therapy for severe vertebral artery orifice stenosis may effectively promote cerebellar function remodeling and exert neuroprotective effects.
