Comparison of QT Correction Methods in the Pediatric Population of a Community Hospital: A Retrospective Study

Objective:Accurate measurement of QT interval,the ventricular action potential from depolarization to repolarization,is important for the early detection of Long QT syndrome.The most effective QT correction(QTc)formula has yet to be determined in the pediatric population,although it has intrinsically greater extremes in heart rate(HR)and is more susceptible to errors in measurement.The authors of this study compare six dif-ferent QTc methods(Bazett,Fridericia,framingham,Hodges,Rautaharju,and a computer algorithm utilizing the Bazett formula)for consistency against variations in HR and RR interval.Methods:Descriptive Retrospective Study.We included participants from a pediatric cardiology practice of a community hospital who had an ECG performed in 2017.All participants were healthy patients with no past medical history and no regular med-ications.Results:ECGs from 95 participants from one month to 21 years of age(mean 9.7 years)were included with a mean HR of 91 beats per minute(bpm).The two-sample paired t-test or Wilcoxon signed-rank test assessed for any difference between QTc methods.A statistically signiﬁcant difference was observed between every combination of two QTc formulae.The Spearman’s rank correlation analysis explored the QTc/HR and QTc/RR relationships for each formula.Fridericia method was most independent of HR and RR with the lowest absolute value of correlation coefﬁcients.Bazett and Computer had moderate correlations,while framingham and Rautaharju exhibited strong correlations.Correlations were positive for Bazett and Computer,reﬂecting results from prior studies demonstrating an over-correction of Bazett at higher HRs.In the linear QTc/HR regression analysis,Bazett had the slope closest to zero,although Computer,Hodges,and Fridericia had comparable values.Alternatively,Fridericia had the linear QTc/RR regression coefﬁcient closest to zero.The Bland-Altman method assessed for bias and the limits of agreement between correction formulae.Bazett and Computer exhibited good agreement with minimal

目的基于framingham评分对安徽省老年人群脑卒中发病风险进行评估,为开展脑卒中精准预防提供科学依据。方法于2019年7—8月采用多阶段分层法从安徽省4个城市抽取60~84岁共5254名老年人作为调查对象,根据framingham风险评估模型评估脑卒中10年发病风险。用SPSS 22.0软件进行χ^(2)检验、t检验,采用多因素logistic回归分析老年人脑卒中患病风险。结果老年人脑卒中总体发病风险中位数为9%(P_(25)~P_(75):5%~13%),男性发病风险中位数为11%(P_(25)~P_(75):7%~17%),女性发病风险中位数为8%(P_(25)~P_(75):4%~11%)。女性服降压药者的收缩压水平[(141.14±20.08)mm Hg]高于男性[(138.48±17.30)mm Hg],男性吸烟率(41.66%)明显高于女性(4.79%),差异均有统计学意义(P<0.01)。多因素logistic回归分析结果显示,女性(OR=0.242,95%CI:0.211~0.278)、居住在阜阳(OR=0.757,95%CI:0.636~0.902)、居住在六安(OR=0.824,95%CI:0.697~0.973)、居住在宣城(OR=0.819,95%CI:0.687~0.977)、年收入6500元及以上(OR=0.857,95%CI:0.742~0.990)的老年人脑卒中发病风险较低,而丧偶(OR=1.679,95%CI:1.096~2.572)和独居(OR=1.275,95%CI:1.044~1.558)的老年人脑卒中发病风险更高。结论脑卒中防治工作是一项综合性的系统工程,依据不同人群制定相应的防控策略,可以使脑卒中预防更具针对性。

目的:基于framingham风险(FRS)评分预测强直性脊柱炎(AS)病人未来10年心血管疾病的患病风险程度,进一步探讨AS病人并发心血管疾病的风险和可能的危险因素。方法:选取2021年1月-2021年12月河北中石油中心医院风湿免疫科收治的AS病人120例作为观察对象,收集病人基本信息资料及体征检查结果、实验室指标、影像学指标,应用FRS评分评估心血管疾病风险,依据患病风险程度将病人分为3组:低危组(FRS评分20%),应用单因素和多因素Logistic回归分析AS病人并发心血管疾病风险高的危险因素。结果:120例AS病人低危、中危、高危心血管疾病风险分别为53.33%、29.17%、17.50%;3组间吸烟比例、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)、25羟基维生素D[25(OH)D]、颈动脉内膜中层厚度(IMT)比较,差异有统计学意义(P<0.05);将单因素分析中显著性因素与AS病人心血管疾病发生风险行多因素Logistic回归分析结果显示,吸烟、LDL-C、CRP、25(OH)D是AS病人心血管疾病风险升高的影响因素(P<0.05)。结论:AS病人并发心血管疾病风险升高与吸烟、LDL-C、CRP、25(OH)D有关。

Assessing 10-year coronary heart disease risk in people with Type 2 diabetes mellitus: framingham versus United Kingdom Prospective Diabetes Study

Aims: Previous studies have suggested that the framingham coronary heart disease risk prediction equation underestimates risk among people with Type 2 diabetes. We compared the 10-year absolute risks of coronary heart disease (CHD) using a framingham equation and a United Kingdom Prospective Diabetes Study (UKPDS) equation in adults with Type 2 diabetes. Methods: Participants were from a cross-sectional survey of a randomly selected population. There were 461 people with newly (n = 132) or previously diagnosed (n = 329) diabetes aged 35 to 74 years with no past history of cardiovascular disease or nephropathy. We examined predicted 10-year CHD risk by age, gender, and newly or previously diagnosed diabetes. Results: Overall the mean 10-year CHD risks predicted by the two equations were similar. Among men, the UKPDS and framingham scores were almost identical below 60 years of age but at older ages, the UKPDS score was 4% - 11% higher than framingham. For women, the framingham score was higher than the UKPDS score between ages 40 and 65 years, but the UKPDS score was about 4% - 5% higher for women aged 70 years and over. The UKPDS equation tended to give higher risk estimates in people with a predicted 10-year framingham CHD risk above 15%. Conclusion: framingham CHD risk scores tended to be lower than UKPDS scores primarily in people above standard thresholds for drug treatment, so the clinical impact of underestimating risk is likely to be limited. Moreover, the UKPDS equation predicted lower risks than framingham for women and newly diagnosed diabetes at otherwise low to moderate CHD risk, which could result in later initiation of therapy in these groups if the UKPDS score was used instead of the framingham score.

Association between Coronary Atherosclerotic Plaque Composition and Cardiovascular Disease Risk

Objective The objective of this study is to determine whether coronary atherosclerotic plaque composition is associated with cardiovascular disease(CVD) risk in Chinese adults. Methods We performed a cross-sectional analysis in 549 subjects without previous diagnosis or clinical symptoms of CVD in a community cohort of middle-aged Chinese adults. The participants underwent coronary computed tomography(CT) angiography for the evaluation of the presence and composition of coronary plaques. CVD risk was evaluated by the framingham risk score(FRS) and the 10-year atherosclerotic cardiovascular disease(ASCVD) risk score. Results Among the 549 participants, 267(48.6%) had no coronary plaques, 201(36.6%) had noncalcified coronary plaques, and 81(14.8%) had calcified or mixed coronary plaques. The measures of CVD risk including FRS and ASCVD risk score and the likelihood of having elevated FRS significantly increased across the groups of participants without coronary plaques, with noncalcified coronary plaques, and with calcified or mixed coronary plaques. However, only calcified or mixed coronary plaques were significantly associated with an elevated ASCVD risk score [odds ratio(OR) 2.41; 95% confidence interval(CI) 1.09-5.32] compared with no coronary plaques, whereas no significant association was found for noncalcified coronary plaques and elevated ASCVD risk score(OR 1.25; 95% CI 0.71-2.21) after multivariable adjustment. Conclusion Calcified or mixed coronary plaques might be more associated with an elevated likelihood of having CVD than noncalcified coronary plaques.

Objectives We sought to determine the clinical factors and heritability associ ated with inflammation measured as circulating levels of soluble-intercellular adhesion molecule-1 (sICAM-1) in a community-based cohort. Background Several prospective studies indicate that circulating sICAM-1 is predictive of future cardiovascular events. However, in some studies this predictive value is lost af ter multivariable adjustment for traditional cardiovascular disease (CVD) risk f actors. We addressed the heritability of sICAM-1 and its relation to CVD risk f actors in a community-based cohort. Methods We examined 3,295 subjects from the framingham Heart Study and measured sICAM-1 levels. We then used linear and st epwise multivariable regression to determine predictors or sICAM-1 levels. Resu lts In age-and gender-adjusted regression models, increased sICAM-1 levels were positively associated with age, total/highdensity lipopro tein cholesterol, systolic blood pressure, body mass index (BMI), blood glucose, diabetes, smoking, and prevalent CVD. In stepwise multivariable regression mode ls, sICAM-1 levels remained associated with age, female gender, total/high-den sity lipoprotein cholesterol ratio, BMI, blood glucose, smoking, and prevalent C VD. The residual heritability of sICAM-1 was 24%. Conclusions In addition to p revalent CVD, established CVD risk factors and non-traditional ones such as BMI were associated with systemic inflammation as determined by sICAM-1 levels. Th ere also is significant heritability of sICAM-1, which suggests a genetic compo nent to systemic inflammation.
