Locking plates in distal humerus fractures： study of 43 patients

Objective： The treatment of multi- fragmentary, intraarticular fractures of the distal humerus is difficult, even in young patients with bone of good quality. Small distal fragment, diminished bone mineral quality and increased trauma-associated joint destruction make stable joint reconstruction more problematic. The anatomically preshaped locking plates allow angular stable fixation for these complex fractures. We evaluated functional results of patients treated with open reduction and internal fixation with distal humerus locking plates for complex distal hu- merus fractures. Methods： Forty-three consecutive patients with ar- ticular fractures of the distal humerus were treated by open reduction and internal fixation with AO distal humerus plate system and locking reconstruction plates. Forty patients were available for the final outcome analysis. According to AO/ASIF classification, there were 2 cases of type A2, 4 cases of type A3, 1 case of type B1, 1 case of type B2, 14 cases of type C1, 7 cases of type C2 and 11 cases of type C3. Open reduction with triceps splitting technique was used in all patients. The clinical and radiographic follow-up was performed and outcome measures included pain assessment,range of motion, and Mayo elbow performance score. Results： Forty patients were available for the final outcome analysis. There were 29 males and 11 females with an average age of 38.4 years （18-73 years）. Clinical and ra- diological consolidation of the fracture was observed in all cases at an average of 11.6 weeks （9-14 weeks）. The average follow-up was 12 months （10-18 months）. Using the Mayo elbow performance score the results obtained were graded as excellent or good results in 33 patients （82.5%）. One pa- tient had superficial infection, and 4 had myositis ossificans. QualiW of life following total hip arthroplasW in patients with acetabular fractures, previously managed by open reduction and internal fixation

Purpose： Total hip replacement （THR） is one of the most successful and cost-effective surgical procedures and remains the treatment of choice for long-term pain relief and restoration of function for patients with diseased or damaged hips. Acetabular fractures managed either conservatively or operatively by fixation tend to present later with secondary joint changes that require THR. In this study we evaluated the functional outcome and quality of life achieved by such patients. Methods： Our study was carried out as a retrospective trial by recruiting patients who underwent THR from June 2006 to May 2012. A total of 32 patients were included with a mean age of 46.08 years ranging from （25-65） years. We evaluated the quality of life in the patients using scoring techniques of Short Musculoskeletal Functional Assessment （SMFA） and the 12-1tem Short Form Health Survey （SF-12）. Functional outcome was assessed using Harris Hip Score （HHS）. Results： The mean HHS of the patients was 84.3 with a range from 56 to 100. The SMFA averaged 13.3. The SF-12 score averaged 49.1. The correlation of the HHS with SF-12 was positive （p - 0.001 ） while with SMFA there was a negative correlation （p - 0.001 ）. Conclusion： From this study it is inferred that the functional outcome of THR and quality of life in pa- tients who had acetabular fractures and were initially managed by open reduction and internal fixation is good.

Biomechanical evaluation of vertebroplasty using calcium sulfate cement for thoracolumbar burst fractures

Objective： To evaluate the biomechanical performance of vertebroplasty using calcium sulfate cement for thoracolumbar burst fractures. Methods： Sixteen bovine thoracolumbar spines （TH- L1 ） were divided into 4 groups （ A, B, C and D）. After burst-fracture model was created, 12 vertebral bodies in Groups A, B and C were augmented with calcium sulfate cement （ CSC ）, calcium phosphate cement （ CPC ） and polymethylmethacrylate （ PMMA ） bone cement, respectively. Each anterior vertebral body height was measured with a caliper at 4 time points： intact conditions （Hint）, post-fracture （ HFr）, post-reduction （HRe） and post-vertebroplasty （HVP）. The f＇dling volume of 3 different bone cements was also measured. Each vertebral body was compressed at 0. 5 mm/s using a hinged plating system on a materials testing machine to 50 % of the postvertebroplasty height to determine strength and stiffness. Difference was checked using t test or One-way ANOVA. Results： The average strike energy was 66. 2 J. Vertebroplasty with different cements could sustain vertebral height. The average f＇dling volume of bone cement in 3 groups was 4.35 ml （ CSC ）, 3.72 nd（ CPC ） and 3.95 ml （PMMA）, respectively, and there was no statistically significant difference among them （ P 〉0.05 ）. Vertebroplasty with PMMA completely restored strength （116%） and stiffness （105%）. CSC or CPC partly recovered vertebral strength and stiffness. However, greater strength restoration was got with CSC （ 1 659 N） as compared with CPC （1 011N, P〈0.01 ）. Regarding stiffness, differences between CSC （ 140 N/ram ±40 N/nan） and the other two bone cements （ CPC ：148 N/nun ±33 N/nun, PMMA.236 N/mm ＋97/N/mm） were not significant （P〉0.05）. Conclusions： For a burst-fracture of calf spine, use of CSC for vertebroplasty yields similar vertebral stiffness as compared with PMMA or CPC. Although augmentation with CSC partly obtains the normal strength, this treatment still can be applied in thocacoloumbar burst fractures with

Percutaneous pedicle screw fixation through the pedicle of fractured vertebra in the treatment of type A thoracolumbar fractures using Sextant system： an analysis of 38 cases

Objective： To prospectively evaluate the feasibility, safety and efficacy of the percutaneous pedicle screw fixation through the pedicle of fractured vertebra in the treatment of type A thoracolumbar fractures using Sextant system in the retrospective non-randomized case-control study. Methods： Atotal of 38 consecutive non-randomized patients with type A thoracolumbar fractures, which had been stabilized posteriorly from December 2006 to March 2009, were examined retrospectively more than 9 months after surgery. Twenty-one patients had been treated conventionally with open pedicle screw fixation （OPSF） and 17 patients received minimally invasive treatment with Sextant percutaneous pedicle screw fixation （SPPSF）. As a method of evaluation, the incision size, the intraoperation and post- operative volume of blood loss, operation time, postoperative hospital stay, blood transfusion, the radiological assessment of the sagittal Cobb＇s angle, vertebral body angle and vertebral body height were recorded and compared. Results： All patients were followed up for 8-24 months （average 11.6 months）. There were significant differences in the incision size, surgical blood loss, surgical draining Joss, operation time, hospital stay after operation, blood transfusion, the proportion of antalgic supplement and postoperative incisional VAS between the two groups （P〈0.05）. Mean preoperative kyphotic deformity was 16.0° and improved by 9.3° after surgery in OPSF group, but 15.2° and 10.3° respectively in SPPSF group. Mean preoperative angle of the fractured vertebral body was 15.9°and improved by 7.9° after surgery in OPSF group, but 14.9° and 6.6° respectively in SPPSF group. Mean anterior vertebral body height （% of normal） was 67.3% before surgery and 95.8% after surgery, but 69.1% and 90.1% respectively in SPPSF group. Treatment of type III middle phalangeal neck fractures through a palmar approach： a case report

Phalangeal neck fractures occur almost exclusively in children. We present the case of a 49 years old man with a dislocated fracture of the neck of the middle phalanx with the distal fragment rotated at 180~, due to a traumatic circular saw injury to the left index, which was solved by anatomical reduction and bone fixation with two 1.5 mm Synthes screws and a temporary transarticular K- wire at the distal interphalangeal joint. Zone I flexor digitorum profundus repair was performed using a modification of the Kessler 4-strands core suture and a full-thickness skin graft from the hypothenar eminence was taken to cover the skin gap. At 6-month follow-up the patient was pain-free and with a total active movement equivalent to 190~. No radio-logical signs of avascular necrosis of the head of the middle phalanx or nonunion of the distal fragment was detectable with recovery to the previous manual work. Owing to the position of the phalangeal head maintained in position by the collateral ligaments an anatomic reduction from dorsal approach is difficult to be performed and a longitudinal trac- tion can render the reduction harder too. The volar approach permits an easier reduction of the fracture through a derotation of the distal fragment facing palmar.

Combined anterior and posterior surgery for treatment of cervical fracture-dislocation in patients with ankylosing spondylitis

Objective： To discuss the pathological characteristics of cervical spinal fracture complicating ankylosing spondylitis （AS） and the effect of combined anterior and posterior operation. Methods： Eighteen AS patients with traumatic cervical fracture-dislocation were treated operatively from January 2000 to January 2006. The symptom duration of AS was 14.5 years on average. Three cases had undergone osteotomy in lumbar spine. There were 4 cases of Grade A, 3 cases of Grade B, 9 cases of Grade C and 2 cases of Grade D according to Frankel＇s score. There were 15 cases of Grade III dislocation and 3 cases of Grade II. All patients underwent surgical procedures by combined anterior and posterior approach. Results： There were 4 anterior-posterior procedures, 8 anterior-posterior-anterior procedures and 6 posterior-anterior procedures. Seven patients had one stage operation and 11 cases underwent two stage surgeries. There was certain extent of neurological improvement in 14 incomplete paraplegic patients, but no improvement in 4 complete paraplegic patients. The follow-up period was 21.2 months on average and the time for bone fusion was 3.6 months. There were 4 complications during operation and a longterm complication in follow-up. Conclusions： The study suggests that anterior combined with posterior approach makes the spine stable and relieves the pressure immediately. It is a reasonable surgical strategy for treatment of cervical spinal fracture-dislocation with AS.

A severely displaced metaphyseal fracture of the proximal humerus with dislocation of the shoulder in a child

Fracture of the proximal humerus meta- physis with coexistent dislocation of the shoulder in chil- dren is a rare injury. The injury often occurs as a conse- quence of high velocity trauma. Most fractures of the proxi- mal humerus commonly associated with the epiphysis in children can be treated with closed reduction. We presented a case of 5-year-old girl who sustained this type of fracture-dislocation of the shoulder. Open reduction and internal fixation with multiple smooth K-wires was performed. At two years follow-up, the patient was pain free and regained full range of motion.

Objective： The treatment of fronto- basilar fractures is a demanding aspect of craniofacial frac- ture management. A sequel of inadequate or improper frac- ture management presents cosmetic and functional prob- lems which are very difficult to correct. The aim of this manu- script was to examine a group of growing patients treated for frontobasilar fractures and provide clinicians a possible therapeutic option for the treatment of these challenging fractures. Methods： In this investigation, 12 patients under the age of 16 years treated for severe injuries to the frontobasilar region were included. Their records were reviewed to evalu- ate the clinical diagnosis, preoperative findings, hospital course, postoperative results, and long-term follow-up. Preoperative and postoperative CT scans were performed in all children treated. Postoperative complications were re- viewed in detail. Surgical procedures were evaluated for type and location of fixation. All complications and treat- ments were recorded. Results： Three of the patients presented with a cere-brospinal fluid （CSF） leak and required pericrartial flaps. Only 3 patients underwent removal of plates and screws due to palpability in 1 patient, loose hardware in 1 patient, and limited disturbance on growth in the rest patient. Seven patients underwent open reduction and internal fixation with resorbable plates, 4 patients with titanium plates, and 1 pa- tient with a combination. Follow-up ranged from 6 months to 5 years. No patients with a preoperative CSF leak deve- loped any recurrence of the leak. Conelusion： According to basic craniofacial principles, reducing and stabilizing the fractures should reconstruct the anterior cranial base. However, the treatment ofpaediatric maxillofacial trauma requires the evaluation of several factors. The facial skeleton of a child is constantly evolving and its growth depends on the balance of basal bone and soft tissues.

Predictors of early outcome in unstable pelvic fractures

Objective： To define the preoperative and intraoperative variables which may affect the immediate postoperative outcome in surgically managed patients with unstable pelvic fractures. Methods： This study was performed prospectively from January 2009 to June 2011 on 36 consecutive patients admitted to the trauma ward of Postgraduate Institute of Medical Education and Research, Chandigarh, with unstable pelvic injuries. Results： In the present study of 36 patients, 29 were managed surgically. Surgical duration was 2 hours in pa- tients operated on within 1 week and 3.4 hours in those operated on after 1 week. The blood loss was 550 ml when surgery was done after a week, but when done within a week it was 350 ml. The average blood loss through Pfanenstial approach was 360 ml, through posterior approach was 408 ml and through combined approach was 660 ml which was significantly high. Conclusion： Anterior approach to the pelvis would cause significantly more amount of blood loss than poste- rior approach and extemal fixation. Surgical approaches do not have any influence on the surgical duration or the infec- tion rate. The blood loss significantly increases when the surgical time is more than 1 h. The infection rate is not influ- enced by the duration of surgery. Presence or absence of associated injuries to the head, chest or abdomen is the main determinants of patient＇s survival and it greatly influ- ences the duration of hospital stay.

Clinical observation of auricular acupoint therapy for pain in early-stage extremity trauma

Objective： To observe the efficacy of auricular acupoint sticking based on conventional treatment in treating pain in early-stage extremity trauma. Methods： A hundred eligible patients with acute soft tissue injury or acute closed fracture were randomized into an observation group and a control group by their admission sequence, 50 cases in each group. The two groups both received routine management including routine checking, external fixing, traction, raising up the affected limb, etc., as well as cold compress with Chinese medication （Xiao Zhong Zhi Tong Powder）. In addition to the routine management, the control group was given oral administration of amidopyrine and phenacetin compound tablet, I tablet per dose, twice a day, which was then taken only when necessary or terminated after pain subsided. The observation group was given auricular acupoint sticking in addition to the routine management. The two groups were compared in terms of numerical rating scale （NRS） score, therapeutic efficacy and adverse reactions after pain was relieved. Results： After the intervention, the NRS scores dropped significantly in both groups （P〈0.01）; the NRS score in the observation group was significantly lower than that in the control group （P〈0.05）. The total effective rate in the observation group was superior to that in the control group （P〈0.05）. There were no severe adverse reactions in the two groups. Conclusion： Based on routine management, auricular acupoint sticking can produce a more significant efficacy in treating pain in early-stage extremity trauma compared to amidopyrine and phenacetin compound tablet; it can effectively reduce pain of the affected limb and prevent complications; it＇s easy-to-operate and safe; patients can learn and understand it easily; its efficacy is confirmed; it enhances the satisfaction degree of the inpatients. Therefore, this method is worth promoting in clinic.
