OBJECTIVE:To investigate the effect of—Xiaoyi Yusi decoction(XYYSD,消异育嗣汤),a Traditional Chinese Medicine(TCM),on in vitro fertilization and embryo transfer(IVF-ET)in patients with endometriosis,and to study the mechanism underpinning the ***:Women who underwent IVF-ET were divided into three groups by simple randomization:the treatment(n=32;with TCM treatment),patient(n=28;with endometriosis alone),and control(n=33;with male factor alone)*** luteal phase short-acting gonadotropin-releasing hormone agonist prolonged protocol was used in all three *** compare the changes in TCM scores and reproductive outcomes before and after TCM intervention in patients with endometriosis,partial least-squares discriminant analysis was used to analyze the follicular fluid samples of each group and screen and compare metabolites using the MetaboA nalyst ***:The clinical data indicated that following TCM intervention,kidney deficiency and blood stasis symptom patterns improved dramatically in patients with endometriosis and that their clinical pregnancy rate increased significantly(71.9%vs 57.1%,P<0.05).Metabolomics showed that the two groups of samples were separated before and after TCM *** TCM intervention,the intervention group was close to the control group,indicating that the TCM had a certain *** analysis revealed that after TCM intervention,the metabolism of glycerin phospholipid,pyruvate,and citric acid was ***:Through the pyruvate and glycerophospholipid metabolism pathways and tricarboxylic acid cycle,the TCM XYYSD successfully improved kidney deficiency and blood stasis symptom pattern,as well as the clinical reproductive outcomes of patients with endometriosis-related infertility.

Effects of Chinese herbs combined with in vitro fertilization and embryo transplantation on infertility:a clinical randomized controlled trial

OBJECTIVE： To assess the effects of using Chinese herbs in assisted reproductive technology. METHODS： Four hundred and thirty-three subjects aged less than 42 years with infertility due to Fallo- pian tube or male-related factors who were willing to undertake in vitro fertilization and embryo trans- plantation were randomly allocated to a Chinese herb intervention group （n=216） or a conventional treatment control group （n=217）. All subjects re- ceived one of four routine ultra-ovulation-promot- ing therapies at the Reproductive Center in the Third Hospital Affiliated to Beijing University ac- cording to their physician＇s assessments. The sub- jects in the intervention group received various Chi- nese herbs depending on their conventional treat- ment. Endometrial thickness, number of acquired eggs, and rates of normal fertility, high-quality em- bryos, biochemical and clinical pregnancy of sub- jects were assessed in both ***： The high-quality embryo rate of 51.9%, biochemical pregnancy rate of 51.0%, clinical preg- nancy rate of 44.2% and endometrial thickness of （10.84± 1.75） mm in the intervention group were all significantly higher than those in the control group [48.7%, 38.9%, 34.8%, and （10.52±1.50） mm, respec- tively; P〈O.05]. The normal fertility rate of 58.5% in the Chinese herb group was also significantly supe- rior to the 54.7% achieved in the control group （P〈 0.01）. There were no statistically significant differ- ences （P〉0.05） in the average number of acquired eggs within a single cycle, incidence of excessive stimulation of ovary, rates of embryo transplanta- tion or early abortion and birth of living babies be- tween the two groups. CONCLUSION： Our findings indicate that Chinese herbs increase endometrial thickness, improve the quality of fertility and embryo, and promote embry- onic nidation, thus enhancing the success rate of in vitro fertilization/intracytoplasmic sperm injec- tion-embryo transplantation cycle. Effect of interval after surgery on in vitro fertilization/intracytoplasmic sperm injection outcomes in patients with stage Ⅲ/Ⅳ endometriosis

Background For patients with severe endometriosis, the spontaneous pregnancy rates have been reported to be near 0 due to extreme distortion of normal pelvic *** is one of the treatment options; however, if patients failed to conceive after surgery, in vitro fertilization （IVF） is *** objective of this retrospective study was to determine the clinical characteristics of IVF/intracytoplasmic sperm injection （ICSI） in patients with stage Ⅲ/Ⅳ endometriosis, and to determine the impact of the interval from surgery to IVF/ICSI on *** One hundred and sixty patients who were diagnosed with stage Ⅲ/Ⅳ endometriosis underwent IVF/ICSI cycles between February 2004 and June 2009 were *** mean interval from surgery to IVF, number of oocytes retrieved, fertilization rate, implantation rate, embryos transferred, and good embryos transferred were compared between two age groups （≤35 years and 〉 35 years）.Results The mean interval from surgery to IVF was （37.9±28.9） months for the group ≤ 35 years of age and （57.6±39.7）months for the group 〉35 years of ***-five IVF/ICSI cycles （12.8%） were performed during the first year after surgery, and 34.9% IVF/ICSI cycles were performed 2 years after *** significant differences existed between the two groups with respect to the fertilization rate, implantation rate, number of embryos transferred, number of good embryos,clinical pregnancy rates, live birth rates, and cumulative clinical pregnancy rates （P 〉0.05）.The probability of cumulative clinical pregnancies was 75%, 50%, and 25% （（29.0±4.8）, （61.0±7.6）, and （120.0±16.9） months after surgery, respectively）.Conclusions For infertile patients with stage Ⅲ/Ⅳ endometriosis, the optimal time to conceive by IVF/ICSI is 〈2 years after surgery; nevertheless, most of the patients took a longer time to conceive.

Awareness and Attitudes Toward In vitro fertilization in Patients with Infertility:A Cross-Sectional Study

Objective:To investigate the awareness and attitudes of patients with infertility toward in vitro fertilization(IVF).Methods:A cross-sectional study was conducted in patients with infertility at the Reproductive Medicine Center of Peking University Third *** collection was conducted using a questionnaire that consists of patient background information,methods of obtaining relevant IVF information,and patients’awareness of IVF treatment as well as the law and ***:A total of 198 participants of different backgrounds were enrolled in the study,and IVF information was obtained through different means;however,most of the participants had a lack of awareness and improper attitude toward *** participants had higher estimations of treatment expenses(Pearson λ^(2)=22.132,P<0.05)and time(Pearson λ^(2)=47.992,P<0.05)during the initial consultation,while those with treatment experience expressed fewer concerns about IVF therapy(Pearson λ^(2)=9.467,P<0.05),particularly about the genetic risks of having ***:Measures should be taken to improve patients’awareness and attitudes toward IVF,and a large-scale and multicenter clinical study should be conducted in the future.

We developed tests of sperm-oocyte interaction:sperm-zona binding,zona-induced acrosome reaction,sperm-zona penetration and sperm-oolemma binding,using oocytes which failed to fertilise in clinical in vitro fertilization(IVF).Although oocyte defects contribute to failure of sperm oocyte interaction,rarely are all oocytes from one wom-an *** or zero fertilization in standard IVFwas usually caused by sperm *** sperm-zona pel-lucida binding was frequently associated with failure of standard IVF and obvious defects of sperm motility or *** size and shape of the acrosome is particularly important for sperm binding to the *** proportion ofacrosome intact sperm in the insemination medium was related to the IVF *** the acrosome reaction with acalcium ionophore reduced sperm-zona *** acrosome dispersal with an acrosin inhibitor prevented sperm-zona ***-zona penetration was even more highly related to IVF rates than was sperm-zona *** patients had low or zero fertilization rates with standard IVF but normal sperm by conventional tests and normalsperm-zona *** of their sperm underwent the acrosome reaction on the surface of the zona and none penetrat-ed the *** contrast,fertilization and pregnancy rates were high with intmcytoplasmic sperm *** call thiscondition defective zona pellucida induced acrosome *** of the nature of the abnormalities in the signaltmnsduction and effector pathways of the human zona pellucida induced acrosome reaction should result in simpler testsand treatments for the patients and also provide new leads for contraceptive development.

Endometriosis Fertility Index for Predicting Pregnancy after Endometriosis Surgery

Background： The endometriosis fertility index （EFI） has a predictive value for pregnancy after surgery. In vitro fertilization and embryo transfer （IVF-ET） is a good treatment to infertility. This study aimed to provide external validation of EFI, assess the factors affecting the ability of EFI to predict cumulative spontaneous pregnancy rates （PRs）, and propose reasonable advice for treatment by evaluating the effect of infertility management combining surgery and IVF-ET. Methods： This retrospective study enrolled 345 endometriosis-related infertile women after laparoscopic surgery from January 2012 to January 2016. Among them, 234 patients tried to conceive naturally and were divided into six groups according to their different EFI scores. Of the 345 patients, 307 with an EFI score 〉5 were divided into non-IVF-ET group （n = 209） and IVE-ET group （n = 98） to compare the cumulative PRs. Cumulative PRs＇ curves were calculated using the Kaplan-Meier product limit estimate and the differences were evaluated by log-rank test. Independent predictive factors for pregnancy were assessed using the Cox regression model. Results： Significant differences in spontaneous PRs among different EFI scores were identified （χ2 = 29.945, P 〈 0.05）. The least function score was proved to be the most important factor for EFI （χ2 = 6.931, P 〈 0.05） staging system. In patients with an EFI score ≥5 after 12 months from surgery, the cumulative PRs of those who received both surgery and IVF-ET were much higher than the spontaneous PRs of those who received surgery alone （χ2 =4.160, P = 0.041）. Conclusions： The EFI is a reliable staging system to predict the spontaneous PR of patients. The least function score was the most influential factor to predict the spontaneous PR. Patients with an EFI score ≥5 after 12 months from surgery are recommended to receive IVF-ET to achieve a higher PR.
