BACKGROUND Crizotinib-induce hepatotoxicity is rare and non-specific, and severe hepatotoxicity can develop into fatal liver failure. Herein, we report a case of fatal crizotinib-induced liver failure in a 37-year-old Asian patient.CASE SUMMARY The patient complained of dyspnea and upper abdominal pain for a week in August 2017. He was diagnosed with anaplastic lymphoma kinase-rearranged lung adenocarcinoma combined with multiple distant metastases. Crizotinib was initiated as a first-line treatment at a dosage of 250 mg twice daily. No adverse effects were seen until day 46. On day 55, he was admitted to the hospital with elevated liver enzymes aspartate aminotransferase(AST)(402 IU/L), alanine aminotransferase(ALT)(215 IU/L) and total bilirubin(145 μmol/L) and was diagnosed with crizotinib-induced fulminant liver failure. Despite crizotinib discontinuation and intensive supportive therapy, the level of AST(1075 IU/L),ALT(240 IU/L) and total bilirubin(233 μmol/L) continued to rapidly increase,and he died on day 60.CONCLUSION Physicians should be aware of the potential fatal adverse effects of crizotinib.

To the Editor:Dieulafoy's disease is characterized by the tortuous arteries of abnormally large caliber running within the submucosa, which is susceptible to ulceration and bleeding.This vascular abnormality usually occurs in the gastrointestinal tract.The involvement in the respiratory system is rare.Massive hemoptysis caused by Dieulafoy's disease of the bronchus is a life-threatening condition.An immediate and effective treatment is required in emergency to improve survival.Here,we report a patient who had such a vascular anomaly in bronchus and presented with recurrent massive hemoptysis.The patient signed information consent form.

Objective:To illustrate the clinical features and investigate the indicators associated with a fatal outcome in adult patients with severe Plasmodium falciparum malaria admitted to the Hospital for Tropical Diseases,Bangkok,Thailand.Methods:We studied 202 adult malaria patients admitted to the Intensive Care Unit.A total of 43 clinical variables were identified by univariate and logistic regression analyses,to eliminate confounding factors.Results:Regarding the statistical methods,only 6 variables-jaundice,cerebral malaria,metabolic acidosis,body mass index,initial respiratory rate,and white blood cell count-were significant indicators of death, with adjusted odds ratios(95%CI) of 15.2(2.1-32.3).4.3(2.3-12.6),3.3(2.3-5.7),2.4(1.9-3.5),2.2 (1.5-2.6),and 1.7(1.2-3.1),respectively.Conclusions:Our study found that jaundice,cerebral malaria,metabolic acidosis,body mass index,initial respiratory rate and white blood cell count were indicators of fatal outcome in severe Plasmodium falciparum malaria.Further studies on the fatal indicators in severe malaria need to be compared with data from different geographical areas,to construct practical measures to address potentially fatal indicators in different settings.

The sting of Giant Asian honeybee(Apis dorsata) or Bambara in Sinhala and Kanmge Kulavi in Tamil is a common environmental hazard in Sri Lanka known to cause immediate allergic reactions,which could be fatal in sensitized individuals.We reported myocardial infarction, bowel gangrene and fatal anaphylaxis in a prospectively proven case series and the association of these uncommon complications with delayed removal of stingers from the patients’skin.

This issue of Asian Pacific Journal of Tropical Medicine has published a case report[1].This report detailed an atypical fatal case of fox-mediated human rabies.It once again reemphasized the importance of zoonotic disease transmission by animals(including wildlife),and a need to look introspectively in order to consider innovative solutions aimed at reducing the burden of zoonoses.Rabies continues to significantly impact human lives.

Introduction: Leptospirosis is a worldwide zoonosis. It is transmitted through the urine of infected animals. Currently, there is an increase of reports in many countries. In humans, it presents an ample clinical spectrum, which goes from an asymptomatic infection up to Weil syndrome, which is generally fatal. Clinical Case: A male, 6 years of age, who started with onset fever and jaundice, handled in private means, diagnosed as viral hepatitis “A” and was referred to an institutional hospital where hepatomegaly and splenomegaly were detected. His evolution was towards graveness and, therefore, he was referred to a third level hospital with reactive Hepatitis diagnosis and to rule out lymphoma. On admission, he presented liver and kidney failure, as well as metabolic acidosis and pulmonary haemorrhage that led to death 6 hours later. Confirmatory tests for hepatitis were negative;biopsies were taken post-mortem for Leptospira diagnosis, which were positive in liver and kidney. Conclusions: Leptospirosis is a disease that may be manifested in multiple ways. It is important the understanding of this disease by the physician to improve the diagnosis and, for general population, to avoid exposure. The examination of the epidemiological history of the patient is essential.

Atypical cutaneous lymphoproliferative disorder (ACLD) is a rare condition that has been associated with HIV infection. Patients with ACLD present with diffuse, erythematous and pruritic skin lesions accompanied by generalized lymphadenopathy. The clinical characteristics of ACLD overlap most notably with several other conditions including Mycosis Fungoides/Sézary Syndrome (MF/SS), a cutaneous lymphoma of T-cell lineage. Unlike Mycosis Fungoides, the noxious infiltrates of ACLD are not monoclonal but polyclonal and consist of cytotoxic CD8+ T-cells instead of CD4+ T-cells or B-cells. Highly active antiretroviral therapy (HAART) has been reported to improve ACLD. We describe the case of a Caucasian man with longstanding HIV infection who presented with severe erythroderma. Skin and lymph node biopsies showed polyclonal CD8+ T-cell infiltrates. Gene rearrangement studies did not reveal an obvious clonal disorder. Hallmark peripheral blood findings consisting of a severe depletion of CD4+ T-lymphocytes and markedly elevated CD8+ cells provided an important diagnostic clue. Despite the purported benefits of HAART in ameliorating this disorder, erythroderma and extreme pruritus improved only after the patient began taking mycophenolate mofetil and hydroxyurea. Unfortunately, he succumbed to complications of methicillin-resistant Staphylococcus aureus septicemia. We alert readers to this rare HIV-associated condition which may mimic other benign and malignant skin conditions and briefly discuss diagnostic and therapeutic options.

Epidemiology of fatal cases associated with pandemic influenza reported in Yemen

Objectives: This study describes the incidence of all fatal cases associated with Pandemic Influenza A (H1N1) in Yemen. It sets out to highlight the factors associated with poor prognosis *The authors declare that they have no competing interests. to enhance the implementation of prevention and control programs. Methods: The study is based on retrospective analysis of available data until 14 March 2010, as compiled by the disease control and surveillance team in Yemen. Results: Between 16 June 2009 and 14 March 2010, a total of 33 laboratory-confirmed death cases associated with pandemic influenza A (H1N1) were reported to the Diseases Control and Surveillance in the Ministry of Public Health and Population. During this period, a total of 6049 suspected influenza A (H1N1) cases were recorded. With this denominator, the case fatality rate (CFR) was 0.54%. During June through August, H1N1 confirmed cases were infrequently detected, including only 30;however, from September through December, over 200 confirmed cases were reported each month. Of the 33 cases recorded, 25 were male (76%) and 8 were female (24%), male to female ratio being 3:1. Overall median age of the death cases was 30.8 years (range 1 - 55). The most common diagnosis upon admission was pneumonia. Out of the deaths, twenty five (75.8%) had no documented underlying diseases. Chronic cardiovascular disease (9.1%) was the most commonly reported disease and 2 deaths (6.1%) were recorded as pregnant women. Conclusions: The most common diagnosis upon admission was pneumonia. Chronic cardiovascular diseases were the most commonly reported underlying conditions, while the most identified risk factor was pregnancy. These findings should be taken into consideration, when vaccination strategies are employed.

Lyell Syndrome with fatal Outcome in Two Pregnant Women Receiving Antiretroviral Therapy in Guinea Conakry

An old woman, 28 years old, weighing 74 kg was referred for skin detachment associated with bubbles and mucosal erosions. This clinical picture had occurred 15 days after the institution of a triple combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 20-week gestation. The woman was in her third gestation. The second case is aged 26, weighing 65 kg. This clinical picture had occurred 20 days after the administration of a triple therapy combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 32-week gestation. It was the fourth gestation. The birth of a stillborn had occurred the day before his admission into our service. The diagnosis of Toxic Epidermal Necrolysis with nevirapine was selected on the basis of the clinical and anamnestic arguments. The biological tests showed the following results: transaminases (ALT and AST) were within normal limits, blood glucose, and urea > 10 mmol/l. The evolution was marked by the patient death to Day 9 (D9) and at D2 of hospitalization. Discussion: NET and the SJS have been reported in pregnant women as potentially dangerous complications that are often associated with suspected drugs. Among these drugs, antiretroviral drugs, prescribed to pregnant women to prevent mother to child transmission of HIV are often reported. Our case is an illustration of the problem of HIV management in pregnant women. It also focuses on the association between pregnancy and Toxic Epidermal Necrolysis. Nevirapine is an effective drug in the regimens proposed in the management of HIV patients in limited resources countries. But the high frequency of toxidermia with this drug should lead to its replacement by other antiretrovirals least providers of toxidermia as anti proteases in the context of PMTCT programs.

Comparative Analysis of fatal Pedestrian Crashes between Kansas and USA

Due to the increasing trend of population growth and urbanization, pedestrians form one of the largest single road user groups. However, they are the most neglected group among all road users. Pedestrian safety is now a growing concern in the USA. Identifying the factors associated with fatal pedestrian crashes plays a key role in developing efficient and effective strategies to enhance pedestrian safety. This study addresses safety issues by identifying contributory factors associated with fatal pedestrian crashes in Kansas and the USA. For Kansas, the study uses KARS (Kansas Accident Reporting System) database while for the USA FARS (fatality Analysis Reporting System) database has been used. Different variables considered in this study are human variables (age, and gender), environmental variables (atmospheric condition and light condition), time (time of day, day of week, and crash month), location (intersection vs. mid-block), and roadway variables (speed limit). Different factors that are found to have an association with fatal pedestrian crashes are male pedestrians, older pedestrians, weekend, off peak hours, winter months, dark hours, non-intersection, clear atmospheric conditions, higher speed limit. The findings from Kansas have been compared with that from the USA. This study helps to implement potential countermeasures by identifying the factors that have an association with fatal pedestrian crashes.
