Postpartum family planning at the Timbuktu Hospital in Mali

Introduction: Postpartum family planning is the prevention of pregnancies during the 12 months following childbirth. Few studies have been devoted to postpartum family planning in Mali. Our work will contribute to reducing unmet need for family planning. Objective: To study the use of contraceptive methods in the postpartum period in the obstetrics and gynecology unit of Timbuktu hospital. Materials and Methods: This was a descriptive and analytical cross-sectional study with prospective collection of data from January 1, 2022 to December 31, 2023. All women who gave birth having chosen and benefited from a contraceptive method were included. The statistical test used was the Fisher test with a significance threshold fixed at 5%. Results: The frequency of contraception in the postpartum period was 17.03%. The average age of clients was 26.14% with extremes of 14 and 45 years. They were paupiparous at 56.4% with an inter-birth interval of less than 12 months at 12.3%. More than half of the counseling (58.5%) was done during postnatal visits. The methods chosen were implants at 48.1%, injectable progestins at 21.3%, intrauterine device at 14.7%, miro-progestin pills at 13.5%, tubal ligation at 1 .4% and condoms at 1%. The regular follow-up rate was 51.1% of cases and 25.6% had no follow-up. Conclusion: The overall rate of postpartum family planning of 17.08% remains low. Improving FP staff skills will reduce unmet needs and contribute to increasing contraceptive prevalence in Timbuktu.

Correlates of Mistimed Pregnancy and Unmet Need for family planning among Women of Reproductive Age in Sandema, Ghana

Background: Globally, an estimated 80 million unintended pregnancies comprising both mistimed and unwanted pregnancies are recorded yearly. Yet only half of the women at risk of mistimed pregnancy use contraceptives. In developing countries, over 100 million females have unmet need, and national surveys in Ghana indicate 23% unmet need rate. Methods: Using a cross-sectional community-based approach, a sample size of 300 women of reproductive age were selected using multi-step cluster sampling techniques. The study was quantitative, using structured interviewer-administered questionnaires. Results: Two-third (66%) of the women in reproductive age still had unmet need, 71% were currently pregnant, and more than a third (36%) confirmed ever having a mistimed pregnancy. Fifty-three percent (53%) of the women confirmed never communicating with their partners on family planning issues, a little below half (45%) took their own health care decisions. Seventy nine percent (79%) ever received family planning services from a health professional. Factors related to unmet needs included mistimed pregnancy, level of education, preferred birth/pregnancy interval, communication between partners and the autonomy to spend self-earnings. Conclusion: Considering that high rates of unmet need results in mistimed pregnancy, improved policies around the influence of unmet need on mistimed pregnancies are needed.

Study on the Perceptions and Social Representations of Husbands on family planning in the Health District of Pout/Senegal

Contraception is a set of procedures that aim to achieve temporary infertility in a woman without her ability to conceive being engaged in the future. It plays an essential role in the health of women and children, the psychological well-being of husbands and allows, among other things, savings for the family. The objective was to study the perceptions and social representations of husbands on family planning. A mixed method was used with a qualitative dominance, carried out in the health district of Pout. The result showed that the perceptions and representations of husbands influence this practice family planning. Even if they have positive attitudes and a good level of knowledge about contraceptive methods, their involvement in FP practices remains low with gaps to be filled. Direct or intermediate variables such as the social, cultural, economic and religious aspects are pitfalls. Indeed, the commitment of men and the consideration of their social attributes are variables to be integrated into the process of information and awareness on family planning;this is crucial in achieving Sustainable Development Goal (SDG) 3 (Good Health and Well-being), including its target 7 focused on sexual and proactive health: a second best move towards the demographic dividend.

Factors Associated with Unmet Need for family planning among Couples Living in Urban and Rural Areas of Guinea in 2019

Introduction: Unmet need for family planning (UNFP) is defined as women with unmet needs who want to stop or delay childbearing but are not using any method of contraception. The objective of this study was to analyze the factors associated with unmet needs for family planning among couples living in rural and urban areas of Guinea in 2019. Methodology: This was a prospective, analytical cross-sectional, multicenter study of a six-month period from August 1, 2018 to January 31, 2019, focusing on couples with unmet needs for family planning. Result: Among 189 couples interviewed, 567 had UNFP (33.3%), the reasons for not using modern contraceptive methods were desire for pregnancy (AOR = 2.74, 95% CI: 1.74, 4.31), husband’s refusal (AOR = 0.23, 95% CI: 0.06, 0.81), spousal attitude (AOR = 0.20, 95% CI: 0.130, 30), birth spacing (AOR = 2.10% to 95%: 1.16, 3.82), difficulty with a new pregnancy (AOR = 0.17, 95% CI: 0.04, 0.74), and spousal attitude (AOR = 0.20, 95% CI: 0.14, 0.30). Conclusion: The involvement of spouses, especially in rural communities, would help achieve family planning objectives and reduce unmet needs for family planning.

Evaluation of Knowledge and Practice regarding family planning among Christians Pregnant Women of Gihanga Attending Antenatal Careat Vyizigiro Health Center, Bubanza, Burundi

Introduction: family planning is an important service for everyone and contributes to the protection of women from unwanted pregnancies. In Burundi, it remains at a low rate due to different factors such as fear of side effects, low education level, and notably religious guidance. The purpose of this study was to evaluate the knowledge and practice regarding family planning among Christians pregnant women of Gihanga attending antenatal care at vyizigiro Health center. Methods: This cross-sectional study of Christians pregnant women attending antenatal care was conducted at Vyizigiro health center of Gihanga, Burundi in May 2021. The data were collected on a small sheet done in English, and translated into Kirundi’s local language. In this study, were included all women fulfilled all criteria, and were excluded from all impregnated women who were not Christians and refused to participate in the study. Data were analyzed using SPSS. Results: In the current study, among 129 pregnant women who attended antenatal care at Vyizigiro Health center in the period of 1 to 30 October 2020, 118 of them were interviewed. The study found that the participants have low knowledge regarding family planning as participants scored less than 50% in all items assessed for knowledge and its practice was hindered by religious guidance (41.7%), while others refused modern contraceptives for fear of side effects (13%), lack of sufficient information about it (7.4%) and 5.6% were denied by their husband. Conclusion: This study found a low knowledge and practice regarding family planning among the Christians pregnant women of Gihanga attending antenatal care at Vyizigiro health center due to their religious beliefs, fear of side effects, and lack of sufficient information or their husband who don’t want family planning services.

Unmet need for family planning and sexual and reproductive health and rights among adolescents in Bangladesh

Every individual has the right to make their own choices about their sexual and reproductive *** planning,including contraceptive information and services,is fundamental to all individuals’health and human rights,including *** made a commitment at the Nairobi summit(ICPD+25)in 2019 to address three zeros,including the unmet need for family planning by 2030-a big challenge where unmarried adolescents are excluded from most Sexual and Reproductive Health and Rights(SRHR)information and ***,unmet need for family planning amongst adolescents in the country is high.A literature review of relevant research papers,survey reports,and policy documents has been conducted(i)to demonstrate the current situation of unmet need and SRHR-FP for adolescents in Bangladesh;(ii)to identify unique barriers and challenges faced by adolescents in accessing SRHR-FP information and using services in Bangladesh;(iii)to analyze and identify gaps in the current policies,strategies,and plans to address unmet need and SRHR in Bangladesh;and to provide recommendations for adapting policies and improving best *** paper demonstrates that although Bangladesh has policies designed to support youth rights and access to comprehensive sexuality education and relevant services,there are immense implementation *** stigma and taboos are overpowering the implementation of policies that need critical ***,interventions are needed to address the significant gap in data on unmarried adolescents and their use of family planning services,which limits the analysis of the current situation of unmarried adolescents.

Unmet Needs for family planning among Adolescent Girls Giving Birth in Three Teaching Hospitals in Yaoundé

Many adolescent girls are pressured into having sex at an early age, which puts them at high risk of unwanted pregnancies and unsafe abortions. The overall objective of this study was to evaluate the unmet needs of adolescents who give birth. A descriptive cross-sectional study was carried out in three university hospitals in Yaoundé, Cameroon: Yaoundé Central Hospital, Yaoundé Gyneco-Obstetrics and Pediatric Hospital and the District Hospital of Biyem-Assi, from February 1, 2020 to June 30, 2020. Included were any teenage mothers speaking English or French. Data were entered using CSPRO 7.3, analyzed by Excel 2010 and SPSS version 23.0. The tools used to express our results were the number, the frequency, the mean, the odds ratio (OR) and the P. P was significant if less than 5%. Of a total of 2692 births recorded, 188 (7%) were from adolescents. Of these, 157 fulfilling our selection criteria were recruited and data analyzed. The average age of the participants was 17.9 ± 1.12 years with extremes of 13 and 19, the average parity was 1.2 ± 0.4 with extremes of 1 and 3. Out of 157 participants, 2 who fell in the age range of 10 to 14 years (100%) and 106 of 155 (68.4%) whose age ranged from 15 to 19 years had unmet need for family planning. Only unmarried participants had unmet needs after multivariate analysis [aOR 2.4 (1.1 - 5.3);p = 0.035)]. Being unmarried was independently associated with the occurrence of unmet needs. The intensification of campaigns for provider behavior changes communication and the creation of services dedicated to the sexual and reproductive health of adolescents would help to reduce the rate of unmet needs for family planning among adolescent girls.

Unmet need for postpartum family planning and influencing factors in China

To explore the rate and influencing factors of unmet need for postpartum family planning(PPFP)in *** conducted a retrospective cohort study at 60 hospitals in 15 provinces that were in eastern,central,and western regions of *** were women who delivered a live birth at the study sites with an interval of 13 to 24 months between delivery and *** selected participants using cluster randomization approach,and the first 300 postpartum women who gave a birth at each study hospital after the initial month that had been selected were *** on the women’s background characteristics,pregnancy history,time when menstruation and sexual activity resumed after childbirth,the adoption of contraceptive method,breastfeeding,and any pregnancy or pregnancy outcome after delivery were *** performed life-table analysis to estimate the rate of unmet need for PPFP and a 2-level logistic regression model to explore factors that influence unmet need for PPFP within the first 24 months postpartum.A total,19,939 postpartum women were screened in this study,of which,17,466(87.6%)were eligible for this *** rates of unmet needs for any FP methods were 23.9%(95%confidence interval[CI]23.3-24.6%),11.8%(95%CI 11.3-12.3%);10.6%(95%CI 10.1-11.1%)at 6,12,and 24 months postpartum;these rates for modern FP methods were 35.5%(95%CI 34.7-36.2%),25.6%(95%CI 24.9-26.2%),and 24.6%(95%CI 23.9-25.2%),*** of 2-level logistic regression analysis showed that less-educated young women,those who had only one child or delivered by vaginal delivery at secondary hospitals,were associated with increased risk of unmet need for *** 31%of women who had unmet need for PPFP reported a pregnancy during the first 24 months postpartum,which was significantly higher than the level for their counterparts(10.0%).The level of unmet need for PPFP in China was high,resulting in a high pregnancy rate within 24 months after ***

Improving Access to Quality Care in family planning:WHO’s Four Cornerstones of Evidence-based Guidance

The four cornerstones of guidance in technique service of family planning are established by WHO based on high quality evidences. They have been updated according to the appearing new evidences, and the consensuses were reached by the international experts in this field. The four documents include Medical Eligibility Criteria for Contraceptive Use, Selected Practice Recommendations for Contraceptive Use, Decision-making Tool for family planning Clients and Providers and The Global Handbook for family planning Providers. The first two documents mainly face to the policymakers and programme managers and were treated as the important references for creating the local guideline. The other two documents were developed for the front-line health-care and family planning providers at different levels, which include plenty of essential technical information to help providers improve their ability in service delivery and counselling. China paid great attention to the introduction and application of WHO guidelines. As soon as the newer editions of these documents were available, the Chinese version would be followed. WHO guidelines have been primarily adapted with the newly issued national guideline, The Clinical Practical Skill Guidelines-family planning Part, which was established by China Medical Association. At the same time, the WHO guidelines have been introduced to some of the clinicians and family planning providers at different levels. In the future, more special training courses will be introduced to the township level based on the needs of grass-root providers.
