Endovascular stent graft repair of abdominal aortic aneurysm(AAA)has undergone rapid developments since it was introduced in the early *** main types of aortic stent grafts have been developed and are currently being used in clinical practice to deal with patients with complicated or unsuitable aneurysm necks,namely,suprarenal and fenestrated stent *** computed tomography angiography has been widely recognized as the method of choice for both pre-operative planning and post-operative followup of endovascular repair(EVAR).In addition to 2D axial images,a number of 2D and 3D reconstructions are generated to provide additional information about imaging of the stent grafts in relation to the aortic aneurysm diameter and extent,encroachment of stent wires to the renal artery ostium and position of the fenestrated vessel *** purpose of this article is to provide an overview of applications of EVAR of AAA and diagnostic applications of 2D and 3D image visualizations in the assessment of treatment outcomes of *** of stent wires with renal blood flow from the hemodynamic point of view will also be discussed,and future directions explored.

Background: Provoked spasm might have influence on cardiovascular events even after successful DES implantation. Methods and results: A consecutive 122 patients who underwent first-generation DES (CypherTM, and TaxusTM) implantation and had no residual stenosis at follow-up coronary angiography were included. All patients also underwent acetylcholine provocation test (ACh) at follow-up angiography. Age, sex, coronary risk factors (smoking, hypertension, diabetes mellitus, dyslipidemia), medication (calcium channel blockers, angiotensin receptor blockers, angiotensin converting enzyme inhibitors, nitrates, β blockers, statins), duration from stent implantation to follow-up angiography (15.5 ± 14.5 months), old myocardial infarction, Ach + were included as variables for multivariate analysis for cardiovascular events (angina pectoris without intervention, target lesion revascularization, acute myocardial infarction, definite or probable stent thrombosis, cardiac death, and stroke). ACh provocation test was positive in 64.8%. The following events occurred during a mean follow-up period of 26.9 ± 9.5 months;angina pectoris without intervention in 12, very late stent thrombosis in 1, acute myocardial infarction in 1, sudden death in 1, and stroke in 4. By Adjusted Cox hazard model, age was the independent predictor of future cardiovascular events. ACh was unbalanced data for Cox hazard model, and by logistic regression model, ACh was the strongest predictor. Conclusions: Coronary endothelial dysfunction at chronic stage is the strongest predictor of further prognosis in patients with first-generation DES.

Impact of the COVID-19 Pandemic on the Management Course of Paediatric Rheumatic Diseases in a Tertiary Care Hospital of Bangladesh

Background: The COVID-19 pandemic had a significant impact on every aspect of life. Like other chronic diseases, pediatric rheumatic diseases (PRDs) also faced unique challenges during the pandemic, including limited access to care, concerns regarding infection risk, drug shortages and social isolation. Objective: The objective of the study was to identify the impact of the COVID-19 pandemic on the management course of children with rheumatic diseases. Methods: This cross-sectional study was conducted in the department of paediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka from April 2021 to March 2022. Two hundred and twenty patients were enrolled. A predesigned questionnaire was completed which included socio-demographic, clinical and laboratory parameters. Disease activity was assessed clinically and from laboratory profile. Data were calculated manually and by using SPSS version 22. Z-proportion test and chi square test were done when applicable. Results: Among the enrolled PRD cases 87.7% were Juvenile Idiopathic Arthritis (JIA), 7.7% were Systemic Lupus Erythematosus (SLE) and 4.6% were other cases. Irregular follow-ups were found in 69.9% JIA cases, 82.4% SLE cases and 60% other PRD cases. Poor drug compliance was present in 18.2% of cases. A significant number of patients had higher disease activity or flare during the COVID-19 pandemic. Conclusion: During COVID-19 pandemic most (70.5%) of the PRD patients were irregular in their follow-ups. Poor drug compliance was also more than in the pre-COVID period. Disease activity status significantly worsened during this period.

Novel Histological Evidence of Collagen and Elastin Regeneration in Fractional RF-Treated Acne Scars

Introduction: Fractional radiofrequency (RF) technology has been shown to be a gold standard therapy for acne scars. We have previously published clinical and histological results from the treatment of 8 patients with active acne and acne related scarring using a fractional RF device. Evidence of safety and efficacy was demonstrated, including long term follow-up clinical results from 4 patients, up to two years post last fractional treatment. In the current article we present histology evidence of acne scar skin renewal 3.5 years post treatment. Methods: Skin biopsies from acne scars in a concealed facial area were taken before, after four treatments and at 3.5 years after the last fractional treatment. Biopsies were examined with standard hematoxylin and eosin (H&E) stain, as well as Verhoeff’s Van Gieson (VVG) and Shikata stains for elastic fibers. Results: Histological findings demonstrate regeneration of elastic fibers and reduction of the dermal fibrosis. Scar depth of Patient #2 was reduced by 80% in the corresponding follow-up period. Conclusion: The long-term histological results further support the previously published findings that fractional RF is a safe and effective treatment for acne related scars.

Revised national tuberculosis control programme in India has limited co-hort-wise information about what happens to patients diagnosed with multidrug resistant TB (MDR-TB). We determined the pre-treatment loss to follow-up (non-initiation of treatment by programme within 6 months of diagnosis) and time from diagnosis to treatment initiation in Bhopal district, central India (2014). Pre-treatment loss to follow-up was 13% (0.95 CI: 7%, 23%), not significantly different from the national estimates (18%) and median time to initiate treatment was seven days, lower than that reported elsewhere in the country. Bhopal was performing well with reference to time to treatment initiation in programmatic settings.

Background: Gestational trophoblastic diseases (GTD) are potentially curable with retention of reproductive function once the correct diagnosis is made and treatment is commenced early with adequate follow up. Objective: The objective of this study was to determine the incidence, clinical presentation, management and treatment outcomes of gestational trophoblastic diseases in a tertiary hospital in Abakaliki, South-east Nigeria. Materials and Methods: This was a retrospective descriptive study of gestational trophoblastic diseases managed at Alex Ekwueme Federal University Teaching Hospital, Abakaliki (AEFUTHA) over a 6-year period. The statistical analysis was done using SPSS version 22. Results: The incidence of GTD was 4.6 per 1000 deliveries. Women who were less than 20 years and more than 40 years of age accounted for 5.9% and 23.5% of cases of GTD respectively. Women who were para 5 and above accounted for 76.5% of those who presented with GTD. All the patients presented with vaginal bleeding. Suction evacuation (76.5%) was the commonest form of treatment offered to women with GTD. The commonest complication was anaemia (94.1%). Maternal death due to GTD was 8.8%. Most (58.8%) of the patients did not turn up for follow-up. Conclusion: Gestational trophoblastic diseases has remained an important cause of maternal morbidity and mortality in our hospital due to poor compliance with follow-up. Call and recall system should be introduced in the management of patients with GTD to improve compliance to management standard.

Objective: Cancer patients consider the oncologist as their main resource insofar as the medical needs generally take precedence over psychological needs. Nevertheless, the psychological intervention is also important. The systematic consultation implemented in our hospital after a diagnosis of cancer is a manner to answer patients’ psychological needs. In a survey, we assessed the satisfaction and expectations of the patients about this consultation. Methods: One year after a diagnosis of breast cancer, 104 patients answered a retrospective questionnaire assessing: socio- demographic data, cancer medical information, systematic consultation satisfaction, patients’ expectations for a systematic intervention, and patients’ characteristics who began a psychological follow-up. Results: 72.1% of the patients were satisfied with having the opportunity to consult a psychologist during a systematic consultation after cancer diagnosis. Their expectations were to have opportunities of emotional expression, reassurance, obtaining additional medical information, talking about their fear over additional treatments, and identifying further support. 28.8% of the patients re-contacted the psychologist for further help after the systematic consultation. They were significantly younger (p upport by medical specialists [1], offering the opportunity to breast cancer patients to discuss inner feelings and treatment expectations during a systematic psychological consultation was useful for most of them. About one third of the patients asked for a further consultation with a psychologist after a first systematic consultation, especially younger patients. This stressed the importance of the role of psychologists for psycho-social support of the patient after cancer treatment.

A New Early Gastric Cancer after Subtotal Gastric Resection for Early Cancer: Case Report and Review of the Literature

Although the prognosis of early gastric cancer (EGC) is considered to be satisfactory, some patients experience tumor relapse after curative surgery. Both pathogenesis and risk factors of recurrence remain unclear. We describe a case report of a 49-year-old male who underwent subtotal gastric resection D2A for angular gastric cancer. Histological examination revealed gastric adenocarcinoma with low grade of differentiation and colloid areas, intramucosal, and absence of neoplastic proliferation in the surgical margins, in omental stroma and in the six examined lymph nodes (pT1, pN0). 11 years later, the same patient underwent D2 total gastrectomy for gastric cancer in the remnant stomach. New histological examination revealed again gastric adenocarcinoma, intramucosal, medium degree of differentiation, no documentable neoplastic proliferation within the limits of surgical resection, in the thirty-three examined lymph nodes and in the omentum (pT1, pN0).
