Objective: To compare the clinical efficacy of artificial femoral head replacement and Proximal femoral nail antirotation (PFNA) in the treatment of unstable femoral intertrochanteric fractures in the elderly. Methods: This study retrospectively analyzed 60 elderly patients with unstable intertrochanteric fractures treated with PFNA and artificial femoral head replacement from 2015.06 to 2018.06, of which 34 were in the PFNA group (Group A) and 26 in the artificial femoral head replacement group (Group B). Statistical analysis of relevant surgical indicators such as surgical time, intraoperative blood loss, postoperative blood transfusion, postoperative time to landing, postoperative infection rate, hospital stay, number of secondary operations, postoperative VAS score, and postoperative Hip function score comparison. Results: All 60 patients were followed up for 1 - 24 months. Compared with the artificial femoral head replacement group, the operation time of PFNA group was shorter, the blood loss during operation was less, and the difference was statistically significant (P 0.05). Conclusion: The hip joint function and pain scores of the artificial femoral head replacement group in the early and follow-up periods are better than those of the PFNA group. The artificial femoral head replacement is more suitable for the treatment of elderly unstable intertrochanteric fractures.

Osteonecrosis is a phenomenon involving disruption to the vascular supply to the femoral head, resulting in articular surface collapse and eventual osteoarthritis. Although alcoholism, steroid use, and hip trauma remain the most common causes, several other etiologies for osteonecrosis have been identified. Basic science research utilizing animal models and stem cell applications continue to further elucidate the pathophysiology of osteonecrosis and promise novel treatment options in the future. Clinical studies evaluating modern joint-sparing procedures have demonstrated significant improvements in outcomes, but hip arthroplasty is still the most common procedure performed in these affected younger adults. Further advances in joint-preserving procedures are required and will be widely studied in the coming decade.

External hernia is one of the commonest causes of intestinal obstruction. femoral hernia is a relatively uncommon hernia. It has a very high complication rate due to delayed diagnosis caused by deceptive symptomatology and subtle signs. CT scan enables a prompt diagnosis. Rigorous resuscitation followed by prompt surgical intervention is the mainstay of treatment. A case of a strangulated femoral hernia in a patient with complicated co morbid medical disorders is presented to highlight the pitfalls in initial clinical assessment.

Objective To evaluate the indications for vascularized iliac bone grafting in the treatment of osteonecrosis of the femoral head and operative results. Methods From October 1993 to August 2000, Twenty-six hips of 18 patients with osteonecrosis of the femoral head underwent vascularized iliac bone grafting. Clinical evaluation was made according to Harris Hip Score and ARCO staging system. Results Twenty-three hips of 16 patients were followed up for 31. 5 months (6-74 months) on average. The mean Harris hip score was 76peints (mean 61.7). Thirteen hips (medial 8 hips,central3 hips,lateral 2 hips). were graded >80 points by Harriship score were. The incidence of radiographic collapse 20% was (medial),40% (central) and 75% (lateral) respectively. Conclusion Vascularized iliac bone grafting is effective for the treatment of osteonecrosis of the femoral head in short and middle term. We recommend this procedure for the patients with ONFH in ARCO stage Ⅰ , stage Ⅱ A, Ⅱ B medial and central, and stage Ⅱ

Objective:To study and analyze the clinical efficacy of fixation of the elastic bandage of immobilization of knee-chest position and Pavlik harness fixation in the treatment of neonatal femoral shaft ***:A retrospective analysis was conducted from May 2010 to March 2017 with a total of 37 cases of femoral shaft fracture of the *** them,17 newborns with femur shaft fracture were treated using this fixation of the elastic bandage of immobilization of knee-chest position,and 20 newborns were treated utilizing Pavlik harness *** patients were followed up for 24 *** was compared between the two groups in length of hospitalization,hospitalization cost,the incidence of complications,fracture healing rate and post-operation angulation,rotation and shortening ***:Compared with the Pavlik harness fixation group,the hospitalization cost of knee-chest elastic bandage fixation group was lower(P0.05),with no statistical ***:Both knee-chest elastic bandage fixation and Pavlik harness fixation are effective methods for the treatment of neonatal femoral shaft *** the former has lower cost and simpler operation,which is worthy of clinical application.

Background: Compromised circulatory and bone/lipid metabolic dysfunction are two major contributors to non-traumatic osteonecrosis of the femoral head (ONFH). Qing’e pill plus Salvia has significant anti-inflammatory and anti-atherosclerotic action, and it could also regulate bone formation and remodeling by suppressing osteoclasts. Case Report: We describe a case of a 62-year- old man with ONFH of idiopathic type in earlier stage that could not be adequately controlled with oral non-steroidal analgesics and restoring bone loss agents for past 3 years. After six months treatment with Qing’e pill plus Salvia, the plasma low-density lipoprotein (LDL), high-density lipoprotein (HDL), apolipoprotein A1 (apo A1), apolipoprotein B (apo B), total cholesterol (TC), triglycerides (TG), and blood viscosity were measured on an empty stomach. The Harris scores were maintained during a six months follow-up after discontinuation of Qing’e pill plus Salvia. Discussion: Our case has shown that plasma levels of inflammatory and atherosclerotic relaxed factors and the Harris scores may be controlled with herbal medication in ONFH of idiopathic type in earlier stage that could not be adequately controlled with oral non-steroidal analgesics and restoring bone loss agents.

Simple femoral neck fractures in dogs and cats are rare in clinic. The cause of the fracture is that the pelvis or femur is directly or indirectly subjected to external force,and it often occurs secondarily to osteoporosis caused by dysplasia of the hip joint. Objective To discuss the treatment of the hip joint dislocation combined with femoral head fracture and its clinical therapeutic *** From July 2001 to July 2008,19 patients with hip joint dislocation combined with femoral head fracture underwent conservative therapy or surgical *** was based on Pipkin's criteria,type Ⅰ four,type Ⅱ six,type Ⅲ six,type Ⅳ *** procedures were taken according to the different types of the fracture,the time of the fracture,and the age of the *** clinical therapeutic effects were *** In type Ⅰ,four were rated as excellent by Harris Hip Score's *** type Ⅱ,two rated as excellent,three as good and one as *** type Ⅲ,three rated as good,two as fair and one as *** type Ⅳ,one rated as excellent,one as good and one as fair,overall rate of excellent and good was 73.7%.Conclusion The treatment of Pipkin fracture should be based on multiple factors such as the type of fracture,the time of fracture,the age of patients and so *** prognosis is directly related with those factors and the way of the treatment.

Valgus osteotomy in delayed presentation of femoral neck fractures using fixed angle simple dynamic hip screw and plate

Purpose: Reduction and fixation in femoral neck fracture in young patients have a problem of nonunion, requiring additional procedures like valgus osteotomy;but fixation devices are technically difficult for in experienced surgeons. We aims to assess the results of valgus osteotomy in femoral neck fracture in our setup. Methods: We reported a series of 20 patients of higher Pauwel's angled fracture of femoral neck fracture presenting late wherein valgus osteotomy was added to reduction fixation which was secured with a commonly available 135 dynamic hip screw and plate. Results: femoral neck fractures united in 16 patients (80%). Excellent to good results (Harris hip score >80) were seen in 70% patients. Angle of correction of preoperative Pauwels has been changed from 68.3 to 34.3. Conclusion: The 135 dynamic hip screw and plate provides rigid internal fixation after valgus osteotomy and being a more familiar fixation device simplifies the procedure with good results.
