BACKGROUND entrapment of the temporal horn (ETH) is a rare pathologic condition. It is a kind of focal hydrocephalus caused by obstruction of flow pathway of cerebrospinal fluid. It is caused by various conditions, but ETH secondary to postoperative gamma-knife radiosurgery (GKS) is extremely rare. CASE SUMMARY A 52-year old previously healthy woman underwent resection of a large intraventricular meningioma. A small fragment of residual tumor with no obvious enlargement of the temporal horn was observed 3 mo after surgery, and she was referred for GKS. Two months after GKS, she complained of headache and progressive paralysis of the left limb. Magnetic resonance imaging revealed enlargement of the temporal horn. There was a second procedure to resect the residual tumor 8 mo after GKS. After the second procedure, she recovered smoothly. As of the date of this writing, she has remained in good condition. CONCLUSION This case reminds us that ETH should be considered in the treatment of intraventricular meningiomas, especially before GKS.

Polylactic acid (PLA)and poly(lactic-co-glycolic)acid (PLGA)are two commonly applied biodegrad- able polymers for the preparation of nanocomposites used in drug-delivery systems.However,these polymers lack desirable attributes such as resistance to aggregation during long-term storage due to lyophilisation.To improve their efficacy,in this work,PLA and PLGA were encapsulated within a shell of poly(N-isopropylacrylamide)(pNIPAM)using a single emulsion technique followed by an aqueous free radical precipitation polymerisation process,yielding core-shell PLA/PLGA-pNIPAM nanocomposites. The nanocomposites were cbaracterised using zeta potential,dynamic light scattering,and transmission electron microscopy analyses and were further applied as a delivery system for ramipril,an antihypertensive drug.The drug-loaded PLGA-pNIPAM core-shell nanoparticles exhibited a higher drug content (91%)and entrapment efficiency (78%)than their PLA counterparts.An in vitro release study of the formulations at pH 7.3in phosphate-buffered saline indicated that PLGA was more efficient than PLA with a sustained release of 86% of ramipril from the polymer matrix within 24h.Furthermore,to determine the release kinetics,the data were fitted to Korsmeyer-Peppas and Higuchi models;the release of ramipril from the polymer matrix followed zero-order rate kinetics and an anomalous (non-Fickian)diffusion mechanism.

A model experiment investigating entrapment of inclusions and bubbles on the solidified shell was performed using molten steel,and the conditions for inclusion and bubble entrapment and mechanism of entrapment were studied. The results were applied to the flow behavior in the casting mold of a continuous caster.At the solid-liquid interface, entrapment of inclusions is greatly reduced by the existence of a low velocity flow,e.g.,0.05m/s.The above-mentioned interfacial flow velocity dependency of inclusion entrapment is considered to be largely influenced by changes in the thickness of the concentration boundary layer,which depend on the interfacial flow velocity.Specifically,bubbles and inclusions which enter the concentration boundary layer are drawn to the solid-liquid interface by a suction force which is several orders larger than the Saffman's force.In addition to the above-mentioned suction force,the so-called cleaning effect is determined by fluid-dynamic forces such as drag force,etc.which act on particles,and furthermore,by resident time of particles at the solid-liquid interface,which depends on the solidification rate.In a FC mold with a 2-stage electromagnetic brake,flotation of bubbles entrained in the jet flow from the nozzle is accelerated with the large DC magnetic field.This is attributed to the braking effect of the DC field on the nozzle jet and the upward flow by the buoyancy of the bubbles.As a result,the interfacial flow velocity can be normalized by increasing the strength of the magnetic field,and entrapment of large bubbles and inclusions can be reduced.

Endocsopic Decompression of Intermetatarsal Nerve (EDIN) for the Treatment of Morton’s entrapment— Multicenter Retrospective Review

Background: The authors present the results of a four surgeon, multicenter study of endoscopic decompression for the treatment of Civinini-Morton’s entrapment of 193 interspaces. Methods: A retrospective review of 193 interspaces, which were decompressed by four different surgeons. Results: The overall success rate was high, with 92% of patients reporting a good or fair outcome. Higher success rates were reported in the 3rd interspace decompression group (95%) compared to the 2nd interspace decompression group (85%). Complications reported were minimal, with only seven cases requiring a revision with traditional neurectomy. No revisions were required in the 3rd interspace decompression group. Five of the revisions were in the 2nd interspace decompression group, with the remaining two revisions in the combined 2nd/3rd interspace decompression group. Conclusions: Endoscopic decompression for the treatment of Civinnini-Morton’s entrapment is a safe and efficacious method of treatment of this nerve entrapment, with very low complication rates and rapid return to normal activity.

We encountered a popliteal artery entrapment syndrome (PAES) patient who was treated with stent placement for popliteal artery without correcting an abnormal muscular structure that compresses popliteal artery in another institution. The popliteal artery stent was occluded one year after the stent placement. The patient was treated with bypass graft by reversed saphenous vein. Interventionists should be cautious with stent placements for popliteal artery without adjusting the anomalous musculotendinous structure in patients with PAES.

Neuropathy is a common complication of diabetes mellitus(DM) with a wide clinical spectrum that encompasses generalized to focal and multifocal forms. entrapment neuropathies(EN), which are focal forms, are so frequent at any stage of the diabetic disease, that they may be considered a neurophysiological hallmarkof peripheral nerve involvement in DM. Indeed, EN may be the earliest neurophysiological abnormalities in DM,particularly in the upper limbs, even in the absence of a generalized polyneuropathy, or it may be superimposed on a generalized diabetic neuropathy. This remarkable frequency of EN in diabetes is underlain by a peculiar pathophysiological background. Due to the metabolic alterations consequent to abnormal glucose metabolism,the peripheral nerves show both functional impairment and structural changes, even in the preclinical stage,making them more prone to entrapment in anatomically constrained channels. This review discusses the most common and relevant EN encountered in diabetic patient in their epidemiological, pathophysiological and diagnostic features.

entrapment of middle cluneal nerves induces low back pain and leg symptoms. The middle cluneal nerves can become spontaneously entrapped where this nerve pass under the long posterior sacroiliac ligament. A case of severe low back pain, which was completely treated by release of the middle cluneal nerve, was presented. entrapment of middle cluneal nerves is possibly underdiagnosed cause of low-back and/or leg symptoms. Spinal surgeons should be aware of this clinical entity and avoid unnecessary spinal surgeries and sacroiliac fusion. This paper is to draw attention by pain clinicians in this unrecognized etiology.
