Using laparoscope to remove an ectopic intrauterine device in the anterior wall of urinary bladder:A case report

BACKGROUND An intrauterine device(IUD)is a contraceptive device placed in the uterine cavity and is a common contraceptive method for Chinese women.However,an IUD may cause complications due to placement time,intrauterine pressure and other factors.ectopic IUDs are among the most serious complications.ectopic IUDs are common in the myometrium and periuterine organs,and there are few reports of ectopic IUDs in the urinary bladder,especially in the anterior wall.CASE SUMMARY A 52-year-old woman was hospitalized due to a urinary bladder foreign body found via abdominal ultrasound and computed tomography(CT)examination.The patient had a 2-year history of recurrent abdominal distension and lower abdominal pain,accompanied by frequent urination,urgency,dysuria and other discomfort.Ultrasound examination revealed foreign bodies in the bladder cavity,with calculus on the surface of the foreign bodies.CT revealed a circular foreign body on the anterior wall of the urinary bladder,suggesting the possibility of an ectopic IUD.After laparoscopic exploration,an annular IUD was found in the anterior wall of urinary bladder,and an oval calculus with a diameter of approximately 2 cm was attached to the surface of the bladder cavity.The IUD and calculus were successfully and completely removed.The patient recovered well after surgery.CONCLUSION Abdominal ultrasound and CT are effective methods for detecting ectopic IUDs.The IUD is located in the urinary bladder and requires early surgical treatment.The choice of surgical method is determined by comprehensively considering the depth of the IUD in the bladder muscle layer,the situation of complicated calculus,the situation of intravesical inflammation and medical technology and equipment.

ectopic Overexpression of Wheat Adenosine Diphosphate-ribosylation Factor,TaARF,Increases Growth Rate in Arabidopsis

Differential gene expression between hybrids and their parents is considered to be associated with heterosis. However, the physiological functions and possible contribution to heterosis of these differentially expressed genes are unknown. We have isolated one hybrid upregulated gene encoding putative wheat ADP-ribosylation factor, designated TaARF. In this study, real-time quantitative reverse transcription-polymerase chain reaction analysis indicated that the TaARF transcript was preferentially expressed in root, node and crown, and the accumulation of TaARF mRNA in hybrid was more than 1.5-fold higher than that in two parents. In order to understand possible roles of the putative wheat ARF gene, TaARF was overexpressed in Arabidopsis, and the transgenic plants were characterized. We show that ectopic overexpression of TaARF in Arabidopsis leads to increased leaf area, increased growth rate and earlier transition to flowering, suggesting that TaARF plays significant roles in growth and development. This study provides evidence demonstrating that TaARF plays important roles in growth and development and we speculate that the upregulated expression of this gene might contribute to the heterosis observed in wheat root and leaf growth.

Endoscopic removal of gastric ectopic pancreas:An initial experience with endoscopic submucosal dissection

AIM:To evaluate the therapeutic usefulness and safety of endoscopic resection in patients with gastric ectopic pancreas.METHODS:A total of eight patients with ectopic pancreas were included.All of them underwent endoscopic ultrasonography before endoscopic resection.Endo-scopic resection was performed by two methods:endo-scopic mucosal resection(EMR)by the injection-and-cut technique or endoscopic mucosal dissection(ESD).RESULTS:We planned to perform EMR in all eight cases but EMR was successful in only four cases.In the other four cases,saline spread into surrounding normal tissues and the lesions becameattened,which made it impossible to remove them by EMR.Inthose four cases,we performed ESD and removed the lesions without any complications.CONCLUSION:If conventional EMR is difficult to remove gastric ectopic pancreas,ESD is a feasible alternative method for successful removal.

Introduction: Fibromyomas are benign tumours of frequent uterine location, most often found in black women, whose diagnosis of certainty is histological. The extra uterine location is rare and the physio-pathogenesis is poorly understood. This location poses diagnostic problems on imaging. Management is based on tumour resection. Observation: The authors report a clinical case of a 31-year-old woman who was seen for non-febrile hypogastric pain with a large abdominopelvic mass on physical examination. There was no particular history. The biology did not show any abnormality. Ultrasound with Doppler found a mass that was difficult to characterize, and was completed by a CT scan and a pelvic MRI. The management was surgical with an evolution marked by complications. Conclusion: ectopic leiomyoma is a rare condition. This location poses diagnostic problems on imaging. Management depends on the type of extra uterine presentation of the myoma.

Effect of rhBMP-2 sustained-release nanocapsules on the ectopic osteogenesis process in Sprague-Dawley rats

Objective:To explore the effect of sustained-release recombinant human bone morphogenetic protein-2(rhBMP-2) on ectopic osteogenesis in the muscle pouches of rats through preparing rhBMP-2 sustained-release capsules by wrapping morphogenesis protein bones-2(BMP-2)using chitosan nanoparticles,and compositing collagen materials.Methods:Twenty four SpragueDawley rats were randomly divided into four groups with six rats in each group,that is Group A(control group),Group B(only treated with collagen),Group C(rhBMP-2+collagen treated group) and Group D(rhBMP-2/cs+collagen treated group).The composite materials for each group were implanted in the bilateral peroneal muscle pouches in rats.The peroneal muscles were only separated without implanting any materials in control group.Rats were sacrificed 2 weeks and 4 weeks post treatment and samples were cut off for general observation,Micro CT scans and histological observation.Results:General observation showed no new bone formation in Groups A and B mice,while new bones were formed in Groups C and D mice.Two weeks after treatment Micro CT scans showed that The bone volume fraction(BVF),trabecular thickness(Tb. Th),bone mineral density(BMD) in Group C mice were all higher than that in Group D(Pectopic osteogenesis.Its bone formation effect is better than that of rhBMP-2 burst-release group.

Adrenocortical cancer(ACC)is an infrequent and often aggressive malignancy with a very poor prognosis.It can be classified as functional or nonfunctional.Nonfunctional ACC is hampered by the absence of specific signs or symptoms;only abdominal pain with or without incidental adrenal occupation is typically present.CASE SUMMARY We report a rare case of a patient with a 30 cm×15 cm×8 cm ectopic ACC on the anterior abdominal wall without organ adhesion.A 77-year-old male was admitted to our hospital because of a huge abdominal mass,which,by ultrasonography,had an unclear border with the liver.Computed tomography showed that the mass was not associated with any organ but was adherent to the anterior abdominal wall.The patient underwent tumor resection,and a postoperative pathology examination showed a neuroendocrine tumor,which was diagnosed as ACC.The patient was disease-free at the 9-mo follow up.CONCLUSION The anterior abdominal wall is a rare site of ACC growth.

Synergistic effect of RhBMP-2 and bFGF on ectopic osteogenesis in mice

Objective:To investigate the synergistic effect and mechanism of the combined application of recombinant human bone morphogenetic protein-2(rhBMP-2) and basic fibroblast growth factor(bFGF).Methods:24 KM male mice were randomly divided into 6 groups with 4 mice in each group,namely,Group A(control group),Group B(only treated with collagen),Group C(treated with2 ng bFGF+collagen).Group D(treated with 4μg rhBMP-2+collagen),Group E(treated with 4 μg rhBMP-2+2 ng bFGF+collagen) and Group F(treated with 4 μg rhBMP-2+4 ng bFGF+collagem.The composites were implanted into the intermuscular septum of hind legs mice:whereas in control group,intermuscular septum of mice was separated and no implantation was performed.General observation,detection of concentration of calcium content,micro computed tomography(Micro-CT).three-dimensional reconstruction scan.measurement of bone mineral density(BMD).bone volume fraction(BVF) and trabecular thickness(Tb.Th).as well as histological observation with HE staining and ALP and CD34 immumohistochemical staining were performed.Results:ectopic osteogenesis was found in Groups D.F and F mice.The difference in concentration of calcium contents was statistically significant between Groups D and E(P0.05).Micro-CT and three-dimensional reconstruction revealed continuous newborn bone substance in external surface of ectopic bone formation,and the center of bone formation did not show obvious substantial filling by bone substance.The differences in BMD,BVF and Tb.Th were statistically significant between Groups D and E or F(Pectopic bone formation,and the bone formation in Groups E and F was better than that in Group D.ALP and CD34 immumohistochemical staining revealed the positive expression mainly at the edge of ectopic bone formation,and area of positive expression in Groups E and F was larger than that in Groups D.Conclusions:r

Small cell lung cancer with panhypopituitarism due to ectopic adrenocorticotropic hormone syndrome: A case report

BACKGROUND Small cell lung cancer(SCLC)accounts for 15%of lung cancers,and it commonly expresses peptide and protein factors that are active as hormones.These secreting factors manifest as paraneoplastic disorders,such as ectopic adrenocorticotropic hormone(ACTH)syndrome(EAS).The clinical features are abnormalities in carbohydrate metabolism,hypokalemia,peripheral edema,proximal myopathy,hypertension,hyperpigmentation,and severe systemic infection.However,it is uncommon that EAS has an influence on hypothalamus-pituitary function.CASE SUMMARY A 62-year-old man presented with complaints of haemoptysis,polyuria,polydipsia,increased appetite,weight loss,and pigmentation.Following a series of laboratory and imaging examinations,he was diagnosed with SCLC,EAS,hypogonadism,hypothyroidism,and central diabetes insipidus.After three rounds of chemotherapy,levels of ACTH,cortisol,thyroid hormone,gonadal hormone,and urine volume had returned to normal levels.In addition,the pulmonary tumor was reduced in size.CONCLUSION We report a rare case of SCLC complicated with panhypopituitarism due to EAS.We hypothesize that EAS induced high levels of serum glucocorticoid and negative feedback for the synthesis and secretion of antidiuretic hormone from the paraventricular nucleus,and trophic hormones from the anterior pituitary.Therefore,patients who present with symptoms of hypopituitarism,or even panhypopituitarism,with SCLC should be evaluated for EAS.

Five different cases of ectopic testes in children:a self-experience with literature review

Background Empty scrotum may be due to many reasons;besides true undescended testis these are monorchia,retractile testis,atrophy of testis due to torsion and ectopic testis.The location of testis in an aberrant site after passing through the inguinal canal and leaving it via external ring is a condition called ectopic testis.The most common aberrant locations of ectopic testes are superficial inguinal pouch(Denis Browne pouch),within anterior abdominal wall,perineum,femoral canal,contralateral scrotum(transverse testicular ectopia)and prepenile area.Methods Description of management of five different cases of ectopic testes over a period of 10 years’experience in pediatric surgery have been done with review of the literature about this rarity.Results Five different cases of ectopic testes were as follows:infants 3 months of age with right ectopic testis in anterior abdominal wall near anterior superior iliac spine,4 months of age with left perineal testis,6 months of age with an ectopic femoral testis on left side,8 months of age with transverse testicular ectopia on left side and 10 months of age with right true undescended testis and left perineal testis.Conclusions Search for ectopic sites of testes should always be in mind during examination of patients with empty scrotum.Early surgical repair of ectopic testes is recommended,as there is no chance of spontaneous correction to the normal location.Long-term follow-up is necessary due to relatively high incidence of infertility and malignant transformation.Surgeons should be aware of the rare possibility of transverse testicular ectopia in cases of inguinal hernia with impalpable testis on the contralateral side.

Cystic jejunal duplication with Heinrich's type Ⅰ ectopic pancreas, incidentally discovered in a patient with pancreatic tail neoplasm

The aim of this study was to present a case of enteric duplication cyst and criteria for a proper differential diagnosis. A 51-year-old male was hospitalized for pancreatic tail neoplasm and distal pancreatectomy with splenectomy was performed. During surgery, a jejunal cystic lesion was incidentally detected and jejunectomy was performed. Microscopically, the cyst was observed to be covered by Keratin 7/Keratin 20 positive intestinal type epithelium and the muscularis layer was shared by the cyst and adjacent jejunum, without a cleavage plane between the cyst wall and jejunal muscularis propria. In the deep muscularis propria, a Heinrich's type Ⅰ ectopic pancreas was also noted. In the pancreatic tail, a low grade intraepithelial lesion(panIN-1a) was diagnosed. This case highlights the necessity for a correct differential diagnosis of such rare lesions. Roughly 30 cases of jejunal duplication cysts have been reported to date in the PubM ed database.
