COMPARISON OF SHORT- AND MID-TERM OUTCOMES BETWEEN CYPHER AND TAXUS stentS IN PATIENTS WITH COMPLEX LESIONS OF THE CORONARY ARTERIES

Background drug-eluting stent （DES） could obviously reduce in-stent restenosis, which has been proved by international multi-center clinical trials. However, the types of the lesions for stenting were highly selected in these trials. Up to now, there has been no large scale study on the effect of DES in treating complex lesions in real world. Although REALITY trial was just reported during American College of Cardiology Congress 2005, the entry criteria for lesions were limited to one or two de novo lesions. This study was conducted to compare the short- and mid-term clinical outcomes between sirolimus-eluting stent （CYPHER stent） and paclitaxel-eluting stent （TAXUS stent） in patients with complex lesion. Methods This is a retrospective study. From April 2002 to June 2004, a total of 1061 patients were treated with DES in Fu Wai Hospital, of which, 611 patients （642 lesions with 698 CYPHER stents） were in CYPHER group, and 450 patients （534 lesions with 600 TAXUS stents） were in TAXUS group. There was no significant difference in clinical data and lesion types between CYPHER group and TAXUS group. Results Success rates of stent implantation were 99.2% and 98.8% in CYPHER and TAXUS stent groups respectively. The major adverse cardiac events （MACE） during in-hospital and 6-8-month follow-up were 0.7% and 2.3% in CYPHER stent group versus 1.3% and 3.2% in TAXUS stent group. There was no significant difference in MACE rate between these two groups. Restenosis rate was a little higher in TAXUS stent group than that in CYPHER stent group （14.0% vs 7.3%）, but there was no significant difference. The incidence of acute occlusion of side branch after implanting DES in main vessel was 6.9% in CYPHER group and 11.9% in TAXUS group （P〈0.05） . Conclusions CYPHER and TAXUS DES were safe and effective in patients with complex lesion. Clinical outcomes of CYPHER stent were better than TAXUS stent in bifurcation lesions. As we known, drug-eluting stents (DES) reduce the risk of restenosis and represent an important advance in coronary intervention. Newer-generation DES with thin struts releasing limus-family drugs from durable or biodegradable polymers have further improved clinical outcomes, as compared with early-generation DES releasing sirolimus or paclitaxel. The risk of stent thrombosis has become exceedingly low, that is, the improved safety profile of newer DES comes without compromising their effectiveness.

A Polymer Coated Cicaprost-eluting stent Increases Neointima Formation and Impairs Vessel Function in the Rabbit Iliac Artery

drug-eluting stents have been successful in reducing in-stent restenosis but are not suitable for all lesion types and have been implicated in causing late stent thrombosis due to incomplete regeneration of the endothelial cell layer. In this study we implanted stents coated with cicaprost, a prostacyclin analogue with a long plasma half-life and antiproliferative effects on vascular smooth muscle cells, into the iliac arteries of rabbits. At 28-day follow-up we compared neointima formation within the stented vessels and vascular function in adjacent vessels, to assess if cicaprost could reduce restenosis without impairing vessel function. Arteries implanted with cicaprost eluting stents had significantly more neointima compared to bare metal stents. In adjacent segments of artery, endothelium-dependent relaxation was impaired by the cicaprost-eluting stent but vasodilation to an endothelium-independent vasodilator was maintained. We conclude that the presence of the polymer and sub-optimal release of cicaprost from the stent may be responsible for the increased neointma and impaired functional recovery of the endothelium observed. Further experiments should be aimed at optimising release of cicaprost and exploring different stent polymer coatings.

Comparison the long-term clinical outcomes of Nano polymer-free sirolimus-eluting stent versus Endeavor durable polymer zotarolimus-eluting stent in patients with acute coronary syndrome

Background Durable polymer drug eluting stent （DES） is confronted with many issues, especially the inci- dence of late stent thrombosis （ST） which is mainly due to delayed healing and re-endothelization by the durable polymer coating. Newer polymer-free DES might have a reduction in late stent thrombosis. Therefore, the aim of this study was to evaluate the efficacy and safety of two different drug eluting stents in patients with acute coro- nary syndromes （ACS） by one year follow-up. Methods This study assessed the results of the Nano polymer- free SES （Lepu Medical Technology, Beijing, China） and Endeavor durable-polymer ZES （Medtronic, Minneapo- lis, MN, USA） in ACS patients by clinical follow-up and coronary angiography analysis. The primary endpoint was the onset of any major adverse cardiac events （MACE） such as cardiac death, myocardial infarction （MI） and target vessel revascularization （TVR）. The secondary endpoints were target lesion revascularization （TLR） and stent thrombosis （ST） at the end of one year follow-up. Results Between April 2014 and June 2015, a total of 513 consecutive patients were enrolled in this trial. There were 259 patients enrolled in Nano group and 253 pa- tients in Endeavor group. Mean age was 62.8 ＋10.3 years old （range： 28 to 87 years of age）, and 65.3% of pa- tients were male. Compared Nano-SES to Endeavor-ZES, the incidence of MACE was 5.4% vs. 7.1%. The hazard ratio （HR） was 1.32 with 95% confidence interval （CI） as 0.64-2.72 （P = 0.45）. Secondary end points showed TLR （1.9% vs. 3.2%; HR, 1.42; 95% CI, 0.45-4.55; P = 0.55） and ST （0.4% vs. 0.8%; HR, 2.03; 95% CI, 0.18- 5.37; P = 0.99）, in the one-year period of follow-up. Conclusion Within one-year, the Nano polymer-free SES has similar safety and efficacy compared with the Endeavor durable-polymer ZES in the treatment of patients with ACS.

Domestic versus imported drug-eluting stents for the treatment of patients with acute coronary syndrome

BACKGROUND:The application of coronary stents,especially drug-eluting stents(DESs),has made percutaneous coronary intervention(PCI) one of important therapeutic methods for CHD. DES has reduced the in-stent restenosis to 5%–9% and signifi cantly improved the long-term prognosis of patients with CHD. The study aimed to investigate the long-term eff icacy and safety of domestic drugeluting stents(DESs) in patients with acute coronary syndrome(ACS).METHODS:All patients with ACS who had undergone successful percutaneous coronary intervention(PCI) in the First Aff iliated Hospital of Zhengzhou University from July 2009 to December 2010 were included in this study. Patients were excluded from the study if they were implanted with bare metal stents or different stents(domestic and imported DESs) simultaneously. The included patients were divided into two groups according to different stents implanted:domestic DESs and imported ***:In the 1 683 patients of this study,1 558(92.6%) patients were followed up successfully for an average of(29.1±5.9) months. 130(8.3%) patients had major adverse cardiovascular events(MACEs),including cardiac death in 32(2.1%) patients,recurrent myocardial infarction in 16(1%),and revascularization in 94(6%). The rates of cardiac death,recurrent myocardial infarction,revascularization,in-stent restenosis,stent thrombosis and other MACEs were not signif icantly different between the two groups(all P>0.05). Multivarite logistic regression revealed that diabetes mellitus(OR=1.75,95%CI:1.09–2.82,P=0.021),vascular numbers of PCI(OR=2.16,95%CI:1.22–3.83,P=0.09) and PCI with left main lesion(OR=9.47,95%CI:2.96–30.26,P=0.01) were independent prognostic factors of MACEs. The Kaplan-Meier method revealed that there was no significant difference in cumulative survival rates and survival rates free from clinical events between the two groups(all P>0.05).CONCLUSIONS:The incidences of clinical events and cumulative survival rates are not statistically

Coronary stenting:A matter of revascularization

In the last few decades, the recommended treatment for coronary artery disease has been dramatically improved by percutaneous coronary intervention(PCI) and the use of balloon catheters, bare metal stents(BMSs), and drug-eluting stents(DESs). Catheter balloons were burdened by acute vessel occlusion or target-lesion restenosis. BMSs greatly reduced those problems holding up the vessel structure, but showed high rates of instent re-stenosis, which is characterized by neo-intimal hyperplasia and vessel remodeling leading to a renarrowing of the vessel diameter. This challenge was overtaken by first-generation DESs, which reduced restenosis rates to nearly 5%, but demonstrated delayed arterial healing and risk for late in-stent thrombosis, with inflammatory cells playing a pivotal role. Finally, new-generation DESs, characterized by innovations in design, metal composition, surface polymers, and antiproliferative drugs, finally reduced the risk for stent thrombosis and greatly improved revascularization outcomes. New advances include bioresorbable stents potentially changing the future of revascularization techniques as the concept bases upon the degradation of the stent scaffold to inert particles after its function expired, thus theoretically eliminating risks linked with both stent thrombosis and re-stenosis. Talking about DESs also dictates to consider dual antiplatelet therapy(DAPT), which is a fundamental moment in view of the good outcome duration, but also deals with bleeding complications. The better management of patients undergoing PCI should include the use of DESs and a DAPT finely tailored in consideration of the potentially developing bleeding risk in accordance with the indications from last updated guidelines.
