The application of laparoscopic doppler ultrasound during laparoscopic varicocelectomy in infertile men

The aim of this study is to evaluate the benefits of laparoscopic doppler ultrasound （LDU） application during laparoscopic varicocelectomy （LV）, and to compare the surgical outcomes and complications between LDU-assisted LV （LDU-LV） and conventional LV for infertile patients with varicoceles; 147 infertile patients were randomly divided into two groups. Operative and postoperative parameters, semen parameters, and the pregnancy rate were compared. There were no differences in baseline demographics. The operative time was significantly longer in LDU-LV group than LV group. The incidence of postoperative hydrocele was 1.4% （1/72） in LDU-LV group versus 10.7% （8/75） in LV group, which showed a significant difference （P〈 0.05）. However, other surgical outcomes, such as postoperative hospital stay, postoperative recurrence, and testicular atrophy, were similar between the two groups. Sperm concentration and sperm motility were significantly increased in both groups at 3, 6, and 12 months after surgery （P 〈 0.01）, and they were higher in LDU-LV than LV group in 12 months after surgery （34.21 ± 6.36 vs 29.99 ± 6.04 for concentration, P〈 0.05; 40.72±8.12 vs 37.31 ± 6.12 for motility, P〈 0.05）. Sperm morphology was comparable between the two groups. The pregnancy rate showed no significant difference （44.4% of the LDU-LV vs 37.3% of the LV, P〉 0.05）. In conclusion, compared with LV, LDU-LV could safely and effectively ligate all spermatic veins and preserve spermatic arteries without leading to high varicocele recurrence and postoperative hydrocele. Given the benefits that sperm counts as well as sperm motility favoring LDU-LV, we recommend that LDU should be routinely used as an effective tool to improve outcomes and safety of laparoscopic varicocelectomy.

BACKGROUND： The early correct diagnosis of acute cerebral infarction （ACI） is very important for choosing therapeutic regimen. The classification of Oxfordshire community stroke project （OCSP） provides guide for easy and rapid diagnosis and choosing therapeutic regimen in clinical practice. But current operation is not satisfied. Only depending on clinical symptoms and body signs do objective evidences lack. Transcranial doppler ultrasound （TCD） examination may provide hemodynamical evidences for the correct classification of OCSP to some extent. OBJECTIVE： To compare the results of OCSP classification and TCD examination for treating ACI in patients and analyze the relationship between them as well as the effect of TCD on the correct classification of OCSP. DESIGN ： Controlled observation.SETTING： Department of Internal Medicine, Shenzhen Hospital of Prevention and Treatment for Occupationa Disease. PARTICIPANTS： Thirty-eight inpatients with ACI including 21 males and 17 females, aged 50 to 81 years, who received treatment in the Department of Internal Medicine, Shenzhen Hospital of Prevention and Treatment for Occupational Disease within 24 hours after onset from October 2002 to October 2005 were involved. The involved inpatients all confirmed to the diagnostic criteria of ACI formulated in the 4^th National Cerebrovascular Diseases Conference. All the cases received skull computer tomography （CT） or magnetic resonance imaging （MRI） examinations at 24 hours after onset. Those patients who had no obvious infarct focus in skull CT examination within 24 hours after onset were performed re-examination to verify ACI within 72 hours after onset. Informed consents of examination method were obtained from all the patients. Use of doppler ultrasound to predict need for transjugular intrahepatic portosystemic shunt revision

Detecting doppler ultrasound embolic signals using the wavelet-based feature extraction algorithm

The non-invasive detection of circulating emboli with the doppler ultrasound technique is of active significance in clinical applications. In order to eliminate drawbacks of artifacts brought by the movement of probes or patients and detect emboli accurately, relevant feature parameters are extracted from two angles of the wavelet transform of doppler signals. The singularity of the signal waveform is analyzed based on its wavelet scalogram; then transverse and longitudinal parameters are extracted to represent the scalogram characteristics. A novel method is proposed based on the adaptive wavelet packet basis, from which several parameters such as the energy, the scale, etc. are extracted to represent the optimized signal approximation features. With all feature parameters in two aspects, a classification system is established for doppler ultrasound embolic signals by solving the generalized Fisher discriminant plane. From experiments on 300 simulated and 298 clinical doppler ultrasound signals of cerebral arteries, it is shown that the proposed system can achieve the emboli detection rates of 99.0% and 98.5% for the training set and the testing set respectively. Therefore this method makes an improve- ment of emboli detection compared to traditional in the automatic detection of clinical emboli. methods and has the possibility to be applied

Assessment of voluntary rhythmic muscle contraction-induced exercising blood flow variability measured by doppler ultrasound

Given recent technological developments, ultrasound doppler can provide valuable measurements of blood velocity/flow in the conduit artery with high temporal resolution. In human-applied science such as exercise physiology, hemodynamic measurements in the conduit artery is commonly performed by blood flow feeding the exercising muscle, as the increase in oxygen uptake (calculated as a product of arterial blood flow to the exercising limb and the arterio-venous oxygen difference) is directly proportional to the work performed. The increased oxygen demand with physical activity is met through a central mechanism, an increase in cardiac output and blood pressure, as well as a peripheral mechanism, an increase in vascular conductance and oxygen extraction (a major part of the whole exercising muscles) from the blood. The increase in exercising muscle blood flow in relation to the target workload (quantitative response) may be one indicator in circulatory adjustment for the ac- tivity of muscle metabolism. Therefore, the determination of local blood flow dynamics (potential oxygen supply) feeding repeated (rhythmic) muscle contractions can contribute to the understanding of the factors limiting work capacity including, for instance, muscle metabolism, substance utilization and magnitude of vasodilatation in the exercising muscle. Using non-invasive measures of pulsed doppler ultrasound, the validity of blood velocity/flow in the forearm or lower limb conduit artery feeding to the muscle has been previously demonstrated during rhythmic muscle exercise. For the evaluation of exercising blood flow, not only muscle contraction induced internal physiological variability, or fluctuations in the magnitude of blood velocity due to spontaneous muscle contraction and relaxation induced changes in force curve intensity, superimposed in cardiac beat-by-beat, but also the alterations in the blood velocity (external variability) due to a temporary sudden change in the achieved workloa

A Fertility Assessment of Infertile Men with Varicocele by Means of doppler ultrasound

The diameters of spermatic veins and blood reflux in sixty-two male patients with infertility caused by varicocele were assessed with doppler ultrasound in this study. Scrotal thermograph, testes volume and semen analysis were also applied to analysis of these patients. Among sixty-two patients, thirty-one received high ligation of internal spermatic vein. The results showed: 1. The diameter of spermatic veins was negatively correlated with fertility index and positively correlated with doppler grading of blood reflux. 2. In operative patients, the effect of surgery was better in patients with blood reflux than in those without blood reflux. It was suggested the diameter of spermatic veins probably reflects the duration and serious degree of blood reflux in spermatic vein, and may be used for assessment of severity of male infertility in combination with semen analysis.

Given the recent technological developments, ultra-sound doppler can provide valuable measurements of arterial blood flow with high temporal resolution. In a clinical setting, measurements of hemodynamics is used to monitor, diagnose and manage changes in blood velocity profile for cardiac valve disease, relatively large vessel stenosis and other cardiovascular diseases. In health science and preventive medicine for cardiovascular disease with exercise therapy, evaluation of cardiac and vascular function is a useful indicator not only at rest but also during exercise, leading to improved exercise tolerance as well as physical activity. During exercise, the increase in oxygen uptake (calculated as product of arterial blood flow to the exercising limb and the arteriovenous oxygen difference) is directly proportional to the work performed. The increased oxygen demand is met through a central mechanism, an increase in cardiac output and blood pressure, as well as a peripheral mechanism, an increase in vascular conductance and oxygen extraction (major part in the whole exercising muscles) from the blood. Therefore, the determination of the local blood flow dynamics (potential oxygen supply) feeding to rhythmic muscle contractions can contribute to the understanding of the factors limiting the work capacity including, for instance the muscle metabolism, substance utilization and vasodilatation in the exercising muscle. Using non-invasive measures of pulsed doppler ultrasound the validity of evaluating blood velocity/flow in the fore- arm or lower limb conduit artery feeding to the mus- cle is demonstrated during rhythmic muscle exercise;however the exercising blood velocity profile (fast Fourier transformation) due to muscle contractions is always seen as a physiological variability or fluctuations in the magnitude in blood velocity due to the spontaneous muscle contraction and relaxation induced changes in force curve intensity. A wavelet space based approach for doppler ultrasound blood signals separation

In medical doppler ultrasound systems, a high-pass filter which is usually employed to filter wall clutter components, will remove the information of the low velocity blood flow. To extract intact doppler ultrasound blood signals, a novel approach is proposed based on the spatially selective noise filtration. The wall signals are firstly estimated by the spatially selective noise filtration from wavelet spatial correlation property. Then the wall clutters are exactly obtained by a wavelet threshold de-noising technique which eliminates the residual blood flow signals. Finally the intact blood flow signals are achieved by subtracting the wall signals from the mixed signals. This approach is applied to both computer simulated and in vivo carotid doppler ultrasound signals. The experiment results show that the wavelet space based approach can exactly extract the blood flow signals, and achieve about 45% lower results in the mean absolute error than that of the high-pass filtering. This approach is expected to be an effective method to remove the wall clutters in doppler ultrasound systems.

Identification of syndrome X using intravascular ultrasound imaging and doppler flow mapping

Background The purpose of this study was to assess the morphological changes and physiological function of coronary arteries in patients presenting with chest pain but having normal coronary angiograms, using intravascular ultrasound imaging (IVUS) and intracoronary doppler (ICD) flow measurements, in order to elucidate the mechanism of syndrome X. Methods A total of 126 patients [67 males, 59 females, mean age (53.1±13.0) years] who experienced chest pain but had normal coronary angiograms were included in this study. ICD flow measurements of the left anterior descending coronary artery (LAD) were performed using a Cardiometrics FloMap Ⅱ system. Coronary flow velocity reserve (CFVR) was defined as the ratio of the average peak velocity during hyperemia to that at baseline, induced by an intracoronary bolus injection of 18 μg adenosine. A 3.2F or 2.9F 30 MHz mechanical rotating ultrasound catheter (CVIS, Boston Scientific) or a 3.0F 20MHz electronic ultrasound catheter (Endosonics) was used for IVUS. Results The mean CFVR value of the LAD was 2.71±0.74. Reduction of CFVR (<3.0) was found in 82 of 126 (65.1%) patients. IVUS images of the LAD were available for 109 patients. Plaque formation was detected in 76/109 (69.7%) patients. Based on the presence or absence of plaque formation as well as the reduction or non-reduction of CFVR, patients were divided into four groups: Group Ⅰ (n=10), normal IVUS findings and normal CFVR; Group Ⅱ (n=23), normal IVUS findings with reduction in CFVR; Group Ⅲ (n=29), IVUS evidence of plaque formation but normal CFVR; and Group Ⅳ (n=47), IVUS evidence of plaque formation with reduction in CFVR. Conclusion This study shows the important clinical value of a combination of IVUS and ICD in diagnosing patients with angiographically normal coronary arteries. Only 10% of patients studied (Group Ⅰ) were found to be truly free of coronary disease, while 20% of patients (Group Ⅱ) would be diagnosed as suffering from syndrome X.

doppler study of hepatic vein in cirrhotic patients:Correlation with liver dysfunction and hepatic hemodynamics

AIM: To elucidate the significance of doppler measurements of hepatic vein in cirrhotic patients and to correlate with liver dysfunction and hepatic hemodynamics. METHODS: One hundred patients with liver cirrhosis and 60 non-cirrhotic controls were studied. doppler waveforms were obtained from right hepatic vein and flow velocity measured during quiet respiration. doppler measurements were also obtained from portal trunk, right portal vein and proper hepatic artery. RESULTS: Hepatic vein waveforms were classified into three classical patterns. Flat waveform was uncommon. Mean hepatic vein velocity was significantly higher in cirrhotic patients (12.7 ± 6.4 vs 5.1 ± 2.1 and 6.2 ± 3.2 cm/s; P doppler waveforms of hepatic vein, which is independent of liver dysfunction, should be obtained during normal respiration. Mean hepatic vein velocity reflects the change in hepatic circulation associated with progression of liver cirrhosis. It can be used as a new parameter in the assessment of liver cirrhosis.
