Dear editor,A recent study based on the use of experimental artificial intelligence(AI)tool showed 70%–80%accuracy in predicting development of severe disease in coronavirus disease 2019(COVID-19)based on predictive parameters alanine aminotransferase(ALT),myalgia and hemoglobin,whilst only 5 of 53 patients developed acute respiratory distress syndrome(ARDS),2 of whom reporting myalgia.

Dear Editor,Since last December,the outbreak of 2019-nCoV in Wuhan has caused ever-increasing attention and public panic all over the world.Up to February 9,2020,40,171 patients had been diagnosed with 2019-nCoV infection,including 6484(16.14%)severe cases and 908 deaths(2.27%).Compared to SARS and MERS,2019-nCoV appears to be much more contagious but less lethal,as most patients have mild symptoms and good prognosis1,2.However,according to the Chinese government’s daily report,13.2–21.3%of patients with 2019-nCoV infection developed into severe or fatal illness(Fig.S1a),which is characterized by the rapid development to acute respiratory distress syndrome(ARDS)or septic shock.Along with an increasing number of confirmed cases,the cumulative total of severe patients with 2019-nCoV is growing(Fig.S1b).Treatment of these critically ill patients is becoming one of the major challenges we are facing.

Tuberculosis(TB)continues to be one of the major infectious diseases threatening millions of lives worldwide.[1]We encountered a 52-day-old male infant with a anterior mediastinal mass caused by tuberculosis.On admission to our hospital,the infant presented with respiratory distress,wheezing,fever,and cough.Decreased breath sounds were audible at the apex of the left chest.His temperature was 38.2℃.

Since severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)emerged,we have followed this pandemic with great interest and concern from Europe(Navarra-Spain)and the United States(Boston-Massachusetts).We were expecting children to present to emergency departments with the“classical COVID-19 symptoms-triad”such as fever,cough,and a range of respiratory distress[1]and we had learned from countries affected earlier by the virus,that it appeared to be less common and relatively mild in children[2,3].

Dear Editor,As the outbreak of coronavirus disease 2019(COVID-19)turns into a pandemic,it has literally caused a worldwide public health crisis.Progressive lymphopenia,especially in T cells,was a prominent clinical feature of severe COVID-19 in addition to dyspnea,hypoxemia,acute respiratory distress,and cytokine release syndrome.1 Recently,several studies revealed a correlation between T cell depletion and increased expression levels of several inhibitory checkpoint molecules on T cells in severe COVID-19 cases.2 Classically,inhibitory checkpoint molecules have been documented as key factors for regulating T cell exhaustion in a variety of chronic viral infections and tumors.Recent studies further implied a pivotal role of inhibitory checkpoint molecules in the pathophysiology of acute viral infections,such as Ebola virus or hantavirus infection.Of note,soluble isoforms of checkpoints can be produced by cleavage of membrane-bound proteins or by alternative splicing of mRNA and competitively regulate the functions of their membrane-bound counterparts.3 Thus it is of great interest to determine whether soluble checkpoint molecules are involved in immune regulation and severity of COVID-19.

Objective:To evaluate the efficacy and safety of protective lung ventilation strategy combined with lung recruitment maneuver (RM) in the treatment patients with acute respiratory distress syndrome (ARDS).Methods:Totally 74 patients with ARDS admitted to the Department of Intensive Care Unit, Changshu Second People's Hospital in Jiangsu Province between September 2010 and June 2013 were selected and randomly divided into lung recruitment group and non-lung recruitment group, and the initial ventilation solution for both groups was synchronized intermittent mandatory ventilation (SIMV). For RM, SIMV mode (pressure control and pressure support) was adopted. Positive end expiratory pressure (PEEP) was increased by 5 cm H2O every time and maintained for 40-50 s before entering the next increasing period, and the peak airway pressure was kept below 45 cm H2O. After PEEP reached the maximum value, it was gradually reduced by 5 cm H2O every time and finally maintained at 15 cm H2O for 10 min.Results:A total of 74 patients with mean age of (49.0±18.6) years old were enrolled, 36 patients were enrolled in lung recruitment maneuver (RM) group and 38 patients were enrolled into non-lung recruitment maneuver (non-RM) group. 44 were male and accounted for 59.5% of all the patients. For the indicators such as PEEP, pressure support (PS), plateau airway pressure (Pplat), peak airway pressure (Ppeak), vital capacity (VC) and fraction of inspired oxygen (FiO2), no statistical differences in the indicators were found between the RM group and non-RM group on D1, D3 and D7 (P>0.05), except that only FiO2 of RM group on D7 was significantly lower than that of non-RM group (47.2±10.0) vs. (52.2±10.5),P<0.05]. For the indicators of blood gas analysis, including pH, arterial oxygen pressure (PaO2), arterial carbon dioxide pressure (PaCO2) and oxygenation index (PaO2/FiO2), PaO2 and PaO2/FiO2 of RM group were significantly higher than those of non-RM group on D7, and the values were [(90.2±16.1) mmHg vs. (76.4±11.3) mmHg,P<0.05] and [(19
