Summary What is already known about this topic?The literature has consistently demonstrated that Distress rumination following a traumatic event has significant implications for mental health.However,the potential association between Distress rumination and suicidality,as well as the underlying mechanisms driving this relationship,remains to be elucidated.What is added by this report?The current study demonstrated a significant,positive correlation between Distress rumination and suicidal ideation in college students who have encountered traumatic events.The findings indicate that somatic anxiety serves as a mediator between Distress rumination and suicidal ideation.What are the implications for public health practice?Interventions aimed at reducing somatic anxiety may contribute to a decrease in suicidal ideation.Assessing and addressing somatic anxiety symptoms in college students experiencing Distressful rumination following traumatic events could potentially lower the risk of suicide.

Neonatal respiratory Distress syndrome(RDS),also known as hyaline membrane disease,is a leading cause of neonatal mortality in the US,which affects 1%of all newborns and 10%of preterm babies[1,2].It is also one of the most common causes of admission to neonatal intensive care unit[3].Preeclampsia(PE)is a common pregnancy complication that affects an estimated 5%of pregnancies across the world[4].It is a significant cause of low birth weight and preterm delivery,which are risk factors for RDS[5].However,in the literature,the relationship between PE and RDS remains controversial[6-9].

Mesenchymal stromal cells(MSCs)have been considered a promising alternative for treatment of acute respiratory Distress syndrome(ARDS).However,there is significant heterogeneity in their therapeutic efficacy,largely owing to the incomplete understanding of the mechanisms underlying the therapeutic activities of MSCs.Here,we hypothesize that the cholinergic antiinflammatory pathway(CAP),which is recognized as a neuroimmunological pathway,may be involved in the therapeutic mechanisms by which MSCs mitigate ARDS.Using lipopolysaccharide(LPS)and bacterial lung inflammation models,we found that inflammatory cell infiltration and Evans blue leakage were reduced and that the expression levels of choline acetyltransferase(ChAT)and vesicular acetylcholine transporter(VAChT)in lung tissue were significantly increased 6 hours after MSC infusion.When the vagus nerve was blocked orα7 nicotinic acetylcholine(ACh)receptor(α7nAChR)-knockout mice were used,the therapeutic effects of MSCs were significantly reduced,suggesting that the CAP may play an important role in the effects of MSCs in ARDS treatment.Our results further showed that MSC-derived prostaglandin E2(PGE2)likely promoted ACh synthesis and release.Additionally,based on the efficacy of nAChR andα7nAChR agonists,we found that lobeline,the nicotinic cholinergic receptor excitation stimulant,may attenuate pulmonary inflammation and alleviate respiratory symptoms of ARDS patients in a clinical study(ChiCTR2100047403).In summary,we reveal a previously unrecognized MSC-mediated mechanism of CAP activation as the means by which MSCs alleviate ARDS-like syndrome,providing insight into the clinical translation of MSCs or CAP-related strategies for the treatment of patients with ARDS.

BACKGROUND: Acute respiratory Distress syndrome (ARDS) causes substantial mortalities.Alveolar epithelium is one of the main sites of cell injuries in ARDS. As an important kind of microRNAs(miRNAs), microRNA-145 (miR-145) has been studied in various diseases, while its role in ARDS has notbeen investigated.METHODS: Lipopolysaccharide (LPS) was intratracheally instilled to establish a rat ARDS model.Cytokines from bronchoalveolar lavage fl uid (BALF) were measured using rat tumor necrosis factor-α andinterleukin-6 enzyme-linked immunosorbent assay kits (R&D Systems), and the pathological structureswere evaluated using hematoxylin and eosin (H&E) staining and transmission electron microscope;thelung miR-145 messenger RNA (mRNA) was detected using quantitative polymerase chain reaction.Bioinformatics focused on the target genes and possible pathways of gene regulation.RESULTS: A rat model of LPS-induced ARDS was successfully established. The miR-145was down-regulated in the LPS-induced ARDS lung, and mitochondrial dysfunction was observedin alveolar epithelial cells, most obviously at 72 hours after LPS. TargetScan and miRDB databaseswere used to predict the target genes of miR-145. A total of 428 overlapping genes were identifi ed,seven genes were associated with mitochondrial function, and Ogt, Camk2d, Slc8a3, and Slc25a25were verifi ed. Kyoto Encyclopedia of Genes and Genomes (KEGG) pathways were enriched in themitogen-activated protein kinase (MAPK) signaling pathway, and Gene Ontology (GO) biologicalprocess was mainly enriched in signal transduction and transcription regulation.CONCLUSIONS: The miR-145 is down-regulated in LPS-induced ARDS, and affects itsdownstream genes targeting mitochondrial functions.

Acute respiratory Distress syndrome(ARDS)was reported for the first time in 1967 in 12 patients with sudden respiratory failure due to a non-cardiogenic pulmonary edema.[1]Even though these patients had no underlying pulmonary disease,they rapidly developed severe hypoxemia,stiff lungs,and pulmonary bilateral infiltrates within a few days after a precipitating factor.Autopsy revealed a characteristic histological pattern of diffuse alveolar damage involving hyaline membranes,edema,necrosis,and cell proliferation.[2,3]The definition of ARDS evolved markedly over time and the Berlin definition,which was proposed in 2012,is currently the most recent one.[4]First,acute onset of respiratory symptoms appearing or worsening within 7 days of a clinical insult,thereby excluding patients who develop respiratory failure over more prolonged periods due to idiopathic pulmonary fibrosis,non-specific interstitial pneumonitis,organizing pneumonia,or pulmonary vasculitis.

Empathy is traditionally thought to be a unique ability of humans to feel, understand, and share the emotional state of others. However, the notion has been greatly challenged by the emerging discoveries of empathy for pain or Distress in rodents. Because empathy is believed to be fundamental to the formation of prosocial, altruistic, and even moral behaviors in social animals and humans, studies associated with decoding the neural circuits and unraveling the underlying molecular and neural mecha- nisms of empathy for pain or Distress in rodents would be very important and encouraging. In this review, the author set out to outline and update the concept of empathy from the evolutionary point of view, and introduce up-to-date advances in the study of empathy and its neural correlates in both humans and rodents. Finally, the author highlights the perspectives and challenges for the further use of rodent models in the study of empathy for pain or Distress.

Background The coronavirus disease 2019(COVID-19)pandemic has had a profound and prolonged impact on healthcare services and healthcare workers.Aims The Australian COVID-19 Frontline Healthcare Workers Study aimed to investigate the severity and prevalence of mental health issues,as well as the social,workplace and financial disruptions experienced by Australian healthcare workers during the COVID-19 pandemic.Methods A nationwide,voluntary,anonymous,single timepoint,online survey was conducted between 27 August and 23 October 2020.Individuals self-identifying as frontline healthcare workers in secondary or primary care were invited to participate.Participants were recruited through health organisations,professional associations or colleges,universities,government contacts and national media.Demographics,home and work situation,health and psychological well-being data were collected.Results A total of 9518 survey responses were received;of the 9518 participants,7846(82.4%)participants reported complete data.With regard to age,4110(52.4%)participants were younger than 40 years;6344(80.9%)participants were women.Participants were nurses(n=3088,39.4%),doctors(n=2436,31.1%),allied health staff(n=1314,16.7%)or in other roles(n=523,6.7%).In addition,1250(15.9%)participants worked in primary care.Objectively measured mental health symptoms were common:mild to severe anxiety(n=4694,59.8%),moderate to severe burnout(n=5458,70.9%)and mild to severe depression(n=4495,57.3%).Participants were highly resilient(mean(SD)=3.2(0.66)).Predictors for worse outcomes on all scales included female gender;younger age;pre-existing psychiatric condition;experiencing relationship problems;nursing,allied health or other roles;frontline area;being worried about being blamed by colleagues and working with patients with COVID-19.Conclusions The COVID-19 pandemic is associated with significant mental health symptoms in frontline healthcare workers.Crisis preparedness together with policies and practices addressing psychological well-being are n

Background:Although the use of extra-corporeal membrane oxygenation (ECMO) has been rapidly increasing,the benefit of ECMO in patients with acute respiratory Distress syndrome (ARDS) remains unclear.Our objective was to investigate the effect of venovenous ECMO (VV-ECMO) on adult patients with severe ARDS.Methods:We conducted a multi-center,retrospective,cohort study in the intensive care units (ICUs) of six teaching hospitals between January 2013 and December 2018.Patients with severe ARDS who received W-ECMO support were included.The detailed demographic data and physiologic data were used to match ARDS patients without ECMO.The primary endpoint was the 28-day mortality.Results:Ninety-nine patients with severe ARDS supported by VV-ECMO and 72 patients without ECMO were included in this study.The acute physiology and chronic health evaluation Ⅱ score was 23.1 ± 6.3 in the ECMO group and 24.8 ± 8.5 in the control group (P =0.1195).The sequential organ failure assessment score was 12.8 ± 3.4 in the ECMO group and 13.7 ± 3.5 in the control group (P =0.0848).The 28-day mortality of patients with ECMO support was 39.4%,and that of the control group was 55.6%.The survival analysis curve showed that the 28-day mortality in the ECMO group was significantly lower than that in the control group (P =0.0097).Multivariate Cox regression analysis showed that the independent predictors of the 28-day mortality were the requirement of vasopressors before ECMO (hazard ratio [HR]:1.006;95% confidence interval [CI]:1.001-1.013;P =0.030) and duration of mechanical ventilation before ECMO (HR:3.299;95% CI:1.264-8.609;P =0.034).Conclusions:This study showed that ECMO improved the survival of patients with severe ARDS.The duration of mechanical ventilation and the requirement of vasopressors before ECMO might be associated with an increased risk of death.
