Background Various methods can be applied to build predictive models for the clinical data with binary outcome variable. This research aims to explore the process of constructing common predictive models, Logistic regression （LR）, decision tree （DT） and multilayer perceptron （MLP）, as well as focus on specific details when applying the methods mentioned above： what preconditions should be satisfied, how to set parameters of the model, how to screen variables and build accuracy models quickly and efficiently, and how to assess the generalization ability （that is, prediction performance） reliably by Monte Carlo method in the case of small sample size.

Relationship Between Serum Zinc Level and Microvascular Complications in Patients with Type 2 Diabetes

Background： Previous studies suggested that zinc level was related to relationship between zinc level and all the microvascular complications in a certain diabetic microvascular complication. However, the type 2 diabetic patients remains unknown. The purpose of this study was to analyze the relationship between zinc level and each diabetic microvascular complication and identify the features related to low serum zinc level. Methods： We included the hospitalized patients with type 2 diabetes （T2D） at our department from May 30, 2013 to March 31,2014. We initially compared the serum zinc levels between patients with specific microvascular complications and those without. We then analyzed the association between zinc level and each microvascular complication. Furthermore, we identified the unique features of patients with high and low serum zinc levels and analyzed the risk factors related to low zinc level. Results： The 412 patients included 271 with microvascular complications and 141 without any microvascular complications. Serum zinc level was significantly lower in patients with diabetic retinopathy （P 〈 0.001）, diabetic nephropathy （DN, P 〈 0.001 ）, or diabetic peripheral neuropathy （P = 0.002） compared with patients without that specific complication. Lower zinc level was an independent risk factor for DN （odds ratio = 0.869, 95% confidence interval = 0.765-0.987, P 〈 0.05）. The subjects with lower serum zinc level had manifested a longer duration of diabetes, higher level of hemoglobin A I c, higher prevalence of hypertension and microvascular complications, and lower fasting and 2-h C-peptide levels. Conclusions： Lower serum zinc level in T2D patients was related to higher prevalence of diabetic microvascular complications, and represented as an independent risk factor for DN. Patients with lower zinc level were more likely to have a longer duration of diabetes, poorer glucose control, and worse β-cell function.

ABSTRACT Objective： To study the effects of the Chinese medicine Jinmaitong Capsule （筋脉通胶囊, JMT） on the pathomorphology of sciatic nerves, ciliary neurotrophic factor （CNTF）, and the mRNA expressions of CNTF in rats with streptozotocin-induced diabetes mellitus （STZ-DM）. Methods： The animal model was established by one time intraperitoneal injection of streptozotocin. The rats were simply divided by random into 5 groups including model group, low-dose JMT group （JL）, medium-dose JMT group （JM）, high-dose JMT group （JH） and neurotropin group. For each of the above 5 groups, a group of 10 normal Wistar rats matched in body weight, age and gender were set as normal group. Intragastric administrations were started after the animal model established. The JL group were administered with five times the JMT dose recommended for a human adult; the JM group were administered with ten times the JMT dose recommended for a human adult; the JH group were administered with twenty times the JMT dose recommended for a human adult. The neurotropin group was administered with ten times the neurotropin dose recommended for a human adult. All rats were given intragastric administration for 16 weeks and then killed. In the 4th, 8th, 12th, 16th week, body weight and blood glucose level were detected before and after the intervention. The morphologic changes of the sciatic nerves were observed by optical microscope and transmission electron microscope. The CNTF- mRNA expressions were detected by real-time fluorescent quantitative polymerase chain protein, and the CNTF protein expressions were detected by immunohistochemical method. Results： The blood glucose levels of the STZ-DM rats were much higher than normal group （P〈0.01）, and there was no apparent difference between any treatment groups and the model group （P〉0,05）. Neurotoxic effects of iron overload under high glucose concentration

Iron overload can lead to cytotoxicity, and it is a risk factor for diabetic peripheral neuropathy. However, the underlying mechanism remains unclear. We conjectured that iron overload-induced neurotoxicity might be associated with oxidative stress and the NF-E2-related factor 2 （Nrf2）/ARE signaling pathway. As an in vitro cellular model of diabetic peripheral neuropathy, PC12 cells ex- posed to high glucose concentration were used in this study. PC12 cells were cultured with ferric ammonium citrate at different concentrations to create iron overload. PC12 cells cultured in ferric ammonium citrate under high glucose concentration had significantly low cell viability, a high rate of apoptosis, and elevated reactive oxygen species and malondialdehyde levels. These changes were dependent on ferric ammonium citrate concentration. Nrf2 mRNA and protein expression in the ferric ammonium citrate groups were inhibited markedly in a dose-dependent manner. All changes could be inhibited by addition of deferoxamine. These results indicate that iron overload aggravates oxidative stress injury in neural cells under high glucose concentration and that the Nrf2/ARE sigfnaling pathway might play an important role in this process.

diabetic peripheral neuropathy (DPN) is a common complication of diabetes. The main objective of the study was to determine the prevalence of diabetic peripheral neuropathy and associated factors in diabetics in the University Clinic of Endocrinology Metabolism Nutrition of the CNHU-HKM, Cotonou, Benin 2021. This was a cross-sectional, analytical study that ran from 23 September to 23 December 2021. Admitted diabetic patients seen in consultation during the study period were included. The DN4 tool was used as the basis for data collection. Data analysis was performed using R software version 3.6.1. Multivariate analysis was used to identify factors associated with DPN. Out of 155 diabetics, 54 patients had diabetic peripheral neuropathy, a prevalence of 34.8%. The average age of our patients was 56.8 years and 56.8% were female. Of the patients, 54.7% had unbalanced diabetes. An association between DPN and gender (p = 0.022), occupation (p = 0.004), education (p = 0.011), hypertension (p = 0.017), smoking (p = 0.031), diabetic imbalance (p = 0.001), diabetic retinopathy (p = 0.020) and dyslipidaemia (p = 0.015) was observed. DPN was also associated with erectile dysfunction in men (p = 0.001). Conclusion: diabetic peripheral neuropathy is common (34.8). Its occurrence is indicative of the presence of associated factors.

Background diabetic microvascular complications are very common in patients with type 2 diabetes mellitus(T2DM) and contribute significantly to the morbidity and mortality of the condition. A growing body of literature suggests that obstructive sleep apnea(OSA) is a recognized risk factor for T2DM, and may also be a potential risk factor for diabetic microvascular complications. OSA and hyperglycemia share many similar molecular consequences. However, whether OSA defined by apnea-hypopnea index(AHI) or oxygen desaturation index(ODI) is associated with development of diabetic microvascular complications remains controversial. Objective The aim of this study was to examine the association between hypoxemia parameters related to OSA and microvascular complications(i.e., diabetic peripheral neuropathy(DPN), diabetic retinopathy(DR) and diabetic nephropathy(DN)). Methods A total of 468 Chinese patients with type 2 diabetes aged 18-85 years were included in the study. Information on demographic features, lifestyle risk factors, family history of diabetes, history and treatment of comorbidities was collected. All participants underwent one-night polysomnography, followed by the screening of diabetic microvascular complications, which were diagnosed by medical specialists. Parameters of nocturnal hypoxemia including AHI, ODI, mean and lowest oxygen saturation(SpO), and time spent with SpOdiabetic microvascular complications after adjusting for potential confounders. Statistical significance was accepted as a two-sided test with an alpha level of 0.05.
