Purpose The relationship between sleep macrostructure and obstructive sleep apnea–hypopnea syndrome（OSAHS） patients has been widely reported,but there are few studies on sleep microstructure marked by cyclic alternating pattern（CAP）.The purpose of this study was to compare the sleep microstructural features of young and middle-aged OSAHS patients with different degree of disease,and investigate the relationship between sleep microstructure destroy and cognitive impairment and daytime sleepiness in these OSAHS *** An overnight night of polysomnography（PSG） test was performed on 133 young and middle-aged snorers（mean age 37.5 ± 7.7 years）.In addition to the analysis of traditional PSG parameters,the arousal and cyclic alternating pattern（CAP） parameters were evaluated and *** cognitive function and daytime sleepiness were assessed by using the Montreal Cognitive Assessment（MoCA） and the Epworth sleepiness scale（ESS）,*** The patients were classified according to the severity of apnea hypopnea index（AHI）: primary snoring group（AHI＜5/h,n=25）,mild OSAHS group（5/h≤AHI≤15/h,n=29）,moderate OSAHS group（15/h＜AHI≤30/h,n=27） and severe OSAHS（AHI＞30/h,n=52）.Arousal index（p＜0.001）,CAP rate（p＜0.001）,total CAP time（p＜0.001）,phase A index（p＜0.001）,number of CAP cycles（p＜0.001）,and average duration of phase A（p＜0.001） and B（p＜0.001） were gradually increased with the increased of the severity of OSAHS *** decreased as the severity of OSAHS increased,and ESS score *** it is closely related to the total CAP time,CAP rate and arousal *** We found a significant change in arousal index and several CAP parameters in young and middle-aged OSAHS patients with different degree of *** these parameters have a certain correlation with the ESS score and MoCA of the *** with the analyzing results above,we can draw a conclusion that the more severe the condition of OSAHS patients,the more unstable the slee

Subjective daytime sleepiness is considered a significant risk factor of hypertension in patients with obstructive sleep apnea（OSA）.In this study,our goal was to examine the joint effect on hypertension of OSA and objective daytime sleepiness measured by the multiple sleep latency test（MSLT）.A total of 1338 Chinese patients with OSA and 484 primary snorers were included in the *** subjects underwent 1 night polysomnography followed by *** MSLT values were classified into 3 categories: ＞8 minutes,5 to 8 minutes,and ＜5 *** was defined based either on direct blood pressure measures or on diagnosis by a *** controlling for confounders,OSA combined with MSLT of 5 to 8 minutes increased the odds of hypertension by 95%（odds ratio,1.95;95% confidence interval,1.10–3.46）,whereas OSA combined with MSLT ＜5 minutes further increased the odds of hypertension by 111%（odds ratio,2.11;95% confidence interval,1.22–3.31） compared with primary snorers with MSLT ＞8 *** stratified analyses,the association of hypertension with MSLT in OSA patients was seen among both sexes,younger ages,both obese and nonobese patients,and patients with and without subjective excessive daytime *** conclude that objective daytime sleepiness is associated with hypertension in patients with OSA.

Evaluation of Sleep Quality in Undergraduated Medical Students

Objective: The aim of this study was to evaluate the sleep quality of medical students to identify the main factors that affect sleep patterns. Methods: According to the methodology, 154 students participated in the research, attending from the first to the sixth year of the course. The Pittsburg Sleep Quality Index questionnaire and the Epworth sleepiness scale were applied. Descriptive statistics techniques were applied and demonstrated in the form of tables and graphs. The comparison of variables was carried out using statistical tests. Results: The results showed that most students do not sleep very well. It was observed that 68.83% of a total of 106 students are bad sleepers. Conclusion: It is concluded that medical students sleep a low number of hours by period, which could interfere in impairment of attention, learning and memory formation, interfering in academic performance. On the other hand, they showed greater daytime sleepiness compared to the general population.

Mandibular advancement devices in the treatment of obstructive sleep apnea and snoring in community dental care: A pilot study on self-reported sleep quality

Background: The high efficacy of continuous positive airway pressure (CPAP) in treating obstructive sleep apnea (OSA) patients is sometimes limited because of intolerance. Mandibular advancement devices (MAD) are proven to be efficient in treating mild to moderate sleep apnea and snoring. We examined patients who had been treated in Community Dental Care with a titrable Herbst-type MAD. The aim of the study was to evaluate the quality of sleep based on self report. Methods: A questionnaire was mailed to patients (n = 184) who had received treatment for OSA or snoring between 2007 and2010 inthe Helsinki Health Centre Oral Care Unit. The patients were referred to the clinic for primary treatment, or because another treatment modality had failed. Results: Of the respondents (n = 142, 78.4%), OSA had been diagnosed in 74%, while the MAD was applied for snoring to the remaining. Among all respondents, 78.4% had worn the MAD at least three nights per week. With the MAD in situ, sleep was felt significantly less disrupted (p daytime tiredness was also markedly less frequent (p < 0.001). Conclusions: Treatment with a MAD improved perceived sleep quality and awoke alertness in mild and moderate OSA patients and in snorers. This study supports such treatment to be initiated and monitored in community dental care.
