A bulging intervertebral disc (IVD) occurs when pressure on a spinal disc damages the once healthy disc,causing it to compress or change its normal *** medicine,most attention has been paid clinically to diagnosis of and treatment for such problems,which little effect has been made to understand such issues from a mechanics perspective,i.e.,the bulging deformation of the soft IVD induced by excessive compressive *** report herein a simple elasticity solution to understand the bulging disc *** simplicity,the soft IVD is modeled as an incompressible circular composite layer consisting of an inner nucleus and outer annulus,sandwiched between two vertebral segments which are much stiffer than the IVD and can be treated as rigid *** adopting any assumptions regarding prescribed displacements or stresses,we obtained the stress and displacement fields within the composite layer when a certain compressive stain is applied via an asymptotic *** asymptotic approach is very simple and accurate enough for prediction of the bugling profile of the *** also performed finite-element modeling (FEM) to validate our solutions;the predicted stress and displacement fields inside the composite are in good agreement with the FEM results.

The aim of this research is the improvement towards the consumption of energy in the field of engineering and industry. The efforts have been paid to the enhancement of heat transmission and cooling process through a nanofluid coating of a nonlinear stretching disc. The combination of Water(H2 O) and multiple walled carbon nanotubes(MWCNT)/single walled carbon nanotubes(SWCNT) have been used as a nanofluid. The spreading of a thin nano-layer with variable thickness over a nonlinear and radially stretching surface has been considered. The estimated results of the problem have been accomplished using the Optimal Homotopy Analysis Method(OHAM). The residual errors of the OHAM method have been shown physically and numerically. The important physical parameters of skin friction and Nusselt number have been calculated and discussed. The other embedding parameters like generalized magnetic parameter, Prantl number, nanofluid volume fraction and Eckert number have been intended and discussed. The obtained results have been compared with the Numerical(ND-Solve) method for both sorts of CNTs. The closed agreement of both methods has been achieved.

This paper investigates the effect of cutter clearance angle on variation of depth of cut and cutting process with an actuated disc cutting(ADC).ADC is a cyclic cutting method with two main characteristics:(i)a disk-shape cutter is used to attack the rock in an undercutting mechanism;and(ii)the cutter is dynamically actuated as it is moved across the ***,the cutting process of such system is periodic,each recurrence known as actuation *** first ADC model,developed in 2016,represented an idealization of the technology with a flat disc cutter,where no clearance angle was *** evolution of the contact between the disc and the rock was,therefore,computed only on horizontal x-y plane,ignoring the effect of normal component of the force acting on *** article reports on a study that incorporates the cutter inclination angle in derivation of cutter/rock interface *** extends the proposed kinematic and geometry based model to take into account the variable depth of cut in estimating the forces associated with cutting in one actuation *** were conducted using Wobble to test the predictions of the improved model at various operating *** model predictions are matched with the experimental results and effects of various factors are analysed.

The disc cutters of tunnel boring machine(TBM) are installed with different polar angles. This causes the cutting depth difference between adjacent disc cutters on the tunnel face. A rock-cutting model was established to study the rock fragmentation law between adjacent disc cutters with different polar angles based on particle flow code(PFC). The influence of polar angle of adjacent disc cutters on rock cracks and stresses under different cutter spacing and penetration was studied. Research shows that polar angle difference leads to the discontinuity of rock-fragmentation process by adjacent cutters. The effect of rock-fragmentation is influenced by the cutting depth difference between adjacent cutters. The effect of rock-fragmentation performed best, meanwhile large rock blocks were flaked when the difference of cutting depth is half of the penetration. Too large or small difference of the cutting depth will cause high specific energy consumption of rock fragmentation. The specific energy consumption is relatively small when the difference of cutting depth is half of the penetration.

Past researches have shown that the non-smooth body surfaces of soil burrowing animals help to reduce soil resistance. In this research, this concept of bionic non-smooth surface was applied to disc ploughs and an experiment was conducted in an indoor soil bin to find out the effects of different bionic units on reducing soil resistance to disc ploughing. Horizontal force acting on the disc plough during soil deformation was measured using a 5 kN sensor. Convex and concave bionic units were used and the material used for making convex ones is ultra high molecular weight polyethylene (UHMWPE) which is hydrophobic. From the experiment results, higher or deeper bionic units always resulted in less soil resistance. Convex bionic units gave the highest resistance reduction reaching a maximum of 19% reduction (from 1715.36 N to 1383.65 N) compared to concave bi-onic units. Also, samples with a bionic unit density of 30% gave the highest resistance reduction compared to the other two, which were either plain or had 10% density. In conclusion, the concept of bionic non-smooth units can be applied to disc ploughs in order to reduce soil resistance.

Predicting the cutter consumption and the exact time to replace the worn-out cutters in tunneling projects constructed with tunnel boring machine(TBM) is always a challenging issue. In this paper, we focus on the analyses of cutter motion in the rock breaking process and trajectory of rock breaking point on the cutter edge in rocks. The analytical expressions of the length of face along which the breaking point moves and the length of spiral trajectory of the maximum penetration point are derived. Through observation of rock breaking process of disc cutters as well as analysis of disc rock interaction, the following concepts are proposed: the arc length theory of predicting wear extent of inner and center cutters, and the spiral theory of predicting wear extent of gage and transition cutters. Data obtained from5621 m-long Qinling tunnel reveal that among 39 disc cutters, the relative errors between cumulatively predicted and measured wear values for nine cutters are larger than 20%, while approximately 76.9% of total cutters have the relative errors less than 20%. The proposed method could offer a new attempt to predict the disc cutter's wear extent and changing time.

Effectiveness and safety of moxibustion in treatment of lumbar disc herniation: a systematic review and Meta-analysis

OBJECTIVE: To evaluate the effectiveness and safety of moxibustion therapy in the treatment of lumbar disc herniation (LDH). METHODS: Four Chinese databases and three English databases were searched from their inception to April *** controlled trials (RCTs) were included if moxibustion was used as the sole treatment or as a part of combination therapy with other treatments in patients with LDH. Two reviewers independently extracted the data and assessed the methodological quality using the Cochrane criteria for the risk of bias. The Meta-analysis was performed using Review Manager 5.3 software. RESULTS: In total, 16 RCTs including 1186 patients with LDH were analyzed. The Meta-analysis showed favorable effects of moxibustion in combination with massage therapy on the visual analog scale score compared with massage therapy alone [mean difference (MD)=? 1.32, 95% confidence interval (CI)(? 2.12,? 0.51), P = 0.001]. The subgroup Meta-analysis failed to show favorable effects of electro-acupuncture plus moxibustion on the efficacy rate compared with electro-acupuncture alone [relative risk (RR)= 1.06, 95% CI (0.98, 1.14), P = 0.15]. However, acupuncture or massage therapy plus moxibustion improved the efficacy rates compared with acupuncture or massage therapy alone [RR = 1.33, 95% CI (1.18, 1.49), P < 0.000 01][χ^2 = 2.76, P = 0.25, I2 = 27%],[RR = 1.15, 95% CI (1.06, 1.25), P = 0.001][χ^2 = 0.00, P = 0.95, I2 = 0%]. With respect to the Japanese Orthopaedic Association (JOA) scores, acupuncture or massage therapy in addition to moxibustion produced results different from those of acupuncture or massage therapy alone [MD = 5.58, 95% CI (4.15, 7.00), P < 0.000 01][χ^2 = 0.58, P = 0.45, I2 = 0%],[MD = 3.61, 95% CI (3.01, 4.21), P < 0.000 01]. There were no significant differences in the JOA score for subjective symptoms, objective symptoms, daily living ability, and other parameters. OBJECTIVE: To investigate the analgesic effect of Gua Sha and its underlying mechanism in rats with noncompressive lumbar disk herniation induced by autologous nucleus ***: A rat model of noncompressive lumbar disk herniation was established and rats were randomly divided into model group, sham group, and Gua Sha group(24 in each group). Gua Sha was performed from the 5 th day after the surgery, once every other day, 3 times for a course of treatment,and totally 3 courses. The thermal withdrawal latency was evaluated using the intelligent hot plate one day before the surgery, and on days 4(the day before the treatment), 10(the end of the firstcourse), 16(the end of the second course) and 22(the end of the third course). On days 4, 10, 16 and22, six rats in each group were picked randomly and their blood samples were drawn to assess the expression of interleukin-1β(IL-1β), interleukin-6(IL-6) and tumor necrosis factor-alpha(TNF-α).RESULTS: Compared to rats in the sham group, the application of nucleus pulposus to right L5 dorsal root ganglion induced prolonged thermal hyperalgesia, and up-regulated the expression of IL-1β,IL-6 and TNF-α in serum(P 0.01).CONCLUSION: The current study showed that Gua Sha might alleviate thermal hyperalgesia in rats with lumbar disc herniation induced by autologous nucleus pulposus via inhibiting the expression of proinflammatory cytokins.

Efficacy and safety of electroacupuncture in treatment of lumbar disc herniation: a protocol for a cohort study

OBJECTIVE: To compare the efficacy of electroacupuncture(deep needling) and general orthopedics in treatment of lumbar disc herniation(LDH), and to evaluate its long-term ***: This trial is a prospective cohort study.A total of 175 participants will be observed. The exposure group is obtained from department of acupuncture and moxibustion and the control group is from orthopedic department in hospital. Patients in exposed group will receive electroacupuncture in Dachangshu(BL 25), Guanyuanshu(BL 26), L4 Jiaji points(EX-B2 L4), L5 Jiaji points(EX-B2 L5) and S1 Jiaji points(EX-B2 S1) in the affected side once a day, 6 times as a treatment course for two courses(12 times). Patients in control group will receive orthopedic conventional therapy. In acute period,the treatment is mannitol injection(250 mL∶50 g) 200 mL, i.v., q.d.; 0.9% sodium chloride injection250 mL, vitamin C injection 2 g, Dexamethasone sodium phosphate injection 10 mg, i.v., q.d.; sterile water for injection 2 mL, adenosine cobalt amine injection 1 mg, i.m., q.d., 12 d. In remission period,the treatment is Voltaren(Diclofenac diethyl lamine emulsion) external, q.d., 12 d. The researchers do not give any other intervention and record the relevant information ***: Modified Japanese Orthopaedic Association Scores, Visual Analogue Scale/Score, the Short Form-36 Quality-of-Life Questionnaire and Oswestry Disability Questionnaire and the second outcome measures(Adverse Event Questionnaire and Relapse Questionnaire) are measured before treatment, after the sixth treatment, and after the twelfth treatment.
