Update on endoscopic management of gastric outlet obstruction in children

＜正＞ The structure of a steel changes with temperature due to phase transformations. This phase change is generally companied by a variation in the specific volume which is exhibited as a departure from the behaviour of the thermal expansion or contraction, at the temperature at which the change occurs. This behaviour can be detected as a change in the dimensions of a suitable test piece, and forms the operating principle of dilatometry. The dilatometric technique may be applicable to the investigation of the phase transformation kinetics if the relation between the fractions of phases, the temperature, the compositions of phases and the dilatation can be built up. Efforts have been made to analyze the dilatation during an isothermal transformation or to calculate the dilatation during the .heating of plain carbon steels using the thermodynamic function. The existing models are, however, not directly applicable to the determination of the phase transformation kinetics from a dilatation curve. A detailed analysis has been made of the length change of a hypoeutectoid steel during a continuous cooling. A model has been developed in which the transient dilatation is calculated based on the fraction of the phases present. The model takes into account the redistribution of carbon and is applicable to the determination of the phase transformation kinetics from the dilatation data during a cooling of a hypo-eutectoid steel. The model was validated by comparing the model results with the experimental results of an interstitial free steel. Experiments have been done with an interstitial-free steel and a C-Mn steel. The model has been applied to the calculation of the transformation kinetics indicated by the dilatation curves. Excellent agreements between the model and the experiments have been obtained.

Endoscopic balloon dilatation(EBD) and surgical intervention are two most common and effective treatments for gastric outlet obstruction. Correction of gastric outlet obstruction without the need for surgery is an issue that has been tried to be resolved in these decades; this management has developed with EBD, advanced treatments like local steroid injection, electrocauterization, and stent have been added recently. The most common causes of pediatric gastric outlet obstruction are idiopathic hypertrophic pyloric stenosis, peptic ulcer disease followed by the ingestion of caustic substances, stenosis secondary to surgical anastomosis; antral web, duplication cyst, ectopic pancreas, and other rare conditions. A complete clinical, radiological and endoscopic evaluation of the patient is required to make the diagnosis, with complimentary histopathologic studies. EBD are used in exceptional cases, some with advantages over surgical intervention depending on each patient in particular and on the characteristics and etiology of the gastric outlet obstruction. Local steroid injection and electrocauterization can augment the effect of EBD. The future of endoscopic treatment seems to be aimed at the use of endoscopic electrocauterization and balloon dilatations.

AIM To investigate the role of a novel minimally invasive endoscopic technique in the management of tight neartotal corrosive strictures of the proximal esophagus involving the hypopharynx.METHODS Two patients with near-total corrosive strictures of the proximal esophagus involving the hypopharynx were managed with the novel endoscopic technique. The technique involved passing a 0.025-inch flexible guidewire across the stricture, and stricture dilatation, using 10 F coaxial diathermy and balloon dilators, followed byelectro-incision of the proximal aspect of the residual eccentric stricture by means of a novel approach using a wire-guided sphincterotome.RESULTS Both patients were successfully managed on an outpatient department basis with the complete relief of symptoms and resolution of strictures on endoscopy and an esophagogram. No adverse events were seen during or after the procedure. There was no recurrence of symptoms at a follow-up of over a year in both cases. There was a significant improvement in the body mass index of both patients after the procedure.CONCLUSION We report a novel flexible endoscopic technique for the management of complex hypopharyngo-esophageal strictures. In experienced hands, the procedure is relatively simple, safe and effective with a durable response.

The author has observed the epithelium and basement membrane of uterus, oviduct and fimbriae in mice under the TEM by oral administration of AgNo3 tracer. The results showed that there was obvious deposit of argentum granules outside the basement membrane of mucosal epithelium of the uterus, oviduct and fimbriae as well as outside the basement membrane of uterine glands. And there was dilatation fully filled with high electron-dense argentum granules at the intercellular space just beneath the tight junction between the mucosal epithelial cells of uterus, oviduet, fimbriae. All these finding suggest that the uterus, oviduct and fimbriae serve as barriers to the heavy metal argentum ions.

We report observations in a 7 year old girl with manifest right accessory pathway (AP) and depressed cardiac function. Her Echocardiogram showed LV dilatation with asynchronous ventricular wall motion and LVEF reduction. The electrophysiological study demonstrated a para-his AP. She underwent successfully ablation and without any complications. The Echo validated that asynchronous ventricular wall motion disappeared after ablation. Her cardiac size and function was normal after follow up of three month. Our findings suggested that manifest AP leading to asynchronous ventricular motion was a possible cause of left ventricular dilation and dysfunction. Catheter ablation could reverse cardiac remodeling in such patient. Case report A seven year old girl was referred to our hospital for fainting episode in last six month. To exclude cephalic syncope, she underwent electroencephalogram, the result was negative. The electrocardiogram (ECG) showed right AP. An apparent delta wave looked like LBBB pattern with short PR interval (80ms) and long QRS duration (180ms) (figure 1 left panel). Echocardiogram (Echo) revealed left ventricle dilatation and left ventricular ejection fraction reduction (LVDd=52mm, LVEF=46%). Asynchronous ventriclular wall motion and moderate mitral valve regurgitation also observed on Tissue Doppler Imaging (TDI). She underwent electrophysiology study (EPS) and ablation procedure on the 5th hospital day. The intracardiac electrogram showed a para-His AP with antegrade conduction. The orthodromic SVT (201 bpm) was induce right ventricular apex programmed stimulation. During SVT episode, her symptom was similar to spontaneous one. After successful ablation, delta wave on ECG disappeared and SVT can not be induced with or without isoproterenol. Normal atrial-ventricular conduction was kept without any complications. Prior to discharge, Echo showed the LVEF slightly improved and TDI indicated that asynchronous ventricular wall motion disappeared. She was discharged without any medication.

AIM To evaluate a new balloon-expandablestainless steel stent (Cordis stent ) in atransjugular intrahepatic portosystemic shunt(TIPS) porcine model and compared withWallstent.METHODS TIPS was performed in 26 normaldomestic pigs weighing 20 kg-30 kg using aCordis stent or Wallstent (13 pigs in each stent).All pigs were sacrificed at the 14th day afterTIPS. The stent deployment delivery system,stent patency, and stent recoil after placementwere evaluated. Proliferative response inrepresentative histological sections from thecenter, hepatic and portal regions of the twostent designs were quantified.RESULTS The shunt was widely patent in 4pigs in the Cordis stent group (4/12, prematuredead in 1 pig), and in 5 pigs in the Wallstentgroup (5/13). All remaining stents of bothdesigns were occluded or stenotic. The meanquantified proliferation including thickness ofthe proliferation and the ratio of proliferation:total area in three assayed regions in Cordisstent and Wallstent was 2.18 mm:2.00 mm, and59.18 mm2: 51.66 mm2, respectively (P ＞0.05).The delivery system and mechanical propertiesof the Cordis stent fuctioned well.CONCLUSION The new Cordis stent isappropriate for TIPS procedure.
