cyclospora cayetanensis infections among diarrheal outpatients in Shanghai： a retrospective case study

cyclospora cayetanensis is a foodborne and waterborne pathogen that causes endemic and epidemic human diarrhea worldwide. A few epidemiological studies regarding C. cayetanensis infections in China have been conducted. During 2013, a total of 291 stool specimens were collected from patients with diarrhea at a hospital in urban Shanghai. C. cayetanensis was not detected in any of the stool specimens by traditional microscopy, whereas five stool specimens （1.72%, 5/291） were positive by PCR. These positive cases confirmed by molecular technology were all in the adult group （mean age 27.8 years; 2.94%, 5/170） with watery diarrhea. Marked infection occurred in the rainy season of May and July. Sequence and phylogenetic analyses of the partial 18S rRNA genes of C. cayetanensis isolated showed intra-species diversity of this parasite. This study showed, for the first time, that C. cayetanensis is a pathogen in outpatients with diarrhea in Shanghai, albeit at a low level. However, the transmission dynamics of this parasite in these patients remain uncertain.

Objectives: cyclospora cayetanensis is an important foodborne pathogen, causing numerous cyclosporiasis outbreaks in North America since the mid-1990 s. The lack of genotyping tools for C. cayetanensis has greatly hampered the cases linkage and population analysis of this pathogen. This study aim to develop a multilocus sequence typing(MLST) tool and analyze population structure of C. cayetanensis. Methods: To identify potential genotyping markers, we sequenced the genome of one C. cayetanensis isolate(CHNEN01) from Henan, China by 454 technology and Illumina technology. Microsatellite and minisatellite repeats were searched from whole genome sequencing date and amplified by nested PCR. Totally, 64 C. cayetanensis-positive specimens, which were from China, Peru, the United States, Nepal, Indonesia and Spain, were used to analyze amplification efficiency and nucleotide sequence polymorphism. Afterwards, population genetics analysis were undertook based MLST data generated from the 64 specimens. Results: Five microsatellite loci(CYC3, CYC13, CYC15, CYC21 and CYC22) exhibiting good amplification and sequence polymorphism were used for genotyping all specimens. Totally, thirty-four specimens had sequence data at five loci, forming 25 multilocus genotypes(MLGs). The linkage disequilibria(LD) in the 64 specimens indicated an overall non-panmictic structure. The disappearance of LD when only considered MLGs confirmed the clonal epidemic structure of the population. Meanwhile, the subpopulation analysis suggested the geographical sub-structuring between Chinese specimens and the other ones mostly from Peru and the United States. Conclusion: The typing tool developed in this study has high resolution and MLST data generated revealed an overall epidemic C. cayetanensis population, which consisting of two remarkable geographical segregated sub-populations.
