Effects of asymptomatic hyperuricemia on vascular endothelial cells and vascularsmoothmuscle cells in patients with primary chronic glomerulonephritis

Objective: To investigate the expression level of constitutive cyclooxygenase (COX-1) and inducible cyclooxygenase (COX-2) expression in progressive psoriasis and their significance. Methods: Applying reverse transcription-polymerase reaction (RT-PCR) technique to semi-quantitatively analyze COX-1 and COX-2 mRNA expression in the Peripheral Blood Monouclear Cells (PBMC) from 30 psoriatic patients and 30 cases of healthy subjects, and the correlation between COX and PASI (psoriatic area and severity index) was valu-ated. Results: Compared with normal controls, there was statistical differences in the COX-2 mRNA expression level of patients (p0.05). There was positive correlation between COX2 and PSAI (r=0.683,p<0.01). However, such a positive correlation was not shown between COX1 and PSAI. Conslusion: The higher level COX-2 expression can stimulate over-production of PGs and may be involved in the pathogenesis of progressive psoriasis.

AIM: To investigate the mechanism for bradykinin(BK) to stimulate intestinal secretomotor neurons and intestinal chloride secretion. METHODS: Muscle-stripped guinea pig ileal preparations were mounted in Ussing flux chambers for the recording of short-circuit current(Isc). Basal Isc and Isc stimulated by BK when preincubated with the BK receptors antagonist and other chemicals were recorded using the Ussing chamber system. Prostaglandin E2(PGE2) production in the intestine was determined by enzyme immunologic assay(EIA).RESULTS: Application of BK or B2 receptor(B2R) agonist significantly increased the baseline Isc compared to the control. B2 R antagonist, tetrodotoxin and scopolamine(blockade of muscarinic receptors) significantly suppressed the increase in Isc evoked by BK. The BK-evoked Isc was suppressed by cyclooxygenase(COX)-1 or COX-2 specific inhibitor as well as nonselective COX inhibitors. Preincubation of submucosa/mucosa preparations with BK for 10 min significantly increased PGE2 production and this was abolished by the COX-1 and COX-2 *** BK-evoked Isc was suppressed by nonselective EP receptors and EP4 receptor antagonists, but selective EP1 receptor antagonist did not have a significant effect on the BK-evoked Isc. Inhibitors of PLC, PKC, calmodulin or Ca MKⅡ failed to suppress BK-induced PGE2 production. CONCLUSION: The results suggest that BK stimulates neurogenic chloride secretion in the guinea pig ileum by activating B2 R, through COX increasing PGE2 production. The post-receptor transduction cascade includes activation of PLC, PKC, Ca MK, IP3 and MAPK.

Hypersensitive(sticky) platelets in JAK2-mutated essential thrombocythemia(ET) and polycythemia vera(PV) with thrombocythemia spontaneously activate at high shear in arterioles, secrete their inflammatory prostaglandin endoperoxides and induce plateletmediated arteriolar fibromuscular intimal proliferation. Constitutively activated JAK2 mutated hypersensitive(sticky) platelets spontaneously aggregate at high shear in the endarteriolar circulation as the cause of aspirin responsive erythromelalgia and platelet arterial thrombophilia in JAK2-mutated thrombocythemia patients. Increased production of prostglandin endoperoxides E2 and thromboxane A2 released by activated sticky platelets in arterioles account for redness warmth and swelling of erythromelalgia and platelet derived growth factor can readily explain the arteriolar fibromuscular intimal proliferation. Von Willebrand factor(VWF) platelet rich occlusive thrombi in arterioles are the underlying pathobiology of erythromelalgic acrocyanosis, migraine-like transient cerebral attacks(MIAs), acute coronary syndromes and abdominal microvscular ischemic events. Irreversible platelet cyco-oxygenase inhibition by aspirin cures the erythromelalgia, MIAs and microvascular events, corrects shortened platelet survival to normal, and returns increased plasma levels of betaTG, platelet factor 4, thrombomoduline and urinary thromboxane B2 excretion to normal in symptomatic JAK2-thrombocythemia patients. In vivo activation of sticky platelets and VWF-platelet aggregates account for endothelial cell activation to secrete thrombomoduline and sVCAM followed by occlusion of arterioles by VWF-rich platelet thrombi in patients with erythromelalgic thrombotic thrombocythemia(ETT) in ET and PV patients. ETT is complicated by spontaneous hemorrhagic thrombocythemia(HT) or paradoxical ETT/HT due to acquired von Willebrand disease type 2A at platelet counts above 1000 × 10~9/L and disappears by cytoreduction of platelets to normal(< 400 ×

细胞毒性化疗起初在缩小肿瘤大小方面是有效的,然而,在一些患者经数次化疗后会逐渐出现耐药性。先前研究已证实,化疗后癌干细胞（CSCs）也会通过强化生存而选择性的重新生长。这里,揭示该现象的一种新的机制：通过人类膀胱癌异种移植物,膀胱癌干细胞如何在2个化疗疗程之间通过一个我们尚不了解的增殖效应来重新填充化疗缩小的肿瘤。进一步的分析证明,静止标记的CSCs池细胞可重新分裂来响应化疗产生的损伤,这类似于在创伤修复期间正常干细胞的动员。在化疗有效地诱导细胞凋亡的同时,相关的前列腺素E2（PGE2）却反常的释放,并促进邻近的CSC再生。
