A novel method of defective vascular reconstruction using 2-octyl-cyanoacrylate and homemade prosthetic component

Background Currently,adhesive technique is popular in vascular repair but not widely used for defective vessels.This study aimed to determine the feasibility and effectiveness of repairing defective vessels with 2-octyl-cyanoacrylate and a homemade prosthetic component.Methods Homemade prosthetic component consisting of expanded polytetrofluoroethylene （ePTFE）,terylene film,and homemade soluble hollow stent mixed with adhesive can replace autologous graft and suture in repairing defective vessels,can fix vessels better using the stent without occlusive bleeding.Forty male mongrel dogs were used,20 for biomechanical tests and 20 for animal experiments.In the biomechanical test,dogs were randomly divided into two groups （n=10 each）,one group repaired on the two sides of the carotid arteries with 2-octyl-cyanoacrylate and homemade component and another group repaired with suture and ePTFE.Of the 40 specimens,10 were used for adhesive and 10 for suture specimens for tension strength test,whereas the remaining specimens were used for bursting pressure test.In animal experiments,dogs were also divided into adhesive and suture groups （n=10）,only of the left carotid artery.Recording the operational time,bleeding or not.Vessels were tested using color Doppler ultrasound,the inner diameter was measured,and the degree of stenosis at 8 weeks was evaluated digital subtraction angiography （DSA） were also performed.Specimens were then analyzed histologically.Results In the adhesive and suture groups,the specimens could afford atension strength of （23.80±1.51） N versus （24.60±1.08） N （P ＞0.05）,the bursting pressure was （52.03±.2.43） kPa versus （50.04±3.51） kPa （P ＞0.05）,and the mean time of anastomosis was （15.20±0.55） minutes versus （25.97±0.58） minutes （P ＜0.05）.One dog in the adhesive group was bleeding from the suture.One dog from each group presented with thrombosis at 1 week.After measuring using ultrasound,the stenosis degree of all dogs were no more than 30％ except the two thromboses.DSA and histological observation show

AIM To evaluate the safety and efficacy of a modified cyanoacrylate [N-butyl-2-cyanoacrylate associated with methacryloxysulfolane(NBCA + MS)] to treat nonvariceal upper gastrointestinal bleeding(NV-UGIB).METHODS In our retrospective study we took into account 579 out of 1177 patients receiving endoscopic treatment for NV-UGIB admitted to our institution from 2008 to 2015; the remaining 598 patients were treated with other treatments. Initial hemostasis was not achieved in 45 of 579 patients; early rebleeding occurred in 12 of 579 patients. Thirty-three patients were treated with modified cyanoacrylate: 27 patients had duodenal, gastric or anastomotic ulcers, 3 had post-mucosectomy bleeding, 2 had Dieulafoy's lesions, and 1 had duodenal diverticular bleeding.RESULTS Of the 45 patients treated endoscopically without initialhemostasis or with early rebleeding, 33(76.7%) were treated with modified cyanoacrylate glue, 16(37.2%) underwent surgery, and 3(7.0%) were treated with selective transarterial embolization. The mean age of patients treated with NBCA + MS(23 males and 10 females) was 74.5 years. Modified cyanoacrylate was used in 24 patients during the first endoscopy and in 9 patients experiencing rebleeding. Overall, hemostasis was achieved in 26 of 33 patients(78.8%): 19 out of 24(79.2%) during the first endoscopy and in 7 out of 9(77.8%) among early rebleeders. Two patients(22.2%) not responding to cyanoacrylate treatment were treated with surgery or transarterial embolization. One patient had early rebleeding after treatment with cyanoacrylate. No late rebleeding during the follow-up or complications related to the glue injection were recorded.CONCLUSION Modified cyanoacrylate solved definitively NV-UGIB after failure of conventional treatment. Some reported life-threatening adverse events with other formulations, advise to use it as last option.

Tag retention,wound healing,wound closure,and tagged fish survival are critically important after surgical implantation of acoustic transmitters in fish.This study compared the use of sutures to cyanoacrylate adhesives and bandages for post-surgery wound closure.Adult rainbow trout Oncorhynchus mykiss were assigned to one of seven treatments:a control group where fish were only handled,two suture groups(tagged with dummy acoustic transmitters or non-tagged),and four cyanoacrylate adhesive groups(non-tagged,tagged with adipose fin applied as a biological bandage,tagged with anal fin applied,and non-tagged with water-proof bandage).Tag retention was significantly improved in adhesive with anal fin(89%)and tagged suture(78%)treatments when compared to only 11%tag retention in the adhesive with adipose fin treatment.Wound redness severity increased over the 14 weeks of the study in the tagged and sutured fish,and in those receiving the waterproof bandage.Wound redness severity was also significantly higher in the waterproof bandage and tagged and sutured treatments than both tagged adhesive treatments at the end of the study.Wound gape was significantly more severe in the adhesive with adipose treatment than in the control,suture,tagged suture,and adhesive treatments,but no significant differences were observed between the adhesive with adipose treatment and the adhesive with anal fin or bandage treatments.Wound gape never completely closed in the adhesive with adipose group,while both the tagged suture treatment and tagged adhesive with anal treatment wounds completely closed between four-and six-weeks post-surgery.The results of this study indicate that cyanoacrylate adhesive used in conjunction with anal fin tissue is a suitable post-surgical treatment.

Endoscopic cyanoacrylate injection for the treatment of gastric varices in children

AIM:To evaluate the efficacy and safety of N-butyl-2-cyanoacrylate in treating acute bleeding of gastric varices in children.METHODS:The retrospective study included 21 children with 47 episodes of active gastric variceal bleeding who were treated by endoscopic injection of N-butyl-2-cyanoacrylate at Asan Medical Center Children's Hospital between August 2004 and December 2011.To reduce the risk of embolism,each injection consisted of 0.1-0.5 mL of 0.5 mL N-butyl-2-cyanoacrylate diluted with 0.5 or 0.8 mL Lipiodol.The primary outcome was incidence of hemostasis after variceal obliteration and the secondary outcome was complication of the procedure.RESULTS:The 21 patients experienced 47 episodes of active gastric variceal bleeding,including rebleeding,for which they received a total of 52 cyanoacrylate injections.Following 42 bleeding episodes,hemostasis was achieved after one injection and following five bleeding episodes it was achieved after two injections.The mean volume of each single aliquot of cyanoacrylate injected was 0.3 ± 0.1 mL(range:0.1-0.5 m L).Injection achieved hemostasis in 45 of 47(95.7%) episodes of acute gastric variceal bleeding.Eleven patients(52.4%) developed rebleeding events,with the mean duration of hemostasis being 11.1 ± 11.6 mo(range:1.0-39.2 mo).No treatment-related complications such as distal embolism were noted with the exception of abdominal pain in one patient(4.8%).Among four mortalities,one patient died of variceal rebleeding.CONCLUSION:Endoscopic variceal obliteration using a small volume of aliquots with repeated cyanoacrylate injection was an effective and safe option for the treatment of gastric varices in children.

A 76-year-old woman with hepatitis C cirrhosis presented with tarry stools and hematemesis.An endoscopy demonstrated bleeding duodenal varices in the second portion of the duodenum.Contrast-enhanced computed tomography revealed markedly tortuous varices around the wall in the duodenum.Several afferent veins appeared to have developed,and the right ovarian vein draining into the inferior vena cava was detected as an efferent vein.Balloon-occluded retrograde transvenous obliteration (BRTO) of the varices using cyanoacrylate was successfully performed in combination with the temporary occlusion of the portal vein.Although no previous publications have used cyanoacrylate as an embolic agent for BRTO to control bleeding duodenal varices,this strategy can be considered as an alternative procedure to conventional BRTO using ethanolamine oleate when numerous afferent vessels that cannot be embolized are present.

cyanoacrylate adhesive and its composites have been widely used in industry and dentistry. According to literature survey done by the authors, there are few papers concentrated on role of nano-sized particles on wear behavior of cyanoacrylate glue. Thus the main goal of current research focused on clarifying the role of nano-sized SiO2 on wear behavior of cyanoacrylate. Pin-on-disk wear test, SEM imaging and microhardness test was utilized to investigate wear performance of cyanoacrylate and its nanocomposites with SiO2. The results indicated that SiO2 nano powders can reduce wear rate of cyanoacrylate and change its wear mechanism. It was also shown that surface hardness of cyanoacrylate is increased by addition of nano-sized SiO2.

Background: 2-octyl cyanoacrylate (2OCA) high viscosity tissue adhesive (Medline Industries, Inc., Mundelein, IL) is a liquid topical skin adhesive. 2OCA offers the same design features and clinical utility in terms of flexibility, strength, and low complication rate as the commercially available 2OCA tissue adhesives. Additionally, 2OCA features high viscosity allowing for better control during the application process, polymerization without the use of an external activator, and a distinctive violet color for ease of application visualization. Objective: The aim of this prospective case series is to descriptively report clinical data with the application of 2OCA as a topical incision closure system in female pelvic surgery. The primary outcomes included: incisional pain, incisional dehiscence, and post-operative bleeding. The secondary outcome included drying times of the adhesive with regard to incision length. Methods: A prospective open-label observational case series study was conducted to evaluate the use of 2OCA in surgical wound closure of the topical skin in adult patients undergoing gynecologic surgical procedures. A total of 50 adult women undergoing gynecologic surgery were enrolled. 2OCA was applied only by surgeons who had undergone product training to the incisions in a standardized, protocol-defined fashion. Drying times for the adhesive and photography were recorded intraoperatively. Post treatment follow-up was conducted with queries of pain level, incisional dehiscence, and incisional bleeding immediately post-operatively, 48 hours, 5 - 10 days and 14 days post-treatment. Adverse events were documented. Results: 2OCA was applied to a total of 154 incisions from the 50 patients enrolled to the study. The procedures included: 16 laparoscopic total hysterectomies, 4 diagnostic laparoscopies, 2 laparoscopic myomectomies, 2 laparoscopic bilateral or unilateral salpingo-oophorectomies, 5 total robotic-assisted laparoscopic surgeries (2 total hysterectomies, 1 supracervical hysterectomy, 1 sacrocolpop

From a mere diagnostic tool to an imperative treatment modality,endoscopic ultrasound(EUS)has evolved and revolutionized safer efficient options for vascular interventions.Currently it is an alternative treatment option in the management of gastrointestinal bleeding,primarily variceal type bleeding.Conventional treatment option prior to EUS incorporation had limited efficiency and high adverse events.The characterization and detail provided by EUS gives a cutting edge towards a holistically successful management choice.Data indicates that EUS-guided combination therapy of coil embolization and glue injection has the higher efficacy for the treatment of varices.Conversely,similar treatment options that exist for esophageal and other ectopic variceal bleeding was also outlined.In conclusion,many studies refer that a combination therapy of coil and glue injection under EUS guidance provides higher technical success with fewer recurrence and adverse events,making its adaptation in the guideline extremely favorable.Endo-hepatology is a novel disciple with a promising future outlook,we reviewed topics regarding portal vein access,pressure gradient measurement,and thrombus biopsy that are crucial interventions as alternative of radiological procedures.The purpose of this review is to provide an update on the latest available evidence in the literature regarding the role of EUS in vascular interventions.We reviewed the role of EUS in variceal bleeding in recent studies,especially gastric varices and novel approaches aimed at the portal vein.

Background Self and mutual-aiding occlusive dressing is a novel method to treat with the wounds in special circumstances.This study aims to prepare a new antimicrobial adhesive for the dressing and evaluate the application effects of the adhesive.Methods The main component of the new antimicrobial adhesive was 5％ triclosan / cyanoacrylate （CA） antimicrobial adhesive.The adhesive was modified with carboxylic multi-walled carbon nanotubes （MWCNTs-COOH）,multi-walled carbon nanotubes （MWCNTs）,hydrophobic nano-silica,nitrile rubber,epoxy resin and polymethyl methacrylate （PMMA） respectively.The bond strength,toughness and viscosity of the modified adhesive in different concentrations were examined to select the optimal modifying material and the best ratio to prepare the new antimicrobial adhesive according to the results.After that,the antimicrobial property of the new antimicrobial adhesive was tested by filter paper method.At last,we disposed the injury models in rats using the new antimicrobial adhesive to examine the application effects.Results In individual tests,the bond strength modification performance of 0.064％ MWCNTS-COOH is the best,the bond strength is （14.71±1.48） Mpa.8％ nano-silica shows the best toughness modification performance,the Tg is （1.10±0.24）℃.The viscosity modification performance of 8％ nano-silica is the best,the viscosity is （15 536.68±28.4） cP.However,consolidating three test results,6％ nano-silica / antimicrobial adhesive has the balanced bond strength,toughness and viscosity.Its bond strength is （14.03±1.92） Mpa,the Tg is （3.60±0.68）℃,and the viscosity is （5 278.87±31.68） cP.The inhibition zone diameter of 6％ nano-silica / antimicrobial adhesive and antimicrobial adhesive group in Day 5 is （28.61±0.91） mm versus （28.24±2.69） mm （P ＞0.05）.In animal studies,both in blood routine test and pathological section,6％ nano-silica / antimicrobial adhesive group shows lower white blood cells count than gauze bandage group （P ＜0.05）.Conclusions 6％ nano-silica has the optimal effect of bond strength
