cortical plasticity and nerve regeneration after peripheral nerve injury

With the development of neuroscience, substantial advances have been achieved in peripheral nerve regeneration over the past decades. However, peripheral nerve injury remains a critical public health problem because of the subsequent impairment or absence of sensorimotor function. Uncomfortable complications of peripheral nerve injury, such as chronic pain, can also cause problems for families and society. A number of studies have demonstrated that the proper functioning of the nervous system depends not only on a complete connection from the central nervous system to the surrounding targets at an anatomical level, but also on the continuous bilateral communication between the two. After peripheral nerve injury, the interruption of afferent and efferent signals can cause complex pathophysiological changes, including neurochemical alterations, modifications in the adaptability of excitatory and inhibitory neurons, and the reorganization of somatosensory and motor regions. This review discusses the close relationship between the cerebral cortex and peripheral nerves. We also focus on common therapies for peripheral nerve injury and summarize their potential mechanisms in relation to cortical plasticity. It has been suggested that cortical plasticity may be important for improving functional recovery after peripheral nerve damage. Further understanding of the potential common mechanisms between cortical reorganization and nerve injury will help to elucidate the pathophysiological processes of nerve injury, and may allow for the reduction of adverse consequences during peripheral nerve injury recovery. We also review the role that regulating reorganization mechanisms plays in functional recovery, and conclude with a suggestion to target cortical plasticity along with therapeutic interventions to promote peripheral nerve injury recovery.

Modified constraint-induced movement therapy(mCIMT)has shown beneficial effects on motor function improvement after brain injury,but the exact mechanism remains *** this study,amplitude of low frequency fluctuation(ALFF)metrics measured by resting-state functional magnetic resonance imaging was obtained to investigate the efficacy and mechanism of mCIMT in a control co rtical impact(CCI)rat model simulating traumatic brain *** 3 days after control co rtical impact model establishment,we found that the mean ALFF(mALFF)signals were decreased in the left motor cortex,somatosensory co rtex,insula cortex and the right motor co rtex,and were increased in the right corpus *** 3 weeks of an 8-hour daily mClMT treatment,the mALFF values were significantly increased in the bilateral hemispheres compared with those at 3 days *** mALFF signal valu es of left corpus callosum,left somatosensory cortex,right medial prefro ntal cortex,right motor co rtex,left postero dorsal hippocampus,left motor cortex,right corpus callosum,and right somatosensory cortex were increased in the mCIMT group compared with the control cortical impact ***,we identified brain regions with significantly decreased mALFF valu es at 3 days *** correlation coefficients with the right forelimb sliding score indicated that the improvement in motor function of the affected upper limb was associated with an increase in mALFF values in these brain *** findings suggest that functional co rtical plasticity changes after brain injury,and that mCIMT is an effective method to improve affected upper limb motor function by promoting bilateral hemispheric co rtical *** values correlate with behavio ral changes and can potentially be used as biomarkers to assess dynamic cortical plasticity after traumatic brain injury.

Nerve transfer helps repair brachial plexus injury by increasing cerebral cortical plasticity

In the treatment of brachial plexus injury, nerves that are functionally less important are transferred onto the distal ends of damaged crucial nerves to help recover neuromuscular function in the target region. For example, intercostal nerves are transferred onto axillary nerves, and accessory nerves are transferred onto suprascapular nerves, the phrenic nerve is transferred onto the musculocutaneous nerves, and the contralateral C7 nerve is transferred onto the median or radial nerves. Nerve transfer has become a major method for reconstructing the brachial plexus after avulsion injury. Many experiments have shown that nerve transfers for treatment of brachial plexus injury can help reconstruct cerebral cortical function and increase cortical plasticity. In this review article, we summarize the recent progress in the use of diverse nerve transfer methods for the repair of brachial plexus injury, and we discuss the impact of nerve transfer on cerebral cortical plasticity after brachial plexus injury.

＜正＞Pain is composed of multiple dimensions including sensory-discriminative,affective-motivational and cognitive -evaluative *** it has been gradually known that nociceptive information is processed through widelydistributed, hierachically-interconnected neural network,referred to as neuromatrix,in the ***,the knowledge of pain perception,emotion and cognition at cellular and network level is surprisingly poor and requires to be gained by experimental research using multiple approaches at the cerebral *** the other hand,nociception,when persists,may result in comorbidities of pain such as anxiety,loss of sleep,amnesia,depression,*** still lack of effective therapeutic approaches in *** studies on the effects of persistent or chronic pain on cerebral cortical functions and structures are still *** this symposium,I would like to present some update data showing how persistent nociception influences cerebral cortices including somatosensory cortex area 1（SS1）,anterior cingulate cortex（ACC） and hippocampal formation（HF） which are believed to be involved in various functions of the *** using in vivo evoked potential recordings,in vitro multi-electrode-array（8x8 MEA） and whole-cell patch-clamp recordings on acutely prepared brain slices of rats suffering from persistent nociception,synaptic responses were significantly enhanced not only temporally （LTP-like） but also spatially（network enlargement） in SS 1 and HF,however in ACC,spatial enlargement of synaptic connections were observed,but LTP-like phenomenon was not significantly ***,the altered synaptic connections and functions at the cerebral levels were demonstrated to be caused by peripheral persistent nociception because blockade of impulses in the periphery could eliminate the cerebral changes *** single synaptic level, it was further shown that imbalance between excitatory（EAA） and inhibitory（IAA） amino acids at pre-synaptic components an

We have characterized cortical plasticity changes in schizophrenia *** findloss of cortical-striatal motor evoked potentials potentiation in these patients when treated with transcranial magnetic stimulation protocols for plasticity ***,the loss of cortical plasticity wascorrelated to impaired motor learning ability in a rotary pursuit ***,the loss of plasticity correlates to clinical schizophrenia symptoms in these *** results suggest that schizophrenia might lead to altered cortical plasticity and functioning.

Fear memory contextualization is critical for selecting adaptive behavior to *** fear conditioning(CFC)is a classical model for elucidating related underlying neuronal *** primary visual cortex(V1)is the primary cortical region for contextual visual inputs,but its role in CFC is poorly ***,our experiments demonstrated that bilateral inactivation of V1 in mice impaired CFC retrieval,and both CFC learning and extinction increased the turnover rate of axonal boutons in *** frequency of neuronal Ca^(2+)activity decreased after CFC learning,while CFC extinction reversed the decrease and raised it to the naïve *** to control mice,the frequency of neuronal Ca^(2+)activity increased after CFC learning in microglia-depleted mice and was maintained after CFC extinction,indicating that microglial depletion alters CFC learning and the frequency response pattern of extinction-induced Ca^(2+)*** findings reveal a critical role of microglia in neocortical information processing in V1,and suggest potential approaches for cellular-based manipulation of acquired fear memory.

We adopted professional diving players as a typical subject pool to explore whether structural brain differences relative to motor skill acquisition exist between highly skilled athletes and *** on the voxel-based morphometric(VBM) technique,structural MRIs of the brains of 12 elite diving players with professional training were analyzed and compared with those of control subjects without any professional physical *** players showed significantly increased gray matter density in the thalamus and left pre-central gyrus than control ***,future researches are needed to prove the contribution of preposition and *** also suggests that athletes as the subject pool could form a new subject pool to explore plastic change induced by motor skill acquisition.
