Vaccination against coronavirus disease 2019 in patients with pulmonary hypertension:A national prospective cohort study

Background:coronavirus disease 2019(COVID-19)has potential risks for both clinically worsening pulmonary hypertension(PH)and increasing mortality.However,the data regarding the protective role of vaccination in this population are still lacking.This study aimed to assess the safety of approved vaccination for patients with PH.Methods:In this national prospective cohort study,patients diagnosed with PH(World Health Organization[WHO]groups 1 and 4)were enrolled from October 2021 to April 2022.The primary outcome was the composite of PH-related major adverse events.We used an inverse probability weighting(IPW)approach to control for possible confounding factors in the baseline characteristics of patients.Results:In total,706 patients with PH participated in this study(mean age,40.3 years;mean duration after diagnosis of PH,8.2 years).All patients received standardized treatment for PH in accordance with guidelines for the diagnosis and treatment of PH in China.Among them,278 patients did not receive vaccination,whereas 428 patients completed the vaccination series.None of the participants were infected with COVID-19 during our study period.Overall,398 patients received inactivated severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)vaccine,whereas 30 received recombinant protein subunit vaccine.After adjusting for baseline covariates using the IPW approach,the odds of any adverse events due to PH in the vaccinated group did not statistically significantly increase(27/428[6.3%]vs.24/278[8.6%],odds ratio=0.72,P=0.302).Approximately half of the vaccinated patients reported at least one post-vaccination side effects,most of which were mild,including pain at the injection site(159/428,37.1%),fever(11/428,2.6%),and fatigue(26/428,6.1%).Conclusions:COVID-19 vaccination did not significantly augment the PH-related major adverse events for patients with WHO groups 1 and 4 PH,although there were some tolerable side effects.A large-scale randomized controlled trial is warranted to confirm this finding.The final approval

Identifying critically ill patients at risk of death from coronavirus disease

BACKGROUND:A pandemic of coronavirus disease(COVID-19)has been declared by the World Health Organization(WHO)and caring for critically ill patients is expected to be at the core of battling this disease.However,little is known regarding an early detection of patients at high risk of fatality.METHODS:This retrospective cohort study recruited consecutive adult patients admitted between February 8 and February 29,2020,to the three intensive care units(ICUs)in a designated hospital for treating COVID-19 in Wuhan.The detailed clinical information and laboratory results for each patient were obtained.The primary outcome was in-hospital mortality.Potential predictors were analyzed for possible association with outcomes,and the predictive performance of indicators was assessed from the receiver operating characteristic(ROC)curve.RESULTS:A total of 121 critically ill patients were included in the study,and 28.9%(35/121)of them died in the hospital.The non-survivors were older and more likely to develop acute organ dysfunction,and had higher Sequential Organ Failure Assessment(SOFA)and quick SOFA(qSOFA)scores.Among the laboratory variables on admission,we identifi ed 12 useful biomarkers for the prediction of in-hospital mortality,as suggested by area under the curve(AUC)above 0.80.The AUCs for three markers neutrophilto-lymphocyte ratio(NLR),thyroid hormones free triiodothyronine(FT3),and ferritin were 0.857,0.863,and 0.827,respectively.The combination of two easily accessed variables NLR and ferritin had comparable AUC with SOFA score for the prediction of in-hospital mortality(0.901 vs.0.955,P=0.085).CONCLUSIONS:Acute organ dysfunction combined with older age is associated with fatal outcomes in COVID-19 patients.Circulating biomarkers could be used as powerful predictors for the in-hospital mortality.

BACKGROUND: The study aims to illustrate the clinical characteristics and development of septic shock in intensive care unit(ICU) patients confirmed with severe acute respiratory syndrome coronavirus 2(SARS-Co V-2) infection, and to perform a comprehensive analysis of the association between septic shock and clinical outcomes in critically ill patients with coronavirus disease(COVID-19).METHODS: Patients confirmed with SARS-Co V-2 infection, who were admitted to the ICU of the Third People's Hospital of Shenzhen from January 1 to February 7, 2020, were enrolled. Clinical characteristics and outcomes were compared between patients with and without septic shock.RESULTS: In this study, 35 critically ill patients with COVID-19 were included. Among them, the median age was 64 years(interquartile range [IQR] 59-67 years), and 10(28.4%) patients were female. The median ICU length of stay was 16 days(IQR 8-23 days). Three(8.6%) patients died during hospitalization. Nine(25.7%) patients developed septic shock in the ICU, and these patients had a significantly higher incidence of organ dysfunction and a worse prognosis than patients without septic shock.CONCLUSIONS: Septic shock is associated with a poor outcome in critically ill COVID-19 patients and is one of the hallmarks of the severity of patients receiving ICU care. A dysregulated immune response, uncontrolled infl ammation, and coagulation disorders are strongly associated with the development and progression of COVID-19-related septic shock.

Comparison of clinical characteristics in patients with coronavirus disease and influenza A in Guangzhou, China

BACKGROUND: This study aims to compare the epidemiological, clinical and laboratory characteristics between patients with coronavirus disease(COVID-19) and influenza A(H1N1), and to develop a differentiating model and a simple scoring system.METHODS: We retrospectively analyzed the data from patients with COVID-19 and H1N1. The logistic regression model based on clinical and laboratory characteristics was constructed to distinguish COVID-19 from H1N1. Scores were assigned to each of independent discrimination factors based on their odds ratios. The performance of the prediction model and scoring system was assessed. RESULTS: A total of 236 patients were recruited, including 20 COVID-19 patients and 216 H1N1 patients. Logistic regression revealed that age >34 years, temperature ≤37.5℃, no sputum or myalgia, lymphocyte ratio ≥20% and creatine kinase-myocardial band isoenzyme(CK-MB) >9.7 U/L were independent differentiating factors for COVID-19. The area under curves(AUCs) of the prediction model and scoring system in differentiating COVID-19 from H1N1 were 0.988 and 0.962, respectively. CONCLUSIONS: There are certain differences in clinical and laboratory features between patients with COVID-19 and H1N1. The simple scoring system may be a useful tool for the early identification of COVID-19 patients from H1N1 patients.

Introduction: This study aimed to describe the clinical and microbiological characteristics of individuals confirmed to have coronavirus disease 2019 (COVID-19) in the laboratory of virological hemorrhagic fever in Guinea. Method: This was a cross-sectional and descriptive study conducted from March 19 to June 19, 2020, in the virological hemorrhagic fever laboratory in Guinea. Samples collected from individuals living in the Conakry region, which were positive for the reverse-transcriptase polymerase chain reaction test for COVID-19, were included in this study. Results: Among the confirmed cases (N = 743), the males (n = 440, 59%) were more than the females (n = 303, 41%). The mean age of the confirmed cases was 33.31 ± 16.08 years. Almost all the confirmed cases (n = 515, 69.3%) had contact with a confirmed case of COVID-19. At the time of collection, 78.7% (481) of the cases were asymptomatic, while 35.3% (262) were symptomatic. At the time of the sample collection, the main symptoms were fever (51.53%), cough (49.24%), physical asthenia (46.18%), sore throat (33.2%), headache (28.62%), anosmia (9.92%), nausea, vomiting, and abdominal pain (5.34%), and diarrhea (1.53%). Only 2% of the cases had comorbidities, including diabetes (n = 7) and hypertension (n = 5). The viral load of the asymptomatic and symptomatic patients was similar with the cycle threshold value, all of which varied from 10 - 19, 20 - 29, and 30 - 36, respectively. In the first test, 27 samples were declared undetermined;two days later, samples were re-taken and re-tested and 18 of them were positive for COVID-19. The average length of hospitalization was 14 days for patients admitted to the hospital. On the other hand, the non- hospitalized patients isolated themselves at home for 22 days. Both coronavirus disease 2019(COVID-19)and severe acute respiratory syndrome(SARS)are epidemic,contagious,sudden,and publicly harmful diseases.The whole-genome nucleotide identity of the pathogens causing the two diseases reached 79.5%.The mechanism and treatment of COVID-19 are still under investigation.Combining the experiences of SARS prevention and treatment in 2003,and the case data and literature of COVID-19,the similarities and differences between the two diseases in terms of causes,susceptible people,characteristics,dialectical mode,and treatment were analyzed.The two diseases are both plagues in terms of Chinese medicine.The cause of SARS was"heat poison,"and its pathogenesis was"heat poison,stasis,and qi and yin deficiency."Therefore the treatment regimen was mainly to clear away heat,detoxicate,and expel evil.While the cause of COVID-19 was"wet poison,"and its pathogenesis was"wet,poison,heat,stasis,close,syncope,and yang collapse."Hence the basic treatment strategy was to declare lung and remove dampness.Treatment of COVID-19 was clearing away evil qi as soon as possible,with the basic treatment regimen,which was declaring lung,removing dampness,and dispelling the evil.Combined with Traditional Chinese medicine’s(TCM’s)understanding and experience in SARS prevention and treatment,and Chinese clinical cured cases,we try to provide strategies for people all over the world to understand and respond to COVID-19,through the analysis and comparison.To improve COVID-19 prevention and treatment regimen and give full play to the advantages of TCM.

Successful Treatment of coronavirus disease with Integrated Traditional Chinese and Western Medicine: A Case Report

Background:coronavirus disease(COVID-19)is a new outbreak infectious disease,caused by severe acute respiratory syndrome coronavirus 2.Nearly 80,000 people have got infected in China in just 2 months.Up to now,there still has no specific effective treatment for this disease.While integrated traditional Chinese and Western medicine has definite curative effect,could significantly reduce the disease mortality and shorten the course of treatment.Case Report:Here we report a 31-year-old female case of COVID-19 in a family cluster in Beijing.She reported low fever,cough with a little white sputum,fatigue,watery diarrhea,anorexia on admission.The COVID-19 in Traditional Chinese Medicine(TCM)belonged to plagues caused by dampness toxin.The patient was treated with TCM from dampness combined with antiviral drug abidol.After four visits,the patient was significantly improved with symptoms disappearing,chest-computed tomography dissipating,viral nucleic acid turning negative.She discharged and donated 200 ml convalescent plasma after 19 days hospitalization.Conclusion:The present case demonstrated that integrated traditional Chinese and Western medicine could treat COVID-19 effectively.However,clinical randomized controlled trials are warranted to verify the precise clinical efficacy.

900 abusers were tested for the presence of drug abuse and coronavirus disease.In the early stage,the throat and noise samples of these patients were examined by polymerase chain reaction(PCR).The mean age of the study group is 42±23.Hematological and serological examinations were performed on 131 patients.All data were analyzed by t-test and one-way analysis of variance(ANOVA).Therefore,Pcoronavirus disease showed positive serological tests and lymphopenic condition.All of urine or blood samples in the 131 patients were positive for the presence of abuse drugs.C-reactive protein(CRP)is one of important serologic test that detected on these patients.This study showed the importance of molecular genetics test such as PCR and also hematology and serology tests in distinguish of coronavirus diseases.
