Due to the continuous development in the field of bariatric surgery, there is an increasing need for postbariatric body contouring surgery. The morbidity of postbariatric patients predisposes them to develop wound healing complications. In this article we describe the preoperative, intraoperative and surgical factors influencing the wound healing and therefore the final outcome. The most common postbariatric body contouring procedures, including brachioplasty, breast contouring surgery, abdominoplasty/circumferential body lift and medial thigh lift are being discussed in terms of wound healing characteristics and subsequent complications. The preoperative preparing as well as special operative techniques are described in order to achieve a low rate of wound healing complications.

Subcutaneous white adipose tissue (sWAT) can be described micromechanically as a foam structure. It is shown that according to this model, mechanical stiffness of this tissue is primarily dependent on the average cell size and is almost independent of the dispersion of cell sizes in a local adipocytes’ population. Whereas the influence of natural fat renewal process with a rate of 10% per year must be of minor importance for mechanical properties of sWAT, induced adipocytes’ death can substantially reduce local sWAT stiffness. The sWAT which contains two or more different subpopulations of adipocytes of varying sizes with a spatially clustered structure can demonstrate significant inhomogeneity of their mechanical properties when compared with those of sWAT consisting of a single population of adipocytes. It is proposed that this effect may be an important pathophysiological feature of cellulite. Transformation of the cell shape from quasispherical to wrinkled or elliptical forms makes adipocytes more susceptible to thermo-mechanical stress reducing the strain needed to achieve the local plastic deformation. These mechanical features of sWAT are essential for understanding the mechanisms of different non-invasive and minimal invasive body contouring procedures.

It is shown that modulation of subcutaneous white adipose tissue (sWAT) volume after application of different physical factors in body contouring procedures cannot be characterized by a single physiological reaction with dispersed reaction times, supposing that several different dynamic processes with different characteristic reaction times must be involved. Quick modulation of sWAT with characteristic times of 103 - 105 s must be connected with such processes as hyaluronan production and consequent water retention in the tissue, as well as with reversible modulation of its mechanical properties. It is supposed that slow processes with characteristic times of up to 3 × 106 s can be caused by modulation of lipolytic activity of adipocytes through changesin their micro-environment or through their transdifferentiation, as well as through death and removal of adipocytes from sWAT.

Facial skeletal contouring surgery such as reduction malarplasty and reduction angleplasty is popular in Asian countries. With rapid economic development, an increasing number of patients receive facial skeletal contouring surgery combined with other facial aesthetic sur geries. From January 2014 to December 2016, a total of 37 patients underwent Facial skeletal contouring surgery combined with facial esthetic surgeries in our department. The modified L-shape reduction malarplasty and reduction mandibular angleplasty were performed using reciprocating saws. The combined facial esthetic surgeries are 4 cases of double eyelid blepharoplasty, 6 cases of augmentation rhinoplasty, 2 cases of autologous fat transplantation and 1 case of augmentation genioplasty. 36 patients were satisfied with the final facial contour and appearance. No severe complications were seen in our series. Facial skeletal contouring surgeries can drastically and permanently change the patient's facial contour. Plastic surgeons should be acquainted with the metabolic aesthetic trends and be familiar with all the surgical techniques. The selection of an appropriate procedure or combined procedures for individual patients should be made according to the cultural background, patient's chief complain, anatomic variations and possible operative sequ elae.

Aim: The post-bariatric patients who undergo torsoplasty often incur postoperative complications that dilate their recovery time. The authors propose the technique of extended abdominoplasty, described in 2012 for aesthetic torso remodeling, in post-bariatric patients in order to reduce complications and hospitalization time. Methods: The authors performed 21 extended abdominoplasties and compared them with 21 torsoplasties in post-bariatric patients during a 1-year period. Data studied was age, gender, duration of the procedure, blood loss, complications and hospitalization duration. Results: Peri and postoperative bleeding led to 3 cases of anemia necessitating blood transfusions in 14.3% of the second group;no blood transfusion was needed in the first group. No other major complications occurred during the postoperative period in both groups. Conclusion: The authors believe that extended abdominoplasty is a viable alternative to the torsoplasty in selected patients presenting a severe adipocutaneous circumferential laxity, as occurs in post-bariatric patients with a significant weight loss.

AIM To evaluate the safety and efficacy of Viora's new multipolar radiofrequency(RF) handpiece. METHODS A group of twelve volunteers(11 females and 1 male) participated in the current study, ranging in age from 23-70 years with Fitzpatrick skin type Ⅱ-Ⅴ. The inclusion criteria for the enrollment were no contraindications for the treatment, body mass index(BMI) < 35 and local fat accumulation or cellulite formation. A total of 19 treatment areas were treated in the study: 9 abdomen, 2 abdomen plus flanks, 2 arms and 6 thighs. The treatment performed with new multi-polar RF handpiece(V-FORM) with 4 levels of RF power(up to 50 W), 4 levels of vacuum pressure intensity(up to 500 mbar) and 4 operational modes(0.8, 1.7 and 2.45 MHz). Circumferential reduction and cellulite reduction treatments were performed once a week(7 ± 1 d) for a treatment series of 3-8 sessions. The clinical assessment of the treatment outcomes included skin moisture level, skin impedance, body temperature, circumferential measurements, clinical photographic assessment and BMI. RESULTS Ten of twelve patients completed the treatment course. No side effects were recorded during the study. The skin responded with slight erythema and sometimes edema, which is considered a positive end-point. All patients maintained a stable weight during the entire period of the study. No patient underwent any treatments or took medications for fat volume reduction during the study. A moderate positive correlation was found between the patient's age and BMI(correlation coefficient 0.54). The initial body temperature increased in average to 34.0 ℃ from 31.9 ℃, the initial skin moisture level increased to an average 40.98% from 38.9% and the initial skin impedance decreased by 3.8%-35.9% by the end of the treatment course. The pre-heating time for all body areas ranged between 1-6 min with negative correlation to the body's end-point temperature(correlation coefficient-0.31). Background: Obesity is currently considered a public health problem. Bariatric procedures have become an important part of obesity management and, consequently, the number of male patients seeking post-bariatric reconstructive procedures have increased. Therefore, the clinical approach and understanding of the body contour of this population have become more relevant. The goal of post-bariatric reconstruction is to enhance the male silhouette through removal of skin and adipose tissue excess, and abdominal rectus diastasis repair. Material and Methods: We conducted a retrospective study at the Plastic and Reconstructive Surgery Department of the National Medical Center “20 de Noviembre”. All male patients referred to our department to start a post-bariatric reconstruction protocol from January 2018 to December 2022 were included in this study. Results: In total, 15 patients who underwent corporal contouring procedures were included;median age was 49.2 years with minimum of 33 years, and a maximum of 57 years. Median Body mass index was 28.4 kg/m2 with minimum of 22 kg/m2 and maximum of 38 kg/m2. All patients were treated 18 months after their bariatric surgery. All patients underwent an abdominoplasty as a body contouring procedure. 4 (26.7%) patients presented complications related to the surgery. Conclusion: We described a comprehensive and systematic approach to massive weight loss for male patients, suggesting an abdominal marking based on the patient’s clinical features and the expected results avoiding feminization of the abdominal body contour.

Terrain medelling is a common task for an engineer engaged in the field of river engineering. In the past three decades, a number of methods for terrain modelling haVe been proposed in surveying, civil engineering and in other fields. However, workers in different fields do not wish to achieve the same objectives. In river engineering, the data features and data collection methods are different from those in other fields. Therefore, a selection of suitable methods for terrain modelling that match the data features to obtain accurate terrain models is of importance. This paper deals with the methods for terrain modelling in river engineering. Attempts are made to summarize the methods. The problems and suitability of the methods associated with river engineering are discussed. Finally. some suggestions as to the selection of the terrain modelling methods are presented.
