Safety and efficacy of percutaneous transhepatic balloon dilation in removing common bile duct stones:A systematic review

BACKGROUND Endoscopic sphincterotomy(EST) is widely regarded as the first choice in the management of common bile duct(CBD) stones. However, for some patients, this treatment is not possible. The percutaneous transhepatic balloon dilation(PTBD)technique has been suggested as an alternative but has yet to gain wide *** To review cases of PTBD for removing CBD stones and explore the safety and efficacy of this *** We conducted a systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched EMBASE,PubMed, and Web of Science for cases of PTBD that underwent CBD stone removal from 1981 to January 2019. We analyzed all relevant articles available in full text. We extracted data on patient's age, gender, overall technique success rate, reasons for technique failure, and the presence and type of major and minor complications. We analyzed the data and reported the results in a table and ***, we retrieved 12 case series and 6 case reports, for a total of 1347 patients. Thirty cases were excluded due to a lack of patient *** The overall technique success rate for removing a CBD stone was 98.5%(1327/1347) and 98.1%(109/111) for removing concurrent CBD and gallbladder stones. Based on available data(n = 1312), mean age of all patients(687 males and625 females) was 68.9 years. The total number of procedures in the remaining 1317 patients(after exclusion) was 3237(average 2.4 procedures per patient). The total number of failures for eliminating a CBD stone was 20, and the reasons for failure included: Stone impaction(n = 10), intrahepatic bile duct stricture(n = 5),large stone(n = 2), severe CBD dilation(n = 1), multiple stones(n = 1), and duodenal perforation(n = 1). Various major complications related to the procedure were reported, but the incidence rate was low(1.4%). No pancreatitis or procedure related mortality was reported. We establish weak and strong convergence of Ishikawa type iterates of two pointwise asymptotic nonexpansive maps in a Hadamard space. For weak and strong convergence results, we drop “rate of convergence condition”, namely (Cn(x)-1)< to answer in the affirma-tive to the open question posed by Tan and Xu [1] even in a general setup.

Strong Convergence of a General Iterative Algorithm for Mixed Equilibrium, Variational Inequality and common Fixed Points Problems

The aim of this paper, is to introduce and study a general iterative algorithm concerning the new mappings which the sequences generated by our proposed scheme converge strongly to a common element of the set of solutions of a mixed equilibrium problem, the set of common fixed points of a finite family of nonexpansive mappings and the set of solutions of the variational inequality for a relaxed cocoercive mapping in a real Hilbert space. In addition, we obtain some applications by using this result. The results obtained in this paper generalize and refine some known results in the current literature.

Amino Acid Composition Reduces Frequency of common Colds. Results of a Controlled Randomised Completely Masked Trial

Purpose: Special nutrients have an impact on common cold infections. In this trial we have investigated the effectiveness of the specific amino acid composition “aminoplus? immun” on the occurrence, duration and severity of common cold. Material and Methods: Our study is a controlled, randomised completely masked trial comparing genuine trial remedy versus placebo in parallel groups. Eligible subjects were people with recurrent and chronic infections of the ear, nose and pharyngeal area. The treatment period was 90 days. The primary outcome was the mean number of days that the patients had symptoms of infections. Results: From Oct. 2009 to Feb. 2010 a total of 145 patients were recruited, of whom 119 had no major protocol violation and were valid for assessment. Two thirds of the patients were female. The mean age of the patients was 45 years. 65% of participants had come into contact with many people and thereby an increased risk of infection. On average the patients had 11.1 infections during the 12 months before the study. Compliance for the intake of the study preparation was 98.9%. Due to randomisation, the groups were very similar and balanced. The primary and all 6 secondary outcome variables showed a clear superiority of the investigated amino acid composition over placebo (p Conclusion: The specific composition “aminoplus? immun is effective in preventing recurrent and chronic infections of the ear, nose and upper respiratory tract.

A Survey of the Experience and Impact of Acute Upper Respiratory Tract Infections on People in Six Countries in the 2011/2012 common Cold and Flu Season

Introduction: Acute Upper Respiratory Tract Infections (URTIs) are the most common infectious diseases of humankind. While usually mild and self-limiting, they are characterized by a series of simultaneously occurring symptoms/ signs that are sufficiently disruptive to sufferers’ normal activities in which medication is frequently sought. While the literature has many examples of epidemiological studies on these infections, there are few reports on patient experience and impact. This study was designed to investigate these aspects of common Cold/Flu across six countries. Methods: A minimum of 500 adults aged 18 and older were recruited in each of six countries (Brazil, China, Germany, India, Russia, and the US) using customary survey research sampling techniques. Single 30-minute (online) or 40-minute door-to-door quantitative questionnaires with c. 50 questions were completed with each participant by the global research firm Ipsos. Main Findings: Across countries, incidence and seasonality of infections reported to this study were consistent with published data. There appears to be a need for patient education on the causes and transmission routes of respiratory infections. Getting good quality sleep and being able to continue with daily activities as an infection resolves are significant drivers to therapy. The most common non-prescription therapies reported were multi-ingredient products in line with the simultaneously occurring multi-symptom nature of the condition(s). Conclusions: This study indicated that acute URTIs exert a significant deleterious effect on sufferers. Public health education, possibly best undertaken by Pharmacists has the potential to impact the extent of virus transmission by ensuring that people know the true cause of the infection, how it is transmitted and how best to combat this. Cooperative Housing and Cohousing in Canada——The Pursuit of Happiness in the common Courtyards

Risk factors for the development of post-endoscopic retrograde cholangiopancreatography pancreatitis in patients with asymptomatic common bile duct stones

BACKGROUND Previous studies have revealed that patients with asymptomatic common bile duct(CBD)stones are at a high risk of developing post-endoscopic retrograde cholangiopancreatography(ERCP)pancreatitis(PEP).However,no studies to date have addressed the risk factors for PEP in patients with asymptomatic CBD *** To examine the risk factors for PEP in patients with asymptomatic CBD *** Using medical records of three institutions in Japan for 6 years,we identified a total of 1135 patients with choledocholithiasis including 967 symptomatic patients and 168 asymptomatic patients with native papilla who underwent therapeutic *** performed univariate and multivariate analyses to examine the risk factors for PEP in the 168 patients with asymptomatic CBD *** The overall incidence rate of PEP in all the patients with during study period was 4.7%(53/1135).Of the 168 patients with asymptomatic CBD stones,24(14.3%)developed *** univariate analysis,precut sphincterotomy(P=0.009)and biliary balloon sphincter dilation(P=0.043)were significant risk factors for *** multivariate analysis,precut sphincterotomy(P=0.002,95%CI:2.2-27.8,odds ratio=7.7),biliary balloon sphincter dilation(P=0.015,95%CI:1.4-17.3,odds ratio=4.9),and trainee endoscopists(P=0.048,95%CI:1.01-8.1,odds ratio=2.9)were significant risk factors for *** ERCP for asymptomatic CBD stones should be performed by experienced *** performing precut sphincterotomy or biliary balloon sphincter dilation in patients with asymptomatic CBD stones,the placement of a prophylactic pancreatic stent is strongly recommended to prevent PEP.

Clinical Application of Primary Suture Following Three-Port Laparoscopic common Bile Duct Exploration: A Report of 176 Cases

Objective: To investigate the feasibility, safety and the clinical value of primary suture following 3-port laparoscopic common bile duct exploration (LCBDE). Methods: From January 2012 to September 2014, 176 patients suffered from choledocholithiasis were treated with primary suture following 3-port LCBDE and the clinical data were retrospectively analyzed. Results: All cases were operated successfully and none was converted to open surgery. The duration of operation was 92.2 ± 18.8 min and the length of postoperative hospital stay was 4.4 ± 3.7 d. Postoperative bile leakage occurred in 2 cases and these patients recovered by simple drainage for 3 to 7 days without re-operation. All patients recovered smoothly without any serious complications. Conclusions: Primary suture following 3-port LCBDE is safe, effective and mini-invasive, which is worthy of further clinical application.

Development and Evaluation of Species-Specific Biomass Models for Most common Timber and Fuelwood Species of Bangladesh

Allometric biomass models are efficient tools to estimate biomass of trees and forest stands in a non-destructive way. Development of species-specific allometric biomass models requires extensive fieldwork and time. Our study aimed to generate species-specific allometric biomass models for the most common fuelwood and timber species of Bangladesh. We also wanted to evaluate the performances of our models relative to the performances of regional and commonly used pan-tropical biomass models. We used semi-destructive method that incorporates tree-level volume, species-specific biomass expansion factor (BEF), and wood density. We considered four base models, 1) Ln (biomass) = a + bLn (D);2) Ln (biomass) = a + bLn (H);3) Ln (Biomass) = a + bLn (D^2H);4) Ln (Biomass) = a + bLn (D) + cLn (H) to develop species-specific best-fitted models for Total Above-Ground Biomass (TAGB) and stem biomass. The best-fitted model for each species was selected by the lowest value of Akaike Information Criterion (AIC), Residual Standard Error (RSE) and Root Mean Square Error (RMSE). The derived best-fitted models were then evaluated with respect to regional and pan-tropical models using a separate set of observed data. This evaluation was conducted by computing ME (Model Efficiency) and MPE (Model Prediction Error). The best-fitted allometric biomass models have shown higher model efficiency (0.85 to 0.99 at scale 1) and the lowest model prediction error (-8.94% to 5.27%) compared to the regional and pan-tropical models. All the examined regional and pan-tropical biomass models showed different magnitude of ME and MPE. Some models showed higher level (>0.90 at scale 1) of ME compared to the best-fitted specific species biomass model.
