combined treatment of penile keloid： a troublesome complication after circumcision

Dear Editor, We herein present three rare cases of penile keloids that formed after circumcision and were effectively treated with a combined modality. We also discuss their possible causes. The current complication rate for male circumcision is 2%-4%.＇ Hematoma, edema, infection and incision dehiscence are encountered more often, while keloid forma- tion, a common complication after skin injury or surgery, is rarely seen after circumcision. This complication may result from improper cir- cumcision and postoperative management. Penile keloids are more difficult to manage than those on other body sites because mechanical pressure and silicone sheeting, which comprise the normal treatment modality for keloids, are generally considered difficuk to apply to the penis.2-5 Using a novel dressing method that enabled the use of the normal treatment modality, three cases of penile keloids secondary to circumcision were effectively treated. All patients have given their informed consent prior to their inclu- sion in the study. One patient was a 32-year-old Chinese man who had a large, pruritic scar with pain on the penis. Two years prior, he underwent circumcision. Infection occurred on the third post- operative day and resulted in incision dehiscence. The wound healed in 1 month. Shortly before wound closure, the scar started to elevate and became hypertrophic and prominent; growth was slowly pro- gressive for more than 16 months. Physical examination revealed a 2.5-era-diameter circumferential scar cord with a colon-like, reddish surface along the coronal sulcus at the circumcision site. The patient also complained of embarrassment （such as that experienced in public lavatories and bathhouses） and the inability to have intercourse. He also had several hypertrophic scars on his lower abdominal wall and left deltoid region that had formed following dermatitis about 20 years prior. ＜正＞ The technics of combined treatment of ion nitrocarburizing-ion sulphurizing of CrMoCu alloy cast iron has been investigated and the compound layer with nitrocarbonide and sulphide has been made on the surface of CrMoCu alloy cast iron. The compound layer is composed of sulfide surface layer and the nitrocarbonide hypo-surface layer and its diffusing layer. The size of sulfide globular grains distributing equably on the surface is in nano-micron-scale, and the phase structure of the compound layer is composed of FeS、FeS2、Fe2C and Fe3N. Under dry sliding condition, the friction-reducing of sulphurized surface is good, but its function time can’t last very long. The nitrocarbonided+sulphurized surface can greatly improve the wear-resistance and the friction-reducing of CrMoCu alloy cast iron, and its integrated friction and wear properties are better than plain and sulphurized surfaces’.

The aim of this study is to investigate the effect of chemical treatment method on the properties of Posidonia fibers. The chemical treatment which is carried out is a combined hydrogen peroxide and sodium hydroxide treatment. First, an investigation of the treatment processes was undertaken. Secondly, the physical properties (linear density, diameter and ratio length per diameter), the mechanical properties (tenacity, elongation) and chemical properties (FT-IR spectra and X ray diffraction) of posidonia fibers were investigated. The optimum operating conditions were identified using a factorial design.

Optimization and Characterization of combined Degumming Process of Typha angustata L. Stem Fibers

Plant derived natural fibers have been widely investigated as alternatives to synthetic fibers in reinforcing *** over the years have explored many plant fibers using different extraction processes to study their physical,chemical,and mechanical *** this context,the present study relates to the extraction,characterization,and optimization of Typha angustata *** *** this purpose,desirability functions and response surface methodology were applied to simultaneously optimize the diameter(D),linear density(LD);yield(Y),lignin fraction(L),and tenacity(T)of Typha stem *** stems have been subjected to both alkali(NaOH)and enzymatic(pectinex ultra-SPL)*** levels of process variables including enzyme concentration(10,15,and 20 ml/L)and treatment duration(10,15,and 20 days)were used to design the experiments according to the factorial *** results were examined by analysis of variance and fitted to second order polynomial model using multiple regression *** Derringer’s desirability function released that the values of process variables generating optimized diameter,linear density,yield,lignin ratio and tenacity are 20 ml/L and 20 days for concentration of pectinex ultra-SPL enzyme and treatment duration,*** was performed and high degree of correlation was found between the experimental and statistical ***,the morphological structure has been investigated by the scanning electron microscope,showing a crenelated structure of ultimate fiber bundles of cellulose composing the Typha *** to Typha stem non-treated fibers(TSNTF),Typha stem combined treated fibers(TSCTF),brings to improve mechanical *** change in mechanical properties is affected by modifying the fiber structure showing alpha cellulose of(66.86%)and lignin ratio of(10.83%)with a crystallinity index of(58.47%).

Although targeted therapies and immunotherapies have been effective againstseveral malignancies, the respective monotherapies are limited by low and/orshort-term responses. Specific inhibitors of oncogenic signaling pathways andtumor-associated angiogenesis can activate the anti-tumor immune responses byincreasing tumor antigen presentation or intratumor T cell infiltration. Additionalinsights into the effects and mechanisms of targeted therapies on the induction ofanti-tumor immunity will facilitate development of rational and effective combinationstrategies that synergize rapid tumor regression and durable response. Inthis review, we have summarized the recent combinations of targeted therapiesand immunotherapies, along with the associated clinical challenges.

BACKGROUND An intradermal nevus is a common skin tumour,and the classical method of removal has a risk of recurrence and *** is a challenge for dermatologists to treat eyebrow intradermal nevi quickly and *** study focused on investigating the efficacy and safety of shearing combined with electrocautery and curettage in the treatment of eyebrow intradermal *** SUMMARY We describe two adult patients with eyebrow intradermal nevi treated by shearing combined with electrocautery and *** patients were followed up regularly after *** follow-up,no recurrence of eyebrow intradermal nevus and no obvious scars or hypopigmentation were found in either *** results indicated that shearing combined with electrocautery and curettage could remove eyebrow intradermal nevus without side effects and confirmed the efficacy and safety of this modality for treating these skin *** Shearing combined with electrocautery and curettage has superior merits,including simple operation,good cosmetic effects,and high patient satisfaction,presenting great application potential for treating intracutaneous nevus.

Indwelling catheter and conservative measures in the treatment of abdominal compartment syndrome in fulminant acute pancreatitis

AIM： To study the effect of combined indwelling catheter, hemofiltration, respiration support and traditional Chinese medicine （e.g. Dahuang） in treating abdominal compartment syndrome of fulminant acute pancreatitis. METHODS： Patients with fulminant acute pancreatitis were divided randomly into 2 groups of combined indwelling catheter celiac drainage and intra-abdominal pressure monitoring and routine conservative measures group （group 1） and control group （group 2）. Routine non-operative conservative treatments including hemofiltration, respiration support, gastrointestinal TCM ablution were also applied in control group patients. Effectiveness of the two groups was observed, and APACHE Ⅱ scores were applied for analysis. RESULTS： On the second and fifth days after treatment, APACHE Ⅱ scores of group 1 and 2 patients were significantly different. Comparison of effectiveness （abdominalgia and burbulence relief time, hospitalization time） between groups 1 and 2 showed significant difference, as well as incidence rates of cysts formation. Mortality rates of groups 1 and 2 were 10.0% and 20.7%, respectively. For patients in group 1, celiac drainage quantity and intra-abdominal pressure, and hospitalization time were positively correlated （r = 0.552, 0.748, 0.923, P 〈 0.01） with APACHE Ⅱ scores. CONCLUSION： combined indwelling catheter celiac drainage and intra-abdominal pressure monitoring, short veno-venous hemofiltration （SVVH）, gastrointestinal TCM ablution, respiration support have preventive and treatment effects on abdominal compartment syndrome of fulminant acute pancreatitis.

Biological nutrient removal by internal circulation upflow sludge blanket reactor after landfill leachate pretreatment

The removal of biological nutrient from mature landfill leachate with a high nitrogen load by an internal circulation upflow sludge blanket （ICUSB） reactor was studied. The reactor is a set of anaerobic-anoxic-aerobic （A^2/O） bioreactors, developed on the basis of an expended granular sludge blanket （EGSB）, granular sequencing batch reactor （GSBR） and intermittent cycle extended aeration system （ICEAS）. Leachate was subjected to stripping by agitation process and poly ferric sulfate coagulation as a pretreatment process, in order to reduce both ammonia toxicity to microorganisms and the organic contents. The reactor was operated under three different operating systems, consisting of recycling sludge with air （A^2/O）, recycling sludge without air （low oxygen） and a combination of both （A^2/O and low oxygen）. The lowest effluent nutrient levels were realised by the combined system of A^2/O and low oxygen, which resulted in effluent of chemical oxygen demand （COD）, NH3-N and biological oxygen demand （BOD5） concentrations of 98.20, 13.50 and 22.50 mg/L. The optimal operating conditions for the efficient removal of biological nutrient using the ICUSB reactor were examined to evaluate the influence of the parameters on its performance. The results showed that average removal efflciencies of COD and NH3- N of 96.49% and 99.39%, respectively were achieved under the condition of a hydraulic retention time of 12 hr, including 4 hr of pumping air into the reactor, with dissolved oxygen at an rate of 4 mg/L and an upflow velocity 2 m/hr. These combined processes were successfully employed and effectively decreased pollutant loading.
