Background:The purpose of this analysis is to evaluate the current evidence with regard to the effectiveness and safety between coiling and clipping in patients with ruptured intracranial aneurysms(RIAs).Methods:We performed a meta-analysis that compared clipping with coiling between July 2000 and September ***,EMBASE,and the Cochrane Library were searched for related articles *** the treatment efficacy and postoperative complications were ***:We identified three randomized controlled trials and thirty-seven observational studies involving 60,875 patients with ruptured cerebral *** summary results showed that coiling was related a better quality of life(mRS0-2;OR=1.327;CI=1.093-1.612;pcoiling was significantly associated with a better life quality(mRS0-2),a lower incidence of postoperative complications,and a higher rate of mortality,rebleeding,hydrocephalous,and a lower rate of complete occlusion than clipping.

Low-carbon steel is widely used for household appliance and automotive panel steel because of its excellent ***,yield point phenomena easily appear in the low-carbon steel produced by a continuous annealing process and cause degradation to the surface quality during *** effect of the coiling temperature(600-750℃)and annealing temperature(740-820℃)on the yield point behavior is *** tests show that coiling temperature has a greater effect on yield point elongation(YPE)and aging index(AI)than the annealing *** observations show that coiling temperature at 750℃would make the micron-sized carbides appearing at the grain boundary disappear and a number of dispersed nanoscale carbides precipitate in grain interior,corresponding to the highest solid solution carbon content in the matrix of 750℃coiled *** experimental results suggest that AI rather than YPE has a positive relationship with the solid solution carbon content of the low-carbon *** YPE has a positive relationship with the upper/lower yield strength.

Subarachnoid hemorrhage （SAH） is a subtype of stroke that is most commonly caused by the rupture of intracranial aneurysm. Endovascular occlusion of intracranial aneurysms has emerged as an alternative to the established microsurgical clipping, The decision process when choosing a treatment for ruptured intracranial aneurysm should comprise an algorithm that was based on the characteristics of the aneurysm and patient status, age, and neurological condition, rather than a blanket assumption that one modality is superior to another.

Posterior circulation intracranial aneurysm (IA) treatment remains challenging, due to the anatomy of the area and the high rupture possibility. Endovascular treatment seems to be more suitable for these aneurysms, but studies focused on endovascular treatment demonstrate a high rate of re-intervention needing. A meta-analysis might offer a clearer view, being useful in a more effective treatment planning. Methods: A systematic search was performed, using the PubMed database platform. The final article pool contained 20 articles. Studied parameters were operative mortality, late mortality, permanent neurologic deficit (PND), and the need for re-intervention (Re-int). We divided patients into two subgroups, those with ruptured and those with unruptured aneurysm. Statistical analysis was performed using appropriate software. Results: In the total population (645 patients), there was a superiority of coiling over clipping in terms of PND and of coiling in terms of Re-int. As regards mortality, there was no clear superiority of one method over the other. Conclusions: The current study came to the conclusion that there is a superiority of coiling over clipping in terms of PND. On the other hand, clipping seems to be superior to coiling in terms of the need for re-intervention. As regards mortality (both operative and late), there is no clear superiority of one method over the other. Studying subgroups of patients (ruptured and unruptured posterior circulation IAs), in terms of PND, there is no superiority of one method over the other. The same goes for Op-Mo on ruptured aneurysms.

Prompt diagnosis and therapy of aneurysms are critical for patients with nontraumatic subarachnoid hemorrhage （SAH）.The aim of our study was to assess the clinical usefulness of multislice computed tomography angiography （CTA） in the surgical and endovascular treatment of intracranial aneurysms.A total of 195 cases with 206 intracranial aneurysms underwent *** （24%） aneurysms underwent surgical clipping while 156 （76%） aneurysms underwent endovascular *** the five missed aneurysms at digital substraction angiography and the nine aneurysms with mass intracerebral hematomas,surgical treatment was successfully performed based on 16-slice CTA alone,and the other 36 aneurysms were clipped on the main basis of the *** intraoperative findings correlated well with the CTA findings and all aneurysms were clipped ***-slice CTA image information has been shown to determine the choice of aneurysm therapy and assist the surgical and endovascular treatment of intracranial aneurysms.

A measure called the Uncertainty in Unruptured Intracranial Aneurysm Patients Undergoing Endovascular coiling Scale (UUIACS) was developed and its validity and reliability were examined. The 49 questions that comprised the original draft of the UUIACS were created based on interview data. Based on data from 172 participants, exploratory and confirmatory factor analyses were conducted. As a result of exploratory factor analysis, the UUIACS e retained 17 items and extracted four factors (“Lack of decision-making cues”, “Lack of information and complexity of information interpretation”, “The ambiguous nature of the disease”, and “The unpredictable living with UIA”). All of the UUIACS items showed adequate internal consistency. Between the UUIAC scale and the Universal Uncertainty in Illness Scale (UUIS), the Health Locus of Control (HLC) scale, and the SF-36v2® (Japanese version), positive correlations were found between the UUIACS and UUIS, and the HLC scale at a 1% significance level indicating concurrent validity. According to confirmatory factor analysis, the UUIACS had an acceptable goodness of fit. Given these findings, the UUIACS was judged to have satisfied the criteria for use in a clinical setting, although further investigation was required.

Clipping versus coiling in Ruptured Aneurysms of the Anterior Cerebral Circulation

Objective: To evaluate and compare the outcomes of microsurgical clipping and endovascular coiling of ruptured anterior cerebral circulation aneurysms. Methods: The current study retrospectively included 61 patients presented with subarachnoid hemorrhage (SAH), treated by clipping or coiling for ruptured aneurysms located in anterior circulation. Anatomic outcome and post-operative ischemic events were evaluated. The primary clinical outcomes were evaluated as operative complications while the secondary clinical outcome was evaluated by modified Rankin scale (mRS) at discharge. Results: Of 61 ruptured aneurysms, 47 and 14 were treated by surgery and coiling, respectively. The average follow-up duration was 19.1 ± 18.6 months for clipping and 21.7 ± 14.8 months for coiling. Complete occlusion was greater in surgery group (80.8%) than endovascular group (21.4%). Re-growth occurred in 2 cases of the coiling group only. Ischemic events were encountered in both groups;silent ones were higher in coiling group (21.4%) than in clipping group (6.3%), while symptomatic ones were higher in the clipping group (49%). Good outcome (mRS 0 - 2) was 51% and 71.4% after clipping and coiling respectively. Postoperative hospital period was longer after clipping (P = 0.04). Conclusions: For anterior circulation ruptured aneurysms, both treatments are feasible, coiling showed lower rate of morbidity while clipping results were better regarding durability and recurrence. Coil embolization may be preferred, which is the current point proved by the RCT like ISAT, but if you choose the patients carefully and find out the optimal candidate for the clipping, the clipping is still enough feasible as a mainstream.
