Coronavirus disease 2019 (COVID-19) is now pandemic worldwide and has heavily overloaded hospitals in Wuhan City, China during the time between late January and February. We reported the clinical features and therapeutic characteristics of moderate COVID-19 cases in Wuhan that were treated via the integration of traditional Chinese medicine (TCM) and Western medicine. We collected electronic medical record (EMR) data, which included the full clinical profiles of patients, from a designated TCM hospital in Wuhan. The structured data of symptoms and drugs from admission notes were obtained through an information extraction process. Other key clinical entities were also confirmed and normalized to obtain information on the diagnosis, clinical treatments, laboratory tests, and outcomes of the patients. A total of 293 COVID-19 inpatient cases, including 207 moderate and 86 (29.3%) severe cases, were included in our research. Among these cases, 238 were discharged, 31 were transferred, and 24 (all severe cases) died in the hospital. Our COVID-19 cases involved elderly patients with advanced ages (57 years on average) and high comorbidity rates (61%). Our results reconfirmed several well-recognized risk factors, such as age, gender (male), and comorbidities, as well as provided novel laboratory indications (e.g., cholesterol) and TCM-specific phenotype markers (e.g., dull tongue) that were relevant to COVID-19 infections and prognosis. In addition to antiviral/antibiotics and standard supportive therapies, TCM herbal prescriptions incorporating 290 distinct herbs were used in 273 (93%) cases. The cases that received TCM treatment had lower death rates than those that did not receive TCM treatment (17/273= 6.2% vs. 7/20= 35%, P = 0.0004 for all cases;17/77= 22% vs. 7/9= 77.7%, P = 0.002 for severe cases). Background:Cryptococcal meningitis(CM)is one of the most common opportunistic infections caused by Cryptococcus neoformans in human immunodeficiency virus(HIV)-infected patients,and is complicated with significant morbidity and mortality.This study retrospectively analyzed the clinical features,characteristics,treatment,and outcomes of first-diagnosed HIV-associated CM after 2-years of follow-up.Methods:Data from all patients(n=101)of HIV-associated CM hospitalized in Shanghai Public Health clinical Center from September 2013 to December 2016 were collected and analyzed using logistic regression to identify clinical and microbiological factors associated with mortality.Results:Of the 101 patients,86/99(86.9%)of patients had CD4 countclinical features and outcomes of first diagnosed HIV-associated CM with 2-year follow-up data.Altered mental status and coinfection with PCP predicted mortality in HIV-associated CM.

Background： Atopic dermatitis （AD） is an inflammatory skin disease characterized by chronic recurrent dermatitis with profound itching. Most patients have personal and/or family history of atopic diseases. Several criteria have been proposed for the diagnosis of AD. Although the clinical features of childhood AD have been widely studied, there has been less large-scale study on adult/adolescent AD. The aim of this study was to investigate the clinical features of adult/adolescent patients with chronic symmetrical eczemaJAD and to propose Chinese diagnostic criteria for adult/adolescent AD. Methods： A hospital-based study was performed. Forty-two dermatological centers participated in this study. Adult and adolescent patients （12 years and over） with chronic symmetrical eczema or AD were included in this study. Questionnaires were completed by both patients and-investigators. The valid questionnaires were analyzed using EpiData 3.1 and SPSS 17.0 software. Results： A total of 2662 valid questionnaires were collected （1369 male and 1293 female）. Of all 2662 patients, 2062 （77.5%） patients had the disease after 12 years old, while only 600 （22.5%） patients had the disease before 12 years old, suggesting late-onset eczema/AD is common. Two thousand one hundred and thirty-nine （80.4%） patients had the disease for more than 6 months. One thousand one hundred and forty-four （43.0%） patients had a personal and/or family history of atopic diseases. One thousand five hundred and forty-eight （58.2%） patients had an elevated total serum IgE and/or eosinophilia and/or positive allergen-specific IgE. Based on these clinical and laboratory features, we proposed Chinese criteria for adult/adolescent AD. Of all 2662 patients, 60.3% were satisfied with our criteria, while only 48.2% satisfied with Hanifin Rajka criteria and 32.7% satisfied with Williams criteria, suggesting a good sensitivity of our criteria in adult/adolescent AD patients. INTRODUCTIONMyasthenia gravis （MG） is an autoimmune neuromuscular disorder caused by anti-acetylcholine receptor antibodies （AChR-Abs） or other etiologies.Ocular symptoms were the first presentations in 40-50％ of MG patients.Ptosis and diplopia were the most common symptoms of ocular MG （OMG）.Limited studies demonstrated that patients presented with initial symptoms of only ptosis in 47％,only diplopia in 14％ and both ptosis and diplopia in 39％ of OMG.[1] Variations of diplopia or ptosis did not significantly affect the physical or mental status of OMG.

clinical features and linkage analysis for a Chinese family with autosomal dominant central areolar choroidal dystrophy

Background A Chinese family with autosomal dominant central areolar choroidal dystrophy （CACD） was identified. The purpose of this study was to collect the clinical findings from the family and to identify the genetic entity by linkage analysis.Methods Forty-three individuals from 3 generations of the family underwent ophthalmologic examinations, including best-corrected visual acuity, examination of the anterior segments, and inspection of the ocular fundus after pharmacologic mydriasis. Affected family members further underwent color vision test, color fundus photography, fluorescein angiography, automated perimetry, and electroretinography. The family was followed up for 30 months. Peripheral venous blood or buccal swabs were collected from each family member and genomic DNA was extracted. Linkage analysis was performed for candidate genes or loci using microsatellite markers.Results Seven family members in 3 continuous generations were diagnosed as having autosomal dominant CACD. The family showed progressive development of the disease, affecting both male and female. Age of onset of visual disturbances varied between 11 and 50 years. Phenotypic variability among affected individuals was apparent and ranged from relatively normal-appearing fundus with mild parafoveal pigment mottling to geographic atrophy of the macula. Fluorescein angiography showed hyperfluorescent parafoveal changes in early stage or well-demarcated area of chorioretinal atrophy with enhanced visibility of the residual underlying choroidal vessels in the late stage. Peripheral retina and visual fields were normal in affected individuals. Electroretinogram showed normal or mild reduction in the photopic amplitude. Eight candidate genes （STGD4, RCD1, peripherin/RDS, GUCA 1A, RIMS1, UNC119, GUCY2D, and AIPL1） and two genetic loci （4p15.2-16.3, and 17p13） were excluded to be responsible for the disease by linkage analysis.Conclusions The clinical findings Of this Chinese family with CACD shared similarities with previously reported famil

BACKGROUND The diagnosis of both cancer and intracerebral hemorrhage(ICH)in the same patient is not uncommon,but the clinical features and pathogenesis of patients with colorectal cancer(CRC)and ICH are still not well known.AIM To investigate the clinical features and underlying pathogenesis of ICH in patients with CRC.METHODS A retrospective review of CRC patients complicated with ICH from three centers between January 2014 and December 2020 was performed.clinical data such as laboratory examinations,imaging features,prognosis,and underlying pathogenesis were analyzed.RESULTS Of 16673 identified CRC patients,20(0.12%)suffered from ICH.There were 13 males and 7 females,with an average age(mean±SD)of 68.45±10.66 years.Fourteen patients(70%)had distant metastases and most patients(85%)showed an elevation of one or more cancer biomarkers.The hemorrhagic lesions in 13 patients(65%)were in the intracerebral lobe.Four patients were completely dependent and 4 died within 30 days after hemorrhage.Intratumoral hemorrhage(50%)and coagulopathy(50%)accounted for the majority of hemorrhages.CONCLUSION Patients with ICH and CRC often have clinical features with lobar hemorrhage,distant metastases and poor prognosis.Intratumoral hemorrhage and coagulopathy are the main causes of ICH in patients with CRC.

PBC-AIH overlap syndrome is an autoimmune liver disease between AIH （autoimmunehepatitis） and PBC （primary biliary cirrhosis）. To investigate the characteristic of the PBC-AIH ovedap syndrome, we have conducted a retrospective study of 12 diagnosed ovedap syndrome patients and compared them with typical PBC and AIH on the basis of clinical, biochemical, serological and histopathological findings.

BACKGROUND Since the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)epidemic,numerous studies have been published on SARS-CoV-2-related encephalitis/meningitis,but it has not been established if there are specific clinical characteristics of encephalitis/meningitis associated with SARS-CoV-2 infection.AIM To identify the specific clinical features of cases of encephalitis/meningitis associated with SARS-CoV-2 infection in the context of this virus infection pandemic and investigate their relationship with SARS-CoV-2 infection.METHODS We searched PubMed,and included single case reports and case series with full text in English,reporting original data of coronavirus disease-19(COVID-19)patients with encephalitis/meningitis and a confirmed recent SARS-CoV-2 infection.clinical data were extracted.RESULTS We identified 22 articles(18 single case reports and 4 case series)reporting on a total of 32 encephalitis/meningitis patients with confirmed SARS-CoV-2 infection.SARS-CoV-2 infection was confirmed through reverse transcriptase-polymerasechain-reaction(RT-PCR)in 96.88%of cases.A total of 22(68.75%)patients had symptoms of SARS-CoV-2 infection in about 1 wk(7.91 d)preceding the onset of neurologic symptoms.The most common neurological symptoms were consciousness disturbance(59.38%),seizure(21.88%),delirium(18.75%),and headache(18.75%).Four cases were confirmed by positive RT-PCR results in cerebrospinal fluid(CSF),one was confirmed by positive RT-PCR results in postoperative brain tissue,and one by the presence of SARS-CoV-2 antibodies in CSF.The mainly damaged targets identified by neuroimaging included the temporal lobe(15.63%),white matter(12.5%),frontal lobe(9.38%),corpus callosum(9.38%),and cervical spinal cord(9.38%).Eighty percent of patients had electroencephalograms that showed a diffuse slow wave.Twenty-eight(87.5%)patients were administered with specific treatment.The majority(65.63%)of patients improved following systemic therapy.CONCLUSION Encephalitis/meningitis is the common neurological com

INTRODUCTIONCerebral fat embolism （CFE） is a rare but potentially lethal disease that is usually complicated by long bone fracture, which can develop over 12-72 h. Since the main fbrm of treatment for CFE is supportive, early diagnosis, prevention, and appropriate treatment can reduce mortality and the development of complications. However, due to symptom heterogeneity and imprecise diagnostic methods, CFE may be masked by other clinical conditions that make early detection challenging. Given its low incidence, most clinical studies of CFE are single cases or small case series. To characterize the features of CFE, clinical manifestations, laboratory examinations, and neuroimaging data were evaluated in a large sample of patients with CFE.

Background:The aim of this study was to retrospectively analyze our experience with the patients who underwent surgical treatment of posterior communicating artery(PComA)aneurysms originating from fetal posterior cerebral artery(fPCA)and analyze the risk factors for the postoperative radiological infarction and outcome.Methods:From 2011 to 2020,we retrospectively reviewed 74 PComA aneurysms originating from fPCA in terms of the clinical and radiological features and obtained the follow-up data from the Department of Neurosurgery,Beijing Tiantan Hospital,Capital Medical University.The relationships between these features and follow-up data were assessed with the univariate and multivariate analysis.Results:In this series,74 aneurysms were occurring at the origin of fPCAs.All the patients showed complete obliteration of their aneurysms.Full fPCA type tends to be a predictive factor for radiological infarction(univariateχ^(2)=5.873,P=0.027;multivariate OR=0.264,P=0.060).Postoperative radiological infarction(univariateχ^(2)=12.611,P=0.001;multivariate OR=6.033,P=0.043),rupture(univariateχ^(2)=4.514,P=0.047;multivariate OR=57.966,P=0.044),and hypertension(univariateχ^(2)=5.301,P=0.024;multivariate OR=24.462,P=0.029)tend to be the independent predictive factors for poor prognosis at 3 months after discharge.Conclusions:In conclusion,we report a series of patients harboring aneurysms originating from the fPCA.Surgical clipping is a reliable strategy.Full fPCA type is related to postsurgical infarction.Postoperative radiological infarction,rupture,and hypertension tend to be the independent predictive factor for poor prognosis at 3months after discharge.
