Background:This study aimed to summarize the clinical characteristics of patients with deep sternal wound infection-induced sepsis after median sternotomy and improve the treatment outcomes of infection-related ***:A retrospective cohort study was conducted on 21 patients with deep sternal wound infection-induced sepsis after median sternotomy who were admitted to the Department of Critical *** clinical manifestations,laboratory test results,infection control,and organ and nutritional support of the patients were summarized,and the follow-up data were ***:The primary symptoms of deep sternal wound infection-induced sepsis included dyspnea,high fever,chills,and altered state of *** test results revealed increased inflammatory markers and decreased oxygenation *** and liver function injury were observed in 8 and 4 patients,respectively;18 and 12 patients demonstrated elevated D-dimer and N-terminal Pro B type natriuretic peptide levels,*** the 8 patients whose wound secretions tested positive for bacteria,Acinetobacter baumannii and Staphylococcus aureus infections were present in 6 and 2 patients,*** of the 6 patients whose blood cultures tested positive for bacteria demonstrated Candida albicans *** patients received ventilator-assisted ventilation and 2 patients received renal replacement *** all the 21 patients,17 were cured,2 died,and 2 were ***:Postmedian sternotomy sepsis attributed to a deep sternal wound infection usually results from a preexisting *** most prominent clinical manifestation is dyspnea,which is sometimes accompanied by the impairment of organ *** prevention,proper nutrition support,and maintenance of healthy organ function are the cornerstones for successful treatment outcomes.

In order to better understand the clinical manifestation of systemic lupus erythematosus （SLE） with intracranial hypertension syndrome （IHS）, we analyzed the clinical features and treatment of a typical SLE patient with IHS. SLE is one of the most unpredictable autoimmune diseases involving multiple organ systems that is defined clinically and associated with antibodies directed against cell nuclei. IHS is an uncommon manifestation of neuropsychiatric SLE （NPSLE） and is characterized by an elevated intracranial pressure, papilledema, and headache with occasional abducens nerve paresis, absence of a space-occupying lesion or ventricular enlargement, and normal cerebrospinal fluid chemical and hematological constituents. IHS has been reported in a few sporadic cases in patients with SLE worldwide, but rarely has been reported in China. In this study, a 34-year-old female SLE patient with IHS was. reported and pertinent literature reviewed. The clinical presentation, image logical features, and investigatory findings were discussed.

Objectives To investigate the clinical feature of acute pulmonary embolism. Methods Retrospective clinical analysis was performed according to the data of 38 cases of pulmonary embolism. Results There were ground diseases and predisposing factors in 36 cases of pulmonary embolism among 38 cases, the ratio was 94. 7%, among the total predisposing factors, tumor, cardiovascular disease, venous thrombosis of lower extremity, smoking and long-term bed were common. There was no specificity in clinical feature, physical sign and rout chest X ray, electrocardiography, and their appearances were diversified. There were specificity and sensitivity in echocardiogram （UCG） and D-dimer to some extent. But, the final diagnosis must depend on some special examinations, such as selective pulmonary arteriography, CTPA, MRA and so on. Conclusions The special examinations must be done to make a definite diagnosis to confirm pulmonary embolism when the high risk factors and ground diseases are existing. It is necessary to some cases when the clinical feature can not be explained by other diseases.

AIM:To investigate the incidence,location,clinical presentation,diagnosis and effectiveness of endoscopic treatment of gastric Dieulafoy's lesion(DL)in China. METHODS:All patients who received emergency upper gastrointestinal(GI)endoscopy due to gastric DL from February 2000 to August 2008 at GI endoscopy center of Renmin Hospital of Wuhan University were included in this *** clinical presentation,medical history,location and characteristics of DL methods and effectiveness of therapy of patients with DL were retrospectively analysed by chart ***-term follow-up data were collected at outpatient clinics or telephone interviews. RESULTS:Fifteen patients were diagnosized with DL,which account for 1.04%of the source of bleed- ing in acute non-variceal upper GI *** comorbidities were found in one patient with hypertension and diabetic *** or combined therapy with hemoclip produced primary hemostasis in 92.8%(13/14) of patients. CONCLUSION:DL is uncommon but life-threatening in *** proved to be safe and effective in controlling bleeding from DL.

Background & Aim: Ulcerative colitis is a form of inflammatory bowel disease (IBD) characterized by idiopathic and chronic inflammation of the intestine. Incidence is highest in Caucasians and Jews and was considered rare in Asians. The exact etiology and pathogenesis of ulcerative colitis still remains elusive and debatable. The main symptoms are diarrhea with or without blood, abdominal pain, rectal bleeding but the symptoms vary with the race and geographical distribution and are dependent on the extent of colonic involvement and the severity of the disease. Endoscopy is the main stay for diagnosis and evaluation of the extent of the lesions in ulcerative colitis. Treatment is mainly with the 5-aminosalicylates and its derivative like sulfasalazine and mesalamine. Corticosteroids are used if the disease does not respond to the primary treatment. Refractory cases are treated with immounomodulators like Aziathropine and 6-Mercaptopurine. Though mortality rates from ulcerative colitis is low, its chronicity makes it a debilitating disease. In this study, we aim to report our experiences in the clinical features, diagnosis and treatment choices of ulcerative colitis.Methods: A retrospective and clinical analysis of 50 patients with ulcerative colitis between 2005 and 2007 was done. The diagnostic criteria was based on the guidelines for diagnosis and treatment of IBD approved in China in 2000. The clinical characteristics including sex, age, presenting complaints, investigations like hemoglobin levels, C-reactive protein levels, serological markers, colonoscopy findings like extent of the lesion and total number of days hospitalized were recorded. The study was designed to retrospectively analyze the severity of disease, its symptoms, extent of the lesions as seen on colonoscopy and treatment findings and to compare these results with similar studies done elsewhere.Results: 50 cases diagnosed with ulcerative colitis between 2005 and 2007 were taken for the

Objective: To study the outcome after the standardization therapeutics of ischemic stroke and the relatively factors which can influence the outcome of patients with stroke. Methods: Using data collected from 312 patients enrolled in the clinical standardization therapeutics trial, and neurological impairment at the hospitalization and the discharge was quantified using the China Stroke Scale (CSS), the outcome at the discharge using the percentage of CSS decreasing. The outcome was rated as cure if the percentage of CSS decreasing was over 89% and dead. Analyses were adjusted for age, history of previous disorders, and the form of onset, blood pressure at onset, complication. Results: The percentage of cure after the thrombolysis was 12.5%, after anticoagulant was 8.23%, after decreasing fibrinogen was 8.57% and after the convention treatment was only 5.66%. And at the same time, the percentage of dead after the thrombolysis was 12.5%, after anticoagulant was 8.23%, after decreasing fibrinogen was 8.57% and after the convention treatment was 10.1%. The other factors had no differently between the variability treatments. The complication could influence the outcome of stroke patients, especially the infect of respiratory, disturbance of kidney and electrolyte. Conclusions: The standardization therapeutics should be contributing the outcome of patients with ischemic stroke.

BACKGROUND:Human CD8 + CD28 - T-suppressor(Ts) cells have been considered to indicate a reduced need for immunosuppression in pediatric liver-intestine transplant recipients and recipients of deceased heart-kidney ***,in adult-to-adult living donor liver transplantation(A-A LDLT)little information is available and the clinical significance is still unknown. METHODS:Flow cytometry was used to detect the population of CD8+CD28 -Ts cells present in peripheral blood in A-A LDLT recipients(n=31),patients with end- stage liver disease(n=24)and healthy controls(n=19). Meanwhile,we tested the graft function and trough levels of immunosuppression in *** clinical and follow- up data of 31 transplant recipients were analyzed. RESULTS:Compared with diseased controls(P=0.007) and healthy individuals(P=0.000),a notable expansion of CD8 + CD28 - Ts cells was found in recipients of A-A *** was associated with graft function,levels of immunosuppression and rejection episodes. CONCLUSIONS:To monitor the CD8 + CD28 - Ts cells levels is important to evaluate the immune state of recipients. Meanwhile,it is also important to promote expansion of CD8+CD28 -Ts cells in recipients of A-A LDLT,not only to sustain good graft function and decrease the dosage of immunosuppressants,but also to reduce the occurrence of rejection.

BACKGROUND: Selection of drugs for antiviral therapy of patients with hepatitis B virus (HBV)-related acute-on-chronic liver failure (ACLF) remains difficult. This study was undertaken to evaluate the short-term efficacy of entecavir versus lamivudine on hepatitis B e antigen (HBeAg)-negative patients with ACLF. METHODS: The data of 182 HBeAg-negative patients with ACLF were retrospectively collected from patient profiles of the hospital. In these patients, 93 HBeAg-negative patients with ACLF were treated orally with 0.5 mg of entecavir and 89 were treated orally with 100 mg of lamivudine every day. The gender and age were matched between the two groups. Biochemical items, the model for end-stage liver disease (MELD) score, and HBV DNA level were matched at baseline between the two groups and monitored during treatment. The 3-month mortalities of the two groups were compared. RESULTS: No significant differences were found in biochemical items, MELD score, and HBV DNA level at baseline (P>0.05). HBV DNA level decreased within 3 months in both groups (P0.05). Pretreatment MELD score was not related to posttreatment HBV DNA levels (P>0.05), but related to a 3-month mortality in both groups (both P<0.001).CONCLUSIONS: In HBeAg-negative patients with ACLF, the short-term efficacy of entecavir versus lamivudine was similar. The degree of pretreatment liver failure significantly affected the outcome of treatment.
