BACKGROUND Amyloidosis is a rare disorder that can be classified into various types,and the most common type is the systemic light chain *** prognosis of this disease is extremely *** general,amyloidosis mainly affects the kidneys and heart and manifests as abnormal proliferation of clonal plasma *** in which the liver is the primary organ affected by amyloidosis,as in this report,are less common in clinical *** SUMMARY A 62-year-old man was admitted with persistent liver dysfunction of unknown cause and poor treatment *** condition persisted,and he developed chronic liver failure,with severe cholestasis in the later stage that was gradually accompanied by renal ***,he was diagnosed with hepatic amyloidosis through liver biopsy and pathological *** Hepatic amyloidosis rarely occurs in the clinic,and liver biopsy and pathological examination can assist in the accurate and effective diagnosis of this condition.

liver failure （LF） is defined as severe dysfunction in hepatic synthesis, detoxification, and metabolism induced by various etiologies. CHnical presentation of LF typically includes severe jaundice, coagulation disorder, hepatic encephalopathy, and ascites. LF can be classified into acute LF, acute-on-chronic LF （ACLF）, and chronic LF. ACLF has been demonstrated as a distinct syndrome with unique clinical presentation and outcomes. The severity, curability, and reversibility of ACLF have attracted considerable attention. Remarkable developments in ACLF-related conception, diagnostic criteria, pathogenesis, and therapy have been achieved. However, this disease, especially its diagnostic criteria, remains controversial. In this paper, we systemically reviewed the current understanding of ACLF from its definition, etiology, pathophysiology, pathology, and clinical presentation to management by thoroughly comparing important findings between east and west countries, as well as those from other regions. We also discussed the controversies, challenges, and needs for future studies to promote the standardization and optimization of the diagnosis and treatment for ACLF.

目的:探讨慢性肝衰竭患者虹膜特征与中医证型相关性。方法:以符合纳入标准的慢性肝衰竭150例,30例体检正常的人用以对照,使用亿衡EH-900U虹膜诊断仪,在清晨10时采集观察对象虹膜图像用于虹膜特征性分析。按照Jensen氏虹膜定位理论,利用CV Advance Analysis System,从右眼虹膜肝脏分区及双眼虹膜全局上坑洞、斑块、裂缝等改变作为观察指标。分析虹膜特征与证型之间的相关性。建立虹膜诊断证型判别模型。基于虹膜诊断理论及总结的虹膜特征与中医证型相关性、CV Advance Analysis System,虹膜诊断证型判别模型,采集慢性肝衰竭患者虹膜图像作出证型判别,并与临床医师的中医辨证进行一致性分析。结果:(1)慢性肝衰竭患者右眼虹膜肝脏分区,均无特征性改变,说明慢性肝衰竭患者在虹膜肝脏分区不存在相关性。(2)慢性肝衰竭患者阳黄组、阴黄组、阴阳黄组三个证型的虹膜特征与对照组比较的存在差异。(3)阳黄证与坑洞、裂缝存在相关性,与斑块不存在相关性,与裂缝这一特征性改变的相关性具有显著性。阴黄证与斑块、裂缝存在相关性、与坑洞的相关性具有显著性。阴阳黄证与坑洞、斑块、裂缝均存在相关性,但与斑块的相关性具有显著性。(4)建立的虹膜诊断判别模型,训练样本回代法示阳黄证判别符合率66.7%,阴黄证判别符合率60.0%,阴阳黄证判别符合率70%。三种证型判别的总符合率为65.6%。(5)虹膜特征证型判别与临床医师中医辨证的证型具有一致性。结论:虹膜诊断对慢性肝衰竭中医证型有意义。
