Analyzing the risk of drug abuse for morphine-containing chinese patent medicines in China

chinese patent medicines（CPMs） are widely used across China, and they are differently administrated from chemical drugs. For traditional reasons, CPMs may contain opium-derived compounds, such as morphine, which may pose the issues of drug abuse and addiction. In the present study, we aimed to evaluate the present risk of morphine-containing CPMs in causing drug abuse and addiction, based on a questionnaire-based approach and from a medicinal administrative perspective. We chose 76 CPMs containing morphine from government document and set them as the study group, and 10 chemical drugs containing morphine were set as the control group. Because there is no ready-made method, we created an evaluation sheet consisting of five questions to evaluate each drug and generate a risk-value score. Crosstabs analysis using SPSS 15.0 was conducted to obtain the influencing factors. We found that the study group obtained a mean score of 2.04 for the risk value, which was five times of that of the control group. Specifically, no maximum limit of morphine or codeine was set in product quality standards for more than four-fifth of the drugs. Moreover, 46.0% of package inserts failed to show any warning about addiction or prohibition over the long-term use of the drugs. The number of ingredients, as well as the functions of the drugs, was found to correlate with the risk value. Our findings suggested that CPMs were more risky in causing drug abuse and addiction than chemical drugs. Approaches, such as i） controlling the number of ingredients, ii） setting up maximum limit of morphine or codeine in quality standard, iii） providing more information on the package insert of drugs and iv） monitoring the drug containing both morphine and ephedrine, will be effective in reducing the risk of drug abuse and addiction.

A capillary electrophoresis with electrochemical detection（CE-ED） method was developed for the quality analysis of herbal medicine products prepared from the same herb of Herba Sarcandrae： Fufang Caoshanhu tablets, Qingrexiaoyanning capsules, and Xuekang oral liquids. Under the optimal analysis conditions, the low detection limit[l.0×10^-7 mol/L（S/N=3）] and the wide linear range（1.0×10^-7-1.0×10^-4 mol/L） were obtained for quality standard compound of isofraxidin. The precisions of the peak current and the migration time（as RSDs） for the real sample analysis were 2.0%-2.6%, and 1.2%-1.8% for isofraxidin, respectively. The contents of isofraxidin detected were 15.77 μg/tablet, 0.48 mg/capsule, 1.2 mg/ampoule（Jiangxi）, and 0.44 mg/ampoule（Dalian） for Fufang Caoshanhu tablets, Qingrexiaoyanning capsules, and Xuekang oral liquids from different manufacturers, respectively. Quality estimate was conducted by comparing the contents of isofraxidin in the herbal medicine products with the demanded values of chinese pharmacopeia. In addition, based on their own unique CE-ED profiles（namely, CE-ED electropherograms） the Xuekang oral liquids from the different manufacturers could be easily identified.

Developmental Toxicity and Potential Teratogenicity of Compound Danshen Tablet, Angong Niuhuang Pill, and Lidan Paishi Tablet in Zebrafish Embryos

Objective Herbal medicines containing toxic herbs or minerals such as Compound Danshen Tablet （CDT）, Angong Niuhuang Pill （ANP）, and Lidan Paishi Tablet （LPT） are avoided or used with caution for pregnant women because of potential teratogenicity. To understand their mechanism, they were chosen as model subjects for the research. Methods Zebrafish embryos were used to evaluate their potential teratogenic risk in vitro. Results All of them showed teratogenic and lethal effects in zebrafish embryos, with the ECs0 values at 351,793, and 220 μg/mL, and LC50 values at 41 7, 596, and 380 μg/mL, respectively. CDT and LPT, displaying week potential teratogenicity as their teratogenicity indexes were greater than 1, induced tail malformation and cardiac edema mainly in zebrafish embryos, respectively. Conclusion The results provide the significant guidance of clinical safety of medication.

Objective To explore ancient and modern medication laws of aromatic chinese medicines in treating angina pectoris, and to provide new ideas for the clinical *** With “angina pectoris” as the key word, ancient books prescriptions and chinese patent medicines related to angina pectoris were collected from China National Knowledge Infrastructure(CNKI), Traditional chinese Medicine Database System, chinese Medicine Prescription Database, New National Proprietary chinese Medicine(2 nd edition), and chinese Pharmacopoeia(2020 edition) from January 1, 2015 to December 31, 2021. Core highfrequency aromatic chinese medicines were defined, and their potential medication rules were analyzed and summarized. Microsoft Access 2010 was used for data management. Data analysis software, including Excel and IBM SPSS Modeler 18.0 were used for drug association rule analysis, and Cytoscape 3.7.2 for visual *** There were 67 ancient books prescriptions and 258 chinese patent medicines containing aromatic chinese medicines treating angina pectoris collected from relevant databases. In ancient books prescriptions, there were nine aromatic chinese medicines with the frequency ≥10, and the most commonly used medicine was Danggui(Angelicae Sinensis Radix), followed by Chenpi(Citri Reticulatae Pericarpium). There were 33 aromatic chinese medicines with the frequency ≥10 in chinese patent medicines, and the most commonly used medicine was Danshen(Salviae Miltiorrhizae Radix et Rhizoma), followed by Chuanxiong(Chuanxiong Rhizoma) and Sanqi(Notoginseng Radix et Rhizoma). In ancient books prescriptions, the medicines mainly belonged to intenal-warming medicines, Qi-regulating medicines, and blood circulation promoting and blood stasis removing *** were eight medicine pairs with confidence equal to 100% in ancient books prescriptions, the most frequently used pairs were Chuanxiong(Chuanxiong Rhizoma) +Danggui(Angelicae Sinensis Radix), and Xiangfu(Cyperi Rhizo
